Abstract

Pneumonia can pose a serious threat to children's health. In pneumonia, increased lung secretions obstruct the airways, affecting breathing, especially in children who cannot expel sputum alone. If not treated immediately, this condition can be fatal. This study aimed to provide nursing care for children with nursing problems of ineffective airway clearance with Pursed Lips. Breathing using balloons can help improve gas exchange, which is observed by increasing oxygen saturation in the body and can help expel secretions. Participants in this study were pneumonia patients aged 3-5 years who were treated in the Bougenville Room of Dr. Haryoto Hospital, Lumajang, with nursing problems of Ineffective Airway Clearance and were not allergic to balloons. Data collection used interviews, observations, and documentation studies. The results of the assessment showed that An. A male, 5 years old, has a weak general condition, ineffective cough, sputum, additional breathing sounds of rhonchi, and an increased respiratory rate. The nursing problem that occurred was ineffective airway clearance. The interventions carried out refer to the Indonesian Nursing Intervention Standards (SIKI) intervention, namely airway management, respiratory monitoring, and Pursed Lips Breathing (PLB) actions. To attract children's interest, PLB uses balloons conceptualized as balloon play therapy, whose mechanism is similar to PLB with an atraumatic care approach. After nursing care for 4 days, the goals were achieved. Namely, changes in effective coughing increased, sputum decreased, additional breath sounds were absent, breathing patterns changed, and breathing frequency improved. Pursed Lips Breathing (PLB) can increase alveolar expansion in each lung lobe so that alveolar pressure increases and secretions can be pushed into the airway during expiration. PLB can be carried out to increase knowledge as an innovative intervention in treating children with pneumonia diagnosed with ineffective airway clearance.

Abstract

A Caesarean section is a surgical procedure through the abdominal wall and uterus to remove the fetus, which generally causes more severe postpartum pain than standard delivery. Pain due to surgery, if not immediately addressed, will hinder the mother's recovery process and the baby's breastfeeding process. One effort that can be done to reduce pain is with non-pharmacological methods, one of which is through the administration of lavender aromatherapy. This study aimed to explore the implementation of lavender aromatherapy administration for post-cesarean section mothers experiencing acute pain in the Lotus Room of Dr. Haryoto Lumajang Regional Hospital. The method used in this study was a case study involving one participant who met the inclusion criteria, namely a post-CS patient treated in the Lotus Room of Dr. Haryoto Lumajang Regional Hospital, exhibiting 5 out of 6 significant signs and symptoms of acute pain with a moderate to severe pain scale. Data was collected on March 18-20, 2025, using interviews, observation, and documentation techniques. The measurement instrument for the level of pain reduction was the Numeric Rating Scale (NRS) and the observation sheet for outcome criteria. Aromatherapy was implemented using a diffuser with five drops of lavender essential oil twice daily for 20 minutes. The study results showed a reduction in pain levels, with the following criteria: pain complaints decreased from 8 (severe) to 2 (mild), decreased grimacing, decreased anxiety, decreased protective behavior, improved pulse rate, and improved sleep quality. Pain management with one of the lavender aromatherapy interventions can reduce pain from 8 to 2 on NRS.

