Abstract

The Effects of Work Shift and Workload towards the Occurrence of Nurses Work Fatigue in Inpatient Ward of Governence Hospital. Introduction: Nurses work fatigue is an important problem in any industrial process, including nursing services industrial sector. Nurses work fatigue can lead to work accidents to patients and nurses themselves. Heavy workload and patterns of non-standard work shift can lead to work fatigue to nurses who work in shifts. Methods: This research aims to determine the effect of work shift and workload on nurses work fatigue. The kind of this research is an analytical survey with cross sectional design. The subjects of this research are 30 respondents. Each respondent was measured its fatigue before and after working. Work shift was measured by using a standard checklist sheet of work shift,workload was measured by using the daily log and work fatigue was measured by using a Raction meter Lakasidaya L 77. Result: Data analysis was performed by using bivariate analysis with Chi-Square test to determine the effect of the independent variables on the dependent variable and multivariate analysis by using logistic regression analysis with Backward LR method.The results of bivariate analysis of the independent variable that influences on the dependent variable (nurses work fatigue) are: morning work shift (p value = 0.030), evining work shift )p value = 0,038), night work shift (p value = 0.042), morning workload (p value = 0.014), evening workload (p value = 0.042) and night workloads (p value = 0.004). On the multivariate analysis, the variables that influence are morning work shift (OR = 0.082), night work shift (OR = 0.053), night workload (OR = 0.028). Recommendation: For Nurse manager, the result this research can be input for humen resource policy in shift system must gain for work shift and work load standard.

Abstract

Perawat merupakan sumber daya manusia yang penting dirumah sakit. Kompetensi perawat akan mempengaruhi kualitas pekerjaan. Kompetensi sosial harus dimiliki perawat dalam memberikan asuhan keperawatan. Kompetensi sosial berdampak pada kepuasan kerja perawat.Desain penelitian menggunakan Quasy Experimental dengan pre post test with control group. Jumlah sampel 18 kelompok intervensi dan 18 kelompok kontrol. Penggumpulan data penelitian menggunakan kuesioner komptensi sosial dan kuesioner kepuasan kerja Minesotta. Rerata usia responden kelompok intervensi nilai mean 28,8bulan dan kelompok kontrol adalah 29,2, kelompok intervensi mayoritas memiliki pendidikan Diploma III Keperawatan sebanyak  83,3% dan kelompok kontrol mayoritas berpendidikan Diploma III Keperawatan sebesar 88,9%. Nilai rata-rata lama kerja kelompok intervensi adalah 4,22 dan standar deviasi 1,39 dan nilai minimal-maksimal 1-6. Pada kelompok kontrol, nilai rata-rata lama kerja adalah  4. Uji analisis pada kelompok intervensi menggunakan Paired t-testdidapatkan hasil p-value sebesar 0,000, hasil tersebut <alpha sehingga dinyatakan terdapat pengaruh intervensi kompetensi sosial terhadap kepuasan kerja perawat. Kata kunci: kompetensi sosial, kepuasan kerja  THE EFFECT OF NURSING SOCIAL COMPTENCE ON NURSING SATISFACTION ABSTRACTNurses are important human resources in hospitals. Nurse competence will affect the quality of work. Social competence must be possessed by nurses in providing nursing care. Social competence has an impact on job satisfaction of nurses. Research design uses Quasy Experimental with pre-post test with control group. The number of samples was 18 intervention groups and 18 control groups. Research data collection using social competency questionnaire and Minesotta job satisfaction questionnaire. The mean age of respondents in the intervention group mean value was 28.8 months and the control group was 29.2, the majority intervention group had a Diploma III Nursing education of 83.3% and the majority control group had a Diploma III Nursing education of 88.9%. The average length of work of the intervention group was 4.22 and the standard deviation was 1.39 and the minimum-maximum value was 1-6. In the control group, the average value of length of work was 4. Discussion: The analysis test in the intervention group using the Paired t-test obtained a p-value of 0,000, the results were <alpha so that it was stated that there was an influence of social competence interventions on nurse job satisfaction. Keywords: social competence, job satisfaction

