Abstract

Abstract: Acne vulgaris is a chronic inflammatory disease of pilosebaceous follicles that commonly occurs in adolescence and can heal itself. The purpose of this study was to determine the profile of acne vulgaris in Dermatovenereology of Prof. Dr. R. D. Kandou Manado in 2009-2011. This study was conducted retrospectively in patients with acne vulgaris who came to the Dermatology clinic in 2009-2011. The data were recorded from  medical records of the amounts of patients, gender, age, occupation / education, lesion location, and the type of acne. The results showed that of a total of 10003 visits in 2009-2011 there of 121 patients (3.59%) were new cases of acne vulgaris, acne vulgaris in patients dominance of female patients by 75 patients (61.9%), the age group most at 15-24 years old is 76 patients (62.8%), the highest educational status on the student group is 73 patients (60.3%), the most lesions location are on the face, most types of acne is papulopustular. Key words: acne vulgaris, inflammation, pilosebaceous    Abstrak: Akne vulgaris adalah penyakit peradangan menahun folikel pilosebasea yang umumnya terjadi pada masa remaja dan dapat sembuh sendiri. Tujuan dari penelitian ini untuk mengetahui profil akne vulgaris di poliklinik Kulit dan Kelamin RSUP Prof Dr R. D. Kandou Manado pada tahun 2009–2011. Penelitian ini dilakukan secara retrospektif pada pasien dengan akne vulgaris yang datang ke poliklinik  Kulit dan Kelamin pada tahun 2009–2011. Dilakukan pencatatan data dari catatan medik mengenai jumlah pasien, jenis kelamin, usia, pekerjaan/pendidikan, lokasi lesi, dan jenis akne. Hasil yang diperoleh menunjukkan bahwadari total 10003 kunjungan pada tahun 2009–2011 terdapat 121pasien (3,59%) merupakan penderita baru akne vulgaris, pasien akne vulgaris di dominasi pasien perempuan sebanyak 75 pasien (61,9%), kelompok usia terbanyak pada usia 15 – 24 tahun yaitu 76 pasien (62,8%), status pendidikan terbanyak  pada kelompok pelajar yaitu 73 pasien (60,3%), lokasi lesi terbanyak yaitu di bagian wajah, jenis akne terbanyak yaitu papulopustuler. Abstract

Abstract: Intertriginous Candidosis is a superficial mycoses affecting skin folds caused by yeast of the genus Candida, mostly Candida albicans. The purpose of this study was to evaluate the profile of intertriginous candidosis in Dermatovenereology clinic of Prof. Dr. R. D. Kandou Hospital Manado during the period from January – December 2012. This study was conducted retrospectively on medical records of new patients diagnosed with intertriginous candidosis in Dermatovenereology clinic of Prof. Dr. R. D. Kandou Hospital Manado during the period from January – December 2012. The result of this study showed that there were 37 patients (3.37%) with intertriginous candidosis among 1096 new patients, significantly higher in female (64.86%) than in male patients, highest rate recorded was within 45-64 age group (45.95%), most skin fold affected was groin (16.22%), and the most commonly used therapy was a combination of topical antifungal and oral antihistamine (29.74%). Keywords: superficial mycoses, intertriginous candidosis, retrospective study    Abstrak: Kandidosis Intertriginosa merupakan mikosis superfisialis yang terkena pada daerah lipatan kulit yang disebabkan oleh jamur dari genus Candida umumnya Candida albicans. Tujuan penelitian ini adalah untuk mengetahui profil kandidosis intertriginosa di poliklinik Kulit dan Kelamin RSUP Prof. Dr. R. D. Kandou Manado periode Januari – Desember 2012. Penelitian ini dilakukan secara retrospektif dari rekam medik pasien baru dengan diagnosis kandidosis intertriginosa di Poliklinik Kulit dan Kelamin RSUP Prof. Dr. R. D. Kandou Manado periode Januari – Desember 2012. Hasil penelitian ini menunjukkan bahwa ada 37 pasien (3,37%) kandidosis intertriginosa dari 1096 pasien baru, lebih banyak ditemukan pada perempuan dibanding laki-laki, kelompok umur terbanyak pada 45-64 tahun, lokasi lipatan kulit paling banyak terkena pada lipat paha, dan terapi yang paling banyak digunakan adalah antifungi topikal dan antihistamin. Abstract

BackgroundMelasma is a highly prevalent chronic pigmentary disorder. The pathogenesis is unknown but melasma often occurs in photo-exposed areas, e.g., cheeks, upper lip, chin, and forehead. Tranexamic acid (TA), a plasmin inhibitor, aids in the inhibition of UV-induced plasmin activity and melanogenesis, making it a favorable therapeutic option for melasma. Tranexamic acid may be administered through various routes, e.g., topical. This study aimed to compare the efficacy of topical 10% versus 5% TA in women with melasma. MethodsThis double-blind randomized controlled trial included 16 females with epidermal type melasma who were randomized into two groups to receive either topical 10% TA (n = 8) or 5% TA (n = 8) applied twice daily for eight weeks. Prior to intervention and at 8 weeks after intervention, the intensity and extension of melasma were assessed based on melasma area and severity index (MASI) score and pigmentation score. ResultsMean MASI and pigmentation scores in both treatment groups were similar at base-line (p>0.05). The reduction in MASI and pigmentation scores in the topical 10% TA and 5% TA groups was similar and statistically not significant after 4 and 8 weeks of treatment (p>0.05). There were no drug-related adverse reactions or complications. ConclusionThis study demonstrated that topical 10% TA and 5% TA were effective in treating women with melasma. The utilization of topical 5% TA for melasma is a promising alternative therapeutic option without the need to increase the concentration of the formulation.

Abstract

Acne vulgaris (AV) is a chronic inflammatory disorder of the pilosebaceous follicle with multifactorial pathogenesis and pleiomorphic clinical manifestations constituting comedones, papules, pustules, nodules, and cysts. Oxidative stress has been reported to contribute in AV pathogenesis. This phenomenon rationalizes antioxidant supplementation as an adjuvant therapy for AV management. Four cases of 22 to 23 yo women with complaints of worsening facial acne for 1 to 6 mo. Initially, acne lesions appeared as skin-colored papules, which increased in number, became reddish papules and pustules, and extended to the jaw and chin. Lesions were accompanied by temporary itching and pain. Dermatological examination revealed multiple circumscribed discrete erythematous papules, miliar to lenticular in size, comedones (+), and pustules (+), with total lesions <30. According to the Lehmann criteria, patients were diagnosed as mild AV. All patients were treated with 0.025% topical retinoic acid cream, while the other two patients received additional oral antioxidant supplementations (zinc and ɑ-lipoic acid) and were followed up every two weeks. Two cases who received additional oral antioxidant supplementations (Group A) exhibited an earlier and higher clinical improvement, characterized by a reduction in the number of lesions on each follow up, till the current report. Oxidative stress in AV pathogenesis causes microenvironment alteration that favours colonization of Cutibacterium acnes. Together with the increase in sebum production, it stimulates the release of pro-inflammatory cytokines, such as interleukin (IL)-1α, IL-8, and tumor necrosis factor-α (TNF α), contributing to the inflammatory response. Antioxidant supplementation plays a role in suppressing the process of lipid peroxidation and inhibiting the expression of pro-inflammatory cytokines. Comprehensive management of AV is based on pathogenesis and the role of oxidative stress. An earlier and higher clinical improvement reduction was noted in Group A, patients who received a combination of 0.025% topical retinoic acid cream and oral antioxidant supplementation.
