Abstract

Latar belakang : Hipoalbuminemia pada pasien rawat inap berkaitan dengan prognosis buruk pasien. Penelitian ini, mengidentifikasi bahwa hipoalbuminemia berat pada awal pasien masuk rawat inap sebagai prediktor andalan penanda laboratorium dalam mortalitas. Metode : Sebuah studi cross sectional pada pasien dewasa dengan hipoalbuminemia (kadar albumin < 3,5 g / dL) pada pasien rawat inap (usia > 18 tahun) pada periode Januari 2013 - Maret 2018. Kami mengevaluasi penanda prediktor kematian. Multivariat dengan regresi logistik diterapkan dalam penelitian ini. Hasil : Dari 747 hipoalbuminemia pada pasien rawat inap dengan rata-rata kadar albumin pada awal adalah 2,0 ± 0,6 g / dL. Sebagian besar pasien (83,4%) memiliki kadar albumin ≤ 2,5 g / dL (hipoalbuminemia berat), 16,6 persen memiliki > 2,5 g / dL (hipoalbuminemia ringan-sedang). Kondisi yang mendasari pasien adalah infeksi HIV / AIDS (26,9%) dan sepsis (26,6%). Proporsi multiple komorbiditas pada kelompok hipoalbuminemia berat adalah 55,1 persen Pada kelompok hipoalbuminemia berat terutama untuk kadar albumin 2,01 - 2,5 g / dL, angka mortalitas adalah 28,3 persen. Berdasarkan model regresi logistik akhir, faktor risiko kematian meliputi kadar albumin pada awal dan lama rawat pasien. Mortalitas lebih tinggi pada pasien dengan hipoalbuminemia berat (rasio odds yang disesuaikan 2,91, 95% CI 1,88-4,50) dibandingkan pasien dengan hipoalbuminemia ringan-sedang. Kesimpulan: Hipoalbuminemia berat pada awal pasien rawat inap sebagai prediktor penanda kematian di rumah sakit. In this study, we attempted to identify that severe hypoalbuminemia at baseline in hospitalized patients is a reliable predictor of laboratory marker for mortality. Methods: A cross sectional study on adults of hypoalbuminemia (albumin level < 3.5 g/dL) in hospitalized patients (aged > 18 years old) in period January 2013 - March 2018. We evaluated the predictor marker of mortality. Multivariate with the logistic regression was applied in this study. Results: Of the 747 hypoalbuminemia in hospitalized patients with the mean albumin level at baseline was 2.0 ± 0.6 g/dL. Most patients (83.4 %) had less than or equal to 2.5 g/dL albumin level (severe hypoalbuminemia), 16.6 percent had over 2.5 g/dL (mild-moderate hypoalbuminemia). The underlying condition of patients was HIV/AIDS infection (26.9%) and sepsis (26.6 %). The proportion of multiple comorbidities in the severe hypoalbuminemia group was 55.1percent. In the severe hypoalbuminemia group especially for 2.01 – 2.5 g/dL albumin level, the mortality rate was 28.3 percent. Based on the final logistic regression model, known risk factors of mortality include albumin level at baseline and length of stay. Mortality was higher among patients with severe hypoalbuminemia (adjusted odds ratio 2.91, 95 % CI 1.88-4.50) than patients with mild-moderate hypoalbuminemia. Conclusion: Severe hypoalbuminemia at baseline in the hospitalized patients was a predictor laboratorymarker of hospital mortality. Keywords: hypoalbuminemia, hospitalized patients, mortality

Abstract

This study describes the concept of network through the dimensions of scape and flow. To build an analytical framework that describes the two dimensions of the network, a literature study from various sources is carried out that discusses the context of tourism promotion. This analytical framework is then elaborated through the results of in-depth interviews with twenty informants who represent the main tourism stakeholders in Bintan Regency. The informants, among others, represent elements of the government, the private sector, academia, and the community. To strengthen the analysis, especially related to the development of tourism promotion which can be directly or indirectly seen from the trend of key tourism indicators in Bintan Regency, a literature review from various books, journals, research references and online news sources from the internet is used. The Result Main finding, established networks seldom play significant role in tourism promotion as they are viewed as already settled. While tourism promotion strategy tends to inherently shows innovation, so it won’t be viewed as repetitive or redundant. This then affect engagement in post-Pandemic tourism, as shown with Bintan Regency case.

Abstract

Waerebo traditional village is one of the villages in Flores that has managed to rise through tourism development. The tourism business in Waerebo is owned and managed by the community. This study aims to determine the process of community participation in the development of community-based tourism. This study uses a qualitative method. Primary data were collected through observation and in-depth interviews with institutions and parties involved in the development. Assistance with a participatory approach through the placement of facilitators to live with the community is carried out by partners from the Waerebo Cultural Conservation Institute. The training activities followed by practice by the community, accompanied by facilitators, have shown good results. The community is involved in all stages, from planning, implementing activities, and evaluating activities. Community participation from planning to implementation is essential in developing CBT and maintaining destination resilience. The CBT approach is able to increase people's income, help maintain traditional houses, forest roads, provide diesel and solar electricity, and improve their children's education.

Abstract

This article discusses the learning systems approach. One approach that can be used to determine. The quality of the educational process is a systems approach. Through systems approach, can be seen various aspects that influence the success of a process. Learning is referred to as a system because it contains components which are interrelated and influence each other. Teachers, students, materials, Methods, media, facilities and infrastructure are components contained in learning. Availability and the effectiveness of each of these components is then determined will determine the success of learning. And the type of research method used is qualitative research where the data we take is based on observation, interviews and documentation.

Abstract

Breast milk is the optimal method to fulfil the nutritional needs of infants for optimal growth and development. However, limited milk production and ejection in the first days after delivery is often a barrier to exclusive breastfeeding. Breast engorgement in breastfeeding mothers is also a common challenge, which can hinder breastfeeding. This study used a case study design with a nursing process approach. The subjects were two breastfeeding mothers over 25 years old who experienced breast engorgement. The interventions provided included a combination of breast care and acupressure, which was carried out for four consecutive days with a duration of 20-30 minutes per session. Data were collected through observation, interview, and cheque sheet. The results showed a significant decrease in the intensity of breast engorgement and an increase in milk production in both subjects. In client I, the frequency of breastfeeding increased from 5 times to 9 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. In client II, the frequency of breastfeeding increased from 6 times to 10 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. Both clients reported decreased pain and increased comfort while breastfeeding. The combination of breast care and acupressure intervention is effective in overcoming the problem of breast engorgement and increasing milk production in breastfeeding mothers. The researcher suggested the combination of breast care and acupressure as part of breast care management in breastfeeding mothers.
