Abstract

Background: Protein Energy Malnutrition (PEM) related to inflammation and physical activities reduction occur commonly among hemodialysis patients. This study aimed to describe Malnutrition-Inflammation Score (MIS) and physical activity of hemodialysis patients.Methods: This study involved 117 patients in the Hemodialysis Unit of Dr. Hasan Sadikin General Hospital Bandung from September to October 2014. Secondary data were collected to examine MIS and physical activity, which was measured by using Baecke Physical Activity Questionnaire. The collected data were presented using frequency distribution.Results: Eighty (68.4%) patients had MIS <6, thirty seven (31.6%) patients had MIS ≥6, and ninety four (80.3%) patients had light physical activity.Conclusions: Most of the patients have MIS <6 and are physically inactive. Abstract

Pengendalian tuberkulosis saat ini terkendala oleh metode diagnostik konvensional yang lambat terutama untuk mendeteksi multidrug resistant tuberculosis (MDR-TB). Deteksi dini MDR-TB sangat penting untuk mencegah penyebaran MDR-TB dan mengurangi angka kematian. Penelitian ini bertujuan menganalisis validitas pemeriksaan polymerase chain reaction genexpert MTB/RIF suatu pemeriksaan molekuler automatis yang cepat untuk mendeteksi MDR-TB. Penelitian dilakukan di Poliklinik Directly Observed Treatment Short- Course (DOTS) Rumah Sakit Dr. Hasan Sadikin Bandung. Subjek penelitian adalah penderita tersangka MDRTB. Sampel penelitian berupa dahak yang dilakukan pemeriksaan mikroskopis, uji kepekaan metode proporsi media Lowenstein Jensen dan polymerase chain reaction genexpert MTB/RIF. Selama periode Agustus 2012 sampai Januari 2013 didapatkan 88 subjek penelitian, kelompok usia terbanyak 25–34 tahun. Hasil pemeriksaan kultur didapatkan 54 sampel tumbuh, 34 sampel tidak tumbuh, dan 3 sampel merupakan Mycobacteria lain dari tuberkulosis. Sebesar 97,5% sampel yang resisten rifampisin juga resisten terhadap isoniazid. Pemeriksaan GeneXpert MTB/RIF menunjukkan sensitivitas 92,3% dan spesifisitas 75% dengan akurasi 88,2%. Simpulan, pemeriksaan GeneXpert MTB/RIF memiliki validitas tinggi untuk mendiagnosis MDR-TB terhadap baku emas uji kepekaan M. tuberculosis metode proporsi pada media Lowenstein Jensen. Pemeriksaan ini disarankan sebagai alat skrining MDR-TB. Early detection of MDR-TB is essential to interrupt MDR-TB transmission and reduce the death rate. The aim of this study was to assess the validity of polymerase chain reaction genexpert MTB/RIF examination, which is a rapid automated molecular examination for the detection of MDR-TB. The study was conducted at the Directly Observed Treatment Short-Course (DOTS) clinic at Dr. Hasan Sadikin General Hospital Bandung. Subjects were patients with suspected MDR-TB. The research sample was sputum which was subjected to microscopic examination, susceptibility test proportion methods in Lowenstein Jensen media and polymerase chain reaction genexpert MTB/RIF examination. During August 2012 until January 2013, 88 subjects were obtained, with most of them were 25–34 years old. There were 54 samples obtained that grew in culture and 34 samples did not grow while 3 samples were other Mycobacteria of tuberculosis. It was also found that 97.5% of rifampin-resistant samples were also resistant to isoniazid. Examination of GeneXpert MTB/RIF showed a sensitivity of 92.3%, specificity of 75% with an accuracy of 88.2%. In conclusion, GeneXpert MTB/RIF examination has high validity for diagnosing MDR-TB against M. tuberculosis gold standard resistance test using the proportion method in Lowenstein Jensen media. Abstract

Severe and critical COVID-19 patients are known to experience hyperinflammatory conditions and endothelial damage primarily characterized by increased levels of IL-6 and D-dimer. This group of patients is also considered at risk of experiencing hemostasis disorders including decreased platelet counts, prolonged PT and APTT, as well as increased fibrinogen. Therefore, this study aimed to determine the correlation between IL-6 and D-dimer in severe and critical COVID-19 patients. The relationship between IL-6 and other hemostasis parameters such as platelet count, PT, APTT, and fibrinogen were also analyzed. A descriptive-correlative observational design was used with a retrospective cross-sectional approach. The subjects were severe and critical COVID-19 patients at Hasan Sadikin Hospital, Bandung treated between January to December 2021 and met the inclusion and exclusion criteria. Secondary data were taken from medical records and the Laboratory Information System (LIS). Correlation analysis between IL-6 and D-dimer as well as hemostasis parameters was carried out using the Spearman test. The results showed that among the total 167 subjects, the median age was 60 years. The number of male subjects was 110 (65.86%), while the most common comorbidity was hypertension (45.51%). The analysis showed a very weak and insignificant correlation between IL-6 and platelets (r= -0.044; p=0.571), IL-6 and PT (r=0.115; p=0.137), IL-6 and APTT (r=0.109; p=0.159), as well as IL-6 and fibrinogen (r= -0.087; p=0.264). However, the correlation between IL-6 and D-dimer was significant (r= 0.199; p=0.010). Interleukin-6 did not correlate with hemostasis parameters but correlated with D-dimer. This means that IL-6 and D-dimer may provide information about the inflammatory response in COVID-19 patients and help monitor disease progression.

