Abstract

Background: Trabeculectomy is a surgical therapy for glaucoma to preserve visual function by lowering intraocular pressure (IOP). In some studies, the success of trabeculectomy in lowering IOP is greater than medication. Success is defined by IOP <21 mmHg, with or without glaucoma medication. Primary glaucoma based on the mechanism of aquous humor outflow is divided into primary open-angle glaucoma (POAG) and primary angle-closure glaucoma (PACG). This study aimed to know the success rate of trabeculectomy in POAG and PACG.Methods: This study was a descriptive study conducted at Cicendo Eye Hospital using medical record of POAG and PACG patients who underwent trabeculectomy surgery on January–December 2013 with minimal one month follow-up. Data collection was conducted during September 2014. Data processed in this study were 100 eyes from 76 patients with diagnosis POAG and PACG.Results: The success rate for trabeculectomy in POAG was 79% and PACG was 86%, failure (IOP ≥ 21 mmHg) 21% in POAG, and 14% in PACG for period 2013 at Cicendo Eye Hospital.Conclusions: The success rate of trabeculectomy at Cicendo Eye Hospital is good in one month, with or without glaucoma medication after surgery. Abstract

Tandur konjungtiva bulbi merupakan baku emas pada pembedahan pterigium yang secara umum metode penempelannya dengan menggunakan jahitan, namun memiliki beberapa kekurangan, di antaranya waktu pembedahan cukup lama, menimbulkan reaksi inflamasi, dan kemungkinan komplikasi. Saat ini dikembangkan penggunaan lem fibrin untuk penempelan tandur konjungtiva bulbi sebagai alternatif prosedur pengganti jahitan. Penelitian ini bertujuan untuk mengetahui derajat hiperemis pascabedah pterigium inflamasi antara teknik lem fibrin otologus (LFO) dan teknik jahitan. Penelitian ini merupakan uji klinis acak terkontrol tersamar tunggal yang dilaksanakan di Pusat Mata Nasional Rumah Sakit Mata Cicendo Bandung dari bulan Oktober−Desember 2010. Subjek penelitian dibagi menjadi dua kelompok secara acak dan hasilnya terdapat 12 penderita kelompok LFO dan 14 penderita kelompok jahitan. Pemantauan dilakukan pada minggu pertama, kedua, dan keempat pascabedah serta dilakukan pengambilan foto lampu celah biomikroskop digital. Benang jahitan disamarkan menggunakan perangkat lunak penyunting foto dan satu orang pengamat menilai secara objektif derajat hiperemis pada foto digital. Analisis statistik dilakukan menggunakan Uji Mann Whitney. Hasil penelitian menunjukkan bahwa derajat hiperemis secara bermakna lebih kecil pada minggu pertama, kedua, dan keempat pada kelompok teknik LFO (derajat hiperemis 2,5; 2; dan 1,5) dibandingkan dengan kelompok teknik jahitan (derajat hiperemis 4; 3; dan 2) (p<0,05). Simpulan, penggunaan LFO untuk melekatkan tandur konjungtiva bulbi pada pembedahan pterigium inflamasi menghasilkan derajat hiperemis yang lebih kecil dibandingkan dengan penggunaan jahitan. [MKB. 2013;45(3):174–9]Kata kunci: Lem fibrin otologus, pterigium inflamasi Comparison of Hyperemia Degree between Autologous Fibrin Glue and Suture Technique Post Inflammed Pterygium SurgeryConjunctival autograft is the gold standard in pterygium surgery which is regularly secured with suture, butthis method has few drawbacks of prolonged operating time, provoke ocular inflammation and potential risk for suture related complication. The use of fibrin glue has become an alternative procedure in conjuntival graft transplantation. The aim of this study was to compare hyperemia degree post inflamed pterygium surgery between autologous fibrin glue (AFG) and suture technique. This was a randomized, controlled, single blind clinical trial that conducted in National Eye Center, Cicendo Eye Hospital Bandung from October−December 2010. Subjects were randomly assigned to two groups and as result 12 patients belong to AFG group and 14 belong to suture group. Digital slit-lamp photographs were taken at 1st week, 2nd week and 4th week postoperatively for observation. Sutures were masked using photo-editing software and one masked observers objectively graded the digital photograph for degree of hyperemia. Statistical analysis was performed using Mann Whitney Test. The results of this study showed that the degree of hyperemia was significantly lower in AFG group (hyperemia degree 2.5, 2 and 1.5) than in suture group (hyperemia degree 4, 3 and 2) at 1st week, 2nd week and 4th week post operatively (p<0.05). In conclusion, the use of AFG for graft fixation in inflamed pterygium surgery produced significantly lower hyperemia degree. Abstract

