Abstract

Radioterapi eksternal merupakan pengobatan utama karsinoma nasofaring. Radioterapi  eksternal menyebabkan disfungsi endotel arteri karotis komunis yang menurunkan produksi nitrogen oksida dan prostasiklin di sel endotel sehingga terjadi penebalan dinding pembuluh darah. Tujuan penelitian ini untuk mengetahui perubahan ketebalan intima media dan diameter lumen arteri karotis komunis akibat radioterapi eksternal  pada penderita karsinoma nasofaring. Tipe penelitian adalah  studi deskriptif dengan rancangan pre-post design. Penelitian dilakukan di Departemen Telinga Hidung Tenggorok-Bedah Kepala Leher-Rumah Sakit Dr. Hasan Sadikin Bandung mulai Maret 2009–Februari 2010. Pada penelitian ini dilakukan pemeriksaan  ultrasonografi  gray scale praradioterapi, pascaradioterapi, dan 10 minggu pascaradioterapi.  Hasil penelitian ini menggunakan uji Fischer dan T berpasangan. Terdapat 25 subjek karsinoma nasofaring dan hasil penelitian ditemukan peningkatan ketebalan intima media sebesar 0,1 mm di intima media arteri karotis komunis kanan dan 0,09 mm di intima media arteri karotis komunis kiri (p<0,001), juga ditemukan pengurangan diameter lumen arteri karotis komunis kanan sebesar 0,384 mm dan 0,342 mm di arteri karotis komunis kiri (p<0,001).  Simpulan, radioterapi eksternal menunjukkan ketebalan intima media dan pengurangan diameter lumen arteri karotis komunis pada penderita karsinoma nasofaring. [MKB. 2012;44(3):179–85].Kata kunci: Diameter lumen, karsinoma nasofaring, ketebalan intima media, radioterapi eksternal                     Intima and Media Tunica Thickness of Common Carotid Artery in Nasopharyngeal Carcinoma Pre and Post External RadiotherapyExternal radiotherapy is the main treatment for nasopharyngeal carcinoma. External radiotherapy  can cause endothelial dysfunction of common carotid artery and reduces nitrogen oxide and prostacyclin by endothelial cell and cause thickening of the vessels walls. Purpose of this study was to find out changes in intima-media thickness and lumen diameter of common carotid artery due to radiotherapy in patients with nasopharyngeal carcinoma. This research was descriptive with pre and post design and performed from March 2009 to February 2010 in the Ear Nose Throat-Head Neck Surgery Department of Dr. Hasan Sadikin Hospital Bandung. Gray scale ultrasound examination was performed before, after and ten weeks after radiotherapy and calculated by using Fischer’s test and paired T test.  They were 25 subjects and was found out 0.1 mm additional thickness in right intima media and 0.09 mm in left intima media common carotid artery (p<0.001), also found reduction in lumen diameter of common carotid artery in which 0.384 mm in the right and 0.342 mm in the left (p<0.001). In conclusions, external radiotherapy can cause increase intima media thickness  and reduction lumen  diameter  in common  carotid artery  in patients with nasopharyngeal carcinoma. Abstract

