Abstract

The problem at RSI Ibnu Sina Padang in 2023 is that the search for medical records is always done manually with the time needed for 1 medical record ± 10 minutes, this is because the medical records are not specified or scattered. The purpose of this study is to find out the design of an outpatient medical record search application at RSI Ibnu Sina Padang in 2023. By adopting the type of research, namely Research & Development (R&D) with the prototype development method. The research was conducted from January to July 2023. The number of informants was 5 people, namely 5 runner officers. Conduct data analysis of potentials and problems, collect data, design products, validate designs, improve designs, and test products. The results showed that the process flow for filling in components, especially in the form of searching was done manually which included outpatient registration proof sheets. Application design can be fully implemented. The test results show that the existing components are in accordance with the hospital format and the application of the search process is faster and more efficient than manual search. Based on the results of the study, it was concluded that the design of the outpatient medical record storage information system was in accordance with the hospital format. It is hoped that this research application design can be used in hospitals and this research can be continued by other researchers.

Abstract

The admission and discharge summary is a medical record form that is used to record a summary of the course of the disease from the time the patient entered the hospital until the patient was discharged. The research used is quantitative research with a descriptive approach. Data collection was carried out at Lubuk Basung Regional Hospital from 27 May to 1 July 2023., and the sample consisted of 88 entry and exit summary forms from the medical records of patients suffering from hypertension. Research through observation with a checklist table. Computerization is used to process data and carry out univariate analysis. The results of the study showed that there were still incomplete filling out of entry and exit forms on patient identification items (40.9%), important patient reports (20.5%), patient permission (12.5%), good record keeping (15.9%) . ), and scribbled and clearly legible (17.05%). The results showed that the summary of admissions and discharges for hypertensive patients was still incomplete. In conclusion, this shows that the summary of admissions and discharges for hypertensive patients is still incomplete. It is recommended to carry out evaluations and reviews, as well as outreach and evaluation with doctors, nurses and other related personnel. It mainly deals with quantitative analysis of incoming and outgoing summaries.

Abstract

Plastic waste has become one of the major problems threatening the environment and human healthIn Indonesia, the use of plastics in daily life, such as shopping bags, water bottles, and food packaging, contributes greatly to the increasing amount of plastic waste (Ditjen PPKL, 2018). This research aims to analyse effective plastic waste management strategies to reduce negative impacts on the environment. The research uses a mixed-method approach, a combination of quantitative and qualitative methods, to gain a comprehensive understanding of the condition of plastic waste and identify the factors that influence the effectiveness of such management. This method is relevant for evaluating waste management policies, the role of the community, and the application of environmentally friendly technologies. This study will be conducted in several areas that have high plastic waste production rates, such as urban, coastal, and areas with community-based waste management initiatives. The results show that community awareness level participation in recycling high 60% p = 0.001 This shows that environmental education is very important to increase community participation. Community-based education is needed to raise this awareness. A massive education campaign is needed, involving community leaders to strengthen positive social norms.
