Abstract

Celah bibir dan langit-langit merupakan kelainan kongenital kedua terbanyak di Indonesia. Perawatan kasus ini adalah koreksi bedah, dalam beberapa tahap kehidupan pasien. Tujuan: Ditujukan untuk mempelajari karakteristik kasus celah bibir dan langit-langit di Cleft Center RS ??Gigi Universitas Padjadjaran Metode: Dalam kohort retrospektif, semua pasien yang terdaftar di Cleft Center (Januari 2018 – Desember 2019) ditinjau. Data yang dikumpulkan terdiri dari umur, jenis kelamin, diagnosis, dan sisi celah. Sebanyak 613 pasien ditinjau, dengan pengecualian 13 kasus karena data yang tidak lengkap. Hasil: Celah bibir dan langit-langit (38%) paling banyak ditemukan, diikuti celah langit-langit (35%), dan celah bibir (27%). Pasien laki-laki (58%) lebih sering diamati dibandingkan pasien perempuan (42%). Sumbing paling banyak ditemukan di sisi kiri (70%) daripada di sisi kanan (30%). Kesimpulan: Celah bibir dan langit-langit adalah kasus yang paling banyak diamati pada penelitian ini. Pasien laki-laki lebih terpengaruh. Celah ditemukan sebagian besar di sisi kiri.

Abstract

Background: The maxillary canine have a key role in the aesthetics and continuity of the dental arch. Impacted canines are the most frequently impacted teeth apart from the third molars of both the maxilla and mandible. The purpose of this study was to determine and obtain a description of the impact profile of maxillary canines at RSGM UNPAD based on age, sex, classification, type of treatment action plan.Method: This research was a retrospective descriptive study conducted from February to March 2021 based on inclusion and exclusion criteria. The research sample was taken using purposive sampling technique and determination of the impacted image using a classification based on Stivaros Mandal, Ghenoima, and Yamammoto, analyzed with ImageJ software by interobserver and intraobserver. and tested using Kappa statistics for reliability.Result: There were 76 patients who had cases of maxillary canine impaction with an age prevalence range of 10-25 years and the majority occurred in women (42.56%) and men (15.20%). Analysis of the Stivaros and Mandall classification of 59.40% grade III canine impacted patients, the greatest prevalence of the Ghenoima classification in Type E 27.00% and the Yamammoto classification of the largest prevalence in Type II 41.40%. The results of the analysis of the action plan for the treatment of patients with exposure surgery had a percentage of 0.03% and Odontectomy 0.03%.Conclusion: The majority of maxillary canine impacted prevalence occurred at the age of 10-25 years with grade III angulation, mesio-angular position between the anterior-inferior maxillary sinus.

Abstract

ABSTRAKPendahuluan: Fraktur kondilus dan Korpus mandibula merupakan fraktur pada mandibula yang cukup sering terjadi pasca trauma. Komplikasi yang sering terjadi akibat fraktur kondilus dan korpus mandibula pada anak adalah ankilosis dan gangguan sendi sendi temporomandibula. Perawatan fraktur mandibula dapat dengan reduksi terbuka atau reduksi tertutup. Penanganan fraktur secara sederhana dan teknik yang tepat dapat mencegah komplikasi lebih lanjut. Tujuan laporan kasus ini memaparkan dan membahas penatalaksanaan kegawatdaruratan pada pasien remaja yang menderita fraktur kondilus dan korpus mandibula dengan reduksi tertutup. Laporan kasus: Pasien perempuan usia 16 tahun datang ke Rumah Sakit Gigi dan Mulut Unpad dengan keluhan perdarahan dari mulut dan sulit membuka mulut akibat kecelakaan lalu-lintas. Pemeriksaan klinis dan radiografis menunjukan adanya fraktur kondilus bilateral dan fraktur korpus mandibula. Pasien dilakukan pembersihan dan penjahitan luka, perawatan fraktur kondilus dengan reduksi tertutup dan fiksasi maksilomandibular dengan arch bar. Kontrol pada minggu ke 12 menunjukkan hasil perbaikan. Simpulan: Penatalaksanaan fraktur kondilus dan Korpus mandibula dengan reduksi tertutup yang tepat menunjukkan hasil yang memuaskan secara anantomi dan fungsi pada saat penatalaksaan kedaruratan trauma.Kata kunci: fraktur kondilus bilateral; fraktur korpus mandibula; reduksi tertutup; fiksasi maksilomandibular ABSTRACTIntroduction: Condyle and mandibular corpus fractures are quite common post-trauma mandibular fractures. Complications that often occur due to condyle and mandibular corpus fractures in children are ankylosis and temporomandibular joint disorders. Mandibular fractures can be treated with open reduction or closed reduction. Simple fracture management and proper technique can prevent further complications. This case report aimed to present and analyse the emergency management of condyle and mandibular corpus fractures in an adolescent patient with closed reduction. Case report: A 16-year-old female patient came to Universitas Padjadjaran Dental Hospital with complaints of oral bleeding and mouth-opening difficulty due to a traffic accident. Clinical and radiographic examination revealed bilateral condylar and mandibular corpus fractures. The patient was cleaned and sutured, treated for condylar fractures with closed reduction and maxillomandibular fixation with arch bars. The improvement showed at the control visit after 12 weeks. Conclusion: Management of condylar and mandibular corpus fractures with properly closed reduction showed satisfactory anatomical and functional results.Keywords: bilateral condyle fracture; mandibular corpus fracture; closed reduction; maxillomandibular fixation