Abstract

Manajemen keperawatan merupakan proses perencanaan, pengorganisasian, pengarahan dan pengawasan untuk mencapai tujuan keperawatan. Kepuasan kerja adalah berbagai persepsi perawat terhadap lingkungan pekerjaannya. Kenyataan pelaksanaannya belum sesuai dengan perencanaan, pengorganisasian, pengarahan dan pengawasan, serta perlu lingkungan kerja yang baik. Tujuan penelitian adalah mengetahui hubungan dan pengaruh persepsi perawat pelaksana dalam perencanaan, pengorganisasian, pengarahan dan pengawasan manajemen keperawatan serta lingkungan kerja terhadap kepuasan kerja perawat di ruang rawat inap RSUD Tugurejo Semarang. Penelitian ini merupakan penelitian observasional deskriptif analitik dengan pendekatan cross sectional. Responden penelitian adalah 114 orang perawat pelaksana. Analisis data dengan analisis univariat, analisis bivariat dan analisis multivariat dengan regresi logistik. Hasil penelitian menunjukkan persepsi manajemen keperawatan berhubungan dengan kepuasan kerja perawat pelaksana adalah perencanaan (p value = 0,0001), pengorganisasian (p value= 0,0001), pengarahan (p value = 0,0001) dan pengawasan (p value= 0,0001), serta didapatkan lingkungan kerja baik (p value = 0,0001). Persepsi manajemen keperawatan yang berpengaruh bersama-sama terhadap kepuasan kinerja perawat pelaksana adalah perencanaan (exp B=27,631), pengorganisasian (exp B=7,549) dan pengawasan (exp B=17,166). Rekomendasi penelitian untuk manajer keperawatan perlu menyusun kembali perencanaan strategis, uraian tugas, tanggung jawab dan wewenang perawat pelaksana, mengoptimalkan pendelegasian wewenang kepada kepala ruang, memberi arahan dan penjelasan kembali kepada perawat pelaksana tentang prosedur pelayanan keperawatan serta memberikan perhatian serta motivasi dalam pengembangan profesional perawat, menjadi mediator yang baik dalam penyelesaian masalah dan melakukan monitoring dan evaluasi berkala program keperawatan di RSUD Tugurejo Semarang. Influence Analysis of Nurses Perception in Nursing Management and Work Environment with Job Satisfaction of Nurses in Ward of Tugurejo General Hospital of Semarang Nursing Management is process of planing, organizing, directing and controlling to achieve the aim of nursing services. Job satisfaction is many perception of nurse with work environment. The nursing management is not matched with planing, organizing, directing and controlling and need good environment of work. The purpose of this study was to describe the correlation and influence of nurses perception in nursing management such as planing, organizing, directing and controlling and work environment with job satisfaction of nurses in ward of Tugurejo General Hospital of Semarang. This study was a analytic describtive observational, cross sectional approach. Respondens were 114 nurses. Analysis of data using bivariat analysis, univariat analysis and multivariat analysis with logistic regression. The results showed nursing management perception related to job satisfaction of nurses was planing (p value=0,0001), organizing (p value=0,0001), directing (p value=0,0001) and controlling (p value=0,0001), and good environment of work (p value=0,0001). Nursing management perception influenced together with job satisfaction of nurses was planing (exp B=27,631), organizing (exp B=7,549) and controlling (exp B=17,166). Recommendations from this research is aimed to nursing management in Tugurejo General Hospital of Semarang to restructure a strategic planing and nurses job description, empower head of ward to delegate responsibility, redirect to nurses about nursing services standard, make attention and motivation to nurses professional development, made as mediator in problem solving and make schedule monitoring and controlling nursing programme.

