Abstract

Hepatocellular carcinoma (KHS) is 90% of primary tumors in the liver. One of natural treatment that have anti-cancer activity is propolis. Its anti-cancer activity works through induction of apoptosis and inhibition of cell proliferation. Bcl2 belongs to the family group B-cell lymphoma 2 protein which acts as an intrinsic pathway apoptotic protein. p21 is a tumor suppressor protein that has a primary function in inhibiting cell cycle progression. This study was aimed to determine the anti-cancer effect of propolis ethanol extract (EEP) originating from Kerjo, Karanganyar on hepatocellular cancer cell culture (Hep G2) through its potential as an anticancer in increasing p21 protein expression, reducing bcl2 protein expression in suppressing apoptosis and reducing cell proliferation . This research is an experimental laboratories with post test with control group design. Performed on Hep G2 cells with the treatment of EEP concentrations (½IC50, IC50, 2IC50), sorafenib IC50, a combination of EEP IC50 + sorafenib IC50, and controls. Observation of Bcl2 and p21 protein expression by immunocytochemistry method, observing cell proliferation with doubling time, observing apoptosis with flowcytometry. We use ANOVA statistical test, followed by the Tuckey post hoc test, were stated to be significant if p 0.05. The inhibition concentration of 50% Hep G2 cells by EEP was 65.2 μg / mL, and sorafenib (6.08 μg / mL). EEP concentration of 2IC50 (p = 0.001), sorafenib 1 IC50 (p = 0.001), 1IC50 EEP + 1IC50 sorafenib (p= 0.001) able to increase p21 protein expression. The 2 EEP concentration (p= 0.040), was able to reduce the expression of Bcl2 protein, with p= 0.001. The strength of EEP 1IC50 is equivalent to sorafenib IC50 in suppressing 24-hour proliferation (p 0.05). The strength of EEP ½IC50 is equivalent to sorafenib IC50 in suppressing proliferation 48 hours (p 0.05). The strength of EEP 1IC50 is better than sorafenib IC50 in suppressing proliferation of 72 hours (p 0.05). All EEP concentrations increase p21 protein expression, with the EEP 2IC50 group providing the strongest effect. All EEP concentrations were able to reduce cell proliferation, with the EEP 2IC50 group, giving the strongest effect. It was concluded that concentration of 2IC50 EEP has an effect on decreasing the expression of bcl2 protein and hepatoma cell apoptosis.Keywords: Ethanol Extract Propolis, Bcl2, P21, Apoptosis, Proliferation, Hep G2 Cells

Abstract

BACKGROUND: Cisplatin is one of the most potent chemotherapy drugs to treat various types of cancer, however the use of cisplatin has some the adverse effect, such as the increase of oxidative stress and inflammation by malondialdehyde (MDA) and nuclear factor kappa B (NF-kB) activation. Since the dosing of cisplatin is critical, we observed the effect of single and multiple doses of cisplatin injection on rats’ inflammation and oxidative stress level.METHODS: Total of 27 male Sprague-Dawley rats were divided into 9 sub-groups, each consisted of 3 rats. The baseline sub-group received no treatments; Group 1 (sub-group 1.1, 1.2, 1.3, and 1.4) were administered one single dose of 5 mg/kg BW/intravenously (i.v) of cisplatin; and Group 2 (sub-group 2.1, 2.2, 2.3, and 2.4) were given 0.2 mg/kg BW/i.v of cisplatin twice a week for two months. Rats were observed for their body weight, NF-kB, and MDA level based on the assigned group. RESULTS: Body weight loss was observed in the 1st week after treatment for Group 1, and 7th week for Group 2. Group 1 and Group 2 showed increasing level of NF-kB and MDA since the 1st observation, which was the 1st week and 5th week, respectively. NF-kB and MDA and levels were also significantly increasing in both groups for every week of observation (p<0.05).CONCLUSION: Cisplatin injection either in single or divided dose can induce inflammation and oxidative stress thus decrease the body weight. However, dividing cisplatin in smaller dose can delay the inflammation effect on subjects.KEYWORDS: cisplatin dose, MDA, NF-kB, body weight, inflammation, oxidative stress