Abstract

Background: There are various nucleic-acid-based diagnostic tests used to detect SARS-CoV-2 in COVID-19 patients such as reverse transcription polymerase chain reaction (RT-PCR) and isothermal amplification real time fluorescence assay (IARTFA), with the first-mentioned is considered as the gold standard. The IARTFA is an alternative method using cross priming amplification (CPA) technique under constant temperature, with a simpler procedure and faster result. This study aimed to determine the concordance between IARTFA and RT-PCR in detecting SARS-CoV-2.Methods: This was an observational cross-sectional study with a random sampling method, conducted from August-October 2021 in Dr. Hasan Sadikin General Hospital, Bandung. Data on patients was obtained from medical records. Cohen's kappa test was used to determine concordance between both methods.Results: A total of 30 subjects were collected, including subjects with positive result on RT-PCR and IARTFA (n=15), negative result on both methods (n=5), and with positive result on RT-PCR but negative on IARTFA (n=10). There were discrepancies between both methods in subjects with cycle threshold (CT) values ≥35. However, there was a significant concordance between both methods (Cohen’s kappa coefficient of 1, p = 0.006).Conclusion: In addition to RT-PCR, IARTFA is an alternative method for detecting SARS-CoV-2 with advantages and excellent concordance. In communicable diseases such as COVID-19, along with healthy lifestyles such as isolation and wearing masks, early diagnosis is important to prevent and manage the spread of the disease.

Abstract

Interleukin-6 (IL-6) is a proinflammatory cytokine that plays an important role in hyperinflammation and cytokine storm in Coronavirus Disease 19 (COVID-19) patients. The neutrophil-lymphocyte ratio (NRL) describes the innate and adaptive immune responses. Elevated IL-6 and NLR levels usually indicate a severe clinical condition in COVID-19 patients. Aim of this study was to determine the correlation of IL-6 and NLR with the severity of illness in COVID-19 hospitalized patients. This was a descriptive correlative observational study with a cross-sectional design using secondary data from COVID-19 patients treated in Dr. Hasan Sadikin General Hospital, Bandung, Indonesia, from November 2020 to October 2021. There were 225 subjects who were classified by the severity and analyzed for IL-6 levels and NLR. Median levels of IL-6 at moderate, severe, and critical levels were 4.1 pg/mL, 20.4 pg/mL, and 38.8 pg/mL, respectively. The median NLR at moderate, severe, and critical grades were respectively 4.41, 9.65, and 17.79. The correlation between IL-6, NLR, and severity was 0.441 (p<0.001) and 0.408 (p<0.001). Meanwhile, the correlation between IL-6 levels and NLR in COVID-19 was 0.230 (p<0.001). Thus, IL-6 and NRL levels have a moderate positive correlation with the severity of COVID-19, while IL-6 and NRL have a weak correlation because IL-6 is not the only factor that affects the NRL.

Abstract

Diabetes melitus merupakan masalah kesehatan masyarakat di seluruh dunia. Hemoglobin A1c (HbA1c) adalah derivat dari hemoglobin A (HbA), dengan penambahan glukosa pada rantai ß. Pada tahun 2010 ADA memasukkan HbA1c sebagai salah satu kriteria diagnosis diabetes. Jum Tujuan dari penelitian untuk mengevaluasi metode-metode standar yang digunakan dalam pengukuran HbA1C serta meninjau bagaimana hasil pengukuran tersebut diterapkan dalam praktik klinis. Metode pemeriksaan HbA1c cukup banyak namun yang telah terstandardisasi oleh NGSP dan saat ini banyak dipakai diantaranya yaitu metode immunoassay, ion-exchange HPLC (High Performance Liquid Chromatography), enzimatik, boronate affinity chromatography, dan capillary electrophoresis. Pemilihan metode sangat dipengaruhi oleh populasi pasien dan biaya. Metode yang dapat dipilih untuk populasi hemoglobinopati yaitu metode boronate affinity HPLC dan enzimatik. Pelaporan hasil HbA1c dalam persen sesuai dengan standar NGSP atau mmol/mol sesuai dengan standar IFCC yang keduanya dapat dikonversi satu sama lain dengan master of equation. Hasil pemeriksaan HbA1c dipengaruhi oleh berbagai faktor yang paling penting adalah Hb varian.Kesimpulan dari penelitian ini yaitu standardisasi metode pemeriksaan HbA1c oleh NGSP dan IFCC telah memastikan konsistensi hasil yang memungkinkan konversi nilai HbA1c antar sistem yang berbeda.

Abstract

Resistansi insulin merupakan kondisi metabolik yang ditandai dengan gangguan respons tubuh terhadap insulin, yang berperan penting dalam metabolisme glukosa. Kondisi ini berkontribusi pada perkembangan berbagai gangguan kesehatan, seperti diabetes melitus tipe 2, hipertensi, dislipidemia, dan penyakit kardiovaskular. Studi ini mengevaluasi metode pemeriksaan laboratorium dalam mendeteksi resistansi insulin, mencakup metode langsung seperti Hyperinsulinemic Euglycemic Clamp (HIEC) dan metode tidak langsung seperti HOMA-IR, QUICKI, dan Indeks TyG. Metode langsung memberikan hasil yang sangat akurat, namun membutuhkan fasilitas dan prosedur yang kompleks, sedangkan metode tidak langsung menawarkan alternatif praktis dengan tingkat prediksi yang baik. Selain itu, indeks pengganti berbasis biomarker seperti adiponektin dan leptin terus dikembangkan untuk meningkatkan efisiensi diagnosis. Studi ini menekankan pentingnya pemilihan metode yang tepat sesuai kebutuhan klinis atau penelitian untuk deteksi resistansi insulin yang lebih efektif.
