Abstract

Mandibular fracture is a discontinuity of mandible bone that usually leads to trauma. The fractured area is mostly the mandibular angle located in the third molar area. Therefore, this study aims to examine the incidence of mandibular angle fractures accompanied by impacted teeth in the oral surgery clinic of Hasan Sadikin Hospital. This is a retrospective observational study and the data used were collected from the medical records of patients with mandibular angle fractures accompanied by impacted third molars at the Oral Surgery Clinic of Dr. Hasan Sadikin Bandung from January 2017 to December 2019. Panoramic radiographs were obtained for confirmation and the data collected were age, gender, fracture aetiology and location, impacted tooth type and classification, as well as treatment. The number of mandibular angle fractures with impacted third molars in male patients (92.8%) was more than in females (7.2%). Fractures caused by traffic accidents and fights were 85.8% and 14.2%, respectively, while all patients were treated with ORIF. The percentage of cases according to the classification of third molars in angle fractures are classes IA (20%), IB (6.7%), IIA (20%), IIB (20%), IIC (13.3%), IIIC (6.7%), and unerupted tooth seeds (13.3%).The incidence of mandibular angle fracture with impacted third molars at the Oral Surgery Clinic, Dr. Hasan Sadikin over the last three years has increased. This case is more common in males due to traffic accidents and is treated with surgery (ORIF).

Abstract

Pendahuluan: Fraktur mandibula merupakan fraktur pada tulang wajah yang paling sering terjadi. Fraktur ini dapat terjadi akibat kecelakaan lalu lintas, olah raga, bermain, perkelahian, kegiatan profesi ,atau bahkan saat latihan peperangan. Berbagai metode perawatan telah dilakukan untuk mempercepat penyembuhan fraktur mandibula. Pemberian stem cells saat ini  merupakan salah satu alternatif perawatan pada fraktur karena stem cells mempunyai kemampuan self-renewal dan diferensiasi yang baik. Penelitian ini bertujuan untuk menganalisis literatur mengenai peranan pemberian stem cells terhadap percepatan regenerasi tulang pada fraktur mandibula. Metode: Rapid review dilakukan pada bulan Februari 2021 hingga April 2021. Pencarian artikel mengacu pada pedoman analisis PRISMA dengan menggunakan  strategi PICO melalui database PubMed, Sciences Direct, dan Google Scholar dengan kriteria inklusi berbahasa inggris dari tahun 2011 hingga 2021 serta membahas peranan pemberian stem cells terhadap percepatan regenerasi tulang pada fraktur mandibula. Artikel diseleksi berdasarkan duplikasi, judul dan abstrak, serta keseluruhan isi artikel. Hasil: Dihasilkan 4 artikel penelitian yang menyatakan bahwa kelompok yang diberi stem cells memiliki durasi penyembuhan dan kemampuan penyatuan tulang yang lebih cepat. Kesimpulan: penelitian literatur ini menunjukkan bahwa stem cells berperan dalam proses regenerasi tulang yang terlihat dari terbentuknya kepadatan tulang serta penyembuhan fraktur lebih cepat.Kata Kunci: Stem cells, Regenerasi Tulang, Fraktur mandibula

Abstract

Introduction: The lingual frenulum is an important oral structure that plays an important role in the act of suction, speech, and feeding. Ankyloglossia (tongue tie) is a congenital anomaly that results when lingual frenulum is too short and adhere to tip of tongue, limiting the tongue movement. This case report aimed to explain management of Ankyloglossia with different severity. Case Report: This case report reported three cases of ankyloglossia with different ages and complaints of difficulty in feeding and speech. The patients were treated for lingual frenectomy procedure under general anesthesia using one haemostats method. The haemostats were used to clamp the upper aspect of frenulum which might have been helpful to guide the incision close to ventral surface of tongue. Care had to be taken not to injure the submandibular ducts when making incision at lower part of tongue. The result after 1 week of surgery: the wound was completely healed. Conclusion: Ankyloglossia will impair the function of suction, speech and feeding that affecting a considerable number of infants and children. It is important that accurate information and guidance are given to parents with regard to the indications and potential benefit of tongue tie revision. These case series offer guidelines which can be used by general and pediatric dentist for examination, diagnosis and treatment of ankyloglossia in different severity.KeywordAnkyloglossia, congenital, lingual frenulum

