Abstract

Coronary heart disease is a catastrophic disease that causes a high burden of medical costs. At Dr.Iskak Tulungagung hospital, coronary heart disease is the disease with the highest cost. The number of coronary heart patients who are hospitalized is also increasing every year. Several Previous studies have shown that the amount of direct medical costs of coronary heart patients reach more than 50% of the total costs of treatment. Therefore, it is necessary to do research on the factors associated with the increasing direct medical costs of coronary heart patients. The purpose of this study was to analyze the factors that influence the amount of direct medical costs in coronary heart inpatients which include: relationship between patient characteristics, medical procedures, and comorbidities with direct medical costs in coronary heart inpatients. This analytical study used a cross sectional design. The research sample was a coronary heart inpatient at Dr.Iskak Tulungagung Hospital who met the inclusion and exclusion criteria, namely 125 samples. Sampling with systematic random sampling. Data were analyzed using Spearman Rho test and Eta test. The results showed that most of the patients were aged 56-65 years, the majority were men with the length of hospitalization mostly for ≤ 3 days. The majority underwent invasive procedures and most had comorbidity. The average direct medical cost in 2020 is IDR 26,788,964 while in 2021 it is IDR 31,553,045. The three biggest costs of cath lab costs, drugs, and doctor and nursing actions. Medical procedures and comorbidity are factors related to that affect the amount of direct medical costs.

Abstract

Puskesmas Silo II merupakan salah satu puskesmas di Kabupaten Jember yang telah melaksanakan pelayanan ANC dan pada pelaksanaannya Puskesmas Silo II sudah melakukan berbagai upaya untuk meningkatkan motivasi ibu hamil, namun capaian K6 di Puskesmas Silo II mencapai target RPJMN tahun 2020-2024 yaitu sebesar 60%. Kurangnya capaian K6 di Puskesmas Silo II menunjukkan bahwa masih banyak ibu hamil yang tidak memanfaatkan pelayanan ANC secara lengkap. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember. Jenis penelitian ini adalah analitik dengan menggunakan desian cross sectional. Sampel Penelitian sebanyak 76 ibu nifas yang pernah mendapatkan pelayanan ANC dengan teknik pengambilan sampel menggunakan simple random sampling. Penelitian ini menggunakan data primer yang diperoleh dari hasil wawancara dengan ibu nifas menggunakan kuesioner. Analisis data menggunakan aplikasi SPSS dengan analisis univariat untuk mendeskripsikan terkait budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas, serta analisis bivariat dengan menggunakan uji chi square untuk mengetahui hubungan antara variabel independent dengan dependen. Hasil penelitian menunjukkan bahwa beberapa faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember adalah budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas.

Abstract

The increasing number of Social Security Agency of Health (BPJS Kesehatan) participants who are inactive or drop out be a threat to participation in the health insurance program in Indonesia. Inactive participants in August 2022 recorded 45,987,956. The most common cause is not paying premiums according to applicable regulations. The purpose of the research is to analyze the factors that influence the willingness to pay for BPJS Kesehatan participants using the contingent valuation method approach. This research used quantitative methods with an analytic observational approach through surveys. The unit of analysis in research was the individual. The reachable population for the study were BPJS Kesehatan participants in the category of non-Recipients of Contribution Assistance (non-PBI) in the BPJS Kesehatan Jember working area (Jember and Lumajang Regencies). The sample size was 167 respondents taken using the quota sampling technique. Data analysis with multiple linear regression. The results showed that the Perceived Benefit Package, Perceived Sacrifice, and Free Mentality contributed to the formation of willingness to pay premiums regularly. Only the Conformity Effect does not significantly contribute of willingness to pay premiums regularly. Perceived Sacrifice and Free Mentality contribute in opposite directions. This means that if the respondent's Perceived Sacrifice and Free Mentality are high, it will affect the low willingness to pay premiums. It is necessary to develop a program that can provide added value for Non-PBI participants who so far rarely or have never used health services. This aims to maintain the motivation of participants to remain willing to pay premiums regularly.

