Abstract

Penebalan lensa kristalina pada katarak senilis mengakibatkan hambatan pada jalur aliran akuos. Pascabedah katarak terjadi peningkatan kedalaman bilik mata depan (BMD) yang memiliki korelasi positif dengan pelebaran sudut BMD serta penurunan tekanan intraokular (TIO). Tujuan penelitian untuk menganalisis perbedaan penurunan TIO pascabedah katarak pada kelompok sudut BMD tertutup dan terbuka. Penelitian ini menggunakan desain pre-post test, untuk membandingkan penurunan TIO pascabedah katarak fakoemulsifikasi pada 26 mata dari 26 orang penderita, yang dibagi menjadi kelompok sudut BMD tertutup dan terbuka masing-masing berjumlah 13 mata. Tempat penelitian Rumah Sakit Mata Cicendo Bandung periode Maret?Juni 2012. Pengambilan data berdasarkan urutan datang penderita yang direncanakan operasi katarak fakoemulsifikasi. Penilaian sudut bilik mata depan prabedah dilakukan menggunakan lensa gonio Sussman 4-mirror. Tekanan intraokular pascabedah diukur saat pemantauan minggu ketiga pascabedah. Penilaian TIO pra dan pascabedah dilakukan menggunakan alat ukur tonometri aplanasi Goldmann. Analisis statistik dilakukan menggunakan uji t. Hasil menunjukkan perbedaan penurunan TIO secara bermakna lebih besar pada kelompok sudut BMD tertutup (19,6%) dibandingkan dengan kelompok sudut BMD terbuka (11,3%) dengan nilai p=0,022. Simpulan, perbedaan penurunan TIO pascabedah katarak fakoemulsifikasi lebih besar pada kelompok sudut BMD tertutup dibandingkan dengan kelompok sudut BMD terbuka. Increased anterior chamber depth had a positive correlation with the widening of the anterior chamber angle and decreased of intraocular pressure (IOP) after cataract extraction. The purpose of this study was to compare IOP reduction after cataract surgery between angle-closure and open-angle group. This pre-post test design study was to compare IOP after phacoemulsification cataract surgery in 26 eyes of 26 patients divided into angle-closure and open-angle groups consisting of 13 eyes each. The study was conducted in Cicendo Eye Hospital Bandung in period of March until June 2012. Patients who planned to have phacoemulsification cataract surgery were recruited consecutively. The anterior chamber angle was measured before surgery using Sussman 4-mirror goniolens. The intraocular pressure were measured before and three weeks after surgery using Goldmann aplanation tonometer. Statistical analysis was done using t test. The results indicated that IOP reduction was statistically significant greater in the angle-closure group (19.6%) compared with open-angle group (11.3%) with p=0.022. In conclusion, IOP reduction after phacoemulsification cataract surgery was greater in the angle-closure group compared with open-angle group. Abstract

