Abstract

Rheumatoid arthritis is a chronic autoimmune disease characterized by joint inflammation, pain, stiffness, and decreased mobility. Ankle pain is one of the common complaints that significantly affects patients' daily activities and quality of life. Non-pharmacological interventions such as warm water foot soaking therapy are considered effective in reducing pain through vasodilation and muscle relaxation mechanisms. This study aimed to determine the effectiveness of warm water foot soaking in reducing ankle pain in patients with rheumatoid arthritis. This research employed a descriptive case study design involving two patients at RS TK II Putri Hijau Medan. The intervention was conducted for three consecutive days using warm water with a temperature of 31–37°C for 15–20 minutes. Pain intensity was measured using the Numeric Rating Scale (NRS). The results showed a significant decrease in pain intensity from a scale of 5 (moderate pain) to 2 (mild pain) in both patients after the intervention. Patients also reported increased comfort and improved mobility. In conclusion, warm water foot soaking is effective as a non-pharmacological nursing intervention to reduce ankle pain in patients with rheumatoid arthritis. This therapy is simple, safe, cost-effective, and can be applied independently by patients.

Abstract

The quality of primary health care services is greatly influenced by the performance of health workers. Service quality and organizational effectiveness are supported by good employee performance. However, employee performance can be affected by increasing workload and limited organizational resources. Only limited empirical evidence has been provided on the roles of motivational climate and career development in improving job performance among primary health care workers, especially in Indonesia. This study was conducted to examine the effects of motivational climate and career development on the job performance of health workers in community health centers. A cross-sectional quantitative design was employed involving 350 health professionals from 18 community health centers in Dairi Regency, North Sumatra, Indonesia. Data were collected by using structured questionnaires. After the classical assumption tests were completed, the data were analyzed by using multiple linear regression. The findings indicate that both motivational climate and career development have positive and significant effects on job performance (p<0.001). Career development emerged as the strongest predictor (β=0.566), followed by motivational climate (β=0.279). It was found that 64.4% of the variance in job performance was explained by the regression model. Strong explanatory power was shown by the model. It is shown by these findings that stronger career development systems and a supportive motivational climate should be developed. These are considered important strategies for improving the performance of health workers in primary health care settings. Keywords: Motivational Climate, Career Development, Job Performance,  Primary Health Care, Workers

Abstract

Diabetes mellitus is a chronic disease that occurs when the pancreas is unable to produce sufficient insulin or when the body cannot effectively use the insulin produced, resulting in increased blood glucose concentration (hyperglycemia). One non- pharmacological nursing intervention that can be used to reduce blood glucose levels in patients with diabetes mellitus is a 30-minute walking activity. This study aims to describe the implementation of a 30-minute walking intervention to reduce blood glucose levels in patients with diabetes mellitus at TK II Putri Hijau Hospital, Medan. This research used a descriptive case study method with a nursing care approach including assessment, diagnosis, intervention, implementation, and evaluation. The study was conducted in April 2025 on two patients with diabetes mellitus. The results showed that after implementing the 30-minute walking intervention, both patients experienced a decrease in blood glucose levels: in Patient 1. from 245 mg/dL to 134 mg/dL; and in Patient 2, from 234 mg/dL to 164 mg/dL. The conclusion from this implementation indicates that a 30-minute walking intervention can be an effective nursing intervention to help reduce blood glucose levels in patients with diabetes mellitus, as demonstrated in both cases

Abstract

Dyslipidemia is a disorder of lipid metabolism characterized by increased levels of total cholesterol, Low Density Lipoprotein (LDL), triglycerides, and decreased High Density Lipoprotein (HDL), which can increase the risk of cardiovascular diseases. The high prevalence of dyslipidemia in the community is caused by unhealthy lifestyles, such as excessive consumption of fatty foods, lack of physical activity, and smoking habits. One non-pharmacological effort that can be applied to reduce blood lipid levels is regular physical activity in the form of aerobic exercise. This study aimed to describe family nursing care for dyslipidemia through aerobic exercise in the working area of UPT Puskesmas Sei Mencirim. The research design used was a case study with a family nursing process approach, including assessment, nursing diagnosis, planning, implementation, and evaluation. The subjects of this study consisted of two families with family members diagnosed with dyslipidemia. The results of the nursing care showed an increase in family knowledge regarding dyslipidemia, positive changes in attitudes toward a healthy lifestyle, and improvement of physical complaints such as neck pain, muscle aches, and discomfort after the regular implementation of aerobic exercise. In addition, families were able to actively participate in decision-making and in providing health care for affected family members. In conclusion, the application of aerobic exercise as part of family nursing care can improve family knowledge and involvement, as well as support the control of dyslipidemia. Therefore, aerobic exercise can be recommended as a promotive and preventive nursing intervention in community health service
