Abstract

Background: Smoking, both active and passive, has been widely recognised as toxic to the human body, since it induces several forms of cancer, including that affecting the oral cavity. Benzopyrene, the carcinogen contained in tobacco smoke, can even lead tocarcinogenesis which potentially affects the regulation of cell apoptosis in both active and passive smokers. Purpose: This study aims to investigate the carcinogenic effects of cigarette smoke on apoptosis of rat tongue mucosae through p53 expression. To determine the risk of malignant transformation through tumor suppressor genes in the apoptotic pathway. Methods: Rattus norvegicus subjects were divided into four groups, namely Treatment Group 1 exposed to sidestream cigarette smoke for four weeks (P1), Treatment Group 2 exposed to sidestream cigarette smoke for eight weeks (P2), Control Group not exposed to sidestream cigarette smoke for four weeks (K2), and Control Group (K) not exposed to sidestream cigarette smoke for eight weeks (K2). The exposure process was conducted using a smoking pump and alternating exposure. Four micron-thick sections of formalin were subsequently fixed together with paraffin embedded biopsy material from tongue mucosa of Rattus norvegicus. The tissue sections from the treatment groups were then analyzed immunohistochemically to compare the expressions of p53 and Bcl-2 proteins with those of the control groups. Results: The T-test results indicated statistically significant differences in the expressions of p53 between the 4-week control group (K1) and the 4-week treatment group (P1) (p=0.01, p<0.05) as well as between the 8-week control group (K2) and the 8-week treatment group (P2) (p=0.03, p<0.05). Conclusion: Exposure to cigarette smoke can induce changes in tumor suppressor genes and also affect the regulation of cell apoptosis, thus changing cell structure and leading to malignancy.

Abstract

Background: Aplastic anaemia is a disease that occurs when the body fails to produce enough blood cells. Oral lesions, such as canker sores, are often used as an early marker of this disease condition. In the COVID-19 pandemic era, a teledentistry method is expected to assist in the management of patients with various diseases including aplastic anaemia. Purpose: This case report discusses the management of patients with aphthous-like ulcers related to aplastic anaemia in a 34-year-old woman, based on history, clinical examination, and blood tests through teledentistry. Case: The patient complained of a bleeding wound on the inner left cheek and corner of the left lip that appeared one month ago. There was an ulcer lesion that was bleeding easily in the buccal mucosa sinistra and the angular sinistra, as well as macules lesion on her hand and foot. The complete blood count test exhibits a low platelet level. Case management: The diagnosis was an aphthous-like ulcer related to aplastic anaemia. Patient was treated with topical antiseptic and anti-inflammatory mouthwashes for the oral lesions, as well as collaboration with the medical team. The lesions improved after the patient received a blood transfusion and drug therapy from the internist while she was hospitalized, but the lesions reappeared in other locations in the oral cavity when the platelet level dropped. Conclusion: Management of oral manifestations in patients with aphthous-like ulcers related to aplastic anaemia in the COVID-19 pandemic era can be done through teledentistry. However, this requires sensitivity from the dentist, patient compliance in following instructions, and the involvement of a multidisciplinary approach, such as collaboration with internists to achieve recovery. Delay in detecting and treating aplastic anaemia will lead to death, as in this case.

