Abstract

Background: Gingival recession has a number of causes, including orthodontic therapy. The degree and extent of gingival recession and orthodontic therapy are strongly correlated. Care must be taken in choosing the best soft tissue grafting technique. Development of a number of new and improved surgical techniques, including the modified tunneling technique. Case Management: A-24-year-old female patient came to Periodontic Clinic Dental Hospital of Airlangga University with a chief complain of gingival recession in mandibular central incisors since 6 months ago. The patient complained about dental sensitivity. The dental history revealed that she had undergone fixed orthodontic therapy for 7 years. She revealed the fixed orthodontic appliance was put off since 8 months ago. The patient concern about her appearance and aesthetic problem. Gingival recession on buccal surface tooth 31 and 41 extending 1,5-2 mm apical of the incisors was found in clinical assessment. This case classified as Miller class III. The doctor planned root coverage procedure using modified tunnel technique. Acellular dermal matrix (Surederm®) was chosen as a gingival graft. Discussion: Creating a "tunnel" through the buccal mucosa to treat gingival recession enables coronal repositioning of the soft tissue with predictable root coverage and a pleasing appearance. Tissue graft implantation is made possible by a horizontal incision. Conclusion: In brief, root coverage using modified tunnel technique is effective and predictable treatment modality. Comparing with other root coverage method it give quite satisfying result. The use of Acellular Dermal Matrix (ADM) considered as an alternative to Connective Tissue Graft (CTG).

Abstract

Introduction: Gingival tissue plays an important role in smile aesthetics, especially in the anterior maxillary region. A gummy smile (GS) is a gingival condition that appears excessive when the patient smiles. The prevalence of gummy smile in individuals aged from 20 to 30 is 10% of population. It is more common in women and can cause aesthetic problems. Gummy smile aetiology is multifactorial. This case report discusses gummy smile case caused by altered passive eruption (APE) and associated with the presence of aberrant frenum. Case Report: A 19-year-old female patient with chief complaints of the excessive gingival display. The diagnosis of the case was confirmed by clinically healthy gingiva in an intact periodontium with gummy smile due to the presence of altered passive eruption type I-B accompanied by an aberrant frenum. The final surgical plan was aesthetic crown lengthening (CL) with flap surgery and osteotomy simultaneous with a frenotomy. Conclusion: The aesthetic crown lengthening surgery and frenotomy can be performed simultaneously to correct the gummy condition and improve the patient’s smile profile. Appropriate examination, diagnosis, and treatment planning help achieve optimal patient outcomes.

Abstract

An excessive gum display (EGD) refers to a smile that displays more than 2 mm of visible gingiva. Among dental patients, it is a prevalent aesthetic issue. It influences both psychological and aesthetic status since it typically lowers self-confidence. EGD is considered unaesthetic when smiling, thus many patients seek treatment to manage this condition. The etiology of EGD varies, with hypermobile upper lip (HUL) as the most predominant. HUL is defined as more than 8 mm of upper lip movement from rest to maximum smile. This article will discuss about the management of EGD caused by HUL, treated with coronally placed vestibulum. Case: A 28-year-old male patient came with a complaint of disproportionate gingival display. The patient felt that his gums were exposed too much when smiling. The patient was not confident with the condition. On maximum smile, 6 mm of gingiva was displayed due to hypermobile upper lip. The diagnose of excessive gum display was established. The treatment was coronally placed vestibulum, which done by relocating the attachment between the mucosa and the attached gingiva closer to the coronal. Follow-up results after 3 months showed a noticeable decrease in excessive gingival display. Discussion: Coronally placed vestibulum is performed on cases of EGD with the etiology of HUL. Through this procedure, the vestibule's limiting muscular pull is narrowed, reducing gingival display when smiling. Conclusion: Coronally placed vestibulum is a less-invasive treatment of choice for EGD caused by HUL.

Abstract

Introduction : In the modern era, dental esthetics has become an important consideration, particularly in cases of gummy smile, which may significantly reduce a patient’s self-confidence. A gummy smile is defined as excessive gingival display of more than 3 mm in the vertical dimension of the maxillary anterior region during smiling. The etiology of gummy smile is multifactorial and may include hyperactivity of the upper lip elevator muscles, vertical maxillary excess, and delayed passive eruption. Case Report: A 32-year-old woman presented with the chief complaint of excessive gingival display during smiling and dissatisfaction with poorly fabricated veneers, which negatively affected her confidence. She also reported halitosis following veneer placement performed at a non-clinical setting. Intraoral examination of the maxillary and mandibular anterior regions revealed poor-quality veneers, hyperemic and edematous gingival margins, and abnormal gingival texture. Discussion: A pleasant smile requires harmony among tooth shape, color, size, gingival health, and an appropriate amount of gingival display during smiling. This case report describes a surgical approach combining periodontal soft tissue reconstruction with smile elevator tissue containment for the correction of gummy smile. The technique aims to reduce excessive gingival display by limiting the excessive retraction of the smile elevator muscles. Conclusion: Periodontal soft tissue reconstruction in combination with smile elevator tissue containment represents an innovative surgical technique for the correction of gummy smile.

Abstract

Background: A ‘gummy’ smile, also known as excessive gingival display, is a dental condition characterized by the excessive visibility of the gums when a person smiles. This condition typically occurs when more than 3–4 mm of gum tissue is exposed, making the teeth appear shorter and affecting the smile’s aesthetic appeal. Purpose: This case report aims to explain the minimally invasive surgery technique for correcting a gummy smile using diamond frenotomy. Case: A 24-year-old woman with the chief complaint that her gingiva was visible excessively when smiling. Case management: Intraoral examination on the maxillary anterior region showed a gingival margin at the normal position, normal texture, standard color of gingiva, and normal crown size. Diamond frenotomy with a double mosquito hemostat was inserted parallel to the attached gingiva and vestibule, then incised until the two mosquito clamps met, producing the remaining frenum tissue to be excised. Following evaluation over six months, the gummy smile was observed to have been significantly reduced. Conclusion: Diamond frenotomy is an innovative surgical technique for gummy smile correction.