Pemeriksaan fundus memerlukan akses visualisasi yang baik. Pada penderita diabetes melitus, pupil sulit lebar menggunakan obat pelebar pupil standar sehingga perlu ditambahkan agen pelebar pupil. Penelitian ini dilakukan di Rumah Sakit Mata Cicendo periode Januari sampai Juli 2014 menggunakan metode double masked rendomized cotrolled trial terhadap 64 mata dari 32 subjek penelitian yang telah didiagnosis menderita retinopati diabetika dengan tingkat yang sama antara mata kiri dan kanan. Dibuat larutan kombinasi campuran 10 mL tropikamid 0,5%/fenilefrin 0,5%, 10 mL fenilefrin 5% dan 10 ml 0,1% Na diklofenak (larutan I) dan campuran 10 mL tropikamid 0,5%/fenilefrin 0,5%, dan 10 mL fenilefrin 5% (larutan II). Setiap subjek mendapat salah satu larutan pada mata dan larutan yang lain pada sebelahnya secara acak. Dilakukan pemantauan dengan cara memotret pupil pada menit ke-15, 20, dan 25 pada kedua mata. Lebar pupil kedua kelompok perlakuan diukur dengan perangkat lunak khusus (image processing). Dari penenelitian terdapat perbedaan bermakna antara penggunaan larutan I dan larutan II dalam melebarkan pupil baik pada menit ke-15 (t=2,02; p=0,047), menit ke 20 (t=2,23; p=0,029), dan pada menit ke-25 (t=2,041; p=0,045). Larutan kombinasi fenilefrin, tropikamid, dan natrium diklofenak menghasilkan efek dilatasi pupil yang lebih baik dibanding dengan larutan kombinasi fenilefrin dan tropikamid saja pada kasus retinopati diabetes. [MKB. 2017;49(3):199?207]Kata kunci: Dilatasi pupil, natrium diklofenak, retinopati diabetika Effectiveness of Sodium Diclofenac Addition to Pupil Dilatation Agent on Diabetic Retinopathy PatientsFundus examination requires good visualization of fundus. In diabetic patients, it is difficult for the pupil to dilate using the standard pupilarry dilating agent. To achieve proper dilation, special agents have to be added to the standard dilation agent. The study was conducted in Cicendo Eye Hospital from January to July 2014, using double masked randomized controlled trial to 32 patients (64 eyes) who were diagnosed as suffering from diabetic retinopathy with the same grade on both eyes. Eye drop solutions were developed beforehand by mixing 10 mL of 0.5% tropicamide/0.5% phenylephrine, 10 mL of 5% phenylephrine, and 10 mL of 0.1% diclofenac-natrium (solution I) and also by mixing 10 mL of 0.5% tropicamide/0.5% phenylephrine and 10 mL of 5% phenylephrine (solution II). Every subject received one drop of one solution on one eye and the other solution on the other eye in randomly fashion. The observation was performed by taking photographs of the pupil on both eyes 15, 20, and 25 minutes of observation. The resulting pupil photographs were then analyzed and measured using special image processing software and compared. It was revealed that there were statistical differences in pupil dilation between solution I and solution II at 15 minutes (t=2.02; p=0.047), 20 minutes (t=2.23, p=0.029), and 25 minutes (t=2.041, p=0.045). Phenilefrine, tropicamide and diclofenac-natrium combination solution provides better dilation effect on the pupil compared to the combination of phenilefrine and tropicamide alone in diabetic retinopathy patient. Abstract

