Abstract

Introduction: Breast cancer is a most commonly diagnosed cancer in women with an estimated 1.67 million cases diagnosed in 2012, i.e. 25% of all cancer cases. As many as 69% of breast cancer incidence was found to have reached stage III and IV, which has lower five-years survival rate. Various factors play a role in the delay of treatment, one of them is an herbal usage history. This study aims to determine whether an herbal usage history is a risk factor of delay in breast cancer patients in Surakarta. Methods: This study design was observational analytic with cross sectional approach. The samples were 45 breast cancer patients with delayed treatment and 55 breast cancer patients without delay treatment in RSUD Dr. Moewardi and RSU Kasih Ibu Surakarta. Samples were taken by fixed disease sampling, which then selected based on inclusion and exclusion criteria. The instrument used is the patient records and supplementary questionnaire. The variables studied were delays in treatment with an herbal usage history along with other confounding factors that may affect the delay of treatment. Data then analyzed using bivariate test and logistic regression. Results: The results of the bivariate analysis showed that an herbal usage history is a risk factor of delay in treatment, with p = 0.002 and OR = 3.69. While the results of the multivariate analysis showed that a low level of knowledge affect the most to delays in treatment of breast cancer with OR = 3.699. Conclusions:Herbal usage history was proved as a risk factor for delay in treatment on breast cancer patients in Surakarta. Keywords:Herbal Usage History, Delay in Treatment, Breast Cancer

Abstract

Previous study from 351 Indonesian women shown that they had breast cancers at younger age compared with western. In this study we investigate role of hormonal contraceptive as risk factor for Indonesian Javanese young breast cancer cases. However, the presence different life style between ethnic alter their risk as causal factors across populations. Diagnostic and prognostic study findings, including breast cancer prediction rules, must therefore be validated in Asian women. We undertook case-control study to determine population-based distributions of breast cancer among young Javanese people, one of the largest populations in Indonesia (Southeast Asia). A total of 500 women diagnosed with breast cancer participated in this study, divided in to two group young (less 40 years old) and mature breast cancer. Data for hormonal contraceptive, clinico-pathological characteristics and other risk factors were collected. We found that young Javanese women who use hormonal contraceptive for more than 10 years had a 4,67 fold increased risk of being diagnosed with breast cancer in young age (p<0,01). We didn’t found any differences between this two groups in menarche and parity. Interestingly for Javanese women who breast feeding more than 18 months increase 1,74 fold increased risk of being diagnosed with breast cancer in young age (p<0,01).

Abstract

Immunohistochemical is used to show hormone receptors on their surface. The aim of this study were to find information about immunohistochemical profile of breast cancer patient on oncology department, Moewardi Hospital, Surakarta Indonesia. A total 1.395 cases of breast carcinoma with Her2 profile were reviewed. The mean age of the patients was 45.7 (Ranged 23-73 years old). At time of diagnosis 739 (53%) patients were under 50 years of age. In this group the frequency of Her2 positive was higher than other. The incidence of patients above 50 years of age were significancy higher in Her2 positive with 363 (47%). Ductal invasive carcinoma is the most common type of breast carcinoma (86.9%). HER-2 Positive subtype was the most common subtype in the woman breast carcinoma (43.6%), which was followed by Basal like (31.3%), Luminal A (16.8%) and (8.3%). For tumor grading, the highest on G3 with the results 764 (54.7%). G3 got the highest results on Her2 positive with 535. We just divide the type of tumor consist of three parts (ductal, lobular, and other). In tumor type obtained the most data on ductal carcinoma with 878 (63%).

Abstract

Breast cancer surgery can impact patients psychologically as well as organically, which can manifest as Quality of life (QOL). Thirty patients, the age ranged between 25 and 60 years and the mean age 41 years. Twenty seven patients have married, a widow, and two unmarried. There was not found a difference in health related quality of life. No differences were seen in level of pain, skin sensitivity and related problems in surgical area. Woman in mastectomy group reported stastitically more problem with their body image. Beyond that, they also have a complaint about the feelings of sexual attractiveness than woman in breast conserving surgery (BCS). Women undergoing breast conserving surgery experience more positive outcomes than women undergoing mastectomy. Women in the mastectomy groups also reported experiencing more physical problems related to their surgery. The score of CES-D is above 16 both breast conserving surgery’s group and mastectomy’s groups. Higher scores are associated with more symptoms. There were also no differences seen among them as measured by the MOS, four main poins consists of emotional support, real support, affection support and good social interactions. There were also no differences seen among them as measured by the RDAS consists of three aspects (concensus, satisfaction, cohession). This can happen because the doctors give well support and education to all patients. Despite, the patients, their families, their partners also give support each other. The women sight of their body images was considered making well social interaction. Most of the study showed the declining of social interactions after the surgery. This happened beacuse they were apprehensive about rejection in social life.

Background : The development of technology has grown rapidly in surgical science, especially the use of endoscopy to develop operations in producing minimal scar tissue. Endoscopic VABS is a surgical technique and began to develop since 1995. The technique has several approaches, namely through the chest wall, mammary and axilla. The descriptive study aims to report our first experience in the management of benign breast tumors with Endoscopic Video Assisted Breast Surgery. Methods : Ten patients with benign breast tumor have performed Endoscopic VABS in the period March 2017 to September 2017 at the Hospital in Surakarta. The largest diameter at the nodule, duration of operation, length of postoperative care, postoperative pain, cosmetic outcomes and complications were observed retrospectively. Result : All patients are women with median age 28 years. The largest diameter that can be taken is 3.5 cm with median 2 cm. One patient performed the conversion into a lumpectomy with conventional techniques due to difficult dissection and uncontrolled bleeding. The median duration of surgery was 60 minute with an average length of treatment for 2 days postoperatively. Average VAS Score was 4. No postoperative complications were found. But cosmetically, all patients are satisfied with the results of surgical wounds. Conclusion : The VABS endoscopic procedure is safe and effective procedure and has excellent cosmetic benefits because it does not show scar tissue in the breast area. This VABS Endoscopic technique has a weakness in terms of longer duration of operation compared to conventional techniques, but this can be overcome with the expertise and experience of the operator.