Sindroma Schwartz-Matsuo adalah suatu kondisi langka yang ditandai dengan ablasi retina regmatogen, peningkatan tekanan intraokular (TIO), dan sel akuos di bilik mata depan. Pada kasus ini, seorang laki-laki berusia 46 tahun datang dengan keluhan penglihatan kedua mata buram. Pasien mempunyai riwayat miopia tinggi yang tidak terkoreksi sejak 30 tahun yang lalu dan maya kanan sudah tidak bisa melihat sejak 20 tahun yang lalu. Dua puluh enam tahun sebelumnya, dia mengalami cedera benda tumpul di mata kirinya. Tajam penglihatan mata kanan nolight perception (NLP) dan 1/300 untuk mata kiri dengan reaksi bilik anterior positif. TIO kedua mata masing-masing adalah 48 dan 28. Pemeriksaan segmen posterior menunjukkan ablasi retina di seluruh kuadran kedua mata. Pasien menjalani operasi pars plana vitrectomy pada mata kiri. TIO mata kiri kembali normal setelah operasi. Kesimpulannya, sindrom Schwartz-Matsuo merupakan penyebab penting glaukoma sekunder. Anamnesis menyeluruh, pemeriksaan mata, dan pencitraan okular diperlukan untuk menegakkan diagnosis dan pengobatan yang akurat.

Abstract

ABSTRACT Cystic Bleb Is An Abnormal Bleb Morphology Following Trabeculectomy Procedure. This Type Of Bleb Characterized By An Elevated, Thin, And Avascular Bleb Surface. Post-Operative Bleb Evaluation Needed To Be Done Thoroughly To Recognize And Prevent Further Complication Which Can Be Sight Threatening. Several Options Of Management Include Medical Therapy And Surgical Repair. To Report The Management Of Cystic Bleb. The Study Design Is A Case Report Study, Which Was Conducted At Indonesia National Eye Centre, Cicendo Eye Hospital, Bandung, Indonesia. Subject Of The Study Was A-17-Years-Old Female That Presented With Discomfort Sensation In The Right Eye, Which Previously Has Undergone Trabeculectomy Procedure With Antifibrotic Agent Application. Ophthalmological Examination Revealed A Cystic Bleb On The Right Eye. After Medical Therapy Was Given, The Symptoms Persist, Then The Cystic Bleb Was Surgically Repaired. Follow-Up Examination Showed An Improvement Of The Patient’s Condition, Measured By No Further Complaint Of Discomfort Sensation, A Good Bleb Morphology, And A Stable Intraocular Pressure. Cystic Bleb Is One Of The Complication From Trabeculectomy Which Could Progress To Devastating Condition. Abstract

Introduction & ObjectivesCyclodestructive laser is one of the last resource managements for refractory glaucoma, especiallywith low visual potential. Conventional laser using CW-TSCPC showed unstable IOP reduction withcollateral tissue damage. MP-TSCPC using on and off cyclic waves is considered to have a gooddegree of IOP reduction and fewer complications. The purpose of this study is to assess the IOPreduction after the MP-TSCPC procedures for patients with refractory glaucoma. MethodsThis is a prospective observational study for refractory glaucoma patients treated with the MPTSCPClaser procedure. Measurement of IOP was carried out preoperatively and on postoperativedays 1, 7, and 30 with the gold standard Goldmann applanation tonometry. ResultsA total of 33 eyes from 33 patients were enrolled in this study. The mean baseline IOP was 48.45 ±12,79 mmHg. The mean degree of IOP reduction on follow-up days 1, 7, and 30 were 14.39 ±23.52%, 45,83 ± 24,80 and 27.09 ± 23.86% respectively. On the 30th day after the laser treatment,no complications were observed. The number of anti-glaucoma medication was reduced from 2.39 ±0.49 at the baseline to 1.91 ± 0.58 at one month follow up. ConclusionMicro-pulse transscleral cyclophotocoagulation laser showed a good degree of IOP reduction with noincidence of complication observed at day 30.