Abstract

Background: The oral manifestation of Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS) is an important indicator because it is a disorder that usually appears first when the immune system is suppressed. Oropharyngeal candidiasis is an opportunistic infection that most often occurs in people suffering from HIV infection or AIDS. It is also an important marker in immunosuppressed states with CD4 values below 200 cells/mm3. This case study aims to evaluate the treatment of oropharyngeal candidiasis in patients with HIV/AIDS with CD4 values below 10 cells/µLCase Presentation: A 30-year-old man complained of a thick and uncomfortable feeling in the entire oral cavity felt since 4 months ago. Clinical examination revealed multiple pseudomembranous, which were removed and left a reddish area at the base of most of the oral cavity to the pharynx. The results of the fungal examination showed that the hyphae were not insulated with the results of candida albicans culture, and the absolute CD4 value was 9 cells/µl. On the second visit, the patient admitted that he was HIV positive by bringing a reactive result on the 3-method anti-HIV examination conducted 3 months earlier. This case was handled by administering an antiseptic mouthwash, topical and systemic antifungal, and referred to the IHAN RSUA polyclinic for ARV therapy.Conclusion: The successful treatment of oropharyngeal candidiasis, in this case, depends on integrated therapy between dentists and internists as well as patient compliance in following doctor's instructions. Latar Belakang: Manifestasi oral Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS) menjadi indikator penting karena merupakan kelainan yang biasanya muncul pertama kali saat kondisi imun tersupresi. Kandidiasis orofaring merupakan suatu infeksi oportunistik yang paling sering terjadi pada orang yang menderita infeksi HIV atau AIDS. Hal ini juga disadari sebagai suatu pertanda penting pada keadaan imunosupresi dengan nilai CD4 dibawah 200 sel/mm3. Laporan kasus ini bertujuan untuk mengevaluasi penanganan kandidiasis orofaring pada pasien (HIV/AIDS) dengan nilai CD4 dibawah 10 sel/µLPresentasi Kasus: Seorang laki-laki berusia 30 tahun mengeluhkan rasa tebal dan tidak nyaman pada seluruh rongga mulut yang dirasakan sejak 4 bulan yang lalu. Pemeriksaan klinis ditemukan pseudomembran multipel yang dapat dihapus dan meninggalkan area kemerahan di dasarnya pada hampir seluruh rongga mulut hingga ke faring. Hasil pemeriksaan jamur menunjukkan hyphae tidak bersekat dengan hasil biakan candida albicans, dan nilai CD4 absolut 9 cell/µl. Kunjungan kedua Pasien mengakui sebagai pengidap HIV dengan membawa hasil reaktif pada pemeriksaan anti-HIV 3 metode yang telah dilakukan 3 bulan sebelumnya. Penanganan kasus ini dengan pemberian antiseptik kumur, anti jamur topikal dan sistemik serta dirujuk ke poli IHAN RSUA untuk melakukan terapi ARV.Kesimpulan: Keberhasilan penanganan kandidiasis orofaringeal pada kasus ini bergantung pada terapi yang terintegrasi antara dokter gigi dan dokter penyakit dalam serta kepatuhan pasien dalam mengikuti instruksi dokter.

Abstract

Background: Orofacial pain is associated with the hard and soft tissues of the head, face, and neck. Knowledge of orofacial pain helps in getting information on clinical symptoms, trigger factors, and risks of orofacial pain, as well as clinical and supportive examinations for identifying the pain. The dental professional students’ knowledge influences the ability to diagnose and decide on an appropriate treatment plan. This knowledge is seen from the intelligence level in obtaining information about orofacial pain. Purpose: To describe the level of knowledge of orofacial pain in the students of the Dental Professional Program, Faculty of Dental Medicine, Universitas Airlangga, based on intelligence level. Methods: This research was a descriptive study. Data was taken using a questionnaire on google form with a simple random sampling data technique and was analyzed using SPSS version 25. Results: The majority understand the general description of postherpetic neuralgia (PHN), as well as the risk factors and triggers (93.4% and 87.8%, respectively) associated with it. Further, 90.6% understand burning mouth syndrome (BMS) and the clinical symptoms of PHN. BMS based on clinical examination and support is understood by 96.1%, and 82.9% know how to manage trigeminal neuralgia (TN). Intelligence level is divided into three categories, namely source of knowledge (55.8% are from lectures, journals, and textbooks), material repetition (77.3% never repeated), and retention of material (65.2% no retention). Conclusion: Students’ knowledge of orofacial pain is good, but the relationship between the level of intelligence and knowledge is not yet known.

Abstract

Background: Xerostomia, or dry mouth, is a subjective sensation often linked to hyposalivation, affecting oral and overall health. With a global prevalence of 5.5% to 46%, it is more common in the elderly due to factors like polypharmacy, systemic diseases, and age-related salivary gland changes. Xerostomia impacts daily activities such as speaking, chewing, and swallowing, reducing quality of life. Rejosari Village in Lamongan Regency, East Java, has a high elderly population and a well-established healthcare center for seniors (Posyandu Lansia), making it an ideal setting for studying awareness and knowledge of xerostomia. Purpose: To describe the level of knowledge of xerostomia among elderly people in Rejosari Village, Lamongan Regency, East Java. Methods: This descriptive observational study utilized an online questionnaire distributed via Google Forms. Participants were selected through consecutive sampling, focusing on elderly individuals who met the inclusion criteria. Of the 75 respondents who completed the questionnaire, 70 were included in the final analysis. Data were analyzed descriptively, categorizing knowledge levels into high, moderate, and low. Results: Among the 70 eligible respondents, 70.00% exhibited a high level of knowledge about xerostomia. Meanwhile, 22.86% were classified as having moderate knowledge, and 7.14% had low knowledge levels. These findings indicate that most elderly residents in Rejosari Village are well-informed about xerostomia, although knowledge gaps persist among certain groups. Conclusion: The elderly’s knowledge about xerostomia in Rejosari Village, Lamongan Regency, East Java is dominated in the high knowledge category.