Abstract

 Hasil  pelaksanaan  penerapan Standar Asuhan Keperawatan  (SAK)  di RSUD  Sunan  Kalijaga Kabupaten  Demak  tahun  2011  sebesar  62,49%  dan 2012  sebesar  69,49%,  kondisi  ini  masih dibawah target SAK RSUD Sunan Kalijaga Kabupaten Demak yaitu 86% yang diharapkan pada tahun  2015  pencapaian  pelaksanaan  SAK  100%, hal  ini menunjukkan  terdapat  kesenjangan pencapaian  standar  asuhan  keperawatan,  yang diduga  belum  optimalnya  manajemen  kepala ruang terhadap pencapaian asuhan keperawatan.Tujuan penelitian adalah menganalisis pengaruh manajemen  kepala  ruang  terhadap pencapaian  asuhan  keperawatan  di  ruang  rawat  inap  RSUD Sunan  Kalijaga  Kabupaten  Demak.  Penelitian  ini  merupakan  penelitian  observasional  dengan pendekatan cross   sectional. Instrumen   penelitian   menggunakan   kuesioner   terstruktur dan checklist observasi terhadap kegiatan asuhan keperawatan. Jumlah sampel 62 perawat pelaksana yang tersebar di 14 ruang rawat inap RSUD Sunan Kalijaga Kabupaten Demak.  Analisis bivariat dengan uji chi square dan  analisis  multivariat  dengan  uji regresi  logistik. Hasil  penelitian menunjukkan    proporsi perawat    pelaksana    yang    memiliki    perencanaan    baik  56,5%, pengorganisasian  baik  58,1%,  penggerakan  baik  67,7%  dan  pengawasan baik 56,5% pada desa tercapai  UCI,  pencapaian pelaksanaan  asuhan  keperawatan  yang  sudah  lengkap  80,6%.  Hasil  analisis  bivariat  menunjukkan  bahwa  perencanaan (ρ=0,014), pengorganisasian (ρ=0,010), dan penggerakan (ρ=0,0001) berhubungan   dengan pencapaian asuhan   keperawatan.   Analisis multivariat  menunjukkan  adanya  pengaruh  penggerakkan  (ρ=0,002;Exp β=10,636)  terhadap pencapaian  asuhan  keperawatan.  Disarankan kepada  pihak  RSUD  Sunan  Kalijaga  Kabupaten Demak  untuk  meningkatkan  komunikasi  secara  jelas  dan  efektif,  terlibat  dalam  kolaborasi  dan koordinasi  dalam  memberikan  asuhan  keperawatan  dan  meningkatkan  manajemen  yang  baik dengan memberikan motivasi sebagai umpan balik yang baik kepada perawat.hospital (RSUD) of Demak district was 62.49% in 2011 and 69.42% in 2012; this situation wasstill below the SAK of RSUD Sunan Kalijaga Demak district (86%). Expectation of SAKimplementation accomplishment in 2015 was 100%.These facts indicated that there was a gap in the accomplishment of the nursing care standard, and it was suspected that the management of room coordinator towards nursing care accomplishment was not optimal. Objective of this study was to analyze the influence of management of room coordinator to the accomplishment of nursing care in the inward room of RSUD Sunan Kalijaga of Demak district. This was anobservational study using cross sectional approach. Study instruments consisted of structuredquestionnaire and observation checklist of nursing care activity. The number of samples was 62nurses that were distributed in 14 inward rooms of the RSUD Sunan Kalijaga of Demak district.Bivariate analysis was done by applying chi square test, and logistic regression test was appliedfor multivariate analysis. Results of the study showed proportion of nurses who had goodplanning (56.5%), good organization (58.1%), good actuating (67.7%), and good supervision(56.5%) on UCI village. The accomplishment of complete nursing care implementation was80.6%. Results of bivariate analysis showed that planning (ρ=0.014), organizing (ρ= 0.010), and actuating (ρ= 0.0001) were associated with the accomplishment of nursing care. Multivariate analysis indicated the influence of actuating (ρ= 0.002; Exp β= 10.636) towards the accomplishment of nursing care. Suggestions addressed for RSUD Sunan Kalijaga of Demakdistrict are to improve distinct and effective communication, to involve in collaboration andcoordination activities in providing nursing care, and to improve good management byproviding motivation as a good feedback to nurses.

