Abstract

TIO mata kiri kembali normal setelah operasi.

Abstract

Diabetic retinopathy (DR) is the major microvascular complication of diabetes mellitus (DM) and responsible as the leading cause of vision loss among working-age adults. With the estimated DM cases reaching 578 million in 2030, public health systems are faced with challenges of increasing costs of implementation and maintenance of DR screening program in people with DM. This study aimed to describe characteristics and risk factors of DR among patients of a Primary Health Care Center in West Java, Indonesia. This was a cross-sectional study during the period of March 2021 until June 2022 on 1,080 participants. Among these participants with DM, 28.89% (25.16–33.12% [95% CI]) were classified to have DR. A total of 32.69% (28.14–36.71 [95% CI]) participants with DR had vision threatening DR (VTDR). The prevalence of DR in this study was higher in women (77.23%) with a mean age of 57.26 ± 9.17 and duration of DM of ≥5-years (56.01%), blood glucose level <200 mg/dL (63.79%), high systolic blood pressure (52.03%), high diastolic blood pressure (39.07%), normal BMI (55.5%), high waist circumference (43.7%), and high HbA1C (3.42%). A total of 1,041 (96.39%) participants were using antidiabetic drugs, and 9.63% of them were currently smoking. This study showed that diastolic blood pressure, abnormal abdominal circumference, and high HbA1C levels were more prevalent in DR and VTDR groups. The findings of this study represent the current characteristics of DR patients in West Java and can be used as a baseline or comparison data for other regions in Indonesia.

Abstract

This study examines the historical trajectory of accounting development in Indonesia and how it has evolved through institutional reform, global convergence, and technological transformation. Using a qualitative historical approach and content analysis of academic and regulatory sources, the study identifies eight major phases of accounting evolution, from the colonial era to the digital transformation period. The findings reveal that Indonesia’s accounting development has been shaped by political transitions, economic reforms, and technological advances—particularly the adoption of International Financial Reporting Standards (IFRS) and automation systems. The study uniquely integrates institutional change theory and global convergence theory to explain how domestic accounting practices internalize international norms while retaining local identity. This research contributes to accounting historiography in emerging economies and highlights the strategic role of accountants in digital governance, education, and sustainable professional development.

Abstract

Introduction : Central serous chorioretinopathy (CSC) is an idiopathic condition mostly affecting men in their 20s to 50s. It is characterized by serous fluid accumulation behind the neurosensory retina resulting in a localized macular detachment. Aldosterone-receptor antagonist and photodynamic laser therapy are the current mainstay of treatments, although most CSC cases can resolves spontaneously within 2 to 3 months. Low power mode (LPM) laser has emerged as a new effective technology to treat CSC. Case Illustration : Case 1: A 50-year-old male was diagnosed with recurrent chronic CSC since 2 months ago. His visual acuity declined even after given oral aldosterone receptor antagonist for two weeks, so he was scheduled to undergo LPM. After three weeks of laser treatment, CSC resolved and the visual acuity improved (Figure1). Case 2 : A 47-year-old male complained of decreased vision and metamorphopsia on his right eye since two weeks ago. He was diagnosed with CSC and underwent LPM. Three weeks after LPM, his vision improved and the metamorphopsia resolved (Figure2). LPM helps the migration of retinal pigment epithelium (RPE) cells to restore the outer blood-retinal barrier through expression of heat shock proteins. We used MC-500ViXi laser system (Nidek Co., Gamagori, Japan) and the laser parameters used in this study were 100?m spot size, 0.01s exposure time, 0.25-0.5 spacing, and 30% LPM ratio. Discussion : Conclusion : LPM is a new yet promising treatment modalities to treat acute and chronic CSC. Its low power nature makes LPM a safe choice of treatment with less damage to the surrounding retina.

Abstract

Ocular tuberculosis occurs in 1.5-18% of all extrapulmonary tuberculosis cases, with orbital tuberculosis being a very small proportion of all cases of ocular tuberculosis. High variability of manifestations of orbital tuberculosis may result in delayed diagnosis. Children are the populations most affected by this disease. This study aims to report a case of orbital tuberculosis in a patient without systemic tuberculosis symptoms. We presents a 13-year-old girl who is reported protrusion of the right eye for four months. She complained diplopia and pain in the right eye radiating to the head. She reported no other systemic symptoms. Ophthalmological examination revealed that her best corrected visual acuity of the right eye was 0.25, and the left eye was 1.0. The movement of the right eye was limited to the superior, temporal, and nasal directions. Anterior segment examinations demonstrated proptosis of the right eye with 3 mm of lagophthalmos. The patient then underwent an orbitotomy and biopsy procedure. Histopathological analysis revealed chronic granulomatous inflammation with Langerhans cells, indicative of tuberculosis. The patient was diagnosed with proptosis of the right eye and multiple cranial nerve palsy due to orbital tuberculosis. The patient was subsequently prescribed an antituberculosis treatment and attended regular monthly follow-ups. Ten months after surgery and completed antituberculosis treatment, the proptosis improves and there is no further growth of the mass. Orbital tuberculosis is a rare condition and is often challenging to diagnose, particularly in healthy individuals. Thorough history taking, physical examination, and ancillary testing are essential to establish an accurate diagnosis. This case emphasizes the necessity for heightened suspicion of tuberculosis in atypical presentations, particularly in pediatric patients in endemic areas, to prevent morbidity and ensure timely treatment.