Abstract

Anak talasemia mempunyai risiko mengalami stress oksidatif. Suplementasi sari kurma bisa menjadi alternatif untuk mencukupi asupan gizi dan antioksidan. Penelitian ini bertujuan untuk menganalisis efek suplementasi sari kurma sebagai sumber antioksidan dan teh hijau terhadap penurunan kadar MDA plasma sebagai salah satu penanda stress oksidatif pada anak dengan talasemia.Penelitian dilaksanakan di Kota Magelang, pada komunitas penderita talasemia β mayor se-eks Karisidenan Kedu yang rutin menjalani terapi di RSUD Tidar Kota Magelang pada bulan Oktober 2014. Metode penelitian menggunakan randomized pretest-posttest control group design. Penderita talasemia mayor usia 10-15 tahun dipilih secara random dan dikelompokkan menjadi kelompok yang diberi suplementasi sari kurma bersama teh hijau dosis 20% dari Angka Kecukupan Gizi dan kelompok kontrol yang hanya diberikan teh hijau saja. Perlakuan selama 28 hari, sebelum dan sesudahnya dilakukan pengambilan sampel darah untuk pemeriksaan kadar MDA plasma.Berdasarkan analisis statistik uji t perpasangan diperoleh hasil bahwa anak dengan talasemia yang diberi suplementasi sari kurma bersama teh hijau mengalami penurunan kadar MDA plasma yang signifikan (t: 3.0222, Sig. (2-tailed): 0.014) dengan rata-rata penurunan 0.71 ± 0.07 μmol/l. Pada kelompok kontrol, hasil t: 2.502, Sig.(2-tailed): 0.034, signifikan dengan rata-rata penurunan 0.65 ± 0.82 μmol/l. Pemberian suplementasi sari kurma bersama teh hijau dan konsumsi teh hijau saja pada anak talasemia berefek menurunkan kadar MDA plasma secara signifikan. Children with thalassemia have a risk of experiencing oxidative stress. Date juice supplementation can be an alternative to improve nutritional and antioxidants intake. This research aims to analyze the effects of date palm supplementation and green tea as a source of antioxidants to decrease MDA levels as one of the markers of oxidative stress in children with thalassemia.The research was conducted in Magelang City, in the community of β major thalassemia patients in Kedu Residency who were routinely undergoing therapy at Magelang Regional Public Hospital Tidar in October 2014. The research used randomized pretest-posttest control group design. Children with major thalassemia age of 10-15 years were selected randomly and grouped into supplemented group with palm juice and green tea with a dose of 20% of the Nutrition Adequacy Rate and control group given only green tea alone. Treatment was given for 28 days, before and after the blood samples were taken for examination of plasma MDA levels. Based on statistical analysis of paired t-test, it was found that children with thalassemia supplemented with date juice and green tea had decreased plasma MDA levels significantly (t: 3.0222, Sig. (2-tailed):0.014) with decrease averages were 0.71 ± 0.07 μmol / l. In the control group, the result shows there is significant decreased of plasma MDA levels (t: 2.502, Sig. (2-tailed): 0.034), average of decreases were 0.65 ± 0.82 μmol / l. Provision of date palm juice supplementation with green tea and consumption of green tea alone in children with thalassemia effected in decreased plasma MDA levels significantly.

Abstract

Elderly is a global concern since the number of elderly populations is increasing, which also become one of the concerns of the national health insurance (JKN). Health insurance for the elderly has a package of benefits according to physical conditions and degenerative diseases. Since the elderly is susceptible to disease, Indonesia has Healthcare and Social Security Chronic Disease Management Program, National Health Insurance known as Badan Penyelenggara Jaminan Sosial Program Pengelolaan Penyakit Kronis (BPJS Prolanis) to guarantee the health of elderly. This study aims to obtain an overview of the implementation of BPJS Prolanis in the community which located in Yogyakarta in order to give the feedback for this program. This research using qualitative method, with phenomenological research approach, which conducted in Yogyakarta during 2020. The result showed that most of the elderly, which is 221 (77.5%) elderly already have the health insurance, but only 38.9% who has BPJS Prolanis. Besides, there are more than 50% of the elderly have disease that often complained of. Hence, this study proposed suggestions for BPJS Prolanis, such as giving a continues education for the elderly through public health center. Besides, public health center can hold a promotive and preventive activities for the elderly to increase the awareness of the elderly on having the BPJS Prolanis.

Abstract

BACKGROUND: Anthrax is a zoonotic disease caused by Bacillus anthracis, whose endospores stimulate the release of pro-inflammatory cytokines and promote oxidative stress. Propolis, a natural resource that can be found in Indonesia, has been proven to have anti-apoptotic and antioxidant role which might be a potential adjuvant therapy for anthrax treatment. Hence in this study we aimed to investigate effect of propolis as an anti-inflammatory, anti-apoptotic, and antioxidant in anthrax rats model by examining the level of tumor necrosis factor (TNF)-α, caspase-3, and malondialdehyde (MDA), respectively.METHODS: This was an experimental post-test only study with 40 male rats weighed 180-200 g that were induced by anthrax spores injected subcutaneously. The rats were divided into one control positive group and four intervention groups that were administered with 200 mg/kgBW propolis extract for 7 to 14 days. The levels of serum TNF-α, caspase-3, and MDA were measured using enzyme-linked immunosorbent assay (ELISA) and analyzed with bivariate analysis.RESULTS: TNF-α, caspase-3, and MDA level were found lower in anthrax rats model given ethanol extract of propolis than the control group. The lowest concentration of TNF-α value was found in group administered with propolis extract 200 mg/kgBW for 7 days before the anthrax induction (6.136±0.205 pg/mL). The results were similar to the MDA serum and caspase-3 which were the lowest when the propolis was administered 7 days earlier (1.893±0.188 nmol/mL and 2.040±0.067 ng/mL). There was significant difference in the TNF-α, caspase-3, and MDA serum levels (p≤0.001) compared to control group.CONCLUSION: Propolis has anti-apoptotic and antioxidant effects which can be used as complementary therapy in anthrax infection.KEYWORDS: propolis, anthrax, anti-apoptotic, antioxidants