Abstract

Background: Low back pain (LBP) is a clinical syndrome in which the main symptom is painful or uncomfortable feeling in lower back area. Work factor is one of many factors that can caused LBP, which may be triggered by carelessness and unergonomic work posture. There is a physical therapy that can be applied to reduce the pain of LBP sufferer namely William’s Flexion Exercise.Objective: To know the effect of William’s Flexion Exercise in reducing the pain of LBP sufferer.Method: This research was a literature review. Literatures were gathered from Google Scholar and PubMed database with low back pain and William’s flexion exercise as the keyword. Data was analysed using PRISMA flow diagram with inclusion criteria such as full text format, published between 2015-2020; written in Bahasa Indonesia and nationally published; written in English and internationally published in Scopus Q1-Q3 indexed journal. While exclusion criteria were literatures which published before 2015 and with double publication.Result: There were seven literatures identified according to the criteria above. From these literatures, it was found that William’s Flexion Exercise was proven in reducing scale of pain of low back pain sufferers and improving joint motion. There were several methods of William’s Flexion Exercise such as pelvic tilting, single knee to chest, double knee to chest, partial sit up, hamstring stretches, bicycling, banding from a chair and squat factor.Conclusion: William’s Flexion Exercise can be an alternative physical therapy to reduce the scale of pain of LBP sufferer. ABSTRAKLatar belakang: Low back pain (LBP) adalah sindrom klinis dengan gejala utama nyeri atau ketidaknyamanan di daerah punggung bawah. Banyak faktor yang dapat menyebabkan LBP, salah satunya adalah faktor pekerjaan. Keluhan nyeri punggung bawah dapat timbul karena kurang berhati-hati dan sikap yang tidak ergonomis selama beraktivitas dalam bekerja. Salah satu terapi fisik yang dapat digunakan untuk mengurangi nyeri pada penderita LBP adalah dengan latihan William Fleksi.Tujuan: Mengetahui pengaruh terapi latihan William Fleksi untuk mengurangi nyeri pada penderita LBP. Metode: Penelitian ini merupakan studi literatur. Literatur dikumpulkan dari database Google Scholar dan PubMed dengan nyeri punggung bawah dan latihan fleksi William sebagai kata kuncinya. Data dianalisis menggunakan diagram alir PRISMA dengan kriteria inklusi seperti format teks lengkap, diterbitkan antara tahun 2015-2020; ditulis dalam Bahasa Indonesia dan diterbitkan secara nasional; ditulis dalam bahasa Inggris dan diterbitkan secara internasional dalam jurnal terindeks Scopus Q1-Q3. Sementara kriteria eksklusi adalah literatur yang diterbitkan sebelum tahun 2015 dan dengan publikasi ganda. Hasil: Ada tujuh literatur yang diidentifikasi sesuai dengan kriteria di atas. Dari literatur-literatur tersebut ditemukan bahwa William's Flexion Exercise terbukti dapat menurunkan skala nyeri pada penderita low back pain dan meningkatkan gerakan sendi. Terdapat beberapa metode William Fleksi, antara lain pelvic telting, single knee to chest, double knee to chest, partial sit up, hamstring stretches, bicycling, banding from a chair dan squat factor.Simpulan: Latihan William Fleksi dapat menjadi alternatif terapi fisik untuk mengurangi skala nyeri pada penderita LBP.

Abstract

Introduction: The nurses’ role within nursing care of tuberculosis patients in hospitals is  very important. Nurses problem are preparing nursing interventions in patients with pulmonary tuberculosis rarely use the standard Nursing Intervention Classification (NIC). Independent nursing interventions are not in accordance with the diagnoses established by nurses, so the impact on the implementation needed by patients is not in accordance with the problems occured. Collaborative intervention is mostly carried out by nurses compared to independent intervention. Nurses must arrange nursing interventions based on the standard of nursing diagnoses that are enforced. The objective study was to describe the interventions compiled by nurses at the hospital.Methods: Research design was qualitative research with observational approach. Sample was 100 nurse’s documentastions  of  pulmonary tuberculosis patient  in TB MDR ward. Data analysis used language and text analysis with content analysis. Results: The results research was the highest 10 interventions written by nurses is intervention about observe general condition of patient was 83 times, collaboration with  doctor’s therapy was 54 times, give comfort position was 54 times, give therapy was 47 times, check vital signs regulerly was 40 times, give oxigen was 34 times, advise to bedrest was 30 times, teach  to effective cough was 25 times, Give position semi fowler was 15% times, and teach to deep breath was 15 times. Conclusion: Nurses must improve their skills and knowledge in preparing treatment plans, because the interventions that have been prepared have not been optimal for dealing with patient problems in hospitals. Interventions to improve patient knowledge and abilities in the management of the disease are important to give to patients.

Abstract

Background: Communication is an interaction process that is highly essential in building nurse-patient interpersonal relationships. Good nurse communication-patient will have a good impact on the treatment plan and recovery for the patient. Improper communication can lead to mistaken nursing care provision and even lead to injury and death. Objective: This literature review aimed to know the efforts to improve nurse-patient communication. Design: This literature review used narrative review design. Data Sources: The databases used in narrative review are Scopus, Science Direct, ProQuest, and PubMed. The journal is limited to the publication year 2016 - 2020 with the area of ​​the nursing medicine journal, full text Portable Document Format (PDF) journal. This literature review uses 4 articles appropriate the inclusion criteria. The study of articles and data extraction were carried out separately by the researcher, if there were differences, the results were taken by consensus. Review Methods: The review method for this literature review using a narrative method by classifying the similar extracted data according to the research findings. Results: The databases used in the literature review are Science Direct, Scopus, PubMed and ProQuest. The journal is limited to the 2016 – 2020 publication year with the area of ​​nursing medicine journals, improving nurse communication, full text open access pdf journals, research designs. This literature review was identified with the number of science direct: 45, Scopus: 242, PubMed: 36 and ProQuest: 492, then screened and found 4 articles that matched the inclusion criteria. The study of articles and data extraction were carried out separately by the researcher, if there were differences, the results were taken by consensus. Conclusion: Nurse communication training has a positive impact where nurses have skills in developing knowledge and capability in improving nurse communication performance. Good communication nurse-patient can increase the patient's cure rate.

