Abstract

This study was to analyze the optimum dose of IL-6 induction in pregnant rats that could trigger an increase in mean arterial pressure and protein as two symptoms of preeclampsia. A total of 25 pregnant rats was divided into 5 groups, including pregnant rats the control group (without induction of IL-6), a group of pregnant rats were given an induction in IL-6 doses of 1.25 ng/day, a group of pregnant rats given doses of IL-6 induction 2.5 ng/day, a group of pregnant rats were given an induction of IL-6 doses of 5 ng/day, and groups of pregnant rats were given an induction in IL-6 doses of 10 ng/day. Induction of IL-6 was performed on the tenth day of gestation for 5 days. The expression of caspase-3, IL-17, and STAT3 were analyzed by confocal laser scanning microscopy. The expression of caspase-3, IL-17 and STAT3 was significantly higher in preeclampsia group than the control group (p<0.05). The expression of caspase-3 decreased significantly in all groups were treated by phycocyanin compared to the preeclamptic group (p<0.05), but has not been able to reach expression comparable to the control group (p<0.05). Expression of IL-17 decreased significantly in the group given the two highest doses of phycocyanin compared to the preeclampsia group (p<0.05). STAT3 expression decreased significantly in all groups were treatedby phycocyanin compared to the preeclamptic group (p<0.05), reached expression comparable to the control group in the group received phycocyanin at doses of 10 and 20 ng (p> 0.05). In conclusion, IL-6 on pregnant rats were able to increase apoptosis through IL-17 and STAT pathway. Inhibition of apoptosis due to phycoyanin treatment not only involve the formation of IL-17, this data is found in phycocyanin doses of 10 and 20 ng.

Abstract

The placenta is a joint organ of the uterus surface mother and fetus. One of cytokines of the placenta is IL-6 which is essential for normal placental development and successful pregnancy. Sgp130 binds Interleukin-6-sIL-6R complex, so Interleukin-6 signal cannot be passed on. Both factors play an important role in pre-eclampsia which has high levels of IL-6 and sgp130. They are expected to be suppressed by administration of C-phycocyanin. The aim of this study to prove effect of C-phycocyanin on sgp130 levels among wistar rats’ trophoblasts exposed to IL-6. The design of the study was Experiment, Post Test Only Control, analyse with One way Anova. The samples were 25 rats divided four treatment groups. Dose of C-Phycocyanin (10mg, 20mg, 40mg). The results significant difference between control and treatment group IL-6, IL-6 + CPC10, IL-6 + CPC20. But there was no significant difference between control group and IL-6 + CPC40, sig value. 0.214 (sig. <0.05). Conclusion. There is an effect of C-Phycocyanin on Sgp130 levels in wistar rats’ trophoblasts exposed to IL-6 at a dose of C-Phycocyanin 40mg.

Abstract

Patients treated with Continuous Ambulatory Peritoneal Dialysis (CAPD) are constantly exposed to microbial invasionof the peritoneal cavity and rapid microbiological diagnosis of peritonitis is essential due to Hospitalization and imposes asignicant burden of morbidity. The aims of this study were to enumerate the association between microorganisms, sensitity,and resistence of antibiotic on CAPD patients with clinical peritonitis.We collected data through medical records by the number of CAPD patients with clinical peritonitis from June 2004 untilJune 2009. The study was analysis with one-way ANOVA. We found 23 patients clinical peritonitis out of 77 CAPD patients,with insidence was 14% per-year, aged 14 ? 65 y (15M; 8F). The chronic pyelonephritic was a leading (16/23) cause of endstages renal disease. Each patients underwent HD prior (5 ? 60 months) to CAPD, with survival time was 2 ? 51 months. Out of23 patients, 4 were returned to hemodialisis, 15 were died, due to cardiogenic shock 46.7%. Aseptic peritonitis was 31.3%, andthe common microorganism was staphylococcus 18.8%. Peritoneal !uid test showed mean score of sensitivity were tetracycline22.93, cipro!oxacin 19.36, piperacillin-tazobactam 17.36, thrimetropin/sulfamethoxazole 16.5, fosfomycin 15.78, consecutivelyand the rest were resistent. Staphylococus was strongly related to insidence peritonitis, and tetracycline was the most highlysensitive antibiotic in CAPD patients.

Abstract

This research investigates the influence of nurses' roles and responsibilities on hospital operational efficiency and the provision of quality care, emphasizing patient centred care as a crucial fac-tor. The study was conducted at a Type C private hospital in Jakarta, with data gathered from 208 nurses possessing over one year of experience. The analysis was performed using SmartPLS®4. The findings reveal that enhancing patient safety, employing evidence-based practice, fostering patient centred care, exhibiting positive interpersonal behaviour, and implementing clinical governance significantly improve hospital operational efficiency. Furthermore, cognitive empathy was found to moderate the effects of evidence-based practice and patient-centred care on operational efficiency, exhibiting both positive and negative impacts. The research indicates that hospital operational efficiency strongly affects the quality of care, with higher nurse perceptions being associated with better quality outcomes. The model demonstrated moderate to strong explanatory power and substantial predictive relevance, underscoring its efficacy in elucidating and forecasting quality care delivery through hospital operational efficiency. These findings underscore the necessity of integrating patient centred care and cognitive empathy into nursing practices to enhance hospital efficiency and the quality of care provided.