Abstract

AbstractIntroduction: Oro-maxillofacial traumas comprise injuries to the face, jaws, or both; they include injury to any of the bony structures, skin, and soft tissues around the face. During the COVID-19 pandemic, human activity was mostly at home, with movement restrictions. The purpose of this study is to find out the types of characteristics and management of oro-maxillofacial traumas during the COVID-19 pandemic at the emergency room. Methods: A cross-sectional study was conducted to evaluate every maxillofacial trauma in patients treated by the oral surgeon in the emergency room at RSUP Dr. Hasan Sadikin Bandung Hospital. Incidence records were taken directly from case reports from January 2020 to December 2021. The variables considered were patient age, gender, etiologies, sites, classifications, and management of the trauma. Then integrated into a table to be analyzed epidemiologically. Results: There were 177 maxillofacial trauma patients registered in the Department of Oral and Maxillofacial Surgery. It was found that the gender ratio, female to male, is equal to 2.3:7.6. The 18-36 age group accounted for the largest subgroup in both sexes. Motorcycle accidents accounted for as much as 80.22% of the etiology of the most common traumas. While dentoalveolar (28.22%) was the most common site of the fracture, followed by the mandible (23.31%). Injuries around the mouth made up the largest portion (62.15%) of the areas that were directly impacted by the injuries, followed by injuries to the face (18.64%) and injuries to the mouth and nose (6.21%). Among the 177 patients mentioned above, 10.17% were hospitalized. Conclusion: Male’s are more likely to be involved in traffic accidents, mainly motorcycle crashes, which are the primary cause of traumas. Dentoalveolar fractures were the most common type of fracture, and the management of the oro-maxillofacial trauma is done according to the standard procedure.

Abstract

ABSTRACTIntroduction: Odontectomy is a surgical procedure to remove an impacted tooth. Some cases of odontectomy are performed under general anesthesia because it has risks and complications that need to be considered, such as a patient with mental or physical disorder, difficulty level of the impacted tooth, number of impacted teeth extracted, and operative duration. This study aimed to determine the distribution frequency of post-odontectomy complications under general anesthesia in Universitas Padjadjaran Dental and Oral Hospital (RSGM Unpad), Bandung. Methods: This study was a cross-sectional descriptive study using retrospective data of medical records of a patient who underwent odontectomy under general anesthesia from 2017 until 2018 in RSGM Unpad. Samples were taken using a purposive non-probability sampling technique by setting inclusion criteria such as undamaged medical records and providing patient data on the first and a week control visit. The variables, including age, gender, classification of impacted teeth, number of teeth, and complications, were assessed in this study. The minimum sample size was determined by using the Slovin formula. The minimum sample required is 94%- 95% confidence level and 5% margin of error. Results: The data that had been collected shows that the number of female patients (67%), male patients (37%), the most common age was 21-30 years (52%), and odontectomy in 4 third molars (73%). Odontectomy that performed on four third molars (73%) with the ordinary post odontectocmy complications found in the female patient, such as trismus (2%), prolonged pain (7%), edema (3%), paraesthesia (3%). Conclusion: Distribution frequency of post-odontectomy complications under general anesthesia in RSGM Universitas Padjadjaran Banding is relatively low.Keywords: frequency; odontectomy; general anesthesia; complications; impacted tooth