Abstract

Permasalahan kematian ibu di Indonesia menjadi perhatian penting, khususnya di Kabupaten Banyuwangi Provinsi Jawa Timur, khususnya di Kecamatan Kabat dengan 13 kematian ibu selama tahun 2019-2022. Kematian ibu dapat terjadi karena beberapa hal, kematian ibu dapat dicegah melalui deteksi dini kelainan pada kehamilan ibu melalui ANC Terpadu. Studi ini bertujuan untuk mengatasi AKI dengan berfokus pada layanan KIA. Penelitian ini menggunakan metode deskriptif, populasi dalam penelitian ini adalah seluruh ibu hamil yang melakukan ANC Terpadu di Puskesmas Kabat sebanyak 646 ibu hamil dengan sampel sebanyak 93 ibu hamil. Studi ini menemukan bahwa komponen input seperti sumber daya manusia dan kualitas layanan belum memenuhi target. Komunikasi, informasi, dan pendidikan belum efektif. Penelitian  juga menyoroti perlunya layanan KIA yang lebih baik, seperti menyediakan informasi KIA, membentuk kelompok kesejahteraan, dan mendorong partisipasi aktif dalam kegiatan KIA. Studi ini juga menyoroti pentingnya keterlibatan masyarakat dalam layanan KIA dan perlunya penyebaran informasi KIA yang efektif.

Abstract

Background: Antenatal care (ANC) service program is one of the efforts to reduce maternal mortality. However, as per the Jember District Health Office report in 2021, the coverage of K1 was 103.64% and K4 was 58.67%, which is still below the target of 100%. The low level of public knowledge regarding ANC visits and incomplete counseling provided by midwives to maternal according to the current guidelines were the main reasons for the failure to achieve the K4 target for Integrated ANC visits and the occurrence of MMR in the working area of Sukowono Health Center. Aims: The purpose of this study is to describe the implementation of Integrated Antenatal Care (ANC) in Sukowono Health Center's working area, Jember Regency. Methods: A descriptive study was conducted in the working area of Sukowono Public Health Center, Jember Regency, focusing on 28 health workers who carried out Integrated ANC. The study recruited a total of 28 midwives through total sampling. The data collection techniques used in this research were interviews and documentation studies. Results: The research and documentation study carried out in the Working Area of Sukowono Public Health Center, Jember Regency, revealed that only two midwives (7.2%) conducted frequent speech interviews (counseling) and mental health assessments in the implementation of 10T in integrated ANC. However, all midwives regularly carried out 10T on K4 in integrated ANC, and only one midwife (3.6%) conducted case management frequently on K6 in Integrated ANC in the Work Area of the Sukowono Health Center, Jember Regency. Conclusion: The implementation of the 10T program in the Integrated ANC of the Sukowono Health Center, Jember Regency, has been evaluated and categorized. The implementation of 10T in K1 has been included in the unfavorable category, while the implementation in K4 has been categorized as good. However, the implementation of 10T in K6 has been included in the unfavorable category. It is crucial for counseling and case management to be carried out properly to ensure the health of mothers and prevent maternal mortality.

Abstract

Background: Health is a basic human right and is guaranteed by the 1945 Constitution and Law Number 17 of 2023. Health centers as first-level health care facilities have the responsibility to improve the quality of service performance. One of the factors that influences nurse performance is the leadership style of the head of Primary Health Care. Aims: This study aims to analyze the relationship between the leadership style of the head of the Primary Health Care and the performance of nurses at the Sedati Primary Health Care, Sidoarjo Regency. Methods: The study used an observational analytical quantitative design with a cross-sectional approach conducted in March–April 2025. A sample of 35 nurses was taken using the total sampling technique. Data collection was carried out through interviews with questionnaires that had been tested for validity and reliability. Results: Data analysis used the Fisher Exact Test. The results showed that the majority of Health Center heads applied a democratic leadership style (57.1%) and most nurses had high performance (65.7%). The Fisher Exact Test showed a p value = 0.001 (<0.05), indicating a significant relationship between leadership style and nurse performance. Participative and communicative democratic leadership style has been proven to increase nurses' motivation and professionalism. Conclusion: This study concludes that effective leadership style has an important role in improving nurses' performance and the quality of health services at the primary level.

