Abstract

Background: The National Health Insurance (Jaminan Kesehatan Nasional/JKN) managed by BPJS Kesehatan aims to provide equal access for the entire communities accross nation. However, its implementation faces various challenges related to ethical and medicolegal aspects, including service limitations, patient rights, and medical personnel responsibilities. Objective: The study aims to analyze the ethical and legal aspects within The National Health Insurance in Indonesia. Methods: This study uses the scoping review method with the PRISMA-ScR approach. Literature searches were conducted through the PubMed and Garuda databases using the keywords National Health Insurance, BPJS Health, Medical Ethics, and Medicolegal. Included articles were those published in the last 10 years and relevant to the topic of study. Results: Our result shows that some major challenges in health care services in Indonesia include limited health facilities, ease of access for communities in remote areas, and administrative challenges in BPJS claims. From an ethical standpoint, the principle of justice is often debated due to the uneven distribution of services. From a legal perspective, there is still ambiguity in the medicolegal responsibilities of health workers within the health insurance system, especially regarding medical disputes and legal obligations in BPJS-based services. Conclusion: The implementation of BPJS Kesehatan faces ethical, legal, administrative, and service quality challenges. Thus, policy evaluation, strengthened oversight, and improved legal protection and service quality are needed to strengthen the system

Abstract

Asphyxia is the third most common case in forensics. Death from asphyxia can occur due to murder, accident, or suicide. This study aims to determine the description of death cases with asphyxia. This research is a descriptive research with observational study. The data collection technique used was total sampling with a sample of all cases of death with asphyxia examined at Dr. Moewardi Hospital in the period 2017-2022. From 2017 to 2022, there were 105 cases of death with asphyxia. The most cases were in 2020 (23%). The majority of victims were male (85%) and aged more than 50 years (39%). A total of 21 cases (20%) were strangulation asphyxia, 18 (17%) cases of suffocation asphyxia, 3 cases (3%) chemical asphyxia, 10 cases (10%) asphyxia due to disease, and 53 (50%) cases were other asphyxia. Signs of cyanosis were found in 91% of cases, petechiae in 56% of cases, edema in 2% of cases, and fine foam in 13% of cases. The most victims were found indoors with 48 cases (46%), followed by outdoors with 46 cases (44%), and in transportation with 1 case (1%). The three regions that sent the most victims were Surakarta city (63%), Sukoharjo district (10%), and Karanganyar district (10%). Most cases occurred in 2020. Males are most common in asphyxia cases. Other asphyxia is the most common type of asphyxia. In the majority of cases, signs of cyanosis were found. The place where the victims were found was relatively the same between indoors and outdoors

Abstract

Trauma (injury) dilihat dari pengertian secara medis sedikit berbeda dari aspek medicolegal dimana secara medis, trauma atau perlukaan diartikan tidak adanya kontuinitas jaringan. Pengetahuan tentang alat atau benda yang berpotensi menimbulkan gangguan kesehatan seseorang merupakan pengertian trauma secara medicolegal. Berdasarkan Permenkes 290/2008, seluruh tindakan kedokteran, termasuk tindakan kedokteran yang mengandung resiko tinggi, memerlukan persetujuan tindakan kedokteran (Informed Consent). Penelitian ini berupa deskriptif observasional dengan teknik total sampling dimana sampel pada penelitiaan ini adalah seluruh kasus pada tatalaksana trauma kepala tahun 2020-2022 yang memiliki informed consent rekam medis di RS UNS. Analisa data dilakukan secara deskriptif. Didapatkan 106 kasus trauma kepala yang di tatalaksana di RS UNS tahun 2020-2022 dengan kasus terbanyak berupa trauma tumpul kepala sebanyak 99,23% berjenis kelamin laki-laki berusia 12-45 tahun dengan persentase data informed consent tertulis pada tatalaksana kasus tersebut sebanyak 84,91%. Prevalensi informed consent tertulis pada kasus trauma kepala yang di tatalaksana di RS UNS tahun 2020-2022 sebanyak 84,91% dengan kasus terbanyak berupa trauma tumpul kepala.

Abstract

Kematian adalah hal alami yang akan dialami oleh semua manusia. Dengan angka harapan hidup Indonesia saat ini yang mencapai 73,55 tahun. Di Indonesia, pada tahun 2018-2020 terdapat 2.713 kasus penemuan jenazah yang tercatat. Walaupun demikian, laporan dari negara kita belum mencatat karakteristik dari jenazah-jenazah yang di temukan. Metode: Penelitian menggunakan metode observasional deskriptif dengan desain potong lintang (cross-sectional) yang merupakan metode penelitian yang memiliki tujuan untuk menggambarkan prevalensi penemuan jenazah dengan ada tidaknya identitas jenazah tersebut. Teknik pengambilan sampel dengan total sampling. Pengambilan data menggunakan VeR sesuai dengan kriteria. Hasil: Didapatkan total sampling 234 identitas jenazah. Menunjukkan distribusi frekuensi kelengkapan identitas jenazah. Mayoritas jenazah pada penelitian ini memiliki identitas, yaitu sebanyak 209 jenazah (89,3%). Sementara itu, sebanyak 25 jenazah (10,7%) tidak memiliki identitas. Kesimpulan: Jumlah subjek dalam penelitian ini adalah 234 jenazah. Secara umum, prevalensi jenazah yang diperiksa di IKF RSUD Dr. Moewardi Tahun 2017-2022 sebanyak 209 jenazah (89,3%) memiliki identitas dan 25 jenazah (10,7%) tidak memiliki identitas