Abstract

ABSTRAKPendahuluan: Infeksi odontogen menjadi salah satu penyakit umum yang paling banyak ditemukan di rongga mulut dan maksilofasial. Infeksi odontogen merupakan keadaan darurat yang harus segera ditangani oleh dokter gigi karena akan menimbulkan permasalahan lain secara signifikan. Dokter gigi adalah praktisi medis yang khusus mendiagnosis dan memberikan perawatan pada gigi, mulut, dan maksilofasial di sekitarnya. Pengetahuan yang memadai dibutuhkan dokter gigi untuk menegakkan diagnosis dalam melakukan perawatan darurat infeksi odontogen. Tujuan penelitian mengukur tingkat pengetahuan dokter gigi mengenai perawatan darurat infeksi odontogen. Metode: Jenis penelitian deskriptif kuantitatif dengan teknik pengambilan sampel cluster sampling. Penelitian dilakukan pada 45 dokter gigi umum dan 47 dokter gigi spesialis yang bekerja di rumah sakit Kota Bandung menggunakan instrumen berupa kuesioner dengan 15 butir pertanyaan. Hasil: Sebanyak 27 responden (29,3%) berada pada kategori cukup dan 65 responden (70,7%) berada pada kategori baik. Rata-rata dokter gigi menjumpai kasus infeksi odontogen setidaknya 1x dalam seminggu baik sebelum pandemi 2019, maupun dalam tiga bulan terakhir dengan persentase 38% dan 39,1% Simpulan: Tingkat pengetahuan dokter gigi di rumah sakit Kota Bandung mengenai perawatan darurat infeksi odontogen berada dalam kategori baik. Pengetahuan mengenai manajemen perawatan infeksi odontogen pada kuesioner tersebut memastikan kasus-kasus dapat ditangani secara efektif.KATA KUNCI: pengetahuan, perawatan darurat, infeksi, odontogen, rumah sakitThe level of Dentist’s knowledge about emergency management of odontogenic infection: quantitative descriptiveABSTRACTIntroduction: Odontogenic infections have been one of the most common diseases in the oral and maxillofacial region. Odontogenic infection is an emergency that must be treated by a dentist immediately because it will cause significant problems. A dentist is a medical practitioner who specializes in the diagnosis and treatment of problem that affect teeth, mouth, and the surrounding maxillofacial structure. Knowledge is needed by dentist to confirm the diagnosis in performing emergency of odontogenic treatment. The purpose of this study was to measure the level of dentists’ knowledge in Bandung hospitals about emergency treatment of odontogenic infections. Methods: This research used a quantitative descriptive method with cluster sampling technique. The study was conducted on 45 general practitioners and 47 specialists who work in Bandung hospitals using an instrument questionnaire with 15 questions. Results:  Total of 27 respondents (29.3%) were in the sufficient category and 65 respondents (70,7%) were in a good category. Dentist average encountered cases of odontogenic infection at least once a week, both before the 2019 pandemic and in the last three months, with percentages of 38% and 39,1% respectively. Conclusion: The level of dentists’ knowledge in the city of Bandung hospitals about emergency treatment of odontogenic infections is in a good category. The description of knowledge about management treatment of odontogenic infections in this questionnaire ensures that cases can be handled effectively.KEY WORDS: knowledge, emergency, treatment, odontogenic, infections, hospital

Abstract

Pengantar : Berdasarkan penelitian sebelumnya, penyebab terbanyak cedera kepala dan fraktur maksilofasial adalah akibat kecelakaan kendaraan bermotor. Tujuan penelitian ini adalah untuk menganalisis kejadian fraktur maksilofasial di IGD Rumah Sakit Hasan Sadikin Bandung, Indonesia. Metode: Penelitian deskriptif data cross sectional ini dilakukan pada pasien di IGD RS Hasan Sadikin Bandung periode antara September 2017 - Februari 2018 dengan kriteria inklusi adalah adanya fraktur maksilofasial akibat kecelakaan kendaraan bermotor. Survei primer dan sekunder dilakukan berdasarkan kronologi trauma dan mengidentifikasi lokasi fraktur maksilofasial dan luka di wajah. Data formulir diisi oleh peneliti sesuai petunjuk. Data dianalisis dengan metode cross sectional untuk observasi. Sistem penilaian tingkat keparahan trauma maksilofasial dilakukan dengan menggunakan Facial Injury Severity Scale (FISS) dan Maxillofacial Injury Severity Score (MFISS). Hasil: Data tentang insidensi fraktur maksilofasial dengan cedera kepala akibat kecelakaan kendaraan bermotor pada 100 pasien didapatkan 16 kasus fraktur sepertiga atas maksilofasial, 73 kasus fraktur sepertiga tengah dan 53 kasus fraktur sepertiga bawah.Kesimpulan: Fraktur sepertiga tengah maksilofasial akibat kecelakaan kendaraan bermotor merupakan insidensi trauma yang paling banyak. Hasil penelitian menunjukkan penyebab terbanyak trauma maksilofasial dan cedera kepala pada pasien di Hasan Sadikin Bandung Propinsi Jawa Barat adalah kecelakaan kendaraan sepeda motor.