Abstract

Propolis is an herbal medicinal plant that contains caffeic acid phenethyl ester. This natural polyphenol compound acts as an antioxidant and can reduce blood glucose levels and increase body weight. This study aims to determine the effectiveness of Gunung Lawu propolis in lowering blood glucose levels and increasing body weight in diabetic rats. The Wistar rats were induced with STZ 45 mg/kg BW and nicotinamide 110 mg/kg BW as diabetic model rats. Research subjects were taken randomly and divided into five groups: (1) Normal, (2) DM with no propolis, (3) DM 14 days + propolis 100 mg/kg BW/day (P1), (4) DM 14 days + Propolis 200 mg/kg BW/day (P2), and (5) DM 0 day + Propolis 200 mg/kg BW/day (P3). Propolis extract was given orally once a day for 14 consecutive days (in groups 3 and 4) and 28 consecutive days (in group 5). Blood glucose levels in the treatment group (P1, P2, and P3) decreased significantly (P1: 115.28 ± 4.7 mg/dL; P2: 98.36 ± 4.8 mg/dL; and P3: 87.36 ± 4, 2 mg/dL) compared with that in the DM group (272.07 ± 3.9 mg/dL). The body weight of the treatment group (P1, P2, and P3) increased significantly (P1: 180.60 ± 5.7 g; P2: 180.60 ± 4.2 g; and P3: 208.00 ± 5.1 gr) compared with that in the DM group (160.00 ± 3.2 gr). In conclusion, Propolis is effective in reducing blood glucose levels and improving (increasing) body weight in diabetic model rats.

Abstract

BACKGROUND: Diabetes, which causes various complications, involves pro-inflammatory cytokines such as tumor necrosis factor-alpha (TNF-α), nuclear factor kappa B p65 (NF-κB p65), interleukin-6 (IL-6), cluster of differentiation 4 (CD4), and matrix metalloproteinase-9 (MMP-9). Magnesium has demonstrated anti-diabetic properties, but its anti-inflammatory effects in preventing cardiovascular complications remain unclear. This study aimed to evaluate the anti-inflammatory effects of magnesium citrate, alone and in combination with metformin, by measuring TNF-α, NF-κB p65, IL-6, CD4, and MMP-9 expression in diabetic model rats.METHODS: Thirty male Wistar rats were divided into five groups: normal control, diabetes control, metformin (treated with 9 mg/200 g/day metformin), magnesium citrate (treated with 3.6 mg/200 g/day magnesium citrate), and combination therapy (treated with 4.5 mg/200 g/day metformin + 1.8 mg/200 g/day magnesium citrate). Diabetes was induced in all groups except the normal control group using streptozotocin (STZ) and nicotinamide (NA). TNF-α, NF-κB p65, IL-6, CD4, and MMP-9 expression levels were measured using enzyme-linked immunosorbent assay (ELISA).RESULTS: Significant differences in TNF-α, NF-κB p65, IL-6, CD4, and MMP-9 expression levels were observed across all groups (p<0.001). The combination therapy group demonstrated the most significant reduction in all parameters compared to the diabetic control group (p<0.001) and other therapy groups. Both metformin and magnesium citrate monotherapies showed moderate reductions in cytokine levels but were less effective than combination therapy.CONCLUSION: Combination therapy with metformin and magnesium citrate exhibited the most potent anti-inflammatory effects, significantly reducing TNF-α, NF-κB p65, IL-6, CD4, and MMP-9 expressions in diabetic Wistar rats. Abstract