Latar belakang: Pedagang kaki lima rentan terkena polusi udara. Hidung merupakan salahsatu organ pelindung tubuh terpenting dan menjadi target utama dari polusi udara. Polusi udara dapatmenurunkan kadar pH hidung dan menyebabkan sistem transpor mukosiliar hidung tidak bisa bekerjaoptimal. Terapi cuci hidung telah digunakan sejak berabad-abad lalu, untuk mengobati penyakit sinuskarena mencegah pembentukan krusta pada rongga hidung. Terapi cuci hidung juga dapat membersihkanpartikel-partikel debu dan polusi yang terperangkap di mukus hidung. Tujuan: Melihat pengaruh cucihidung dengan NaCl 0,9% terhadap peningkatan rata-rata kadar pH cairan hidung pada pedagang kakilima yang rentan terkena debu dan polusi udara.Metode: Penelitian ini bersifat pra-eksperimental denganone group pretest-posttest design. Populasi penelitian adalah seluruh pedagang kaki lima yang berjualan dikawasan kampus Universitas Sumatera Utara dan sampel adalah populasi yang memenuhi kriteria inklusidan eksklusi. Data hasil penelitian diolah dengan uji normalitas Kolmogorov-Smirnov dan dilanjutkandengan uji Wilcoxon.Hasil: Berdasarkan uji normalitas data Kolmogorov-Smirnov didapati nilai P pretest0,000 dan P posttest 0,001. Kedua data tersebut tidak terdistribusi normal, kemudian dilanjutkan denganUji Wilcoxon. Didapatlah hasil Wilcoxon dalam peningkatan kadar pH cairan hidung adalah peningkatanyang bermakna (p=0,000; p<0,05) dan memiliki peningkatan rata-rata pH cairan hidung yaitu sebesar0,0824.Kesimpulan: Terdapat peningkatan rata-rata kadar pH cairan hidung setelah dilakukan cucihidung dengan NaCl 0,9% sebanyak dua kali sehari, selama sepuluh hari pada pedagang kaki lima. Parapedagang kaki lima di lingkungan Universitas Sumatera Utara, dianjurkan untuk melakukan cuci hidungsetiap hari untuk menjaga higiene saluran napas.Kata kunci: pH cairan hidung, cuci hidung, polusi udara, pedagang kaki lima ABSTRACTBackground: Street vendors in Medan and other cities in Indonesia are prone to outdoor airpollution that may compromising the nose as well as the entire respiratory system. Outdoor air pollutantmay reduce nasal pH level causing an in-efficient mucociliary transport system. Nasal washing as oneof modality in personal hygiene aims to clean hazardous particles and pollutants trapped in the nasalmucous blanket. Purpose: This study aimed to find-out that average of pH levels of nasal fluid affectedby nasal washing using NaCl 0,9%. Method: The study design was a pre-experimental with one grouppretest-posttest design. Population of the study were all vendors in surrounding Universitas SumateraUtara, and the subjects were having 8 hours a day exposure to outdoor air pollutant without any evidenceof ongoing infectious inflammation. Research data processed by the normality test, Kolmogorov-Smirnovtest, and followed by Wilcoxon test. Result: Based on Kolmogorov-Smirnov test, result obtained P pretest0.000 and P posttest 0.001. Both were not distributed normally, and then followed by Wilcoxon Testwhich found a significant increase of pH level of nasal fluid (P<0.05) with average pH level of 0.0824.Conclusion: An increase in nasal fluid average pH levels was significantly observed after washing thenose using NaCl 0,9% solution twice a day for ten days. Street vendors in Universitas Sumatera Utarasurroundings are suggested to do daily nasal washing as their new life style to maintain personal hygiene.Keywords: Nasal fluid pH, nasal wash, air pollution, vendor

Abstract

All level of communities should be taught about basic life support especially for workers who related to giving first aid, one of them is the airport. Ideally, everyone knows the basic first aid techniques and takes regular training to ensure that the knowledge stays on. Basic life support is an effort made to maintain the life of someone who is in danger which can get worse or death if it’s not treated quickly. Basic life support in the first hour can reduce up to 85% of mortality.

Abstract

Background. Gastroesophageal Reflux Disease (GERD) is a common gastrointestinal disorder caused by reflux of stomach contents into the esophagus, as a result, it causes various symptoms or complications . GERD is a commonly prevalent gastrointestinal disorder in medical students. Based on data from the 2006 US National Health and Wellness survey observing respondents who experienced heartburn at least twice a month, 89% of respondents experiencing nightime GERD symptoms, it showed that the presence of nighttime GERD was associated with decrease quality of sleep. This study aimed to analyze the relationship between GERD and students’ sleep quality in Faculty of Medicine USU 2020-2022. Method. Cross sectional design with total sampling method. Data collection was carried out using an online questionnaire in the form of a google form. The questionnaires used were GERD-Q, FSSG, PSQI, and ISI. The analysis used was univariate and bivariate analysis with Chi-square. Results. This study involved 378 respondents, 114 people (30.2%) with GERD, 67 people (58.8%) with severe of GERD, 286 people (75.7%) had a bad quality of sleep, and 43 people (15%) with moderate and severe of insomnia. The results of statistical analysis showed an association between GERD and quality of sleep. Conclusion. There was a correlation between GERD and quality of sleep of USU Faculty of Medicine students class 2020-2022