Abstract

Pendahuluan: Recurrent Aphthous Stomatitis (RAS) merupakan salah satu penyakit pada mukosa rongga mulut yang paling sering dijumpai. RAS timbul dengan berbagai faktor predisposisi, disertai rasa nyeri, dengan waktu penyembuhan yang berbeda-beda. Tes darah lengkap adalah tes darah mengukur beberapa komponen darah termasuk sel darah merah, sel darah putih, hemoglobin, hematokrit dan trombosit. Di antara faktor etiologi RAS diketahui faktor inflamasi yang berperan. Secara teori, diketahui bahwa neutrofil dan limfosit bertanggung jawab terhadap terjadinya inflamasi. Penelitian ini bertujuan untuk menganalisis apakah terdapat korelasi antara profil hitung darah lengkap dan waktu penyembuhan RAS. Metode: Penelitian ini adalah penelitian analitik observasional dengan rancangan potong-lintang (cross-sectional). Metode pengambilan sampel dalam penelitian ini adalah total sampling. Penelitian ini menggunakan 20 pasien yang sedang mengalami RAS. Data pasien didapatkan dari rekam medik RSGM Universitas Airlangga. Data yang diambil lengkap dengan tes darah pasien serta waktu penyembuhan RAS. Data dari kedua variabel dianalisis menggunakan uji korelasi statistik. Korelasi Pearson digunakan ketika data terdistribusi secara normal sedangkan Spearman's Rho digunakan ketika data tidak terdistribusi secara normal. Hasil: Hasil uji korelasi Pearson antara limfosit dan waktu penyembuhan RAS dengan nilai r=-0,459 ; p=0,042 menunjukkan nilai korelasi negatif yang berarti semakin tinggi kadar limfosit maka semakin rendah waktu penyembuhan RAS. Limfosit, trombosit, eosinofil dan LED juga menunjukkan nilai korelasi negatif tetapi tidak signifikan karena p>0,05. Tidak ada nilai signifikan yang ditemukan pada variabel lain kecuali limfosit dan MCHC. Simpulan: terdapat korelasi antara profil darah lengkap (limfosit dan MCHC) dengan waktu penyembuhan RAS.The correlation between complete blood profile and healing time of Recurrent Aphthous Stomatitis: analytical observationalIntroduction: Recurrent Aphthous Stomatitis (RAS) is one of the most common oral mucosa diseases. RAS occurs with various predisposing factors, accompanied by pain, and has varying healing times. A complete blood count is a blood test that measures several blood components, including red blood cells, white blood cells, hemoglobin, hematocrit, and platelets. Among the etiological factors of RAS, inflammatory factors are known to play a role. In theory, it is known that neutrophils and lymphocytes are responsible for inflammation. Methods: This study is an observational analytical study with a cross-sectional design. The sampling method in this study is total sampling. This study used 20 patients who were experiencing RAS. Patient data has been obtained from the medical records of RSGM Universitas Airlangga. The data is complete, including patient blood tests and RAS healing time. Data from both variables were analyzed using statistical correlation tests. Pearson correlation is used when the data is normally distributed, while Spearman's Rho is used when the data is not normally distributed. Results: The results of the Pearson correlation test between lymphocytes and RAS healing time with a value of r = -0,459; p = 0,042 showed a negative correlation value, meaning that the higher the lymphocyte levels, the lower the RAS healing time. Lymphocytes, platelets, eosinophils, and LED also showed negative correlation values but were not significant because p>0,05. No significant values were found in other variables except lymphocytes and MCHC. Conclusion: a correlation exists between complete blood profile (lymphocytes and MCHC) and RAS healing time.
