Abstract

Pulmonary tuberculosis (TB) is an infectious disease commonly found in India, Indonesia, China, the Philippines, and Pakistan. According to the 2021 global report, Indonesia recorded 824,000 TB cases, but only 393,323 cases (48%) were detected, treated, and reported. In Bandar Lampung, with a population of 1,184,949 in 2021, there were 1,463 suspected TB cases in 2022. The case detection rate (CDR) in Bandar Lampung reached 40%, with a cure rate of 43.2% (1,153 patients). There were 48 deaths during treatment (1.8%). In Panjang District, which consists of eight sub-districts, 187 TB cases were identified, with a cure rate of 38.4%. Based on surveys and information from active TB cadres, poor medication adherence is a major factor in low cure rates. Patients often forget to take their medication or stop taking it once they feel better, increasing the risk of multidrug-resistant TB (MDR-TB). The main challenges include a lack of active TB cadres as treatment supervisors (PMOs), the absence of training programs, and the unavailability of a logbook for monitoring treatment. The proposed solution involves recruiting active TB cadres, providing training, offering guidance, and supplying logbooks. The implementation method includes recruitment interviews, cadre training, and mentoring. Each TB cadre is responsible for five TB patients in the continuation phase of treatment. This community service program's outcome was forming a TB cadre group consisting of 10 PMOs in Panjang District. As a result of the cadres' efforts, 45 TB patients (90%) completed their treatment and were declared cured, 3 patients (6%) continued treatment at a referral hospital, and one patient (2%) passed away. This program highlights the vital role of cadres in improving pulmonary TB recovery rates.

Abstract

Abstrak: Tujuan penelitian mengetahui efektivitas handrub berbahan alkohol dengan handrub benzalkonium chlorida terhadap angka kuman pada tangan perawat. Jenis penelitian adalah quasi eksperimen dengan desain one group pre test – post test. Subjek penelitian adalah perawat yang bekerja di ruang rawat inap RS. Imanuel Kota Bandar Lampung, yang dibagi 2 kelompok yaitu kelompok handrub berbahan alkohol dan kelompok handrub benzalkonium chloride, dengan waktu 30, 60, 90, dan 120 detik dan pengulangan sebanyak 6 kali. Bakteri diidentifikasi sampai genus/spesies bakteri. Data dianalisis dengan uji t berpasangan. Hasil penelitian menunjukkan Penurunan angka kuman terbanyak setelah mengaplikasikan handrub berbahan alkohol yaitu pada waktu 30 detik (73,91%), sedangkan setelah mengaplikasikan handrub benzalkonium klorida yaitu pada waktu 120 detik (37,00%). Efektivitas waktu pada 30, 60 dan 90 detik didapatkan perbedaan yang signifikan penggunaan handrub berbahan alkohol dibandingkan handrub benzalkonium klorida, dengan p-value 0,000; 0,001 dan 0,03. Sedangkan waktu 120 detik didapatkan p-value 0,044. Genus/spesies bakteri terbanyak adalah Bacilus sp (28,8%) diikuti S. epidermidis (21,7%) dan P. mirabilis (13,1%), yang paling sedikit P. peneri (9,1%).

Abstract

Abstrak: Target pemerintah dalam program penanggulangan TB untuk eliminasi penyakit TB yaitu pada tahun 2035 dan bebas TB pada tahun 2050. Jumlah penderita TB paru di Bandar Lampung (2019) sebanyak 3.485 kasus, dan tahun 2020 ada penambahan 777 kasus. Kasus covid-19 di Kota Bandar Lampung terus meningkat, pada tahun 2020 telah mencapai 4.034 kasus, dengan angka kematian sebanyak 189 orang. Kelurahan Kangkung merupakan salah satu kelurahan kumuh di Bandar Lampung, dan merupakan daerah kantong TB, serta masih banyak warganya yang tidak menjalankan protocol kesehatan dengan baik, seperti tidak memakai masker, jarang mencuci tangan dan tidak menjaga jarak. Tujuan pengabdian masyarakat yaitu pemberdayaan kader TB dalam masa pandemic covid-19, untuk mengurangi kasus TB di Kelurahan Kangkung. Kegiatan pengabmas ini dilaksanakan selama 8 bulan pada tahun 2021, dengan kegiatan yaitu perekrutan kader TB, pelatihan kader TB, penyuluhan oleh kader TB, penjaringan suspek TB, pemberian alat cuci tangan, masker, handsanitizer, leaflet dan nutrisi kepada kader TB. Hasil kegiatan ini yaitu telah dilatih 10 orang kader TB, peningkatan pengetahuan kader tentang penyakit TB, kader TB telah dapat melakukan penyuluhan kepada pasien TB, dan penjaringan suspek TB ditengah masyarakat, serta kader TB mendapatkan bantuan alat cuci tangan, masker, handsanitizer dan leaflet. Covid-19 cases in Bandar Lampung City continue to increase, in 2020 it has reached 4,034 cases, with a death toll of 189 people. Kangkung Village is one of the slum villages in Bandar Lampung, and is a TB enclave, and there are still many residents who do not follow health protocols properly, such as not wearing masks, rarely washing their hands and not keeping their distance. The purpose of community service is to empower TB cadres during the COVID-19 pandemic, to reduce TB cases in Kangkung Village. This community service activity was carried out for 8 months in 2021, with activities namely recruiting TB cadres, training TB cadres, counseling by TB cadres, screening TB suspects, providing hand washing equipment, masks, hand sanitizers, leaflets and nutrition to TB cadres. The results of this activity were that 10 TB cadres were trained, increased knowledge of cadres about TB disease, TB cadres were able to provide counseling to TB patients, and screened TB suspects in the community, and TB cadres received assistance with hand washing equipment, masks, hand sanitizers and leaflets.