Abstract

Background: Functional endoscopic sinus surgery (FESS) is a surgical procedure for chronic rhinosinusitis (CRS). Keros and Gera classification can be used to evaluate the height and angle of the lateral lamella (LL) on computed tomography (CT) scan of the paranasal sinuses (PNS) which is the most dangerous area and at risk of injury during FESS. This study aimed to analyze the difference between Keros and Gera classifications on CT scans of PNS.Methods: This study was a cross-sectional study using the results of PNS CT scans from 94 patients at Prof. Dr. Chairuddin Panusunan Lubis Universitas Sumatera Utara Hospital, Medan, Indonesia for the period January–December 2022. The Keros and Gera classification of PNS CT scan were compared based on age and gender and then analyzed using the Chi-Square test. The results were significant if the p-value < 0.05.Results: The most common type of right-sided Keros classification was type 2 (54.3%) and left-sided was type 1 (56%), while the most common type of right-sided and left-sided Gera classification were type 2 (67% and 80.8%). A significant difference was found between the Keros and Gera classification types (p <0.001).  The higher the Keros type did not guarantee the higher Gera type, each type of Keros could be paired with each type of Gera.Conclusions: There is a significant difference between Keros and Gera classification on the results of PNS CT scan of patients. Keros classification alone is not enough to identify the high-risk areas at the skull base, therefore, a combination with Gera classification needs to be evaluated by a radiologist to help the clinician in planning a safer FESS.

Abstract

Tympanic membrane perforation and continuous discharge are hallmarks of chronic suppurative otitis media, a chronic inflammation of the middle ear. Through the malfunction of the Eustachian tube brought on by allergic inflammation, allergic rhinitis may contribute to the development of CSOM. This study aimed to analyze the relationship between allergic rhinitis and CSOM, identify common allergens, and assess the association between eosinophil levels and CSOM. A case-control study was conducted at RSUP H. Adam Malik and RS Murni Teguh Medan involving 72 participants: 36 CSOM patients and 36 healthy controls. Skin prick tests (SPT) and eosinophil counts were used to assess allergic status. Statistical analysis used Chi-square and Fisher’s exact test with significance set at p < 0.05. The results showed a significant association between allergic rhinitis and CSOM (p = 0.001; OR = 6.192). Common allergens found in CSOM patients were D. pteronyssinus (66.7%), D. farinae (63.9%), and cockroach (47.2%), with significant associations (p < 0.05). No significant relationship was found between allergic rhinitis and CSOM sides or type (p > 0.05). Elevated eosinophil levels were more common in the CSOM group but were not statistically significant (p = 0.624). In conclusion, allergic rhinitis is significantly associated with CSOM. Allergen screening is recommended for CSOM patients to improve diagnosis, treatment, and recurrence prevention.

Abstract

Background: Allergic Rhinitis is a symptom disorder that mainly occurs in the nasal, induced after exposure to allergens and causes inflammation of the nasal mucosa mediated by Immunoglobulin E. Data quoted from the American Academy of Allergy Asthma & Immunology shows that 10-30% of the world’s population is affected by allergic rhinitis and still increasing in numbers. To screen and evaluate the severity of allergic rhinitis with ease, two questionnaires have been formed, namely Score For Allergic Rhinitis and Total Nasal Symptoms Score. Objective: To identify the relationship between Score For Allergic Rhinitis and Total Nasal Symptoms Score in individual with suspected allergic rhinitis. Methods: This study uses a descriptive analytic research method with a cross-sectional study approach. The sample individual was selected using a non-probability sampling technique, namely purposive sampling with a sample size of 95 people. Results: Using Spearman Correlation Test, a significant relationship was found (p = 0.000) and a correlation coefficient value shows (p=0.535) there is a strong relationship between these two questionnaires. Conclusion: There is a strong and positive relationship between Score For Allergic Rhinitis and Total Nasal Symptoms Score questionnaire in patients with suspected allergic rhinitis.
