Abstract

The Covid-19 virus is transmitted from human to human through close contact with infected people through their respiratory droplets. A person can also be infected by touching an object contaminated with SARS-CoV-2 and then followed by reaching the eyes, nose, or mouth. Implementing the health protocol is necessary to reduce the number of virus transmissions. In addition, to increase the community's immune against Covid-19, WHO and the government have been developing a vaccination program. However, the recommendation from the latest research stated that people still have to obey the health protocol even after being vaccinated. The purpose of this community service was to help the government to increase people's understanding of the importance of implying health protocols, even after vaccination, to suppress Covid-19 cases. The method used is by holding an online event called a Webinar. It followed by a statement with audiences to commit to implementing the health protocol led by the Rector of Andalas University, then called a SIAGA SAKATO declaration. The activity began with a pre-test questionnaire for initial information collection, followed by education about Covid-19 prevention from the experts to the 512 students. The activity ended with the final data collection using a posttest questionnaire to test the student knowledge after the education. Paired t-test analysis showed a significant increase in student commitment after attending the Webinar (P≤0.05). Based on these results, the Webinar increases students' understanding of the importance of the health protocol in reducing the Covid-19 cases.

Abstract

Background. The pathological process underlying myocardial infarction is atherosclerotic thrombosis that causes endothelial dysfunction. In the AMI-STE process, eNOS has functions as the body's defense mechanism to prevent more severe damage to the endothelium. Decreased eNOS levels were found in modifiable AMI risk factors. This study aimed to determine the association of eNOS levels based on modifiable risk factors for AMI-STE patients. Method. This study was an analytical observational study with a historical cohort design. The samples were patients with AMI-STE diagnosis who had undergone PPCI at M. Djamil Hospital Padang from March to June 2022. The eNOS levels were taken from the blood samples. Modifiable AMI risk factors were taken from patient’s medical record. The T test was carried out to determine statistical analysis. Results. The average age of the 38 subjects was 62.53 ± 8.2 years, and 92.1% among them were male, and 52.6% subjects were diagnosed with anterior AMI-STE. The average eNOS level in subjects was 47.47 ± 23.88 (normal level is 48.16 pg/mL). More than half (73.5%) of the subjects in low eNOS group had diabetes mellitus as risk factor (P<0.0001) and most subjects (78.9%) in group with low eNOS had hypertension (p=0.022). Other risk factors, namely dyslipidemia and smoking, were also more common in group with low eNOS. However, statistical tests showed that there was no statistically association between eNOS levels with dyslipidemia and smoking. Conclusion. There was statistically significant association between eNOS levels with diabetes mellitus and hypertension as modifiable risk factors for AMI-STE.

Abstract

Pendahuluan. Emboli paru (EP) merupakan kondisi serius yang sering terjadi pada pasien dengan kanker, seperti adenokarsinoma paru, dan penyakit paru obstruktif kronis (PPOK). Faktor risiko untuk EP termasuk imobilisasi, kemoterapi, dan kondisi hiperkoagulasi yang diinduksi oleh tumor. Diagnosis cepat dengan skoring Wells dan Geneva, serta konfirmasi dengan CT Pulmonary Angiography (CTPA), sangat penting. Elektrokardiogram (EKG) dan ekokardiografi juga membantu dalam mengevaluasi kondisi jantung terkait, seperti hipertensi pulmonal. Kasus ini bertujuan untuk mempresetansi kasus emboli paru pada adenokarsinoma paru kanan beserta penanganannya. Ilustrasi Kasus. Seorang wanita berusia 54 tahun dengan PPOK dan adenokarsinoma paru kanan T4N3M1a stage IVA PS ECOG 1 mengeluhkan sesak napas, nyeri dada, dan batuk hilang timbul selama satu tahun. Pemeriksaan menunjukkan takikardia, takipneu, leukositosis, peningkatan trombosit dan D-dimer, serta indikasi EP dan hipertensi pulmonal dari radiologi dan ekokardiografi. Kesimpulan. Kasus ini menyoroti pentingnya evaluasi klinis dan diagnostik yang komprehensif dalam penanganan EP pada pasien dengan kanker paru dan PPOK untuk meningkatkan hasil klinis dan kualitas hidup pasien.
