Abstract

Temporomandibular joint osteoarthritis (TMJ OA) is a degenerative condition that affects the temporomandibular joint, causing pain, stiffness, and limited jaw movement. Conventional therapies such as medications, physical therapy, and surgery often only provide temporary solutions and cannot permanently restore joint function. Therefore, regenerative therapy has emerged as a promising approach to address TMJ OA by harnessing the body's natural healing potential. This literature review aims to provide a comprehensive overview of the development of regenerative therapies for TMJ OA, including therapeutic strategies, challenges, and future opportunities. Regenerative therapies for TMJ OA encompass various strategies, such as stem cell injections, biomaterial scaffolds, growth factors, gene therapy, and combination therapies. Although regenerative therapies have shown promising results in preclinical studies, several challenges remain, such as safety, efficacy, and translation into clinical applications. It can be concluded that regenerative therapy offers an innovative and potential approach to addressing TMJ OA by utilizing the body's natural healing capabilities, with the hope of restoring joint function and improving the quality of life for TMJ OA patients in the future.

Abstract

Introduction: Indonesia is in an aging population era marked by an increase in elderly population above 7%. An enhancement of elderly population presents health problems due to organ function deterioration and requires extraordinary attention. General health, nutrition, and oral health status are the most problems claimed by elderly, particularly retired. Objective of this study was to analyze the correlation between the general health, nutrition, and oral health status of elderly. Methods: Analytical descriptive research with cross sectional method was carried out on 36 respondents in Paguyuban Purnabakti, Patrang, Jember, East Java Province. The sampling was done by purposive sampling. Information about social-demography and general health status of respondents were obtained from the questionnaire. The social-demography investigation comprised sex, age, education level and status of residence. While general health observation, it comprised smoking status, general check-up, presence of systemic diseases, blood pressure and blood biochemical parameters.[CS1]  Nutritional status was examined by body mass index (BMI). Oral examination for identifying oral health status included the number of the teeth remaining in the oral cavity, use of dentures, oral hygiene index (OHI-s), and periodontal tissue health (CPITN). Results: The result exhibited that most of the respondents presented good general health status. However, there was no significant correlation between routine general health observation and age (p=0.906). Moreover, there was no significant relationship between blood biochemical profile (blood glucose, total cholesterol, and blood pressure level) with nutrition status (p=..., p=..., and p=... respectively), and oral health status (p=...), except with oral hygiene (p=...). Conclusion: This study is limited to Paguyuban Purnabakti, Patrang, Jember, East Java Province. Although this study could not be generated in all communities, this study concluded no relationship between general health (smoking, systemic disorders, and routine general health observation), nutritional status, and oral health in the elderly. [CS1]Disesuaikan dengan table 2

Abstract

Background: Non-steroidal anti-inflammatory drugs (NSAID) are widely used in temporomandibular joint osteoarthritis management. However, the side effects of NSAIDs on multiple organs need to be anticipated. Curcumin is known for its anti-inflammatory and analgesic potential, comparable to that of NSAIDs. In a previous study, structurally modified curcumin increased the pharmacological effect and simultaneously reduced the toxicity and side effects of curcumin. Pentagamavunon-0 (PGV-0) is one of the active components synthesized by the structure modification of curcumin. Purpose: In this study, we identify the potential target of PGV-0 on the pathogenesis of temporomandibular arthritis characterized by inflammation. Methods: We used a bioinformatics approach to compare the PGV-0 target with curcumin and diclofenac sodium as controls. We identified overlapping gene targets of bioactive compounds (PGV-0, curcumin, or diclofenac sodium) retrieved from the SwissTargetPrediction and GeneCards platforms, specifically for temporomandibular arthritis. An interaction model among targets was developed using the STRING database and Gene Ontology Panther to expound on the bioactive compound’s function on the key signaling pathway. Finally, we formulated a molecular docking prediction between the bioactive compound and the target protein marker derived from the previous analysis using Molecular Operating Environment tools. Results: This study found that curcumin and PGV-0 targeted different molecular pathways in temporomandibular arthritis compared to diclofenac sodium. Curcumin and PGV-0 shared a similar pathway to curcumin by modulating metalloproteinases (MMPs), especially MMP-9 and MMP-13. Moreover, diclofenac sodium influenced cyclooxygenase metabolism. Conclusion: In this study, PGV-0 targeted metalloproteinase in temporomandibular arthritis pathogenesis. This finding underlines the PGV-0 advantage in preventing metalloproteinase-related tissue damage in temporomandibular arthritis.
