Abstract

Introduction: Patients with Diabetes Mellitus type 2  tend to have ABI values lower than the normal range due to lack of physical exercise and uncontrolled hyperglycemia. This needs to be prevented through joint movement exercises with the Buerger Allen exercise which can increase blood perfusion in the lower extremities so that there is an increase in the value of the ankle-brachial index (ABI). Objective: This study aims to determine the effect of Buerger Allen exercise on the value of the ankle-brachial index (ABI) in patients with Diabetes Mellitus type 2 based on a review of research journals. Methods: This study uses a literature review method. Search literature using 5 databases, namely Portal Garuda, Sinta, ProQuest, Scopus, and Google Scholar with keywords according to MeSH, namely "Buerger Allen Exercise", "Ankle Brachial Index", and "Diabetes Mellitus Type 2". The PICOS framework was used to adjust the inclusion criteria which were divided into 5 English journals and 5 Indonesian journals and then analyzed narratively based on similarity analysis. Results: The findings of the ten journals stated that the Buerger Allen exercise had a significant effect on the value of the ankle-brachial index (ABI). Discussion: Buerger Allen exercise is a physical exercise to improve foot peripheral perfusion that can increase the value of the ankle-brachial index in type 2 DM patients. Conclusion: Buerger Allen exercise has a significant effect on increasing the ankle-brachial index (ABI) score so that it can be used as a reference source for standard operating procedures and nursing interventions. Keywords: Buerger Allen; Exercise; Ankle Brachial Index; Diabetes Mellitus

Abstract

Type 2 DM is a metabolic disorder disease characterized by an increase in blood sugar due to insulin resistance and can cause Diabetic Foot Ulcer (DFU) which can be cured by wound cleansing. The study used a Pre-Post control group design with a measurement time of 16 days involving 40 patients who were divided into 2 groups and selected randomly. The method of analysis used the Wilcoxon and Mann-Whitney. The instrument uses standard operating procedures for wound cleansers made from the decoction of Arabic bidara leaves and the Bates-Jensen Wound Assessment Tool (BWAT). Data analyzed using Mann-Whitney  (p= 0.586) showed that there was no difference in the mean after wound cleansing in the intervention group and after wound cleansing in the control group, but there was a difference in the decrease in indicators.  The intervention group reduced 8 BWAT indicators consisting of wound edges, Necrotic Tissue Type, Necrotic Tissue Amount, exudate type, Exudate Amount, Skin Color Surrounding the Wound, Peripheral Tissue Edema & Induration. while the control group decreased 6 BWAT indicators consisting of Necrotic Tissue Type, Necrotic Tissue Amount, Exudate Amount, Skin Color Surrounding the Wound, Peripheral Tissue Edema & Induration. Arabic bidara leaves contain flavonoids as anti-inflammatory and antioxidants as well as alkaloids, tannins, and Christine as antibacterial. This research can be used as an evidence-based practice for DFU wound care.

Abstract

Autonomic neuropathy is a complication of diabetes mellitus that affects the autonomic nervous system, causing death. Inappropriate nursing practices are caused by a lack of knowledge about diabetic foot care. This study aims to determine the relationship between level of knowledge and foot care in diabetes patients. This research is analytical research which is included in the Cross Sectional Study. Data analysis in this study used the Spearman Rho Test. because both variables use an ordinal data scale. The data collection tool used in this preliminary study was a questionnaire. The results of the study showed that 41 people (961.2%) had a level of knowledge about autonomic neuropathy foot care, but the majority of respondents' autonomic neuropathy foot care skills were lacking, 47 people (70.1%). There is a significant relationship between the level of knowledge of the patient's autonomic neuropathy foot care and the patient's ability to care for the patient's autonomic neuropathy foot. It is hoped that suggestions for further research can contribute to knowledge, especially for diabetes patients, about managing diabetic foot care, one of which is by increasing the level of knowledge about foot care.

Abstract

Introduction: Diabetes Mellitus is a chronic disease that has a high prevalence globally and often causes serious complications such as diabetic foot ulcers. Diabetic foot ulcers are one of the leading causes of nontraumatic amputations and significantly affect the quality of life and health of diabetic patients. Treatment of diabetic foot ulcers is a challenge for healthcare because it involves various factors that hinder the healing process. The purpose of this study was to evaluate the effect of topical propolis as an adjuvant in the treatment of diabetic foot ulcers. Methods: Data were analyzed from quality studies indexed in the SCOPUS Quartil 1 database. Data visualization is done using Forest plots. Results: The results of the analysis showed that topical propolis made a significant contribution to the healing process of diabetic foot ulcers. Topical administration of propolis increases connective tissue deposits. It promotes better tissue regeneration, as well as reducing oxidative stress, which is a major contributor to the pathogenesis of diabetic foot ulcers. Conclusion: Topical propolis offers a new and promising approach to the treatment of diabetic foot ulcers. With its anti-inflammatory, antibacterial, and antioxidant properties, propolis can speed up the wound healing process, improve patients' quality of life, and reduce the economic burden associated with diabetic foot ulcer treatment. Although more research is needed to understand the mechanism of action of propolis thoroughly, these results lend support to the potential of propolis as an effective adjuvant therapy in the management of this chronic disease.