Abstract

AbstractIntroduction: Necrotizing fasciitis is a fast-paced infection, severely damaging tissue. Surgical removal of tissue must be performed promptly, and aggressively to remove all involved soft tissue. The wound left after debridement necrotomy is wide enough to close primarily and should be treated by suitable dressings to accelerate wound healing. Objective: This case report aims to understand the concept of moist wound healing, the choice of dressing material, and the principles of optimal wound intervention for the wound healing process. Case: A 58 years old female patient presented with a complaint of swelling of the right lower jaw extended to the chin region. About a month earlier the patient complained of a lower right toothache, the patient went to the public health center, the lower right tooth was extracted but there was a residual root left behind. Then the patient still felt pain in the area of the extraction and the swelling in the lower jaw got bigger, the fragility of the superficial fascia, and the absence of pus. Then the patient was referred to the emergency room at Hasan Sadikin Hospital in Bandung. Case management: The patient was performed necrotomy debridement and tooth extraction causing the infection in local anesthesia, then the wound was treated with modern dressings. After 1 month of the procedure, the wound was closed properly. Conclusion: Modern dressings are wound care using the principle of balanced moisture (moisture balance). Choosing the right dressing material is a key concept to support the wound healing process.

Abstract

Introduction: Maxillofacial fracture is a condition of the facial bones discontinuity in the mandible, maxilla, nasal, zygoma, palate, frontal and orbital. Treatment method of maxillofacial fractures is a close reduction or open reduction by internal fixation using titanium plates. This study was aimed to describe the fixation of maxillofacial fractures using titanium plates and also, to determine the further use of titanium plates as a fixation device at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry Universitas Padjadjaran and Hasan Sadikin Hospital. Methods: The research was a descriptive retrospective study with accidental sampling technique on the maxillofacial fracture fixation cases using titanium plates at the Department of Oral and Maxillofacial Surgery Hasan Sadikin Hospital. The samples were 135 medical records diagnosed with maxillofacial fractures. Results: The result shows that the use of titanium plates were 82.24% of the total maxillofacial fracture cases. Conclusion: There are 135 patients with a diagnosis of maxillofacial fractures treated at the Oral and Maxillofacial Surgery, Faculty of Dentistry Universitas Padjadjaran and Hasan Sadikin Hospital in 2006-2010. Most cases were treated with open reduction (108 cases) and fixed by using a titanium plate (88 cases) as well as the combination of a titanium plate with a wire (12 cases).

Abstract

Introduction: Taro (Colocasia esculenta (L.) Schott) leaves have been reported to possess wound healing, anti-inflammatory, antibacterial and antioxidant activities. One of the factors that affect wound healing is infection in the wound, wherein the wound is infected by  bacteria, especially Staphylococcus aureus that is mostly found in the oral cavity, the wound healing process will be hampered and become longer healed. The objective of this research was to analyse the efficacy of Colocasia esculenta (L.) Schott extract on the healing process of wounds contaminated with Staphylococcus aureus. Methods: This experimental study on Sprague dawley rats was carried out at the Veterinary Medical Teaching Hospital of the Faculty of Veterinary Medicine of IPB University. Circular excision wounds were applied on 24 Sprague dawley rats with a diameter of 2 cm on the dorsum, and Staphylococcus aureus specimens were contaminated with a dose of 3.4x108 LAC. Rats were randomly divided into 2 groups. Group I (control group) was given a placebo, and group II (treatment group) was given a 25% concentration of Colocasia esculenta (L.) Schott extract on the wound once a day. Four rats from each group were euthanized on the 3rd, 7th, and 14th days. The number of neutrophils, macrophages, re-epithelialization and FGF-2 expression were measured by microscopic observation with visual field control, and comparative data were analysed using the Kruskal-Wallis statistical test. Results: The number of neutrophils on the treatment group on the 7th day was less than the control group (p=0.040). The number of macrophages on the 3rd day was found more in the treatment group than the control group (p=0.032), and on the 14th day, the treatment group was less than the control group (p=0.040). Epithelial cells on the 14th day of the treatment group was found more than the control group (p=.0.017). The level of FGF-2 expression of the treatment group on the 7th day was higher than the control group (p=0.044). Conclusion: Application of Colocasia esculenta (L.) Schott leaf extract is efficacious for healing wounds contaminated with Staphylococcus aureus in the proliferative phase.