Penilaian segmen anterior, khususnya pemeriksaan sudut bilik mata depan memegang peranan penting dalam mendiagnosis dan penatalaksanaan penyakit glaukoma. Pentacam dan ultrasound biomicroscopy (UBM) mampu mengukur sudut bilik mata depan secara kuantitatif dan objektif, namun pemeriksaan UBM lebih invasif sedangkan Pentacam tanpa kontak dengan permukaan bola mata. Tujuan penelitian untuk mencari kesesuaian antara Pentacam dan UBM dalam mengukur sudut bilik mata depan. Penelitian ini merupakan studi analitik deskriptif dengan uji kesesuaian antara dua metode pengukuran, dengan subjek penderita glaukoma dan bukan glaukoma di Rumah Sakit Mata Cicendo Bandung, periode November–Desember 2010, berjumlah 42 mata dari 31 orang. Dilakukan pemeriksaan pada area nasal dan temporal sudut bilik mata depan menggunakan kedua alat tersebut. Uji kesesuaian menggunakan Bland and Altman dan uji hipotesis menggunakan uji-t berpasangan. Secara statistik tidak terdapat perbedaan bermakna pada sudut terbuka, hasil pengukuran sudut bilik mata depan area nasal dan temporal antara Pentacam (37,51; 40,49; p=0,55) dan UBM (38,87; 40,76; p=0,22), namun limit of agreement yang didapatkan menunjukkan rentang yang luas, area nasal (dari 11,94 sampai -18,14) dan area temporal (dari 11,51 sampai -15,31) pada taraf kepercayaan 95%. Pada sudut tertutup terdapat perbedaan bermakna hasil pengukuran sudut bilik mata depan area nasal dan temporal antara Pentacam (27,33; 36,65; p<0,001) dan UBM (31,47; 37,34; p<0,001), sedangkan limit of agreement yang didapatkan menunjukkan rentang pada area nasal (dari 4,51 sampai -16,65) dan area temporal (dari 2,98 sampai -14,73) pada taraf kepercayaan 95%. Simpulan, pengukuran sudut bilik mata depan dengan menggunakan Pentacam memiliki kesesuaian yang tidak baik dibandingkan dengan UBM pada kelompok sudut terbuka, dan tidak memiliki kesesuaian pada kelompok sudut tertutup. [MKB. 2014;46(1):28–33]Kata kunci: Glaukoma, Pentacam, sudut bilik mata depan, ultrasound biomicroscopy Compatibility of the Outcomes in Measurement of the Anterior Chamber Angle between Using Pentacam and Ultrasound BiomicroscopyThe assessment of anterior segment, especially the examination of anterior chamber angle, plays an important role in diagnosing and managing glaucoma. Pentacam and ultrasound biomicroscopy (UBM) were able to measure the anterior chamber angle quantitatively and objectively; however, UBM examination is invasive, where as Pentacam is without contact with eye globe surface. The aim of this study was to seek the conformation between Pentacam and UBM in measuring the anterior chamber angle. The study was analytic descriptive study with the agreement of both measurements in 42 eyes of 31 patients with glaucoma and non-glaucoma Cicendo Eye Hospital Bandung, in period of November to December 2010. The examination used both instruments in nasal and temporal area of anterior chamber angle with equal illumination. The agreement test using Bland and Altman and hypothesis using paired t-test. Statistically, there were no significant differences in anterior chamber open angle of nasal and temporal area between using Pentacam (37.51; 40.49; p=0.55) and UBM (38.87; 40.76; p=0.22), while the limit of agreement indicated wide range in nasal area (11.94 to -18.14) and in temporal area (11.51 to -15.31) in 95% confidence interval. In closed angle of anterior chamber, there was a significant difference at nasal and temporal area between using Pentacam (27.33; 35.65; p<0.001) and using UBM (31.47; 37.34; p<0.001), while the limit of agreement showed (4.01 to -16.65) for nasal area (2.98 to -14.73) for temporal area in 95% confidence interval. In conclusion, the measurement of the anterior chamber in open-angle group using Pentacam indicated poor agreement with using UBM; and there is no agreement in closed angle group. Abstract

Glaucoma is a second leading cause of blindness after cataract. Glaucoma caused by unbalance absorption of aqueus humour so it increase intraocular pressure. As a result, it surpresses nerve cells so that nerve cells can not get enough blood flow as nutrition intake and can lead to permanent blindness. One of the treatment for glaucoma is by surgical procedure, called trabeculectomy. After the surgery a slightly lifted tissue due to passing fluid, called bleb, should appears. Bleb assesment is necessary to examine the successful of trabeculectomy surgery. One of standard assesment is Indiana Bleb Appearance Grading Scale (IBAGS). Ophthalmologist used this standard to grade the bleb images manually so the result is subjective. This work offered a new approach to standardize the system of bleb assessment by computer software. Features related to bleb height, width and vascularity were extracted from the bleb image by using image processing algorithm. The KNN algorithm then used to classify the image according the IBAGS. The proposed method has successfully increased the Cohen’s kappa coefficient from 0.56 to 0.63. Therefore, it potentially reduced the subjectivity of the bleb grading.

