Abstract

Kalsitonin,  suatu  obat  anti  resorptif  yang  selama  ini  banyak  digunakan  dalam  terapi  osteoporosis,  terbukti  dapat mengurangi turnover tulang subkondral yang meningkat pada osteoarthritis (OA), mengurangi derajat kerusakan tulang rawan sendi pada beberapa model OA dan merubah komposisi biokimia pada matriks tulang rawan sendi. Mekanisme kerja kalsitonin dalam terapi osteoarthritis belum sepenuhnya diketahui. Apoptosis kondrosit dikaitkan dengan deplesi dari  matriks  tulang  rawan  pada  OA.  Penelitian  ini  bertujuan  meneliti efek  Kalsitonin  terhadap  penurunan  indeks  apoptosis kondrosit  pada  sendi  yang  mengalami  gangguan  biomekanik.  Sampel  hewan  coba  kelinci  dibagi  dalam  3  kelompok, kelompok  1  dan  2  dilakukan  perlakuan  pemotongan  ligamentun  cruciatum  anterior  (ACL T)  dan  menisektomi  untuk membuat  instabilitas  sendi  lutut  kiri  dan  setelah  24  jam  perlakuan  diberikan  injeksi  kalsitonin  pada  kelompok  1, sedangkan kelompok 2 diberikan injeksi normal saline selama 6 minggu, kelompok 3 (kontrol) dilakukan  operasi sham. Setelah  6  minggu,  sampel  diambil  dari  tulang  rawan  sendi  lutut  bagian  femur  dan  tibia  untuk  dilakukan  pemeriksaan apoptosis. Gangguan biomekanik instabilitas meningkatkan apoptosis kondrosit dengan bermakna baik pada sisi femur (x= ̅27) maupun tibia (x= ̅32.2) dari sendi dibandingkan kontrol.

Abstract

Air rebusan selama ini masih dipakai sebagai cairan irigasi untuk debridement fraktur terbuka di IRD RSSA Malang, sedangkan cairan irigasi yang direkomendasikan adalah normal saline yang merupakan cairan fisiologis. Alasan memakai air rebusan ini adalah selain harga noemalsaline relatif mahal juga karena belum ada penelitian mengenai kejadian infeksi denganmenggunakan air rebusan tersebut. Tujuan penelitian ini adalah untuk mengevaluasi penggunaan air rebusan sebagai cairan irigasi terhadap risiko terjadinya infeksi dan inflamasi. Metode yang digunakan adalah eksperimental klinis. Pasien dengan fraktur kruris terbuka grade II yang memenuhi kriteria inklusi dibagi menjadi 2 kelompok secara acak. Satu kelompok dilakukan debridement dengan cairan irigasi normal saline dan kelompok yang lain dengan air rebusan. Efek yang dipantau adalah kadar CRP, jumlah lekosit, angka pertumbuhan kuman dan angka reaksi inflamasi pada luka operasi. Hasil yang didapat berupa kecenderungan peningkatan kadar CRP, jumlah lekosit, angka pertumbuhan kuman dan angka inflamasi luka yang pada luka dilakukan debridement dengan air rebusan dibanding dengan yang dilakukan debridement dengan normal saline. Kesimpulan yang didapat adalah air rebusan tidak dapat dipakai sebagai cairan irigasi pada debridement fraktur terbuka karena secara klinis dan laboratories meningkatkan kejadian infeksi dan inflamasi.

Abstract

Objective: The aim of this study was to compare the swelling behaviour of Hand Polyvinyl Acetate/Polyvinyl alcohol (HANP/PVA) and HANP/PVA/Poly Lactic-co-Glycolic Acid (PLGA) composites.Material and Methods: This study was divided into 2 groups, HANP/PVA and HANP/PLGA/PVA composites which prepared by freeze drying methods. HANP 20% (w/w) was combined with PLGA 20% (w/w) and dissolved in PVA solution at 50oC. Swelling behaviour was obtained by measured initial weight (Wi) and the weight of composites after immersing in PBS pH 7.4; 37oC for 7 days (swollen weight (Ws)). Statistical analysis based on Independent T-test (p<0.05).Results: The Wi of HANP/PVA and HANP/PLGA/PVA composites are 467±4.73 and 292±9.52. The Ws of HANP/PVA and HANP/PLGA/PVA composites are 897±26.61 and 860±32.36. Furthermore, the water retention of HANP/PVA composite is 92.07% (p<0.05) and HANP/PLGA/PVA composite is 194.52% (p<0.05).Conclusion: The addition of PLGA enhance swelling behaviour of HANP/PVA composites. The strong interaction between HANP and PVA decrease the swelling re. HANP is inert in nature, but the combination of HANP/PLGA/PVA can be a good substitute for alveolar preservation.