Abstract

Febrile seizures in children, if prolonged and repeated, can cause serious complications for the child. Hyperthermia is a major nursing problem in children with febrile seizures, which can increase the risk of recurrent seizures if not treated immediately. A family-centered care approach is important in child care. The purpose of this study was to evaluate the use of tepid water sponges within a family-centered care approach to improve thermoregulation in children with febrile seizures. The research method was a case study. The participant in this study was a 3-year-old boy who was treated in the Bougenville Ward of Dr. Haryoto Lumajang Regional Hospital, with a medical diagnosis of complex febrile seizures, with nursing problems of hyperthermia and no history of allergies/extreme cold, the child did not have open wounds or severe infections, no dermatitis/severe eczema and the child did not experience severe respiratory disorders or unstable circulation. Data collection was conducted through interviews, observation, and documentation. The assessment results showed that the patient experienced increased body temperature, red skin, increased skin temperature, and tachycardia. The nursing problem that emerged was hyperthermia. The intervention carried out refers to the Indonesian Nursing Intervention Standards (SIKI), namely the management of external cooling hyperthermia using compresses soaked in tepid water, with a family-centered care approach. Family involvement is carried out through education and direct assistance during the action to increase family participation in child care. After 2 days of implementation, the results were a decrease in body temperature of 0.9–1.3°C, decreased redness, improved skin temperature, and decreased tachycardia. A tepid water sponge can help initiate peripheral vasodilation, increasing heat loss and lowering the child's body temperature. A tepid water sponge with a family-centered care approach can be used as a non-pharmacological intervention in the treatment of hyperthermia in children with febrile seizures

Abstract

Stunting has long-term impacts on physical growth, cognitive development, and increased risk of disease in children. In Lumajang Regency, the stunting prevalence in 2024 reached 23.4%, highlighting the need for community-based prevention efforts. This community service aimed to enhance cadres’ capacity in stunting prevention through the utilization of moringa as a local food resource in the agronursing area of Kutorenon Village. The location was selected based on the availability of moringa and the readiness of local health cadres. The program applied an Asset-Based Community Development (ABCD) approach with a pre-experimental design (pre-test and post-test) involving 50 cadres. Activities included education, discussion, demonstration, and mentoring in preparing moringa-based healthy foods such as moringa ice cream and nuggets.The results showed a substantial increase in cadres’ knowledge, with mean scores rising from 60 to 88, along with improved practical skills in nutritious food preparation. The program also improved complementary feeding practices at community health posts, contributing to better child nutritional intake as a protective factor against stunting. Evaluation across nine posyandu indicated that six (66.7%) developed sustainable moringa-based menu innovations. This program positively impacted community nutritional behavior and has the potential to reduce stunting risk through improved child nutrition based on local resources.