Abstract

Latar belakang: untuk mengetahui keberhasilan operasi trabekulektomi pada glaukoma sekunder pasca vitrektomi pars plana dengan silicone oil intravitrealMetode: data dari 7 pasien dengan glaukoma sekunder pasca vitrektomi pars plana dengan silicone oil intravitreal yang telah dilakukan pengambilan silicone oil dan trabekulektomi dengan atau tanpa mitomicin C di rumah sakit Cicendo dari Januari 2009 sampai dengan Februari 2011. Tekanan intraokuler (TIO) sebelum operasi, 1 hari, 1minggu, 1 bulan dan 2 bulan pasca operasi dicatat dan dianalisis. Hasil: empat (57,14%) dari 7 pasien berusia diatas 45 tahun, dengan rata-rata usia 46,14 tahun (rentang usia 29–63). Semua pasien laki- laki. Lima (71,43%) subjek afakia. Onset glaucoma sekunder rata-rata 78 hari (rentang waktu 2–270). rata rata TIO 1 hari pasca bedah 7,86 mmHg , 1 minggu pasca bedah 25,83 mmHg , 1 bulan pasca bedah 17,50 mmHg, 2 bulan pasca bedah 16,33 mmHg. Satu pasien mengalami keberhasilan tanpa pemberian antiglaukoma topikal pasca bedah, 3 pasien mengalami keberhasilan namun dengan penambahan antiglaukoma topikal pasca bedah dan 2 pasien mengalami kegagalan dan 1 penderita tidak diperoleh data TIO setelah 1 hari pasca bedah. Enam subjek (85,71%) terdapat silicone oil di bilik mata depan, empat diantaranya telah terjadi emulsifikasi, 57,1% mengalami keberhasilan penurunan TIO dan 28,4% mengalami kegagalan. Lima subjek dilakukan trbekulektomi dengan MMC dan empat (57,1%) diantaranya mengalami keberhasilan (p=0,427).Simpulan: usia, status lensa, adanya silicone oil di bilik mata depan dan penggunaan antifibrotik tidak mempengaruhi keberhasilan operasi trabekulektomi pada glaukoma sekunder pasca vitrektomi pars plana dengan silicone oil intravitrealKata kunci: Trabekulektomi, glaucoma sekunder, silicone oli, tekanan intraokuler

Abstract

Introduction : The longterm use of topical antiglaucoma might cause ocular surface instability due to active substance or preservative used. Impression cytology examination may reveal superficial epithelial cells on conjunctiva and cornea, including goblet cells. Goblet cell density decrease is the most important parameter on evaluation of ocular surface disorder. Objective : This study was to understand ocular surface remodeling due to active substance of topical antiglaucoma with impression cytology examination among the patient prior and 3 months after therapy. Methods : This was a randomized controlled trial study with single blind masking. A total of 45 eyes from 31 patients were used as subject and distributed onto three groups treatment, which were timolol maleat 0.5%, latanoprost 0.005%, and latanoprost-timolol maleat fixed combination. All topical antiglaucoma in this study were preservative free. Result : There were differences between 3 groups in goblet cells density after 3 months therapy (p=0,030). Goblet cell density in timolol group was lower than latanoprost (p=0,041) and fixed combination (p=0,045). There was no significantly difference between 3 groups in conjunctival epithelial metaplasia degree (p=0,706) and cell to cell contact degree in corneal epithelial cells (p=0.66) after 3 months therapy. Conjunctival epithelial metaplasia degree were increased among group of timolol (p=0,008) and fixed combination (p=0,046). Conclusion : Timolol maleat 0,5% caused lower goblet cell density after 3 months therapy compare with latanoprost and fixed combination. There was no significantly difference in conjunctival epithelial metaplasia and cell to cell contact degree in corneal epithelial cells among these glaucoma treatment groups.

Abstract

Pendahuluan : Glaukoma sudut terbuka primer saat ini masih merupakan kasus utama yang menyebabkan morbiditas okular di negara berkembang. Glaucoma drainage device (GDD) merupakan salah satu penatalaksanaan operasi dalam menangani kasus glaukoma. Tindakan operasi GDD memiliki faktor resiko. Erosi pada implant di konjungtiva dan tube exposed merupakan komplikasi paling sering dalam tindakan operasi GDD. Tujuan : Untuk melaporkan kasus penatalaksanaan tube exposed glaukoma implant Presentasi Kasus : Seorang pria, 50 thn datang dengan keluhan tidak nyaman pada mata kanan. Pasien memiliki riwayat operasi glaukoma implant pada mata kanan 5 tahun yang lalu. Pada pemeriksaan opftalmologis mata kanan didapatkan tube exposed disertai dengan injeksi siliar. Pasien kemudian dilakukan penatalaksanaan untuk memperbaiki glaukoma implant dengan menggunakan graft sklera pada mata kanan. Kesimpulan : Tindakan operasi GDD menjadi salah satu tindakan yang biasa dilakukan untuk penatalaksanaan glaukoma , Tube exposed pada GDD dapat di tatalaksana dengan menggunakan graft sklera.

