Abstract

Nursing care is a series of professional activities aimed at delivering comprehensive services to patients. Nursing care documentation is a critical component of professional nursing practice because it reflects the nurse’s legal and ethical accountability for the interventions provided. In many healthcare settings, documentation completeness remains suboptimal, requiring structured supervisory approaches to improve quality. This study aims to analyze the effectiveness of a reflective supervision model in improving the completeness of nursing care documentation at Hospital X, Kupang. A quantitative approach was employed using a quasi‑experimental pretest–posttest control group design. A total of 60 nurses participated across both groups. Data were collected through direct observation using a validated documentation completeness checklist based on hospital accreditation standards. Statistical analyses included proportion tests, mean difference tests, and logistic regression. Most respondents were female nurses aged 25–35 years, holding a Bachelor’s degree in Nursing and PK II certification. The intervention group showed an improvement in documentation completeness scores from a mean of 23.07 before the intervention to 35.26 after the intervention. There was a significant difference in the proportion of complete documentation between the intervention and control groups (p = 0.000), and a significant increase in the mean documentation score before and after the intervention (p = 0.000). Multivariate analysis indicated that reflective supervision was the main predictor of improved documentation completeness (p = 0.002). In conclusion, reflective supervision is effective in improving the completeness of nursing care documentation. Hospitals should institutionalize reflective supervision as part of routine nursing development programs through director-level decrees or internal nursing policies. Training for ward heads and supervisors is recommended, including 1–2 working days of facilitator preparation covering reflective communication techniques, constructive feedback, and documentation evaluation based on accreditation standards. Keywords: reflective supervision model; nursing care documentation; nurses

Abstract

Suboptimal supervision by ward heads has been identified as a contributing factor to low retention and high turnover among hospital nurses. Strengthening supervisory practices is therefore essential to enhance performance, job satisfaction, and workforce stability within nursing services. This study aims to analyze the influence of ward head supervision on job satisfaction and organizational commitment among hospital nursing staff. This systematic literature review employed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework and the PICOT strategy. Literature searches were conducted through Google Scholar, PubMed, ScienceDirect, ProQuest, and Gale using keywords related to ward head supervision, job satisfaction, organizational commitment, and hospital settings. The review followed a rigorous and structured process, including critical appraisal using the Critical Appraisal Skills Programme (CASP) to assess methodological quality. From the 15 articles reviewed, most studies demonstrated that ward head supervision and leadership exert a positive influence on nurse performance, quality of work life, job satisfaction, and organizational commitment. Structured, supportive, and development‑oriented supervision was shown to reduce burnout, improve compliance with standard operating procedures, and strengthen nurse retention. However, the effectiveness of supervision is highly dependent on the quality, consistency, and contextual appropriateness of its implementation. In conclusion, ward head supervision and leadership play a pivotal role in improving nurse performance, adherence to standard operating procedures, and patient safety. Supportive and well‑structured supervisory practices, combined with effective leadership, contribute significantly to service quality and the overall well‑being of nurses in hospital settings. Keywords: Ward head supervision; Nurse job satisfaction; Nursing service quality.

Abstract

Patients with acute gastroenteritis (GAE) accompanied by hypokalemia require continuous nursing monitoring due to the risk of fluid and electrolyte imbalances that can worsen their clinical condition. Although the use of checklists in nursing rounds has been widely reported, the application of the Primary Care Round Checklist (PCR) as a guide for systematic reassessment in GEA patients with hypokalemia is still rarely reported. This case report aims to describe the implementation of PCR in two adult patients with GEA and hypokalemia who were treated in Ward 7 of X Hospital, South Tangerang in 2026. PCR was implemented by the Nurse in Charge of Care (PPJA) using a structured checklist in each nursing round that covered general condition, gastrointestinal symptoms, hydration status, intake-output balance, electrolyte levels, therapy, patient safety, and continuity of care. The results showed that PCR facilitated systematic documentation and evaluation of patient condition developments, including changes in diarrhea frequency, hydration status, fluid balance, electrolyte levels, response to therapy, and potential safety risks. The use of the checklist also supported communication between nurses in continuity of care. Both patients showed improvement in clinical condition during treatment according to the therapy provided. These findings suggest that PCR has the potential to be a supporting instrument for systematic nursing review and documentation, although research with a larger sample size is still needed.

Abstract

Background: Patient safety and quality of nursing care are key indicators of hospital service performance. The implementation of the Primary Nursing model requires a supporting tool that facilitates coordination, continuity of care, and timely clinical decision-making. The Primary Nurse Control Board (PNCB) is a visual management tool designed to support nursing care management. Objective: To describe the implementation of the Primary Nurse Control Board (PNCB) in improving nursing performance and patient safety among postpartum patients in the maternity ward of Hospital X. Methods: This study employed a case study design involving two spontaneous postpartum patients admitted to the maternity ward of Hospital X in 2026. The PNCB was implemented throughout the nursing process, including assessment, prioritization of nursing problems, intervention planning and implementation, clinical monitoring, interprofessional coordination, and evaluation of nursing outcomes. Results: The implementation of PNCB improved nursing care management by providing systematic patient monitoring, structured shift-to-shift communication, and patient allocation based on dependency levels. In the first case, pain intensity decreased, breastfeeding effectiveness improved, and no signs of infection were observed. In the second case, PNCB facilitated early identification of post-episiotomy bleeding, enabling prompt medical intervention and preventing further complications. Furthermore, PNCB supported discharge planning and enhanced continuity of nursing care. Conclusion: The implementation of the Primary Nurse Control Board contributed positively to improving nursing performance by strengthening team coordination, continuity of care, appropriate workload distribution, and early detection of clinical deterioration, thereby enhancing patient safety among postpartum patients.
