Abstract

Background. Panfacial fractures involves fractures of several bones of face. They are associated with malocclusion, dish face deformity, enopthalmos, diplopia, cerebrospinal fluid leak and soft tissue injuries. Purpose. The purpose of this paper is to present a case of minimizing surgical wound and morbidity. Case. A 40 year old female presented with severe maxillofacial injuries caused by motor vehicle collisions about 5 days prior to admission. The assessment of the patient is mild head injury, panfacial fractures, lacerated wound at face,  rupture of globe of occular sinistra. An open reduction and internal fixation  (ORIF) and enucleation of globe occular sinistra was performed.  Intraoral vestibular incision is made in the upper and lower vestibular region. Mucoperiosteal flap elevation of vestibular will exposure of the anterior maxilla and mandibular fractures. Intermaksilary fixation within 3 week and restore aesthetic with prosthesis fitting eyeball and denture. Discusion. The goal of  treatment of  panfacial fracture is to restore both the functions and pre-injury 3-dimensional facial contours. To achieve this goal two common  sequences of management of Panfacial fractures are proposed, “Bottom up and inside out” or “Top down and outside in”. Other sequences exist but there are variations of these two major approaches. Conclusion. A minimally invasive approach to  the fracture site is an alternative method  to manage panfacial fracture with a simple, effective and lower complication rate.

Abstract

Introduction. Odontomas are the most common type of odontogenic tumors and generally they are asymptomatic. These tumors are formed from enamel and dentin, and can have variable amounts of cement and pulp tissues. According to radiographic, microscopic, and clinical features, two types of odontomas are recognized: Complex and compound odontomas. Complex odontomas occur mostly in the posterior part of the mandible and compound odontomas in the anterior maxilla. Case Report. A young girl patient, 9 years old came to Department of Oral and Maxillofacial Surgery with a slow growing and asymptomatic swelling in her left posterior mandible for 5 years in his history taking. The panoramic radiograph show  a radioopacity and radiolucent lesion at the lower second molar region, with well-corticated limits. An insisional biopsi   confirmed  as compound odontoma. The surgery  performed with simple enucleation and curettage under general anaesthesia. Discussion. Compound odontomas are usually located in the anterior maxilla, over the crowns of unerupted teeth, or between the roots of erupted teeth. In this case report, Compound odontomas are found in the posterior mandible. Conclusion. Compound odontomas in the posterior mandible is a rare. The treatment of odontomas depends on the size of the lesion. The early diagnosis, the treatment of choice is conservative surgical enucleation and curettage and prognosis is excellent.

Abstract

Deoxyribonucleic acid (DNA) is a genetic material which is found in all living organisms. On the human cell or eukaryotes cell, the DNA is found in the nucleus cell and the mitochondria. The DNA arrangement on each cell in human body is the same, that is why, for the analysis meaning, DNA can be isolated from any cell in the body. The source of DNA to be analyzed usually coming from the blood sample by an injection method, such a way resulting in pain and bringing about constraint. Therefore, a study was carried out to look for an alternative of DNA isolation. The aim of this experimental study was to get an optimal DNA isolation method by using oral mucous smear method with a purpose to get a quick and easy DNA isolation. The investigation materials were in the form of samples which were taken from the oral epithelial mucous cells out of three different subjects. The epithelial cells were obtained by the oral mucous smear method which in a variation of two, four and six times of smear applications, respectively. The DNA was then isolated using buffer extraction method. The concentrations of DNA were measured by using ultraviolet spectrophotometer at 260 nm wavelength. The results of DNA isolation were analyzed by Polymerase Chain Reaction (PCR) technique. The optimal DNA isolation could be analyzed by PCR technique. The experimental results show that from three different subjects of study, DNA can be isolated optimally by oral mucous smear method with six times of smear applications.