Abstract

Introduction: Glaucoma in children is a condition that potentially cause blindness. Management of uveitic glaucoma is challenging because of the many mechanisms involved in its pathogenesis Purpose: To report clinical characteristic and management of uveitic glaucoma in children with Blau Syndrome. Methods: A 13 years old boy came to the hospital with chief complain blurred vision in both eyes. The patient was diagnosed as Blau Syndrome one year ago. He had history of trabeculectomy on his left eye. Scalling skin and swollen joints was found on physical examination. Visual acuity was hand movement in both eyes, intraocular pressure was 34 (right) and 40 (left). Gonioscopy of the right eye was schwabe line in superior and peripheral anterior synechia in three quadrants. At the left eye, there was scleral spur in all quadrant. At the anterior segment of right eye, there was band keratopathy, posterior synechia 360 o , peripheral anterior synechia, and cataract. While at the left eye, there was bleb, band keratopathy, posterior synechia, peripheral iridotomy, and cataract. Trabeculectomy, 5 fluorouracil, synechiolysis, iris retractor, and membranectomy was performed for the right eye. Conclusion: Uveitic glaucoma in children poses a significant risk of blindness and needs an aggressive treatment to control intraocular pressure and amblyopia therapy to preserve vision.

Abstract

Introduction: Angle recession is a common finding after blunt trauma and involves a tear between the longitudinal and circular fibers of ciliary body. The incidence of angle recession was 24.3%. It may occur months to years after ocular trauma. Purpose: To report clinical findings and management of a patient with angle recession glaucoma. Case report: A 51 years old female came with chief complaint of blurry vision of left eye since five months earlier. There was a history of trauma in left eye 26 years ago. She had been treated with antiglaucoma medication by an ophthalmologist. Visual acuity of right eye was 1.0 and left eye was light perception. Applanation Tonometer Goldmann of left eye was 42 mmHg. Slit lamp examination revealed traumatic iritis and lens opacity. Gonioscopy revealed widening of ciliary body band in three quadrants. Funduscopy showed cup/disc ratio enlargement and RNFL thinning. This patient was diagnosed as angle recession glaucoma with traumatic iritis and traumatic cataract. Combined phacoemulsification-trabeculectomy with intraocular lens implantation was performed. One month after surgery, intraocular pressure decreased with improvement of visual acuity. Conclusion: Classically clinical findings of angle recession glaucoma were unilateral glaucoma with history of trauma and widening of ciliary body band. Surgery is needed in uncontrolled intraocular pressure with medication. Combined phacoemulsification-trabeculectomy decreases intraocular pressure as well as improves visual acuity.

Abstract

Sindroma Schwartz-Matsuo adalah suatu kondisi langka yang ditandai dengan ablasi retina regmatogen, peningkatan tekanan intraokular (TIO), dan sel akuos di bilik mata depan. Pada kasus ini, seorang laki-laki berusia 46 tahun datang dengan keluhan penglihatan kedua mata buram. Pasien mempunyai riwayat miopia tinggi yang tidak terkoreksi sejak 30 tahun yang lalu dan maya kanan sudah tidak bisa melihat sejak 20 tahun yang lalu. Dua puluh enam tahun sebelumnya, dia mengalami cedera benda tumpul di mata kirinya. Tajam penglihatan mata kanan nolight perception (NLP) dan 1/300 untuk mata kiri dengan reaksi bilik anterior positif. TIO kedua mata masing-masing adalah 48 dan 28. Pemeriksaan segmen posterior menunjukkan ablasi retina di seluruh kuadran kedua mata. Pasien menjalani operasi pars plana vitrectomy pada mata kiri. TIO mata kiri kembali normal setelah operasi. Kesimpulannya, sindrom Schwartz-Matsuo merupakan penyebab penting glaukoma sekunder. Anamnesis menyeluruh, pemeriksaan mata, dan pencitraan okular diperlukan untuk menegakkan diagnosis dan pengobatan yang akurat.