Abstract

Background: Strong organizational commitment is needed by hospitals to attract and retain nursing staffs in order to consistently deliver good quality of nursing services. The decrease in organizational commitment among nurses can cause many losses to the organization, including increased organizational spending, breakdown in patient care, and cause performance trends that appear not for the benefit of the organization or unit, but more for personal self-interest. Objective: To analyze the relationship of the application of Islamic Based Caring (IBC) model with organizational commitment among nurses. Methods: This was a correlation analytic research with cross sectional design. There were 108 nurses selected using a proportionate stratified random sampling. Islamic Based Caring was measured using a questionnaire developed based on the theory of Suhartini Ismail (2016), and organizational commitment was measured using a questionnaire developed based on the concept of Caldwell, O’Reilly & Chatmann (1990) and Mowday, Porter dan Steers (1982) in Asmaningrum (2009). Logistic regression and forward stepwise (conditional) method were used for data analysis. Results: There was a statistically significant correlation of a healing presence (p=0.000), caring relationship (p=0.010), caring environment (p=0.045) and belief in God (p=0.000). Belief in God (Allah SWT) has the highest correlation (OR=6.660) with organizational commitment among nurses. Conclusion: There is a positive and significant relationship between the implementation of Islamic Based Caring with the organization's commitment among nurses.

Abstract

Background. Writing nursing goals and outcome criteria found in nursing care documentation was very varied and not appropriately, it needs to be explored more deeply about the phenomenon. The Study objective was to determine the writing of nursing goals and the outcome criteria for nursing diagnoses. Methods. Research design was qualitative research with a direct observation approach. Data was taken in the MDR TB ward, samples were 100 documentation of pulmonary TB patients. Sampling technique was nonprobability sampling. Result. The goal of nursing diagnosis of ineffective airway clearance was airway clearance effectively again, the problem was resolved, the patient's airway returned to normal and coughing the patient returns to normal, etc. The goal of nursing diagnosis of ineffective breathing patterns was effective breathing patterns, ineffective breathing patterns resolved, etc. The outcome criteria of nursing diagnosis ineffective airway clearence was negative cough, normal respiratory rate, normal vital signs, negative sputum, shortness of breath, coughing resolved, comfortable patients, patients can demonstrate coughing effectively, sputum can come out, breath was relieved, sputum can come out, etc. The outcome criteria for nursing diagnosis ineffective breathing patterns are normal respiratory rate, normal vital signs, respiratory rate was 20 x/minute, it was not weakness, It was not nausea, etc. Recomendation. Nurses are advised to be given trainings, sosialisation, or workshops related to the goals and outcome criteria of nursing diagnoses, and are expected to use NANDA and NIC-NOC references.

Abstract

Tuberculosis is one of the biggest health problems in the world. This disease is also the highest cause of death and morbidity in developing countries, such as Indonesia. Most nurses do not carry out treatment plans for pulmonary tuberculosis patients because there is no standard structure even though there are guidelines for nursing management of pulmonary TB patients. The purpose of this study was to determine nursing diagnoses that are often enforced by nurses in pulmonary tuberculosis patients. The study was conducted in the inpatient hospital. This study used descriptive observational and descriptive explorative approaches. The samples were the documentation of the TB patients by using nonprobability sampling, namely purposive sampling saturation. Data analysis used univariate analysis with frequency distribution. The results of the study show that the formulation of nursing diagnosis is ineffective airway clearance(52%), the writing of 1 (one) diagnosis of ineffective airway clearance into 13 different diagnostic names. The nurse wrote the diagnosis of ineffective breathing patterns into 7 forms of diagnosis. Nurses also wrote diagnosis of acute pain into 5 forms of nursing diagnosis, and hyperthermy diagnosis into 2 forms of nursing diagnosis. The research recommendation is that hospital management is expected to improve nurses' skills in writing the latest nursing diagnoses, monitoring and evaluating both qualitatively and quantitatively  in the writing of nursing diagnoses, applying the latest standards of nursing diagnoses. Nurses are expected to apply the latest nursing diagnoses in accordance with the use of NANDA.
