Abstract

Objective: This study aims to validate the Indonesian version of International Prostate Symptom Score (IPSS)to ensure similar purpose and quality as the original version. Material & method: Validation and reliability of IPSS-Ina was performed on 174 subjects divided into 87 benign prostate hyperplasia(BPH) patients and 87 non-BPH subjects as a control group. A total of 33 patients in the BPH group then underwent TURP. Validation was assessed by correlating the IPSS score-Ina with a quality of life score (QoL). Reliability was tested by internal consistency with Cronbach α and test-retest trials. Sensitivity to changes in IPSS-Ina post-therapy was tested by comparing the mean IPSS before and after transurethral resection of the prostate (TURP) therapy. Results: Correlation between IPSS-Ina score with quality of life (QoL) was 0.85 (p <0.001),showing strong correlation.Results of Cronbach's α 0.93; while test-retest trials with the intraclass correlation coefficient (ICC) 0.99; which indicates a very good reliability. The mean initial IPSS-Ina (20.97 ± 4.4) with post-TURP (4.82 ± 1.77) differed significantly (p <0.001). Conclusion: IPSS-Ina proved excellent validation and reliability and was sensitive to symptomatic changeafter therapy. IPSS-Ina have the same goals and the quality of the original version.Keywords: International prostate symptom score, Indonesia, validation, reliability.

Abstract

Indonesia merupakan salah satu negara yang sering dilanda bencana dan menempati urutan ke-38 dari 181 negara rawan bencana. Maka dari itu, penting untuk memiliki pengetahuan, sikap, dan praktik yang baik agar dapat mengurangi dampak dan risiko terjadinya bencana. Sebagai garda terdepan dalam merespon bencana, tenaga kesehatan semestinya memiliki kemampuan yang mumpuni dalam penaggulangan bencana. Penelitian ini bertujuan untuk mengetahui gambaran pengetahuan, sikap, dan praktik tenaga kesehatan dalam manajemen pra bencana di Puskesmas Kecamatan Tilango Kabupaten Gorontalo Tahun 2022. Penelitian ini merupakan penelitian deskriptif kuantitatif. Adapun pengambilan sampel menggunakan teknik total sampling dengan sampel sejumlah 67 tenaga kesehatan di Puskesmas Kecamatan Tilango. Penelitian dilakukan di Puskesmas Kecamatan Tilango pada bulan Oktober sampai November tahun 2022. Instrumen penelitian menggunakan kuesioner yang sudah pernah digunakan pada penelitian yang dilakukan oleh Ari Susilawati tahun 2018. Hasil untuk pengetahuan sebagian besar dengan kategori cukup (76,1), sikap dengan kategori cukup (50,7%), dan praktik dengan kategori kurang(43,3%). Gambaran pengetahuan dan sikap responden tergolong cukup memadai adapun praktiknya masih tergolong kurang. Diharapkan kedepannya dapat memperbanyak pengalaman dengan turun langsung dalam kegiatan tanggap bencana serta aktif mengikuti pelatihan terkait kebencanaan agar dapat memperbanyak pengalaman dan meningkatkan pengetahuan tentang kebencanaan.

Abstract

In order to treat non-muscle invasive bladder cancer, this study will compare and contrast conventional transurethral resection of bladder tumors (TURBT) with En-bloc resection (ERBT). It will also assess the efficacy, safety, and recurrence rates of these two methods. We searched PubMed and Sience Direct for relevant articles published between 2013 and 2023. Articles from randomized controlled trials, retrospective analyses, and prospective studies comparing non-muscle invasive bladder cancer therapies employing en bloc versus conventional TURBT are included in this analysis. Measuring effect size was done using Mean Difference (MD) and Odd Ratio (OR) with 95% CI. There is statistical significance when the p-value is less than 0.05. Fourteen studies were eligible for inclusion. With fewer tumor recurrences (OR 0.66; 95% [CI]  0.52, 0.84; I2 = 6%; P = 0.0007), ERBT proved to be superior to TURBT. There was a decrease in the occurrence of perioperative complications and outcomes associated with ERBT, including bladder perforation and obturator nerve injury.  According to the fourteen articles that were included, patients receiving ERBT experienced much fewer complications, recurrences, hospital stays, and catheterization time than those receiving conventional TURBT.
