Abstract

Background: Obesity and periodontal diseases are two highly prevalent chronic conditions in Indonesia. Both obesity and periodontitis have been recognized as a major risk factor for a number of systemic diseases. Obesity related to the etiology and progression of periodontal disease was well known, but a recent study describes the role of periodontitis to obesity.Purpose: The aim of this article to explain possible bidirectional relationship of periodontitis and obesity, so that dental practitioners may play an important role in management both obesity and periodontal disease.Literature review: The concept of infectobesity refers to obesity of infectious origin was accepted for the possible role of oral bacteria in the development of obesity. Excessive storage of adipose tissue in obese individuals leads to the release of adipokines which produce inflammatory. Relentless release of pro-inflammatory cytokines into the systemic circulation from adipose tissue in obese individuals and periodontal bacteria and its product provides a systemic inflammatory overload and may be a reservoir for bidirectional relationship of periodontitis and obesity. Maintaining good oral health is also fundamental for obese individuals. Dental practitioners should play important role in managements by educate their obese patients and refer the patient to physician and dietitian for a proper maintaining obesity.Conc1usion: There is a bidirectional relationship between obesity ang periodontal disease.

Abstract

Pregnancy is a unique time in a woman’s life, accompanied by a variety of physiologic, anatomic and hormonal changes that can affect how oral health care is provided. Good oral health during pregnancy is important to the overall health of both the expectant mother and her baby. Unfortunately, there may be times when pregnant women and family, obstetricians and on occasion dentists are skeptical of dental care during pregnancy owing to prejudices about the safety of dental treatment, resulting in delay of the dental treatment. This article aims to review general considerations of pregnant patients’ dental treatment, and possible dental complications of pregnancy and their management. Search for information through google scholar with the keyword’s “pregnancy” and “dental treatment". With proper technique, dental treatment, dental radiographs and drug use in dental treatment do not place the fetus at risk and should be taken if they are of potential benefit. Preventive care should be delivered throughout pregnancy, and elective routine care is best delivered during the second trimester. Dentists and health agencies should provide and distribute information to women about the importance of maintaining oral health during pregnancy.

Abstract

The development of science and technology in the field of periodontology in digital era has experienced quite rapid development. However, this development is not comparable with community's periodontal tissue health that is fairly high. Data from RISKESDAS 2018 shows the percentage of periodontitis cases in Indonesia is 74.1%. Periodontal disease is an oral disease that ranks first in the 2001 world record book record as the most common disease in humans. The high periodontal prevalence data can be due to lack of patient’s knowledge of periodontal health or dental health practitioners who do not emphasize the importance of the health of periodontal tissue. The risk assessment of periodontal disease is still rarely done by dental health practitioners when compared with the assessment of the risk of dental caries. This article aims to introduce periodontal disease risk assessment methods and tools for dental health practitioners. Search for information on periodontal disease risk assessment methods and tools through PubMed with the keywords "periodontal charting" and "periodontal risk assessment". The periodontal disease risk assessment tool is chosen based on open access and free of charge. Periodontal charts and Periodontal Risk Assessment (PRA) are simple applications that can be used by all dental and oral health practitioners in explaining, evaluating and motivating patients with periodontal disorders. Periodontal chart and PRA can be used as material for evaluation and education of patient's periodontal tissue for dental health practitioners.

Abstract

Overweight and obesity has become an epidemic in many developed and developing countries, and its prevalence is increasing globally. Overweight and obesity obesity are associated with many diseases such as cardiovascular diseases, some forms of cancer, type 2 diabetes mellitus and chronic oral diseases. It is important for dentists to understand and recognize overweight and obesity, educate their patients on its associated risks, promote a healthy lifestyle for their patients, and modify any necessary dental treatments and postoperative procedures for overweight and obese patients.  A multidisciplinary approach of overweight and obesity is needed for optimal control. Professional dentist must be prepared to manage the overweight and obesity-related effects of co-morbidities and oral diseases and working in conjunction with other health care providers. However, the majority do not routinely measure weight and height, nor calculate body mass index for comprehensive care patients in their daily dental practice, thus the dentist role in overweight and obesity is rare. The purpose of this article is to review the assessment of overweight and obesity, the relevance of overweight and obesity to oral helath and dental practice, and highlight some of the actions dentist can to prevent and decrease the risk of both these diseases.