Abstract

ABSTRAKPasca-sectio caesarea (SC), pasien diharapkan melakukan mobilisasi secara bertahap untuk mencegah bahaya fisiologis dan psikologis yang mungkin dapat terjadi. Namun, masih sedikit informasi yang melaporkan tentang pelaksanaan mobilisasi dini di lapangan. Tujuan penelitian: Menggambarkan asuhan keperawatan pada pasien pasca-SC, khususnya pada aspek hambatan mobilitas fisik. Keluhan utama: Pasien 1 (21 tahun) ditemani suami datang ke RS atas rujukan puskesmas. Mereka sudah memahami bahwa harus melahirkan secara SC karena panggul sempit. Selanjutnya, dilakukan operasi SC dengan anestesi lumbal, insisi melintang, luka ± 10 cm. Pasien 1 mengalami anemia sehingga dilakukan transfusi darah. Pasien 2 (32 tahun), datang ke RS ditemani suami, dirujuk karena panggul sempit dari puskesmas, kemudian dilakukan operasi SC dengan anestesi lumbal, insisi melintang, luka ± 10 cm.  Pengkajian melalui wawancara, obervasi, pemeriksaan fisik, dan studi dokumentasi.  Hasil:Intervensi untuk mobilisasi dilakukan setelah tindakan operasi selesai secara bertahap selama tiga hari perawatan. Hari pertama dilakukan pemberian informasi dan ambulasi awal di tempat tidur. Hari kedua pengaturan posisi semi-fowler, membantu perpindahan, latihan keseimbangan berdiri, berjalan. Hari ketiga pasien melakukan aktivitas mandiri tanpa bantuan, penyuluhan perawatan di rumah setelah operasi. Diskusi: Peran perawat pada pasien dengan melakukan mobilisasi secara bertahap serta berkolaborasi dengan tenaga kesehatan yang lain dapat mendukung percepatan pasien melakukan tindakan mobilisasi secara mandiri. Simpulan: Perawat perlu meningkatkan mobilisasi pasien secara bertahap dengan memerhatikan faktor ibu dan keluarga. Perawat disarankan meningkatkan pengetahuan, misalnya mengenai ERAC dan dalam proses asuhan keperawatan menggunakan rangkaian standar pelayanan untuk penyelenggaraan praktik keperawatan di Indonesia, yaitu SDKI, SIKI, SLKI, dan SPO.Kata kunci: mobilisasi dini, sectio caesaria             Gradual Mobilization in Post Cesarean Section: A Case Study at Dr. Haryoto Hospital of Lumajang ABSTRACTAfter the caesarean section (CS), patients are expected to mobilize gradually to prevent physiological and psychological hazards. However, there is still little information reporting about the implementation of early mobilization. Objective: to describe nursing care in post-CS patients, especially regarding physical mobility barriers. Primary complaint: Patient 1 (21 years old), accompanied by her husband, came to the hospital based on the referral of the Public Health Center. They already understood that they had to give birth by cesarean section because the pelvis was narrow. Next, SC surgery was performed with lumbar anesthesia, transverse incision, and wound ± 10 cm. Patient 1 was anemic, so a blood transfusion was performed. Patient 2 (32 years) came to the hospital accompanied by her husband and was referred by the Public Health Center because of a narrow pelvis; then, an SC operation was performed with lumbar anesthesia, transverse incision, and wound ± 10 cm. Assessment through interviews, observations, physical examinations, and documentation studies.  Results: interventions for mobilization were performed after the surgery was completed in stages for 3 days of treatment. On the first day, information was given and early ambulation was performed in bed. On the second day, the activities included adjusting semi-Fowler's position, assisting the movement, exercising balanced standing, and walking. On the third day, the patient did independent activities without assistance and counseling on home care after surgery. Discussion: The nurses play a role by gradually mobilizing patients and collaborating with other health workers to support patients' acceleration to mobilize independently. Conclusion: Nurses need to increase patient mobilization gradually by paying attention to maternal and family factors. Nurses are advised to increase knowledge, for example, about ERAS and the nursing care process by using a series of service standards for implementing nursing practice in Indonesia, namely SDKI, SIKI, SLKI, dan SPO.Keywords: early mobilization, cesarean section

Abstract

Kematian maternal masih menjadi permasalahan kesehatan global yang signifikan, khususnya di negara berkembang. Rendahnya literasi kesehatan ibu hamil terkait faktor risiko menjadi salah satu penyebab utama. Pemanfaatan teknologi digital seperti aplikasi kesehatan menjadi inovasi strategis dalam meningkatkan edukasi. Aplikasi eMAMA dikembangkan sebagai media edukasi kesehatan ibu hamil, namun efektivitasnya perlu ditinjau dari sisi penerimaan pengguna. Tujuan penelitian ini adalah untuk menganalisis penerimaan aplikasi eMAMA sebagai media edukasi risiko kematian maternal pada ibu hamil menggunakan pendekatan Technology Acceptance Model (TAM). Penelitian kuantitatif dengan desain cross-sectional. Sampel berjumlah 21 ibu hamil yang menggunakan aplikasi eMAMA, dipilih menggunakan teknik purposive sampling. Instrumen berupa kuesioner berbasis konstruk TAM meliputi perceived ease of use dan perceived usefulness. Analisis data dilakukan secara deskriptif dan inferensial. Sebanyak 81% responden menyatakan aplikasi mudah digunakan dengan nilai mean 44,95 (SD=5,172). Sebanyak 90,5% responden menyatakan aplikasi bermanfaat dengan median 48 (39-55). Temuan menunjukkan bahwa kemudahan penggunaan berkontribusi terhadap persepsi kebermanfaatan aplikasi dalam meningkatkan pemahaman risiko kematian maternal. Integrasi TAM efektif dalam mengevaluasi penerimaan aplikasi eMAMA. Aplikasi ini berpotensi menjadi media edukasi digital yang mendukung peningkatan literasi kesehatan ibu hamil.
