Abstract

Increased prevalence of chronic kidney failure from time to time will increase, causing significant economic pressure, both for patients and their families and other stakeholders. This study aims to determine the main factors and several advanced CRF risk factors and indirect costs incurred from a household perspective in CRF patients with hemodialysis at Dr. Drajat Prawiranegara Serang Regional Hospital. This type of research is Case Control Study. Data analysis is carried out using three stages, namely univariate, bivariate, and multivariate. The results of bivariate statistical analysis show that of the 21 CRF risk factors there are 15 factors that are statistically significant with P-value 00 0.005. These factors consist of sociodemography, biophysiology, lifestyle. The results of multivariate analysis, the most dominant causing chronic kidney failure (GGK) is kidney stone disease with a risk of 9.6 times compared to without a history of this disease. Then, NSAID consumption history is 7.8 times, herbal consumption. Indirect costs from the household perspective are seen from the transportation costs of each arrival, the cost of eating during hemodialysis therapy and the purchase of multivitamins or medicines that are not borne by insurance/BPJS, other costs such as lodging and personal care. The total minimum cost for each arrival is Rp. 47,250 Maximum Rp. 1,970,000. The monthly fee is 8 times the minimum total expenditure, which is Rp. 96,000 and maximum spending Rp. 9,120,000.

Abstract

Program Jaminan Kesehatan Nasional (JKN) yang diselenggarakan sejak 2014 telah membuat jumlah pasien di rumah sakit, termasuk RS Amira Purwakarta, meningkat tinggi. Peningkatan akan berlanjut seiring pencapaian cakupan semesta JKN pada 2019 ini harus diantisipasi dengan perencanaan pemenuhan sumber daya manusia (SDM) yang memadai guna memastikan tercapainya kualitas pelayanan kesehatan minimum. Untuk menghitung jumlah keperluan SDM dilakukan dengan metode WISN (Work Load Indikator Staff Need) dan analisis kualitas pelayanan farmasi menggunakan metode wawancara dan survei. Hasil penelitian menunjukkan masih kurangnya personel, yaitu 1 tenaga administrasi dan 3 tenaga teknis kefarmasian (TTK) dan penambahan kebutuhan SDM diharapkan akan memperbaiki kualitas pelayanan kefarmasian. Menurut kepala IFRS dapat dilakukan dengan pelatihan teknis kepada karyawan, pembenahan dan penambahan fasilitas fisik, perluasan area instalasi farmasi dan sistem distribusi terdesentralisasi, dan Evaluasi SOP, serta Evaluasi Penerapan Formularium

Abstract

Abstract Background: The increasing incidence of Coronavirus Disease 2019 (COVID-19) in Indonesia affects the need for hospital inpatient services. One of the parameters of service quality and the use of hospital resources is the length of stay (LOS).Objective: To find the determinants of COVID-19 that affect LOS in hospitals.Methods: Retrospective observational data collection using a cross-sectional study design. The sampling method used was total sampling during the January-August 2021 hospitalization period in the Isolation Room of the General Hospital of Jend. A. Yani Metro City. The data used is medical record data. Bivariate analysis uses Chi Square, while multivariate analysis uses Multiple Logistics Regression.Results: The sample consisted of 443 patients who were treated during the period January-August 2021 in the COVID-19 isolation room at RSUD Jend. A. Yani Metro City. The median LOS is 6 days. Multiple logistic regression analysis shows that the determinants of LOS include severity, diabetes mellitus, heart disease, and D-Dimer.Conclusion: Severity, comorbid diabetes mellitus, comorbid heart disease, and D-Dimer value are the determinants of LOS in confirmed COVID-19 patients in the COVID-19 isolation room at General Ahmad Yani Hospital, Lampung Province.Keywords: Determinants, Coronavirus Disease, LOS, hospital Intisari Latar belakang: Angka kejadian Coronavirus Disease 2019 (COVID-19) yang mengalami peningkatan di Indonesia mempengaruhi kebutuhan layanan rawat inap rumah sakit. Salah satu parameter mutu layanan dan penggunaan sumber daya rumah sakit adalah length of stay (LOS).Tujuan: Untuk mencari determinan COVID-19 yang berpengaruh terhadap LOS di rumah sakit.Metode: Pengambilan data secara retrospektif observasional menggunakan desain studi cross sectional. Metode pengambilan sampel menggunakan total sampling selama periode rawat Januari-Agustus 2021 di Ruang Isolasi RSUD Jend. A. Yani Kota Metro. Data yang digunakan adalah data rekam medis. Analisis bivariat dengan menggunakan Chi Square, sedangkan analisis multivariat menggunakan Regresi Logistik Ganda.Hasil: Sampel berjumlah 443 pasien yang di rawat selama periode Januari-Agustus 2021 di ruang isolasi COVID-19 RSUD Jend. A. Yani Kota Metro. Median LOS adalah 6 hari. Analisis regresi logistik ganda menunjukkan bahwa determinan LOS meliputi; tingkat keparahan, diabetes mellitus, penyakit jantung, D-Dimer.Kesimpulan: Tingkat keparahan, komorbid diabetes mellitus, komorbid penyakit jantung, nilai D-Dimer merupakan determinan terhadap LOS pasien COVID-19 terkonfirmasi di ruang isolasi COVID-19 RSUD Jenderal Ahmad Yani Provinsi Lampung.Kata kunci: Determinan, Coronavirus Disease, LOS, rumah sakit