Abstract

Metabolism of the bone healing process can be monitored by Bone formation marker, such as alkaline phosphate (ALP). Simvastatin is known to increase bone marker formation in the repair phase, but its effects on the ALP level have not been known yet. This study aimed to determine the effect of oral simvastatin administration on the expression of ALP level in femur fracture healing in a rat model of femur fracture. The research method was experimental post-test only using 18 Rattus norvegicus rats which were divided into 3 groups, namely the control group for femur fracture (Control), femoral fracture group and giving simvastatin 0.36 mg/day for 2 weeks (Treatment I), and femur fracture group and administration of simvastatin 0.36 mg/day for 4 weeks (Treatment II). ALP levels were measured at 0, 2, and 4 weeks. The results showed an increase in ALP levels in the simvastatin treatment group compared to the control group (p<0.05). It can be concluded that the administration of simvastatin increased ALP levels in rat femoral fractures.

Abstract

Introduction: The peroneus longus (PL) tendon is one of the autograft choices for knee ligament injury because of its safety, length, and strength. However, studies that evaluated donor site morbidity after PL tendon graft are still limited. This case series aims to evaluate the functional outcomes of donor site morbidity after full-thickness PL tendon harvesting for knee ligament reconstruction.Presentation of Case:This study evaluated four patients who had a revision of knee ligament reconstruction surgery using PL tendon autograft from August until September 2018. The functional outcome of the donor site was evaluated before surgery, two weeks, and three months after surgery by using functional scores assessment of foot and ankle.Discussion:There are some donor sites of autograft, but several studies show that the PL tendon autograft has proven to be more favorable methods for knee ligament reconstruction based on the accessibility, safety, and strength of the harvested tendon. The studies also found that using PL tendon as an autograft, has a good result in donor site morbidity evaluation compared to others. This study provides a satisfying result on the functional outcome of the donor site morbidity evaluation.Conclusion:This study showed satisfactory functional outcomes of the foot and ankle after harvesting full-thickness PL tendon. All functional score assessment provides an excellent result in three months after surgery, although lateral ankle bulging has occurred in all patients. However, gait analysis and isokinetic test with a more significant number of patients and longer follow up periods are necessary for further study.

Abstract

Background: In order to ameliorate the burden the coronavirus disease 2019 (COVID-19) pandemic had on the medical resources, and to ensure the safety of orthopaedic patients and medical personnel, some hip and knee arthroplasties, as forms of elective surgeries, were suggested to be canceled or postponed. This review aims to analyze the available literature on the impact of COVID-19 pandemic on all areas surrounding hip and knee arthroplasty service. Materials and Methods: A systematic literature search was conducted in five databases from July 5th to 10th, 2020. We included studies that assessed any impacts the COVID-19 pandemic had on the patients who require hip or knee arthroplasty, hip and knee surgeons, and any other areas that are related to hip and knee arthroplasty service. We excluded studies that do not report complete clinical results, reviews, editorials, and letters.Results: After the screening, a total of nine articles were selected. Two studies evaluated the impact of COVID-19 on the hip and knee arthroplasty patients, one study on public interest in hip and knee arthroplasty, three studies on the hip and knee arthroplasty surgeons, two studies on surgical volume, and one study on industry partners. Of 360 surveyed patients, 60% felt anxious about the uncertainty of the rescheduled arthroplasty. Of 1770 surveyed surgeons, 61.9%, 54.7%, and 20.9% of them reported canceled elective inpatient procedures, outpatient procedures, and all types of surgeries, respectively.Conclusions: COVID-19 pandemic had some impacts on the patients, public interest, surgeons, industry partners, and hip and knee arthroplasty volume.

Abstract

The number of cases handled by PJI is projected to rise in the coming years. The diagnosis of PJI relies on evaluating microbiology, inflammatory response, and pathology. However, the accuracy of the diagnosis is compromised by previous exposure to antimicrobial agents, the possibility of contamination, and the lack of specificity of inflammatory markers. Although new testing methods, such as molecular techniques, hold the potential for a swift diagnosis, they are constrained by the risk of contamination and the absence of susceptibility results. Interestingly, emerging synovial fluid markers exhibit promise as an additional tool in diagnosing PJI. The management of each PJI case, both in terms of surgery and antimicrobial treatment, requires an individualized assessment. It is imperative to conduct high-quality studies that aim to determine the most effective route and duration of antimicrobial treatment for each surgical approach.  This review provides an overview of the diagnostic tests and treatment options for prosthetic joint infection, offering a practical approach to managing this complex clinical condition.
