Abstract

Herpes zoster is often associated with neurological manifestation but less frequently associated with motoric nerve involvement. The most common motoric involvement is paresis, seen only in 1-5% of all zoster cases. This case report a 39-year-old male, with recurrent abdominal liposarcoma under chemotherapy, presented with symptoms of fever, and painful and weak right leg for 2 weeks. Vesicles eruption was seen on his right leg nine days after these symptoms occurred. Physical examination revealed groups of haemorrhagic vesicles with erythematous base on the right lower leg. Lumbosacral spine x-ray showed spondylosis with radiculopathy. Electromyography (EMG) examination revealed lower motor neuron total denervation corresponding to right L5 radix. The patient was diagnosed as herpes zoster on right L5-S1 segment, herpetic neuralgia, and segmental zoster paresis with recurrent liposarcoma under chemotherapy. He was treated with oral acyclovir 800 mg five times a day and gabapentin 300 mg twice a day. Physical therapy and rehabilitation were started concurrently. Paresis is a rare complication of herpes zoster. Radicular pain and weakness preceded the skin lesion potentially lead to misdiagnoses. The most frequent diagnosis for patient suffering pain and weakness in the extremities is spinal disorder, such as stenosis and disc herniation. EMG can be helpful to recognise motoric involvement of herpes zoster, and preclude other diagnoses.

Abstract

Efforts to provide electrical energy in remote areas are by utilizing available natural potential such as solar radiation and water flow rates, so that designing a hybrid power plant based on solar power and micro hydro becomes appropriate. This research aims to design an optimal hybrid power generation system by utilizing each energy source. The design method includes analysis of potential solar radiation and river water flow, selection of main components and system design. The measurement results over a period of seven days show an average light intensity of 5106 lux and an average water discharge of 1.24 m3/s. Based on system test results, the PLTS power is 110.27 Watts and the PLTMH power is 170.09 Watts, so the total energy is 280.36 Watts (0.280 kW). In conclusion, the application of hybrid power plants with solar power and micro hydro power in remote areas has great potential.

Abstract

The purpose of this study is to improve learning activities and to improve learning outcomes by implementing the listening team learning model for students of Class VIIIA on the Material of Faith in the Book of Allah in the Subject of Islamic Religious Education at SMP Negeri 1 Nguntoronadi, Wonogiri Regency in the 2024/2025 Academic Year. This study is a Classroom Action Research (CAR) with the application of the Listening Team learning model on the material of Faith in the Book of Allah SWT in the subject of Islamic Religious Education. This learning model has advantages including being able to develop students' cognitive abilities, make children think critically during the discussion process, make children with interpersonal and intrapersonal abilities develop better. Based on the study, it was obtained that in the first cycle it began to be seen that students' abilities mastered the learning material with an average value of 67.33 with a sufficient level of ability. However, there are still students who are less active, less cooperative, less motivated in learning Islamic Religious Education, especially the material of Faith in the Book of Allah SWT. This is because students' mastery of the material and concentration on the material are still lacking. In cycle II with the same material, it can be seen that the learning outcomes of students have increased, where the average value of student learning outcomes is 80.67 with a level of mastery of learning materials classified as good and above the Learning Objective Criteria Completion. Based on the results obtained, the learning outcomes of students in mastering learning materials have made significant progress.

Abstract

Introduction: According to the pathophysiology, weight loss is a specific marker for suspected malignancy. However, this is rarely found in cases of brain tumors. The presence of the blood brain barrier is thought to play a role in differentiating nutritional status in primary and metastatic brain tumors. Therefore, it is necessary to do this research. Aim: Knowing the description, prevalence, differences in nutritional status of patients with primary and secondary brain tumors, and the affecting factors. Methods: Cross-sectional study in patients with primary and secondary brain tumors at Cipto Mangunkusumo General Hospital in 2017-2019. Malnutrition was diagnosed using the GLIM (Global Leadership Initiative on Malnutrition) criteria. Data analysis used Chi-Square/Fisher and multivariate logistic regression. There were 333 subjects: 246 primary and 87 secondary brain tumors. Description of nutritional status of patients with primary brain tumors: 47.6% obese; 6.1% malnutrition; the rest were normal, while in secondary brain tumors: 25.3% were malnourished; 24.1% obese; the rest is normal. Secondary brain tumor at risk of malnutrition with RR 1.257 (95% KI 1.108-1.426), p<0.001. Factors influencing malnutrition were the type of primary/secondary brain tumor, gender, age, decreased consciousness, anorexia, gastrointestinal complaints, intraaxial lesions, multiple lesions, and the location of the lesions involving the frontal lobes. The independently related factor was multiple lesions with an aOR of 3,423 (95% KI 1.124-10.426), p 0.03. Conclusion: The nutritional status of patients with primary and secondary brain tumors was different, with higher levels of malnutrition in secondary brain tumors and higher obesity in primary brain tumors. The number of multiple lesions in the brain affects the occurrence of malnutrition.
