Abstract

Quality health services are an important factor in increasing patient satisfaction and loyalty. The 1945 Constitution of the Republic of Indonesia affirms the right of the community to get adequate health services, which is the responsibility of local and central governments. Hospitals as the main healthcare providers must meet the expectations of patients not only in terms of treatment but also service experience. This study examines the relationship between service quality, satisfaction, and patient loyalty in hospitals. With increasing competition in the healthcare sector, hospitals must improve the quality of services to retain patients. The study also highlights external factors such as cost, accessibility, and socioeconomic conditions that influence patients' responses to service quality. The results of the research are expected to provide insight for hospital management to formulate better policies and programs, so that the services provided continue to improve and create patient satisfaction and loyalty.

Abstract

Background: The high sepsis mortality rate is a significant problem that must be managed properly and comprehensively. Studies showed 30% mortality due to sepsis, 50% for severe sepsis, and 80% for sepsis shock. Data in Indonesia showed that 30.29% of sepsis patients were admitted, and 11.56% to 49% died. Vitamin D deficiency is associated with an increased risk of infection in sepsis. Objective:This study aims to determine the factors that play a role in the death of sepsis patients. Methods: This study used a cross-sectional analytic design. Data were tak-en from 42 patients who died from sepsis in the Intensive care unit of Dr. M. Djamil Padang Hospital from July 2022 to Sep-tember 2022. Patients were previously checked for vitamin D levels—data analysis using the Man Whitney test. Results:The average age of sepsis patients is 56 years old, female (59.9%), vitamin D deficiency (82.5%), obesity (64.3%), most comorbid with chronic kidney injury (CKD) (16.7%). Sepsis pa-tients who died of vitamin D deficiency had a mean Hb of 11.5 g/dl; leukocytosis (14.490/mm3), thrombocytopenia (225.000/mm3), hypoalbumin (2.4 g/dl) and high risk to sepsis based on procalcitonin levels (15.47 ng/ml). There was no sta-tistically significant relationship to the laboratory results (pValue>0.05).Conclusion:Vitamin D deficiency, female, obesity, CKD, thrombocytopenia, and hypoalbumin increase the mortality risk of sepsis patients in the intensive care Unit. The importance of recognizing demo-graphic and laboratory risk factors helps clinicians in preventing sepsis mortality. Background: The high sepsis mortality rate is a significant problem that must be managed properly and comprehensively. Abstract

Background: Tuberculosis (TB) and lung cancer cause significant morbidity and mortality worldwide and pose a global health threat. Each year these two diseases account for more than 1.6 million deaths worldwide. The incidence of both diseases is still high in many developing countries, especially in Asian countries. TB and lung cancer are often confused and misdiagnosed, especially in countries with diagnostic challenges of low TB incidence and risk of missed diagnosis.Case Presentation: The following is a case report of a 53-year-old male patient diagnosed with pulmonary TB accompanied by right lung cancer, and the same respiratory complaints can be had by lung cancer and TB. However, the presence of facial edema (part of the superior vena cava syndrome) causes clinicians to focus more on lung cancer so that the diagnosis of TB is often overlooked. Conclusion: Tuberculosis should be a significant concern, especially in patients with malignancies such as lung cancer and located in TB endemic areas. Delay in diagnosis and or miss diagnosis will affect the patient's outcome.
