Abstract

The implementation of maintenance medical record documents in RSUP Dr. Sardjito is not optimal. This statement was proven by the damage of medical record documents about 35,05%. The purpose of this research is to review the implementation of maintenance medical record documents uses management elements 5M (man, money, material, machine, and method). The research type is qualitative and the data collection techniques use interviews, observations, and documentations. The subjects of this research are filling officers. The result from man factor shown that filling officers have not received maintenance medical record documents training, meanwhile money factor shown that filling officers did not know about the budget management of maintenance medical record documents. The result from material factor show that the materials for medical records forms and folders have been provided optimally, meanwhile machine factor shown that the type of rack from combination of iron and wood was less suitable because it can made medical record documents were damage. The result from method factor show that standard operational procedure about implementation of maintenance medical record documents was not available. The solutions are giving socialization about implementation of maintenance medical record documents to filling officers, providing standard operational procedure about implementation of maintenance medical record documents, coating around the sharp part of rack using rubber tires that can prevent the damage of medical record folder.

Abstract

Job satisfaction is an important factor that can affect employee performance. If the employee feel satisfiedwith their work, the employee will produce good performance, and vice versa. Based on observations andinterviews with one of the medical recorders, it is known that they never get a reward if they work well inaccordance with applicable procedures. In addition, facilities that support their work such as dumbwaiter andtroli sometimes suffer damage. This condition resulted in work performed by medical recorders at RSUP Dr.Sardjito Yogyakarta is not optimal, so it will affect job satisfaction. The impact of this condition was found alateness providing the patient's medical record to the health services unit or sometimes wrong distribution ofmedical record files that are not in accordance with the patient's destination services unit. This conditioncauses the examination of patients by health professional also hampered. The purpose of this research is toidentify the relationship of job satisfaction with medical recorder at RSUP Dr. Sardjito Yogyakarta. The methodof data collection in this research is a questionnaire. Analysis of the data used is univariate analysis andbivariate analysis using the Spearman rank correlation test. The results showed that the level of jobsatisfaction of medical recorders at RSUP Dr. Sardjito Yogayakarta is classified as moderate (83.3%), whilethe level of performance of the medical records officer at RSUP Dr. Sardjito Yogyakarta is classified as high(50%). Spearman rank correlation test results indicate that there is a relationship with a moderate categorybetween job satisfaction with the performance of medical recorders at RSUP Dr. Sardjito Yogyakarta with asignificance value or p value 0.006 and a correlation coefficient of 0.487.

Abstract

Occupational stress is recognized as a global health issue that affects all professions, no other profession inthe health field i.e. medical record registration section. RSUP Dr. Sardjito Yogyakarta has a total data visitannually on an outpatient registration, which is in 2018 as much as 469813 and in 2019 as many as 518806.The number of visits explained at least the registration staff should serve 200-300 patients every day. Knownerrors that occur in SIMETRISS also harm the officer because inevitably officers should double job. TheStress of work felt by the officers of the list is often felt muscle tension, feel tension, fast anger, feel anxiety,and difficulty concentrating in the work. Stressful work if not resolved will have an impact on activities in thescope or out of work. The research aimed to figure out the work stress overview of the registration officers inRSUP Dr. Sardjito Yogyakarta. The collection methods used are interviews. The analysis of the data used inthis study was qualitative. The results of the study showed work stress experienced by outpatient registrationofficers, that is caused by a physical work environment that was less comfortable so later would cause a lackof concentration by the officers and the declining of officers performance; the conflict between family demandsand the institution demands such as the lack of break time and the presence of a sustained shift; performancedemands to work quickly and precisely however the support facilities, Simetriss, which was still error alsoraised the pressure for the worker. RSUP Dr. Sardjito Yogyakarta has reduced some problems by conductingeffective consultation on outpatient registration officers.

Abstract

Based on the results of a preliminary study conducted at Hasan Sadikin Hospital, it is known that fromFebruary to March 2020 the achievement of the 20 files per day assembling task target has not been fullyachieved. 7 out of 9 assembling officers work 7 to 15 files and there was an inconsistency of the additionalform's location that assessed by each officer. This resulted in narrow space and files that were stacked toolong causing the medical record file to be damaged and the location of the additional forms to be different.This study aims to describe the level of work productivity based on the environment and the work motivationof assembling officers Dr. Hasan Sadikin 2020. This type of research uses quantitative by observing anddistributing questionnaires to respondents. The results obtained are that the assembling officer at RSUP Dr.Hasan Sadikin who has a higher level of productivity are officers with male sex, age categories 35-49 years,length of work for more than 10 years, last education diploma IV / S1, and with marital status.

Abstract

The implementation of the National Health Insurance program in hospitals as referral Health Service Provider in collaborate with the Indonesia Health Insurance Implementing Agency to provided public health services by using a prospective payment system. Based on preliminary study, it known the inpatient claim file had delayed as much 11.38%. It’s not according to hospital standards that claimed should be 100%. This research aimed to provided a preview the causing factors of delayed inpatient claims at RSUP Dr. Hasan Sadikin Bandung based on 5M management elements. This research was descriptive qualitative with expose the results of interviews and observations that had been done in comparison of the theories available. The results showed that the officers still found difficulties to read the diagnosis and the patient's actions due to doctor’s handwrinting difficult to read, incorrect diagnosis codes and actions due to differences in perception between the coder and the doctor, incomplete claimed files such as the result of the patient's supporting examination. There was no standard operational procedure as regulation of inpatient BPJS claimed requirements, computers that been used in the process of filing claims often experiencing long loading and often experiencing network disruptions so it could reduce the performance of officers. In response to those factors, it needs to make a standar operational procedure for completing claim files, to conduct training specifically about National Health Insurance coding at least once a year and doing maintenance on the computer at least once a month.