Abstract

Pelayanan prevention of mother to child HIV transmission (PMTCT) sangat penting karena epidemi human immunodeficiency virus (HIV) dan aquired immune deficiency syndrome (AIDS) meningkat dengan cepat. Di Indonesia jumlah kasus AIDS pada Desember 2009 sebanyak 17,699 kasus dan HIV sejumlah 6.668 kasus. Seiring dengan bergesernya epidemi dari kelompok berisiko ke kelompok masyarakat umum, HIV dan AIDS pada perempuan usia reproduktif dan anakanak juga meningkat. Walaupun prevalensi HIV pada perempuan di Indonesia hanya 16%, tetapi karena mayoritas (92,54%) orang dengan HIV/AIDS (ODHA) berusia reproduksi aktif (15-49 tahun) diperkirakan jumlah kehamilan dengan HIV positif akan meningkat (1,2). Di negara berkembang tercatat sebesar 40% penularan dari ibu ke bayi. Masa penularan ialah pada saat dalam kandungan (intra uterine) 25- 35%, intrapartum (labor and delivery) 70-75% atau postpartum (breastfeeding) 14% (6). Di Uganda prevalensi penularan dari ibu ke bayi tanpa PMTCT sebesar 1,53%, akan tetapi setelah pelayanan dan jangkauan oleh PMTCT ditingkatkan hingga 80% (dengan pemberian nevirapin 48%) dapat mencegah sejumlah 13.000 infeksi selama 2005-2010, menurunkan prevalensi HIV pada neonatal menjadi 1,19% hingga 2010 (3).

Abstract

Diabetes Mellitus Gestasional (GDM) merupakan bentuk hiperglikemia selama kehamilan yang dapat memicu perubahan imunologis kompleks, termasuk ketidakseimbangan sitokin inflamasi. Kondisi ini umumnya ditandai oleh peningkatan sitokin proinflamasi seperti IL-6 dan penurunan sitokin antiinflamasi seperti IL-4. Ketidakseimbangan ini berperan dalam memperburuk inflamasi pada lingkungan kehamilan. Penelitian ini bertujuan untuk mengevaluasi pengaruh ekstrak buah pare (Momordica charantia L.) terhadap kadar IL-4 dan IL-6 pada kultur sel trofoblas dalam kondisi hiperglikemik. Tujuan dari penelitian ini adalah mengetahui pengaruh pemberian ekstrak buah pare terhadap kadar IL-4 dan IL-6 pada kultur sel trofoblas suasana hiperglikemia. Jenis penelitian yang digunakan adalah eksperimental laboratorik pada kultur sel trofoblas. Penelitian ini merupakan studi eksperimental in vitro menggunakan kultur sel trofoblas yang diisolasi dari plasenta normal hasil persalinan pervaginam di salah satu rumah sakit swasta di Surabaya, dengan persetujuan pasien. Sebanyak 30 sampel dibagi menjadi lima kelompok: kontrol negatif, kontrol positif (hiperglikemia tanpa perlakuan), dan tiga perlakuan ekstrak pare (0,2; 0,3; dan 0,8 mg/mL). Kadar IL-4 dan IL-6 diukur menggunakan ELISA dan dianalisis dengan uji ANOVA dilanjutkan LSD. Terdapat perbedaan bermakna kadar IL-4 dan IL-6 antar kelompok (p < 0,05). Kelompok perlakuan dengan dosis 0,8 mg/mL menunjukkan peningkatan kadar IL-4 tertinggi (rata-rata 5429,60 ng/mL) dan penurunan kadar IL-6 terendah (rata-rata 1273,60 ng/mL), dibandingkan kelompok lain. Ekstrak buah pare berpotensi sebagai agen antiinflamasi dengan meningkatkan IL-4 dan menurunkan IL-6 pada sel trofoblas dalam kondisi hiperglikemik. Abstract

Community-Acquired Pneumonia (CAP) or pneumonia acquired outside health facilities is one of the main causes of morbidity and mortality in the elderly age group. As we age, immune function decreases and is accompanied by comorbidities such as diabetes, heart disease, and chronic lung disease that worsen the course of the disease. This study aims to find out the clinical picture and management of Community-Acquired Pneumonia (CAP) in the elderly. This study uses qualitative research methods. The data collection technique in this study is by studying case report documents. The data that has been collected is then analyzed in three stages, namely data reduction, data presentation and drawing conclusions. The results show that Community-Acquired Pneumonia (CAP) in the elderly is a complex condition with challenges in diagnosis and management, as the symptoms are often atypical and involve various risk factors. Clinical guidelines emphasize the importance of basic supporting examinations such as thoracic imaging and organ function evaluations, as well as prompt and appropriate administration of empirical antibiotics. Treatment should be adjusted to the severity of the patient's frailty, comorbidities, and frailty status. Supportive therapy plays an important role in recovery, while an individualized approach based on risk factors such as malnutrition, sarcopenia, and aspiration is needed to improve prognosis and lower mortality in the elderly with CAP.