Abstract

During the Covid-19 pandemic, there was an increase in pregnancy rates in East Java. Besides efforts to suppress pregnancy rates, it is also necessary to ensure the health of the mother and fetus in order to be born healthy. The prevalence of periodontal diseases in pregnant women is high, while the dental visit of pregnant women is low. Periodontal disease is one of the risk factors of preterm birth as well as low-birth-weight (LBW) infants. In this community service, socialization held through lecture methods, QnA, discussion and self-practice procedures to maintain oral health procedures. The goal is to improve pregnant women's knowledge of the importance of dental and oral care, especially periodontal tissue during pregnancy and enhance skills of pregnant women in detecting early symptoms of periodontal diseases and to perform oral dental care independently at home. The knowledge and skills of pregnant women can be improved during this community service. By healthy periodontal tissue and teeth, it is expected that the risk of LBW infants and also preterm birth can be decreased.abstrakSelama masa pandemi Covid-19, terjadi peningkatan tingkat kehamilan di Jawa Timur. Selain usaha menekan tingkat kehamilan, perlu juga dipastikan kesehatan ibu dan janin agar lahir dengan sehat. Prevalensi kelainan periodontal pada ibu hamil tinggi sementara kunjungan ibu hamil ke dokter gigi rendah. Penyakit periodontal merupakan salah satu faktor resiko bayi lahir prematur dan juga berat badan lahir rendah (BBLR). Dalam pengabdian masyarakat ini dilakukan sosialisasi melalui metode ceramah, tanya jawab, diskusi dan praktek mandiri prosedur menjaga kesehatan rongga mulut. Tujuannya adalah meningkatkan pengetahuan ibu hamil mengenai pentingnya perawatan gigi dan mulut khususnya jaringan periodontal selama kehamilan dan meningkatnya ketrampilan ibu hamil dalam mendeteksi gejala awal kelainan periodontal dan cara melakukan perawatan gigi mulut mandiri di rumah. Pada pengabdian masyarakat ini pengetahuan dan ketrampilan ibu hamil dapat ditingkatkan. dengan jaringan periodontal dan gigi serta rongga mulut yang sehat, diharapkan resiko BBLR maupun kelahiran prematur dapat diturunkan.

Abstract

Salah satu dampak nyata dari adanya pandemi Covid-19 dan kebijakan bekerja dari rumah adalah peningkatan angka kehamilan di masyarakat. Jawa Timur sebagai salah satu provinsi dengan jumlah penduduk terbanyak di Indonesia menunjukkan peningkatan angka kehamilan selama masa pandemi. Tingkat kelahiran prematur di Jawa Timur sebesar 23.3%, sementara pada tahun 2018, 885 kelahiran bayi di Surabaya memiliki berat badan lahir rendah (BBLR). Salah satu faktor resiko dari kelahiran bayi prematur dan BBLR (berat badan lahir rendah) adalah adanya kelainan jaringan rongga mulut yaitu kelainan periodontal. Kelainan periodontal yang umum ditemukan pada ibu hamil diantaranya adalah gingivitis dan periodontitis. Di Kota Surabaya, studi yang dilakukan pada delapan puskesmas menunjukkan 73% ibu hamil mengalami gingivitis dan 36% ibu hamil mengalami periodontitis. Mengingat pentingnya kesehatan periodontal, gigi dan mulut selama masa kehamilan, pada pengabdian masyarakat kali ini dilakukan sosialisasi kesehatan jaringan periodontal, gigi dan mulut pada ibu hamil. Target sasaran dari pengabdian masyarakat ini adalah ibu hamil yang melakukan kontrol kehamilan pada klinik utamaarea kecamatan Tambak Sari. Pengabdian masyarakat dilakukan melalui metode pemutaran video, ceramah, diskusi dan peragaan prosedur kesehatan periodontal dan rongga mulut. Keberhasilan dari program pengabdian masyarakat ini dinilai melalui indikator peningkatan nilai pada tes tertulis yang diberikan sebelum dan sesudah pelaksanaan sosialisasi. Melalui pengabdian masyarakat ini, pengetahuan dan prilaku sadar periodontal sehat bagi ibu hamil dapat ditingkatkan, sehingga diharapkan dapat mengurangi resiko penyakit periodontal bagi kehamilan.

Abstract

Background: Smoking is well known risk factors that promote periodontal tissue destruction. Both smoking and periodontitis nowadays consider as a common health problem globally. As smoking habit evolution, electronic cigarettes (E-cigs) have emerged as an alternative to cigarettes. The number of E-cigs smoker or vaping users around the world are increasing. However, information about the effect of E-cigs on periodontium is very lacking lately. The study aimed to compare the effect of cigarette consumption on periodontium by clinical health parameters between all smokers’ type.Method: This review follows the PRISMA guidelines. Document search was carried out in PubMed, Ebsco Host and Scopus using the keywords or phrases: (periodontal health) AND (((((electronic cigarette) OR Vaping) OR e-cig) OR electronic nicotine delivery system) OR nicotine delivery system). A total of 137 articles were obtained after the duplicates were eliminated and five articles were considered met the eligibility criteria for systematic review. Result: This review found that E-cigs are less harmful compared to tobacco cigarettes, with comparable result between E-cigs smoker and non-smoker on some clinical periodontal health parameters for mean probing depth, clinical attachment loss, score of plaque index (PI) and papillary bleeding index (PBI) as well. In contrast, the study reveals that bleeding on probing level was higher among non-smokers than smokers.Conclusion: E-cigs are less harmful compared to tobacco cigarettes on some clinical periodontal health parameters. This result should be interpreted with caution because there is currently insufficient data to investigate the effect of E-cigs on periodontal health through clinical parameters.