Abstract

In order to overcome hypertension which has become a serious public health problem, the Government through BPJS Health is taking a proactive approach. One of them is by involving first-level health facilities (FKTP), especially Health centers, to carry out promotive and preventive efforts known as the Chronic Disease Management Program (Prolanis). Prolanis' goal is to encourage people with chronic diseases, for example, hypertension, to achieve an optimal quality of life so as to prevent complications. This study aims to find out that the implementation of Prolanis can indeed increase patient satisfaction with mild and moderate hypertension at the Health center so that it is feasible to be funded by BPJS Health by knowing the characteristics and satisfaction of hypertensive patients at two Puskesmas, namely the Baros Health Center which has implemented Prolanis and the Rangkasbitung Health Center which has not implemented Prolanis. Data were obtained through questionnaires distributed to hypertensive patients in two health centers, namely the Baros Health Center and Rangkasbitung Health Center. The independent variables used in the study are patient characteristics Meanwhile, the dependent variable is patient satisfaction. Data were analyzed by means of different test analyses using SPSS version 23.0. The results showed that the characteristics of the patients were different for the Baros Health Center and Rangkasbitung Health Center. Judging from satisfaction based on its characteristics, age has different satisfaction both at the Baros Health Center and at the Rangkasbitung Health Center. Judging from the patient's satisfaction related to knowledge and also Prolanis services in general for the Baros Health Center area it is higher (satisfied) and similarly, the blood pressure is more controlled compared to the Rangkasbitung Health Center.

Abstract

Penyakit Ginjal Kronis (PGK) merupakan masalah kesehatan global dengan biaya tinggi. Terapi Pengganti Ginjal (TPG) yang paling sering dilakukan yaitu hemodialisis (HD). Saat ini sebagian besar pasien PGK yang melakukan HD dijamin pembiayaannya oleh Badan Penyelengara Jaminan Sosial Kesehatan (BPJS-K) dengan besaran jaminan sesuai tarif Indonesian Case-Base Groups (Ina-CBGs) yang pernah mengalami penurunan pada tahun 2016. Penelitian ini bertujuan untuk mendeskripskan biaya medis langsung pasien yang menjalani hemodialisis untuk mengetahui rata-rata biaya pelayanan selama satu tahun (real cost), mengetahui besaran biaya apabila pelayanan diberikan sesuai standar pelayanan menurut Perhimpunan Nefrologi Indonesia, PERNEFRI (ideal cost) serta membandingkan ideal cost dengan tarif Ina-CBGs untuk RS pemerintah tipe B. Penelitian ini merupakan penelitian deskriptif dari sudut pandang pemberi pelayanan (Rumah Sakit) pada pasien yang memenuhi kriteria inklusi. Data biaya diambil dari Sistem Informasi Manajemen Rumah Sakit (SIMRS) dan data klaim InaCBGs. Analisis data dilakukan untuk melihat rata-rata biaya medik langsung dan membandingkannya dengan biaya untuk pelayanan ideal. Dari 93 pasien yang dianalisis, 51,6% berjenis kelamin laki-laki, usia rata-rata (minimal-maksimal) 48 (20-75) tahun, 86% menikah, pendidikan sekolah dasar atau dibawahnya 49,5%. Biaya medis langsung Rp. 791.967; 9,9% lebih kecil dari biaya klaim InaCBGs. Namun biaya ideal HD rata-rata Rp 958.467; 8,7% lebih besar dari biaya klaim InaCBGs. Kesimpulan penlitian adalah biaya InaCBGs HD masih lebih besar dari real cost, namun lebih kecil dari ideal cost.

Abstract

The era of National Health Insurance (JKN) brought changes in the health care system, which required hospital pharmacists to adapt to drug regulations and planning. Procurement planning and procedures have a significant impact on the availability of drugs and the financial situation of the hospital in drug management. One of the most crucial aspects of the hospital is the availability of guaranteed goods and sufficient drug supplies in order to provide the best service. This research was conducted to evaluate the procurement and availability of drugs at X Hospital as a public hospital and Y Hospital as a private hospital in Cirebon Regency. The analysis used in this study is descriptive-evaluative, namely the research method used to obtain data in a descriptive form and a systematic analysis is carried out to evaluate based on the descriptive data. Data collection was carried out by means of in-depth interviews with the hospital and distributors as external parties who also contributed to fulfilling the availability of drugs. The data is also complemented by the results of field observations and review of document reports related to the National Formulary (Fornas), Hospital Formulus (Forkit), and Drug Needs Plan (RKO). The results of this study found that the Drug Needs Plan, procurement and availability of drugs at X Hospital and Y Hospital were in accordance with the standards. This shows that most of the drugs available and given to patients are in accordance with the drugs listed in the National Formulary. There is a difference in the pattern of procurement. Hospitals conduct procurement twice a month, but private hospitals conduct procurement patterns twice a week. The two hospitals both use the SIMRS and Teramedik management information systems for the evaluation process of drug procurement and availability