Abstract

Background: Tooth extraction is one of minor oral surgical procedure and antibiotics are commonly prescribed before or after the tooth extraction procedure. Antibiotic is used as therapy or to prevent infections after tooth extractions. Recent studies discovered that misuse of antibiotics by dentists has been an issue nowadays. The purpose of this study is to investigate the frequency and varieties of antibiotics post tooth extraction used by dentists in Bandung City.Method: This study uses a quantitative descriptive method with a cross-sectional approach using a questionnaire. The samples in this study are general dentists and specialists in Bandung. The questionnaire was sent to dentists using proportionate stratified random sampling. Result: The most frequently used antibiotics in Bandung City are Amoxicillin 500 mg (83,1%) and Clindamycin (24,3%). Most respondents prescribed antibiotics after tooth extractions for 5 days.Conclusion: A small percentage of Dentists in Bandung City still prescribed antibiotics in conditions that do not always require antibiotics, such as simple post-extraction, irreversible pulpitis, and reversible pulpitis. Broad-spectrum antibiotics are more commonly prescribed to prevent infections. Furthermore, antibiotic usage after simple tooth extraction needs to be done by considering the patient’s condition, diagnosis, and difficulty level of extraction.

Abstract

Introduction: Necrotizing fasciitis is an infection with very rapid progress affected subcutaneous tissue, spread to the fascia lead to subcutaneous blood vessels thrombosis. Hence, caused necrosis of skin. Frequently polymicrobial, caused by β-haemolytic bacteria such as Streptococcus, Staphylococcus spp. and obligate anaerobes. Necrotizing fasciitis due to fungal infection is a rare case, only five cases were recently reported where Candida albicans was identified as necrotizing fasciitis microorganism Case Report: A 51 years old male patient was referred to Emergency Room at Hasan Sadikin General Hospital, Bandung complaints of swelling accompanied by pain in the left cheek extended to eye and neck, with clinical features shown necrosis of the skin, fluctuations and crepitations. The patient was diagnosed as necrotizing fasciitis. The patient has a history of systemic disease which is diabetes mellitus type 2 with hyponatremia and chronic kidney disease. The patient performed necrotomy debridement and incision drainage at submental, temporal and bukal region in general anaesthesia, then followed by wound care with modern dressings. Bacteria culture shown the growth of C.albicans Conclusion: Necrotizing fasciitis caused by C. albicans is a rare case. Several factors that can increase Candida virulence lead to invasive soft tissue infections, the importance was anti-fungal therapy in necrotizing fasciitis beside broad-spectrum antibiotic therapy of this case. Additionally, postoperative wound care was necessary for encourage wound healing

Abstract

Pendahuluan: Pandemi COVID-19 telah berdampak pada sistem kesehatan di seluruh dunia. Penularan COVID-19 yang terjadi melalui kontak, aerosol, droplet dan fomite membuat dokter gigi termasuk dokter gigi spesialis bedah mulut dan maksilofasial, memiliki resiko tinggi terinfeksi COVID-19. Pemerintah Indonesia mengeluarkan kebijakan PSBB dan PPKM dalam pengendalian penyebaran COVID-19. Hasil beberapa penelitian, peraturan di beberapa negara seperti karantina kewilayahan atau lockdown, mempunyai dampak terhadap kasus bedah mulut dan maksilofasilofasial. Penelitian ini bertujuan memberikan informasi ilmiah mengenai karakteristik kasus bedah mulut dan maksilofasial sebelum dan selama pandemi di RSGM Unpad. Metode: Penelitian ini menggunakan metode studi retrospektif deskriptif menggunakan data sekunder berupa rekam medis pada Maret 2018 hingga Februari 2022 di Poli Paviliun RSGM Unpad. Data yang diambil berupa usia, jenis kelamin, diagnosis, dan tatalaksana. Teknik pengambilan sampel menggunakan simple random sampling dengan pembobotan. Hasil: Jumlah kunjungan sebelum pandemi 264 (45.83%), selama pandemi 312 (54.17%). Jenis kelamin pasien sebelum pandemi diantaranya laki-laki 115 (43.56%), perempuan 149 (56.44%) dan selama pandemi diantaranya laki-laki 119 (38.14%), perempuan 193 (61.86%).  Kategori usia terbanyak sebelum pandemi yaitu dewasa 86 (32.58%) sedangkan selama pandemi yaitu remaja 124 (39.74%). Kasus terbanyak sebelum dan selama pandemi yaitu impaksi gigi (sebelum 40.53%, selama 47.12%). Jenis tatalaksana terbanyak sebelum pandemi yaitu ekstraksi gigi (33.33%), sedangkan selama pandemi yaitu odontektomi (31.41%). Simpulan: Jumlah kunjungan selama pandemi lebih banyak daripada sebelum pandemi. Kasus terbanyak sebelum dan selama pandemi diantaranya impaksi gigi, kontrol pasca operasi, dan abses rongga mulut. Tatalaksana terbanyak sebelum dan selama pandemi diantaranya ekstraksi gigi, odontektomi, dan konsultasi dokter gigi spesialis.
