Abstract

Diabetes mellitus patients often have hypercoagulable blood, as evidenced by the increased coagulation, impaired fibrinolysis,endothelial dysfunction and platelet hyperactivity. Hyperactive platelet is the major determinant of pro thrombotic state in DM. Byassessing the MPV and platelet aggregation, which is a marker of platelet activity, in patients with type 2 DM, it is expected to help theprediction of acute events. This research is aimed to know the differences of MPV and the aggregation of platelet between poor glycemiccontrol as well as good the control group in type 2 DM patients. This study was conducted in cross sectional method using 22 people withgood glycemic control and 28 people with poor one (glycemic control) from June to August 2013. Fasting blood samples were analyzedfor CBC, HbA1c, TG and platelet aggregation. MPV and platelet aggregation value were compared between groups using independentt-test. Based on this study, there is no significant difference in MPV and platelet aggregation between groups (p=0.598, p=0.464 (1 μM),p=0.868 (2 μM), p=0.984 (5 μM), p=0.401 (10 μM)). Mean Platelet Volume (MPV) correlate significantly with platelet aggregationat 1 μM and 5μM ADP concentration in good glycemic control group (r=0.591; p=0.004 at 1 μM ADP and r=0.521; p=0.013 at 5 μMADP). Mean platelet volume correlate significantly with the platelet aggregation at 2 μM ADP and the concentration in poor glycemiccontrol group (r=0.405; p=0.033). There are no significant differences in MPV and platelet aggregation between groups, but there is asignificant correlation between them (MPV and platelet aggregation) in the good glycemic control of the type 2 DM group.

Abstract

D-dimer terlibat di tahap awal patofisiologi proses Infark Miokard Akut (IMA). Kenaikan Kadar D-dimer di IMA mencerminkanadanya trombosis. Penelitian ini bertujuan untuk mengetahui perbedaan kadar D-dimer plasma di IMA dengan ST elevasi dan tanpaST elevasi. Penelitian ini berupa analitik observasional dilakukan secara potong lintang di Instalasi Gawat Darurat RSUP. AdamMalik Medan, masa waktu bulan April–September 2015. Sampel dikelompokkan menjadi 2 kelompok yaitu sampel dengan diagnosaNSTEMI berjumlah 18 sampel dan sampel dengan diagnosa STEMI berjumlah 18 orang. Semua sampel diperiksa Kadar D-dimer plasma.Penelitian ini menunjukkan ada perbedaan kadar D-dimer plasma di IMA dengan ST elevasi (STEMI) dan tanpa ST elevasi (NSTEMI)yaitu kadar D-dimer di kelompok NSTEMI adalah 440,39±209,33 dan kelompok STEMI adalah 654,89±229,88 (nilai p<0,05). Kadarrerata D-dimer di kelompok STEMI lebih tinggi daripada kadar D-dimer di kelompok NSTEMI.

Abstract

Elevated fibrinogen levels is related to the blood hyperviscosity, that causes low blood velocity. Non-contrast-enhanced transcranialDoppler (TCD) is used to evaluate blood flow from the cerebrovascular system. To know the relationship between fibrinogen levels andexamination of TCD in acute ischemic stroke through evaluation. A cross sectional study was admitted from July 2012-Juny 2013.The researchers determined the differences between fibrinogen and TCD in the stroke group and control. The researchers examinedthe relationship between fibrinogen and TCD examination in the stroke group. The fibrinogenwas measured with Clauss method. TheTCD was examined due to middle of the cerebral artery (MCA) and the Internal Carotid one Artery (ICA). The patients were diagnosedas ischemic stroke from head CT-scan. Statistical analyses employed the Independent T test, Anova test and Pearson correlation. Theresearchers had 24 patients and 24 controls that the Fibrinogen levels in stroke group is 549.16±104.84 mg/mL and in the control groupis 385.64±16.80 mg/mL. The researchers examined MCA in the stroke as well as the control and found the mean velocity 43.12±21.03and 56.97±6.22 (p=0.05), the peak velocity 74.17±32.58 and 94.55±14.11 (p=0.05) end diastolic velocity 23.27±12.66 and35.30±7.34 (p=0.00). In ICA, the stroke group and control and found the mean velocity 31.40±8.86 and 43.07±8.06 (p=0.00), thepeak velocity 54.99±11.50 and 75.04±16.04 (p=0.00) end diastolic velocity 18.23±7.67 and 25.64±5.24 (p=0.00). The correlationbetween fibrinogen and TCD in the stroke group was not significant on MCA and ICA, P>0.05. It can be concluded that the differencesbetween fibrinogen levels and TCD in the stroke group and control are significant. But there is no correlation between the fibrinogenand TCD in the stroke group.

