Abstract

Cadres at Sucopangepok village play a crucial role in reducing stunting by educating about essential practices for toddlers, such as exclusive breastfeeding, proper complementary feeding, and maintaining hygiene to ensure adequate nutrition and prevent infections. The cadres offer health nutrition education to pregnant women and parents of toddlers by monitoring their growth at the posyandu, helping to detect stunting early. They serve as the primary line of defense in improving public health, as they understand their community's characteristics. This study aimed to describe organizational factors on performance in implementing posyandu for toddlers. This study used descriptive research to describe organizational factors including infrastructure, rewards, job design, and leadership on cadre performance. Involved 45 cadres as respondents taken by total sampling. Data was collected using interviews questionnaires, observation, and documentation, then the data was analyzed using descriptive data analysis which will describe the characteristics of each. Facilities infrastructure and work design have good results even though in the facilities and infrastructure, there are facilities that need to be replaced and repaired. The rewards in the category are inappropriate. Leadership is in the adequate category due to the lack of closeness between the leader and the cadres. Cadre's performance during toddler posyandu activities showed promising results. The description of organizational factors in facilities and infrastructure and job design is good, rewards in the category are inappropriate, and leadership is in the adequate category with good cadre performance.

Abstract

Jember Regency is included in the five cities/regencies with the highest prevalence of stunting in East Java, with a percentage ranging from 40-45%. One of the interventions for toddlers to reduce the prevalence of stunting is the Recovery Supplemental Feeding (PMT-P). The study aimed to study the implementation of the Recovery Supplemental Feeding (PMT-P) program for toddlers to reduce the prevalence of stunting. This is a qualitative approach. The data was collected by in-depth interviews and documentation. The results showed that the implementation of the PMT-P Program was not fully in accordance with the 2011 PMT-P Guidelines from the Indonesian Ministry of Health, the program implementers had good insight, knowledge, and response in implementing the PMT-P program. The output of the PMT-P program implementation was not in accordance with David C. Korten's theory of suitability. It is suggested to cook together among midwives, cadres, and mothers of target toddlers, create a community of toddlers’ mothers as a sharing platform about the implementation of Recovery Supplemental Food, and increase monitoring in the provision program for toddlers to ensure that supplemental food is actually consumed by the target toddlers.

Abstract

Coronary heart disease is a catastrophic disease that causes a high burden of medical costs. At Dr.Iskak Tulungagung hospital, coronary heart disease is the disease with the highest cost. The number of coronary heart patients who are hospitalized is also increasing every year. Several Previous studies have shown that the amount of direct medical costs of coronary heart patients reach more than 50% of the total costs of treatment. Therefore, it is necessary to do research on the factors associated with the increasing direct medical costs of coronary heart patients. The purpose of this study was to analyze the factors that influence the amount of direct medical costs in coronary heart inpatients which include: relationship between patient characteristics, medical procedures, and comorbidities with direct medical costs in coronary heart inpatients. This analytical study used a cross sectional design. The research sample was a coronary heart inpatient at Dr.Iskak Tulungagung Hospital who met the inclusion and exclusion criteria, namely 125 samples. Sampling with systematic random sampling. Data were analyzed using Spearman Rho test and Eta test. The results showed that most of the patients were aged 56-65 years, the majority were men with the length of hospitalization mostly for ≤ 3 days. The majority underwent invasive procedures and most had comorbidity. The average direct medical cost in 2020 is IDR 26,788,964 while in 2021 it is IDR 31,553,045. The three biggest costs of cath lab costs, drugs, and doctor and nursing actions. Medical procedures and comorbidity are factors related to that affect the amount of direct medical costs.

Abstract

Puskesmas Silo II merupakan salah satu puskesmas di Kabupaten Jember yang telah melaksanakan pelayanan ANC dan pada pelaksanaannya Puskesmas Silo II sudah melakukan berbagai upaya untuk meningkatkan motivasi ibu hamil, namun capaian K6 di Puskesmas Silo II mencapai target RPJMN tahun 2020-2024 yaitu sebesar 60%. Kurangnya capaian K6 di Puskesmas Silo II menunjukkan bahwa masih banyak ibu hamil yang tidak memanfaatkan pelayanan ANC secara lengkap. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember. Jenis penelitian ini adalah analitik dengan menggunakan desian cross sectional. Sampel Penelitian sebanyak 76 ibu nifas yang pernah mendapatkan pelayanan ANC dengan teknik pengambilan sampel menggunakan simple random sampling. Penelitian ini menggunakan data primer yang diperoleh dari hasil wawancara dengan ibu nifas menggunakan kuesioner. Analisis data menggunakan aplikasi SPSS dengan analisis univariat untuk mendeskripsikan terkait budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas, serta analisis bivariat dengan menggunakan uji chi square untuk mengetahui hubungan antara variabel independent dengan dependen. Hasil penelitian menunjukkan bahwa beberapa faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember adalah budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas.

Abstract

The increasing number of Social Security Agency of Health (BPJS Kesehatan) participants who are inactive or drop out be a threat to participation in the health insurance program in Indonesia. Inactive participants in August 2022 recorded 45,987,956. The most common cause is not paying premiums according to applicable regulations. The purpose of the research is to analyze the factors that influence the willingness to pay for BPJS Kesehatan participants using the contingent valuation method approach. This research used quantitative methods with an analytic observational approach through surveys. The unit of analysis in research was the individual. The reachable population for the study were BPJS Kesehatan participants in the category of non-Recipients of Contribution Assistance (non-PBI) in the BPJS Kesehatan Jember working area (Jember and Lumajang Regencies). The sample size was 167 respondents taken using the quota sampling technique. Data analysis with multiple linear regression. The results showed that the Perceived Benefit Package, Perceived Sacrifice, and Free Mentality contributed to the formation of willingness to pay premiums regularly. Only the Conformity Effect does not significantly contribute of willingness to pay premiums regularly. Perceived Sacrifice and Free Mentality contribute in opposite directions. This means that if the respondent's Perceived Sacrifice and Free Mentality are high, it will affect the low willingness to pay premiums. It is necessary to develop a program that can provide added value for Non-PBI participants who so far rarely or have never used health services. This aims to maintain the motivation of participants to remain willing to pay premiums regularly.