Abstract

Background: The treatment of severe periodontitis must consider immune responses and local conditions, including the aesthetical aspects. The tooth loss in the anterior area can lead to a psychological issue in some patients, and the daily use of dexamethasone contributes greatly to the severity of aesthetical aspects. Periodontal treatments must be cautious of these aesthetical effects. Purpose: To report the aesthetical consideration in patient management of severe periodontitis aggravated by oral dexamethasone. Case: The 44-year-old female patient reported having tooth mobility in the upper right and left central incisor and lower right and left posterior. Due to the condition, the central anterior needed to be extracted. The patient had seafood allergies and consumed oral dexamethasone periodically to prevent allergic reactions for two years. Case management: The initial periodontal therapy was designed prior to the tooth extraction, socket preservation, and immediate denture on teeth 11 and 21. A metal frame combined with an acrylic denture was designed to support the tooth splint and replace the teeth on the mandible. The patient was treated with 20 mg of sub-antimicrobial-dose doxycycline twice a day for three months, and vitamin E was prescribed once a day. Since dexamethasone may contribute to immune response and osteoclastogenesis, dexamethasone was replaced by cetirizine. Conclusion: The treatment of severe chronic periodontitis must consider immune responses, local conditions, and aesthetical aspects. In this case, the use of dexamethasone might worsen the periodontal breakdown. However, the periodontal treatment, use of host modulation therapy, and replacement of dexamethasone with cetirizine are expected to improve these conditions.

Abstract

Background: Periodontitis is a chronic inflammatory disease of the periodontal tissue that is characterized by alveolar bone resorption. This occurs due to an imbalance of osteoblast and osteoclast during the bone formation and resorption processes. In order to obtain complete regeneration of periodontal tissue, bone grafting is frequently used in periodontal surgical therapy. Although each material has disadvantages, safe graft materials derived from animal sources can be employed as an alternative to bone graft materials. Osteoblast, osteoclast, calcified bone matrix, type I collagen, osteonectin, and hydroxyapatite can all be found in gourami scales, a form of food waste. BMP4 has osteoinduction functions, which are important in bone metabolism. Through angiogenic activity, FGF2 also contributes to periodontal regeneration. Purpose: The aim of the study was to assess the expression of BMP4 and FGF2 after the treatment group had been given gourami fish scale extract. Methods: Thirty-two experimental three-month-old male Wistar rats (150-200g) were randomly divided into four groups: a seven-day control group, a seven-day treatment group, a 14-day control group, and a 14-day treatment group. One mandibular incisor was extracted from each Wistar rat. The post-extraction socket was filled with blood for the control group and collagen extract for the treatment. Results: The one-way ANOVA test showed a significance level of 0.000 (p = <0.05). Conclusion: The expression of BMP4 and FGF2 increased after the application of collagen extract from gourami scales.

Abstract

Kota Pasuruan merupakan salah satu kota yang masih ditemukan kasus stunting. Adanya Pandemi COVID-19 turut memberikan dampat negatif pada status stunting di Kota Pasuruan yang pada tahun 2021 tercatat sebanyak 23 persen dari tahun sebelumnya sekitar 19 persen. Salah satu sasaran primer pencegahan stunting adalah ibu hamil. Pada ibu hamil, kesehatan rongga mulut berperan besar terhadap kesehatan secara umum, termasuk untuk pencegahan stunting pada calon buah hatinya. Pada pengabdian masyarakat ini, dilakukan pengabdian kepada masyarakat dalam upaya mencegah stunting melalui pencegahan kelahiran Bayi dengan Berat Badan Lahir Rendah (BBLR) dengan cara meningkatkan pencegahan kelainan didalam mulut pada ibu hamil melalui pelatihan deteksi dini dan pencegahan kelainan periodontal pada ibu hamil dan kader kesehatan sebagai upaya pencegahan stunting di Puskesmas Kandangsapi Kota Pasuruan. Tahapan pengabdian dilakukan meliputi tahap persiapan kegiatan, mengorganisasi kegiatan, tahap edukasi dan pelatihan target sasaran, tahap pembentukan kepemimpinan, koodinasi intersectoral dan tahap akhir evaluasi kegiatan. Peningkatan pengetahuan target sasaran menjadi indikator kuantitatif keberhasilan program yang dievaluasi melalui tes tulis sebelum dan setelah pengabdian dilaksanakan. Kegiatan pengabdian masyarakat dapat meningkatkan kemampuan kognitif maupun psikomotor ibu hamil dan kader kesehatan untuk melakukan deteksi dini dan pencegahan kelainan periodontal sebagai upaya pencegahan stunting.