Abstract

Background: Maternal mortality is still a health problem in Indonesia. Antenatal care as a form of prevention program to prevent maternal death. The low coverage of ANC services can be caused by midwives' poor performance in providing services. Permenkes Number 4 of 2019 concerning Health SPM, the performance achievements of district/city regional governments in providing ANC services are assessed from the coverage of health services for pregnant women according to standards in their working areas. Based on Presidential Regulation Number 18 of 2020, the percentage that must be achieved in ANC service coverage is 95%, while data from the NTB Provincial Health Office, the coverage of antenatal care services in Mataram City is below this target with an average K1 and K4 from 2019 - 2021 is 92.67% and 81.66%. Dasan Agung Health Center is a health center located in the city of Mataram, with an average coverage of antenatal care services for K1 and K4 which is still below standard, namely 78.5% for K1 and 60.1% for K4.Aims: the purpose of this study was to determine the factors related to performance in antenatal care services at the Dasan Agung Community Health Center, Mataram City. Method: descriptive research design, the number of samples is 11 people with total sampling. Results: the human resource and leadership variables at the Dasan Agung Health Center tend to be good, the reward and organizational structure variables tend to be quite good. Meanwhile for the performance variable, it was found that the performance of midwives at the Dasan Agung Health Center tended to be good. Conclusion: the results of the study show that the majority of midwives at the Dasan Agung Health Center in Mataram City have good performance in providing antenatal care services

Abstract

AbstrakGancang Aron (Gugus Antisipasi Cegah Antrian Panjang dengan Antar Obat ke Rumah Pasien) merupakan program inovasi RSUD Blambangan untuk mengurangi waktu tunggu pelayanan obat. Penelitian ini bertujuan untuk menganalisis tingkat kepuasan pasien pengguna program Gancang Aron dengan metode Importance Performance Analysis di RSUD Blambangan Kabupaten Banyuwangi. Penelitian ini merupakan penelitian deskriptif dengan pendekatan kuantitatif. Jumlah sampel sebanyak 96 responden yang ditentukan dengan teknik simple random sampling. Hasil penelitian menunjukkan bahwa sebagian besar atribut dianggap penting namun kinerjanya masih ada yang dianggap kurang baik dan belum memenuhi harapan, yaitu petugas kurang tepat waktu dalam melayani pasien, petugas kurang menerima dan mendengarkan keluhan pasien dengan baik, instalasi farmasi memiliki persediaan obat yang kurang lengkap, dan ruang tunggu kurang memadai. Tingkat kepuasan pasien pengguna program Gancang Aron ditinjau dari dimensi kompetensi teknis, akses terhadap pelayanan, kelangsungan pelayanan, keamanan, efektivitas, hubungan antar manusia, dan efisiensi ialah sangat puas, sedangkan ditinjau dari dimensi kenyamanan ialah puas, namun atribut ruang tunggu memadai dan tersedianya AC/televisi/majalah di ruang tunggu belum memenuhi harapan pasien. Berdasarkan diagram kartesius diketahui bahwa atribut tersebar dalam empat kuadran. Atribut yang dianggap penting dan telah baik kinerjanya sebanyak 7 atribut, sedangkan atribut yang dianggap kurang penting dan belum baik kinerjanya sebanyak 8 atribut.Kata kunci: diagram kartesius, gancang aron, kepuasan pasien AbstractGancang Aron (Anticipation Group to Prevent Long Queues by Delivering Medicines to Patients' Homes) is an innovation program at the Blambangan Hospital to reduce waiting time for drug services. This study aims to analyze the level of patient satisfaction using the Gancang Aron program using the Importance Performance Analysis method at Blambangan Hospital, Banyuwangi Regency. This research is a descriptive research with a quantitative approach. The number of samples as many as 96 respondents determined by simple random sampling technique. The results showed that most of the attributes were considered important but their performance was still considered unfavorable and did not meet expectations, namely officers were less punctual in serving patients, officers did not receive and listen to patient complaints properly, pharmacy installations have inadequate drug supplies, and inadequate waiting rooms. The level of patient satisfaction using the Gancang Aron program in terms of the dimensions of technical competence, access to services, service continuity, security, effectiveness, human relations, and efficiency is very satisfied, while in terms of the comfort dimension it is satisfied, but the attributes of the waiting room are adequate and the availability of air conditioning/television/magazines in the waiting room have not met the patient's expectations. Based on the cartesian diagram, it is known that the attributes are spread in four quadrants. Attributes that are considered important and have good performance are 7 attributes, while attributes that are considered less important and have not performed well are 8 attributes. Keywords: cartesian diagram, gancang aron, patient satisfaction