BACKGROUND: Methanol intoxication is associated with significant morbidity and mortality, particularly when acute kidney injury (AKI) developed. Emerging evidence implicates Endothelin-1 (ET-1) and Nucleotide-binding domain leucine-rich repeat-containing pyrin receptor 3 (NLRP3) inflammasome in renal injury, but their roles in methanol-induced AKI remain unclear. To date, no studies have examined whether hydroxychloroquine or folinic acid, which are known to modulate ET-1 and NLRP3 signaling, could mitigate renal injury in this setting. This study evaluated their therapeutic effects in a rabbit model of methanol-induced AKI.METHODS: The animals subjects were randomly assigned to four groups: control group receiving aquabidest, folinic acid group receiving 2 mg/kg body weight (BW) intraperitoneal folinic acid, hydroxychloroquine group receiving 30 mg/kg BW oral hydroxychloroquine phosphate, and combination group receiving both folinic acid and hydroxychloroquine at the same dosages. Histopathological evaluation of tubular injury scores and immunohistochemical analysis of ET-1 and NLRP3 expression were then conducted.RESULTS: Expressions of ET-1, NLRP3, and tubular injury scores were significantly lower in the hydroxychloroquine, folinic acid, and combination therapy groups compared to the control group (p<0.001). Expression of ET-1 was lowest in folinic acid group (59.38±0.71%), followed by combination group (62.23±1.98%) and hydroxychloroquine group (62.43±1.81%), compared to control group (72.14±1.02%). Expression of NLRP3 was lowest in combination group (58.94±1.05%), followed by folinic acid and hydroxychloroquine group, which showed equal values (60.57±1.38%), compared to control group (72.15±1.02%). Tubular injury scores were also lowest in combination group (27.07±3.16%), followed by hydroxychloroquine group (45.29±1.75%) and folinic acid group (48.38±2.49%), compared to control group (77.15±1.66%).CONCLUSION: Expression of ET-1 and NLRP3, as well as tubular injury scores, are significantly lower in all treatment groups compared to control, suggesting hydroxychloroquine and folinic acid demonstrated renoprotective effects in methanol-induced AKI, likely through modulation of ET-1 and NLRP3 pathways.KEYWORDS: methanol intoxication, acute kidney injury, hydroxychloroquine, folinic acid, endothelin-1, NLRP3 inflammasome, experimental animal models, rabbits

Abstract

Latar Belakang : Diabetes Melitus (DM) merupakan kelompok penyakit metabolik kronik dengan karakteristik hiperglikemi yang meningkat prevalensinya di berbagai negara. Keadaan hiperglikemia pada DM menyebabkan terjadinya stres oksidatif yang dapat memacu terjadinya komplikasi baik mikro maupun makro vaskuler dengan indikator antara lain gula darah puasa dan kolesterol-LDL ˃ 100 mg/dL. Stres oksidatif dapat dihambat dengan mengkonsumsi makanan yang mengandung antioksidan yang tinggi. Buah naga merah mengandung sumber antioksidan yang tinggi. Penelitian ini bertujuan untuk mengetahui pengaruh konsumsi buah naga merah terhadap kadar glukosa darah puasa dan kolesterol-LDL pada penderita DM tipe 2.Metode : Penelitian ini adalah penelitian eksperimental klinis  dengan rancangan pre and post randomized controlled group design. Subjek penelitian adalah penderita DM tipe 2 di klub diabetisi Prolanis Poliklinik Rejosari Husada, Delanggu, Klaten. Kelompok perlakuan diberi buah naga merah dalam bentuk potongan sebanyak 180 gram/hari selama 15 hari. Kadar glukosa darah puasa dan kolesterol-LDL diukur sebelum dan sesudah perlakuan. Data dianalisis dengan uji Kolmogorov-Smirnov untuk uji normalitas data kadar glukosa darah puasa dan kolesterol-LDL, uji paired t-test untuk mengetahui perbedaan rerata pretest dan posttest dalam satu kelompok setelah dilakukan perlakuan, uji independent t-test untuk mengetahui perbedaan selisih rerata pretest dan posttest antara kelompok yang diberi perlakuan berbeda dan asupan gizi dari makanan hasil 24 hours food recall dianalisis dengan menggunakan program Nutrisurvey.Hasil  : Tidak terdapat penurunan kadar glukosa darah puasa baik pada kelompok perlakuan maupun pada kelompok kontrol. Terdapat penurunan signifikan kadar kolesterol-LDL sebelum dan sesudah perlakuan pada kelompok perlakuan (p 0,02), terdapat penurunan kadar kolesterol-LDL pada kelompok kontrol tidak signifikan (p 0,31) dan asupan energi dari makanan subjek rata-rata 134,46% kebutuhan rata-rata.Kesimpulan : Konsumsi buah naga merah dapat menurunkan kadar kolesterol-LDL pada penderita DM tipe 2. Konsumsi buah naga merah tidak dapat menurunkan kadar glukosa darah puasa pada penderita DM tipe 2. Pengaturan makan untuk pada pasien DM dengan pengendalian asupan terutama sumber energi penting dalam kendali kadar glukosa darah dan kolesterol-LDL. Keywords : buah naga merah, penderita DM tipe 2, glukosa darah puasa, kolesterol-LDL