Abstract

One of the most difficult cases to be managed in dentistry is an odontogenic infection. The study was aimed to finding the description of the odontogenic infection cases at the Oral Surgery Polyclinic of Hasan Sadikin Hospital, Bandung, Indonesia, according to the odontogenic infection types, genders, ages, sources of infection, and type of treatment given. This study was a retrospective-descriptive study with a survey method conducted from January 2009 to December 2010. The results of this study showed that there were total 8 types of odontogenic infection cases. The periapical abscess was the most frequent abscess found in 89 patients (30.6%). Male patients were having more often cases than women with the ratio of 53:47. The most frequent age group was the 21-30 years old age group in 57 patients (22.7%). The highest number of patients from 8 different cases of odontogenic infection (30.2%). The source of odontogenic infection was found most commonly by the mandibular first molar teeth, with as much as 26.6%.

Abstract

Celah bibir dan langit-langit merupakan kelainan kongenital kedua terbanyak di Indonesia. Perawatan kasus ini adalah koreksi bedah, dalam beberapa tahap kehidupan pasien. Tujuan: Ditujukan untuk mempelajari karakteristik kasus celah bibir dan langit-langit di Cleft Center RS ??Gigi Universitas Padjadjaran Metode: Dalam kohort retrospektif, semua pasien yang terdaftar di Cleft Center (Januari 2018 – Desember 2019) ditinjau. Data yang dikumpulkan terdiri dari umur, jenis kelamin, diagnosis, dan sisi celah. Sebanyak 613 pasien ditinjau, dengan pengecualian 13 kasus karena data yang tidak lengkap. Hasil: Celah bibir dan langit-langit (38%) paling banyak ditemukan, diikuti celah langit-langit (35%), dan celah bibir (27%). Pasien laki-laki (58%) lebih sering diamati dibandingkan pasien perempuan (42%). Sumbing paling banyak ditemukan di sisi kiri (70%) daripada di sisi kanan (30%). Kesimpulan: Celah bibir dan langit-langit adalah kasus yang paling banyak diamati pada penelitian ini. Pasien laki-laki lebih terpengaruh. Celah ditemukan sebagian besar di sisi kiri.

Abstract

Left untreated or not properly managed odontogenic infection can cause spreading into facial spaces. Severe oral and maxillofacial infection can spread systemically and cause sepsis.1 Sepsis can induce unfavorable condition for the kidney, which is Acute Kidney Injury (AKI).2,3 The aim of this paper is to report a rare case of an oral and maxillofacial  infection with sepsis-induced AKI and upper airway obstruction which have high mortality rate. The standard treatment consists of proper monitoring of vital function, fluid resuscitation, drainage of accumulated purulence, empirical use of antibiotics chosen to cover the spectrum of potential pathogens until culture results are available, and supplemental oxygen.1,4 This is a case report of  a 48 year old female patient with oral and maxillofacial infection suffers sepsis-induced AKI and upper airway  obstruction. This patient was diagnosed as right submandibular abscess  which has extended into right buccal space, with sepsis, stage I AKI, and upper airway obstruction due to retropharyngeal abscess. We managed this patient with oxygenation, fluid resuscitation, antibiotics and analgesic administration, incision and drainage, and also extraction of mandibular right third molar. Tracheostomy, incision and drainage of retropharyngeal abscess were supposed to be performed by ENT department, but the patient refused. However the patient still survived. In case of oral and maxillofacial infection, the involvement of distant organ should be investigated, because this conditions result in high mortality rate. Early diagnosis and prompt treatment are important and able to lead to a better survival.

Abstract

Odontogenic myxofibroma is a rare, benign, but locally aggressive neoplasm found exclusively in the jaws. It has a predilection to occurs in the 2nd–3rd decade. The lesion often grows without symptoms and presents as a painless swelling. Most common clinical variant is associated with the impacted tooth and shows local invasion. The surgical treatment of these tumors consists of complete enucleation or radical excision. 15th year old girl with a slow growing, painless swelling in the left mandibular region since 2 years. Results of a radiological examination revealed a multilocular radiolucency extending from first molar up to coronoid process with impacted 3rd molar. Biopsy was done, and the histopathological examination showed a myxofibroma. Hemimandibulectomy was done and the tumor mass was resected along with it. Then the mandibular was reconstructed with AO plate. In this case hemimandibulectomy was done due to high rate of recurrence, surgical treatment through bone resection is the most indicated treatment modality and the patient must be followed‑up closely for years. AO Plate for mandibular reconstruction was considered because it was safe, rapid, easy and tissue tolerance is good. The neoplasm diagnosed on the basis of radiographic imaging modalities and histopathological examination.