Abstract

Registration of loans and refunds of medical records at RSD Mangusda Badung still there is an obstacle that is the loan and refunds data of medical records in the period of 1x24 hours hours is not complete and, there is no description of loan and return time  of medical record documents in the expedition. This research aims to design and create information system for loans and refunds the  medical records by using waterfall method at RSD Mangusada Badung. This research was conducted in RSD Mangusada Badung. The method of this research used is a qualitative method with the waterfall system development method. The data collection methods used are interviews, observations and documentation. The result of this research is the design and creation of a loan information system for the return of medical record documents using waterfall method is expected to facilitate the officer filing at Puskesmas Banjarsengon to do their duties.

Abstract

Performance is a work that can be achieved by a person or a group of people in one organization, inaccordance with the authority and responsibilities of each, in order to achieve organizational goals. Thefactors that affect performance are motivation, opportunity, ability. Based on the results of a preliminarystudy at RSUP Dr. Hasan Sadikin, Bandung, the performance of the coding officer and JKN claims wereconsidered to be less than optimal, because there were still many medical record files that were not yetencoded. Resulting in a delay in BPJS payments to hospitals. The purpose of this study was to evaluate theperformance of the coding officer and JKN claims for inpatients at Dr. RSUP. Hasan Sadikin Bandung. Thistype of research is a qualitative study by describing the situation at the time of the study with the aim ofproviding a clear picture of the performance of the coding officer and inpatient JKN claims. The results ofthis study were the first results coding officers and inpatient JKN claims never get a reward from theleadership let alone get praise and certificates of appreciation for their performance in accordance withexisting procedures. Both the coding officer and the inpatient JKN claim have attended training and 3seminars on medical records especially about codification. Thirdly, the existence of SPO regarding theimplementation of coding and claims of JKN is hospitalized so that it can facilitate the officers in carrying outtheir duties.

Abstract

One of the accreditation instruments is Patient and Family Rights (HPK) 6.3 which states that patients andtheir families were provided with an explanation of the scope of the general consent. Based on preliminaryresearch at the outpatient registration unit at RSUP dr. Sardjito Yogyakarta, the average new registeredpatients reached 43.25 patients per day and 89% of them did not receive an explanation about thecontents of the general consent form. This research aims to identify the current general consent form foroutpatients in 2020, identify the procedure applied in delivering information of the general consent foroutpatients, and identify factors obstructing the application of the general consent. This research wasqualitative. The data were collected through interviews and observations. The subjects of the researchinvolved officers of the inpatient registration unit at RSUP dr. Sardjito Yogyakarta and the head of themedical record and health information department. The results of this research show that the registrationofficers do not inform the general consent information for new patients because they have more concernon the speed of registration process without telling the content of general consent and unavailability ofSOP in providing general consent information that cause less proper work implementation.

Abstract

Puskesmas Patrang merupakan salah satu puskesmas yang sudah menggunakan aplikasi Primary Care (P-Care). Aplikasi ini digunakan untuk mengentrikan data sosial, data pemeriksaan pasien dan pembuatan surat rujukan untuk pasien BPJS. Dalam pelaksanaannya masih ditemui berbagai kendala yang dialami oleh pengguna,yaitu  seperti aplikasi p-care yang mengalami error,  pembuatan surat rujukan hanya dilakukan oleh 1 petugas serta pengguna harus melakukan double entry data guna untuk kebutuhan laporan yang dibutuhkan oleh puskesmas. Tujuan dari penelitian ini adalah mengevaluasi implementasi aplikasi  Primary   Care dengan   menggunakan    metode   TTF   (Tasks  Technology  Fit) . Jenis penelitian ini adalah penelitian kualitatif dengan pendekatan deskriptif. Hasil evaluasi implementasi aplikasi Primary care   di Puskesmas Patrang menunjukkan bahwa dari 5 responden menyatakan karakteristik  tugas sudah sangat baik, semua tugas yang dilakukan sudah terfasilitasi disetiap menu aplikasi. Kriteria karakteristik dalam penerapan teknologi yang digunakan sudah baik, meskipun ketersediaannya belum merata. Kriteria kesesuaian tugas yang dirasakan oleh pengguna sudah sesuai dengan teknologi yang digunakan. Kriteria kebermanfaatan diperoleh bahwa pengguna merasakan butuh dengan aplikasi ini. Serta kriteria dampak kinerja dalam penerapan aplikasi ini yaitu ini sudah dirasakan dengan baik. Upaya penambahan kebutuhan pendukung seperti komputer dan scanner, penggunaan SIMPUS serta pembuatan SOP perlu diterapkan agar implementasi aplikasi primary care bisa berjalan dengan baik.

Abstract

The process of borrowing and returning medical record at Ambulu Public Health Center still use manual book and make return of medical record documents that are not in accordance with SOP (Standard Operational Procedure). The purpose of this research is to design and create an information system of borrowing and returning the medical records based SMS gateway at Ambulu Public Health Center. The method used in this research is waterfall. The stages of the waterfall is requirements analysis, system design, create system, and system testing. The data collected by interview, observation, and FGD (Focus Group Discussion). The results of this research is the information systems of borrowing and returning the medical records based SMS gateway as message or automatic reminder. The suggestion for Ambulu Public Health Center is use information system of borrowing and returning the medical record based SMS gateway can make simple the reports.