Abstract

Latar belakang dan tujuan : Kanker merupakan penyebab kematian tertinggi setelah penyakit kardiovaskuler (jantung dan stroke). Penyakit kanker banyak memakan korban meninggal karena kebanyakan baru terdeteksi saat sudah memasuki stadium lanjut sehingga sulit disembuhkan.. Pada wanita kanker yang terbanyak adalah kanker payudara dan kanker leher rahim. Oleh karena itu perlu usaha pencegahan dan deteksi dini pada kanker payudara dan kanker leher rahim apalagi pada masa pandemic dimana akses ke fasilitas kesehatan terbatas.Metode Pelaksanaan : Kegiatan pengabdian masyarakat berupa seminar daring untuk memberikan pemahaman tentang kanker payudara dan kanker leher rahim meliputi gejala-gejala awal, cara deteksi dini, pencegahan dan tatalaksana apabila sudah didapatkan kanker. Peserta yang terdaftar sejumlah 164 orang dari berbagai kalangan masyarakat dan berbagai wilayah di Indonesia. Pada hari H seminar peserta yang hadir mencapai 121 orang, peserta antusias mengikuti presentasi dan diskusi hingga akhir acara.  Hasil dan Pembahasan : Setelah acara dilakukan survey tentang sikap dan pemahaman mengenai kanker payudara dan kanker leher rahim. Sebanyak 89,6 persen khawatir kalau-kalau menderita kanker dan sebanyak 96,9 % responden berusaha mencari informasi tentang kanker, dengan demikian seminar daring ini dapat menjebatani masyarakat untuk mencari informasi yang tepat tentang kanker. Dari hasil survey pemahaman tentang kanker payudara dan kanker leher rahim diperoleh nilai rata-rata 90,73 namun ada beberapa hal penting seperti penyebab kanker leher rahim, penyeberan dan factor yang berpengaruh pada kanker payudara masih belum paham (yang menjawab benar 11 s/d 20 % responden). Peserta webinar  mendapatkan sertifikat dan juga doorprize berupa masker.  Setelah acara seminar peserta memberikan respon positif untuk ditindaklanjuti dengan acara berikutnya,Kata kunci : penyuluhan, kanker payudara, kanker leher rahim, deteksi dini

Abstract

Introduction: Percutaneous Ethanol Ablation (PEA) has been recommended as the first-line treatment option for symptomatic benign cystic or predominantly cystic thyroid nodules, and it has been shown to be more effective and safer than other techniques in previous studies. Here, we present a case of a 44-year-old man with thyroid nodules who underwent PEA.Case Presentation: We report the case of a 44-year-old man with a painless mass that is getting bigger on his right neck. The physical examination measured 3.0 × 3.0 × 3.0 cm, mobile on swallowing, no pain on palpation. Thyroid function tests show a euthyroid state (TSH 2.4 mIU/L, FT4 1.2 ng/dL). The ultrasound examination in January 2020 showed a colloid cyst on the right thyroid, measured 3.03 × 2.82 × 3.56 cm (TIRADS 1). Cytological examination showed the results of non-infectious cystic fluid. The patient underwent US-guided PEA using 7 ml of ethanol by the moving-shot technique. The procedure was well tolerated by the patient and no adverse events were noted. A week after the procedure, there was a visible and progressive reduction of the nodule with no complaint from the patient. A month after the procedure, the patient came back with no recurrences, complaints, or other complications.Conclusions: PEA for thyroid lesions is an effective and safe method. PEA should be the first option for the treatment of pure cystic and predominantly cystic nodules. PEA is a safe procedure that is easy to repeat and to be performed on an outpatient basis.

Abstract

Introduction: Multiple primaries have been reported in patients with breast cancer in the range of 4.1% to 16.4%. This report followed the standard Surgical Case Report (SCARE) and discussed two histologically distinct breast cancer malignancies. This paper also explored the risk factor, pathogenesis, treatment, and prognosis of multiple primary malignancies in the same organs, especially in the breasts.Case Presentation: The patient was a 45-year-old woman with a lump in the left and right breasts for five years. The patient claimed that the lump was enlarged and painless. The tumor size of the left breast was 3 x 2 x 3 cm, the right breast was 4 x 2 x 4 cm, and the patient had no enlargement of lymph nodes. The patient underwent a biopsy examination of the left breast showing invasive ductal carcinoma NST (No Special Type) 3rd grade with LV1 (+). The biopsy examination of the right breast showed malignant phyllodes tumor. Thus, the patient must undergo chemotherapy and mastectomy as definitive therapy. The patient has already completed the treatment and is currently under evaluation.Conclusion: This report looked for the rare case of double primary malignancies in the breasts. It highlighted double primary cancers starting to increase these days but in a particular way because it happens in the same organ, the breasts. However, medical literature rarely presented a synchronous malignancy case, prompting the publishing of this case. With improved cancer treatment and more prolonged survival, the number of patients with double cancers is increasing.