Abstract

Varieties of the laboratory test can be applied in the detecting of iron status. These test are divided into conventional hematologicindex and biochemical iron serum. With the development of an hematological analyzer, new parameter has been found to monitor theiron status with reticulocytes hemoglobin analysis (Ret-He). To know the diagnostic accuracy of Ret-He and to conclude whether Ret-Hecan be used as a parameter to detect iron deficiency in pregnant women by obtaining certain information. This study used analyticalcross sectional was conducted at Haji Adam Malik Hospital Medan, in March−June 2013. The Applying samples consisted of 64 thirdtrimester pregnant women which divided into two (2) groups: the noniron deficiency group using 28 subjects and in the iron deficiency36 subjects. In this study, the researchers found AUC for Ret-He about 77.7% with the best cut off value of <31.4 pg. Using the cut offvalue the researchers obtained the sensitivity about 61.1%, specificity 75.0%, Positive Predictive Value (PPV) 75.8%, Negative PredictiveValue (NPV) 60.0%, Likehood Ratio (LR) (+) 2.44 and LR (–) 0.51%. Based on this study, can be concluded, that the diagnostic accuracyRet-He is a good parameter to detect iron deficiency in the pregnant women.

Abstract

Thalassemia is a genetic disorder disease which spread in the different parts of the world, including Indonesia. The incidence of β-thalassemia trait in Indonesia is between 3−8%. The objective of this study is to know the incidence of β-thalassemia trait in studentswho performed medical check-up, which obtain by using capilary electrophoresis, to describe the characteristics and to obtain theaverage value, of MCV, MCH and HbA2. This study was an observational study by cross-sectional method, which was carried out at theDepartment of Clinical Pathology Haji Adam Malik Hospital in Medan, performed on July to September 2012, consisting of 560 subjects.The examination included FBC to get a microcytic hypochromic sample (MCV <80 fl, MCH <27 pg), then a quantification of HbA2from microcytic hypochromic sample was done using electrophoresis. From the 560 samples 54% were male and other 46% were female.The average age was 19.25±0.25 years, the percentage based on the races tribe, which including Bataknesse 57.78%, Javanesse 15.74%,Acehnesse 10.73, Malay 9.12%, Karonesse 3.93%, Padangnesse 2.32% and Nias 0.35%. The average of the hematological indices, in the50 subjects were 71.63±7.68 for MCV and 23.27±2.85 for MCH. The quantification of HbA2 in micrositic hypochromic subject showed10 β-thalassemia trait subject (1.8%) with average of HbA2 (4.34±0.25), MCV (62.66±3.41), and MCH (20.11±2.18) as well as four(4) hemoglobin E subjects (0.7%). Based on this performed research on 560 students with the incidence of β-Thalassemia Trait was 1.8%with the average of HbA2 quantification was 4.34±0.25.

Abstract

The hyperactivity of platelet had been seen in patients with metabolic syndrome which can be caused by several factors, such as:insulin resistance, obesity, dyslipidemia and hypertension. The hyperactivity of platelet leads to its aggregation that can be increased therisk of cardiovascular disease. This study is aimed to know the platelet aggregation and mean platelet volume in patients with metabolicsyndrome and obesity by determination 30 patients were choosen for this cross sectional study, those whom attended to the laboratoryand policlinic at H. Adam Malik Medan Hospital, between May 2013 until August 2013. The diagnosis used of metabolic syndromecriteria established by the International Diabetic Federation 2005. From the 30 patients with 15 metabolic syndrome and 15 obesity,four patients were excluded because of their tryglyceride were more than 200 mg/dL. There is no significant differences between theplatelet aggregation with the agonist adenosin difosfat (ADP) in patient with metabolic syndrome and obesity. And there is no significantdifferences of the mean platelet volume values between the metabolic syndrome (9.6±0.93) and the obesity group (9.73±0.74), with pvalue 0.846. Based on this study there is no significant differences between the platelet aggregation and the Mean Platelet Volume values(MPV) in the metabolic syndrome and the obese group

Abstract

Introduction : Hepcidin plays important role in regulating ferrum circulation and ferrum toxicity in thalassemia patient. Soluble transferrin receptor(sTfR) is an ineffective eritropoetic measurement parameter which is more accurate. To find out correlation between Hepcidin and sTfR in •-thalassemia trait patientMethods : This is a cross sectional study which conducted on 28 patient with•-thalassemia trait who consulting in RSUP Haji Adam Malik Medan in March-May 2016. Analysis included full blood count, electrophoresis Hb, Hepcidin,and sTfR serum. Results : There was correlation between Hepcidin and sTfR serum level in •--thalassemia trait patient, this study showed strong correlation (r= 0.613). Statistical analysis found significant correlation between Hepcidin and sTfR level ( p = 0.001). Conclusion : This study showed strong correlation between Hepcidin and sTfR level in •-thalassemia trait patient. Keywords : •-thalassemia trait, Hepcidin, sTfR
