Abstract

Background –Uncontrolled diabetes mellitus will cause various complications, one of which is diabetic ulcers. The incidence of diabetic ulcers in DM patients is still high, namely in 2020 it has reached 25% throughout their lives. Patients will be susceptible to severe infections if they do not understand how to properly care for wounds, so that patients with diabetic ulcers can experience problems in the form of impaired tissue integrity. The development of wound care with modern dressing methods has become a trend now, because this wound treatment uses the moist concept so that it speeds up the wound drying process.Objectives –The case study aims to provide nursing care to patients with grade III diabetic ulcers with the main problem being impaired tissue integrity.Method –This type of case study uses a qualitative research design with a case study approach. The sample in this study consisted of 2 diabetic ulcer patients with conventional wound care for 6 days.Results –The results of case management for 6x8 hours showed that the wound condition had decreased in the BJWAT assessment score, but the wound healing process had not been resolved, namely patient 1 on the first day the BJWAT score was 38 then the sixth day it became 28 , while patient 2 on the first day was 39 then days sixth to 27. The wound healing process does take quite a long time, which can reach 12-20 weeks.Conclusion– The conclusion of this scientific paper is that after treating the wound using 0.9% NaCl solution compressed with metronidazole and gentamicin ointment for 6x8 hours, the wound healing process has not been resolved, but the BJWAT wound assessment score has decreased

Abstract

Diabetic ulcers are a complication of impaired bodily function resulting from high blood glucose that occurs for a long time and is not well controlled. There is a close relationship between diabetes and an increased risk of arteriosclerosis and peripheral arterial disease. The risk increases in active smokers. Macrovascular disorders occur mainly in the infragenicular vessels (tibial and peroneal arteries). Quantitative research using cross sectional and observation on 14 grade II diabetic ulcer patients at the Kitamura Clinic Pontianak to see how smoking habits influence the wound healing process. The smoking habit is one of the main risk factors that can slow down or even inhibit the healing process of diabetic ulcers

Abstract

Diabetic ulcers are open sores on the skin layer to the dermis caused by hyperglycemia and neuropathy. Infection is a barrier to the wound healing process, the incidence of infection in wounds with modern treatments of hydrocolloid and hydrogel type dressings is still high. Ozone has high antibactericidal and antimicrobial properties, is able to penetrate bacterial capsid walls and is expected to kill bacteria on the wound surface.To find the right way to cure diabetic ulcers using a combination of modern methods of dressing and ozonetherapy bagging.Experimental, non-equivalent control group design pre test, post test, population and infinite sample of 25 respondents. Modern dressing used has antibacterial properties and ozone concentration of 60-100ug / ml for 15 minutes using a limb bag was carried out until the 13th day with five times the measurement of the number of bacterial colonies using themethod swab and colonicounter. Data processing using a computer system with the General Linear Model. There was an average difference in the number of bacterial colonies between groups (505.73 ± 172.069 vs 322.21 ± 132.778, p = 0.0061).A combination of modern dressing and ozonetherapy bagging can reduce the number of bacterial colonies and speed up the healing process of the inflammatory phase II diabetic ulcer so that it can be applied in nursing care for diabetic ulcer patients.

Abstract

Background: The 2023 Indonesian Health Survey found that the prevalence of hypertension in the population aged >18 years was 30.8%. Hypertension is the leading cause of death worldwide. The initial symptoms of hypertension are asymptomatic, characterized only by an increase in blood pressure. Most people with hypertension complain of tight necks to headaches that create discomfort in the head area. Objective: To explain nursing care in hypertensive patients with warm compresses and slows stroke back massage in lowering the scale of headaches. Methods: This case study uses a descriptive method by applying an evidence-based practice that focuses on one of the nursing problems. Results: In a case study conducted on 3 respondents who experienced headaches due to hypertension by being given a combination of warm compresses and slow stroke back massage for 6 meetings, there was a decrease in pain scale. In Mrs. W, the initial pain scale was 4 after the intervention to a pain scale of 1. In Mr. K, the initial pain scale of 5 after the intervention became 1. In Mr. M, the initial pain scale was 4 after the intervention, the pain scale became 1. Conclusion: The application of a combination therapy of warm compresses and slow stroke back massage with olive oil is effective in reducing pain.Keywords: Warm Compress; Slow Stroke Back Massage; olive oil; headache; Hypertension.

Abstract

Latar Belakang : Gagal Ginjal Kronik (GGK) merupakan salah satu penyakit yang membutuhkan terapi hemodialisis. Hemodialisis dapat berdampak pada kondisi psikologis pasien dan dapat menyebabkan depresi. Depresi yang tidak ditangani dapat menurunkan kualitas hidup pasien. Tujuan penelitian untuk mengetahui hubungan depresi dengan kualitas hidup pasien gagal ginjal kronik yang menjalani hemodialisis. Metode : Penelitian ini menggunakan desain penelitian kuantitatif dengan metode analisis cross-sectional, dilakukan pada 42 sampel yang dipilih dengan teknik purposive sampling. Skor depresi diukur menggunakan kuesioner BDI-II dan kualitas hidup pasien diukur menggunakan kuesioner KDQOL SF 36. Data penelitian dianalisis menggunakan uji Spearman Rank. Hasil : Hasil penelitian menunjukkan bahwa rata-rata usia responden 54 tahun, berjenis kelamin perempuan (54,8%), tingkat pendidikan terakhir SMA (45,2%), mayoritas tidak bekerja (71,4%) dan lama menjalani hemodialisis >12 bulan (66,7%). Rata-rata responden tidak mengalami depresi (skor=9,93) dan mempunyai kualitas hidup baik (skor=69,97). Terdapat hubungan signifikan skor depresi dengan kualitas hidup pasien gagal ginjal kronik yang menjalani hemodialisis di RSUD dr. Gondo Suwarno Kabupaten Semarang (p=0,001), dengan koefisien korelasi r = -0,489 yang menunjukkan kekuatan hubungan sedang dan arah hubungan negatif. Kesimpulan : Semakin menurun skor depresi, maka kualitas hidup pasien akan meningkat. Disarankan agar institusi pelayanan kesehatan tetap melakukan pengkajian dini mengenai depresi pada pasien GGK yang menjalani hemodialisis karena depresi berpengaruh terhadap kualitas hidup.