Abstract

Abses di spasia submandibula paling sering ditemukan. Infeksi ini dapat menyebar secara unilateral atau bilateral ke ruang di sekitar leher bagian dalam dan dapat berakibat fatal atau mengancam jiwa, biasanya ditandai dengan pembengkakan. Kasus ini jarang terjadi dengan gambaran klinis yang disertai infeksi sekunder. Pada kasus ini, laki-laki usia 62 tahun datang dengan keluhan pembengkakan pada rahang bawah kiri, demam, nyeri di sekitar wajah, disertai mata kiri, hidung, dan bibir atas melepuh. Pada hasil pemeriksaan darah lengkap tidak ditemukan ada kelainan kecuali nilai SGOT sedikit rendah. Pada kasus ini dilakukan tindakan pencabutan gigi sebagai fokus infeksi, pemberian antibiotik dan antivirus baik secara oral maupun topical. Penyakit virus pada rongga mulut adalah jenis patologi menular yang mempengaruhi jaringan mulut. Pemeriksaan klinis dan pemeriksaan penunjang padakasus ini penting untuk menentukan perawatan yang tepat. Prognosis kasus ini sangat baik. Terlihat berkurangnya pembengkakan disertai lesi vesikel yang minimal. Penanganan yang tepat mengurangi resiko penyebaran infeksi yang dapat menyebabkan sepsis dan kegagalan organ.

Abstract

Speech quality is an essential output in assessing the success of a palatoplasty. The goal of a palatoplasty is not merely to create a simple anatomical closure of the palate, but also to create an adequate velopharyngeal mechanism for a normal speech outcome and to prevent abnormal maxillofacial development after surgery. The aim of this study is to find out the difference in speech outcome between post-cleft palate closure patients and patients without cleft palate. An analytical retrospective study was conducted on 22 children (n = 22) with complete unilateral cleft palate, who had been treated using two flap push back technic of palatoplasty during 2014-2017 by purposive sampling method, and 22 children without cleft palate as the control group. The evaluation of speech outcome was done using an assessment of perception by doing a speech pathologist and instrumental examination by taking a lateral cephalometry radiograph. The perception was assessed by the articulation pattern, hypernasality, and speech intelligibility. The lateral cephalometry radiograph was taken at /i/ phonation to measure the distance velum to the posterior pharynx wall. Data were analyzed using Mann Whitney test. The velopharyngeal competence in post-palatoplasty group consisted of 22.8% adequate result, 0.1% marginal result, and 68.1% inadequate result. Meanwhile, in the control group, there were 72.7% adequate and 27.3% inadequate competence. According to the result of the statistical test, this study concluded that there was a significant difference in speech outcome based on articulation pattern, hypernasality, speech intelligibility, and velopharyngeal distance between post-cleft palate closure patients and patients without cleft palate (p < 0.05). Majority of patients after cleft palate closure with two flaps pushback technique had inadequate velopharyngeal competence with moderate-severe hypernasality, severe nasal emission, abnormal speech intelligibility, and velopharyngeal distance ≥ 5.0 mm, whereas the majority of control group had an adequate velopharyngeal competence.

Abstract

Facial soft tissue trauma is a common case in the emergency department. Handling this case requires special attention because the face is a prominent part and a marker of identity. Thalassemia is a blood disorder that causes the sufferer to experience anemia, where thalassemia patients must undergo blood transfusions for life. This case will describe a 14-year-old girl who came to the Emergency Department of Hasan Sadikin Hospital Bandung with complaints of bleeding on the eyelids due to a motorcycle accident, from the patient's history it is known that the patient is a thalassemia patient who routinely undergoes transfusions every month. As a result of this hemorrhage caused a significant decrease in hemoglobin value in the patient, so that emergency management was carried out, namely suturing the patient's traumatized eyelid and blood transfusion. Emergency management of facial soft tissue trauma in children with thalassemia must be done carefully to control bleeding and restore the function and aesthetics of traumatized facial tissues, besides that we also collaborate with pediatric colleagues for the management of anemia due to thalassemia in this patient.
