Abstract

Keberhasilan vaksin BCG dalam memberikan perlindungan terhadap tuberkulosis (TB) pada orang dewasa di Indonesia belum optimal (37%) sehingga diperlukan vaksin alternatif yang lebih efektif. Protein rekombinan fusi ESAT6-CFP10 merupakan kandidat vaksin yang potensial. Penelitian dilakukan untuk menguji efektifitas protein rekombinan fusi ESAT6-CFP10 dalam meningkatkan ekspresi IL2 dan IL10 sel T CD8 yang memainkan peran penting dalam respon imun melawan TB. Pengujian kandidat vaksin dilakukan secara in vitro pada peripheral blood mononuclear cell (PBMC) dari kelompok sehat endemik TB, kelompok kontak TB, dan kelompok pasien TB dengan melihat persentase IL2 dan IL10 CD8. Setiap kelompok diberi perlakuan tanpa antigen, PPD, dan protein rekombinan fusi ESAT6-CFP10. Persentase IL2 meningkat secara signifikan dari kelompok sehat, kontak TB, hingga Pasien TB. Sebaliknya peningkatan persentase IL2 antar kelompok yang dipaparkan PPD tidak signifikan secara statistik (p=0,396). Persentase IL10 tidak menunjukkan perbedaan yang signifikan antar kelompoknya baik tanpa paparan antigen (p=0,617), PPD (p=0,351), maupun protein rekombinan fusi ESAT6-CFP10 (p=0,257). Didapatkan persentase IL2 yang tidak berbeda secara signifikan antar perlakuan pada kelompok sehat (p=0,309), kelompok kontak TB (p=0,318), dan kelompok pasien TB (p=0,424). Demikian juga dengan persentase IL10 yang tidak berbeda secara signifikan antar perlakuan pada kelompok sehat (p=0,908), kelompok kontak TB (p=0,352), dan kelompok pasien TB (p=0,776). Hal ini menunjukkan bahwa protein fusi rekombinan ESAT6-CFP10 dapat meningkatkan persentase IL2 tetapi tidak dengan IL10 meskipun secara statistik tidak signifikan.

Abstract

Mengendalikan epidemi tuberkulosis pada anak adalah prioritas kesehatan global. Efikasi vaksin Bacillus of Calmette and Guerin (BCG) sangat bervariasi sehingga memerlukan pengembangan vaksin baru. Antigen rekombinan 38 Kda Mycobacterium tuberculosis sebagai kandidat vaksin harus melalui uji imunogenitas. Tujuan studi ini untuk mengidentfikasi apakah antigen  38 Kda dapat menstimuli ekspresi IFN-γ dan IL-4 limfosit TCD4+ pada kultur PBMC anak dengan kontak TB. Studi ini dilakukan pada kultur PBMC dari 8 kontak TB dan 8 anak sehat yang diinduksi oleh antigen 38 Kda (kelompok 1), PPD (kelompok 2) dan tanpa perlakuan (kelompok 3). Ekspresi IFN-γ dan IL-4 limfosit TCD4+ diukur dengan flowcytometry. Rerata kadar IFN-γ untuk kontak TB tertinggi pada kelompok 3 (p=0,76), sedangkan rerata IL-4 tertinggi pada kelompok 2(p=0,68).RerataIFN-γ untukkelompok sehattertinggi pada kelompok3(p=0,78) sedangkanrerata IL-4 tertinggi pada kelompok 2 (p=0,32). Rerata ekspresi IFN-γ dan IL-4 yang diinduksi oleh antigen 38 Kda, masing-masing lebih tinggi pada kontakTB daripada subjek sehat (p=0,62 dan 0,39). Pengaruh respon imun yang protektif ditunjukkan oleh rasio ekspresi IFN-γ dan IL-4 yang lebih dari 1, baik pada kontak TB dan sehat (1,22 dan 1,28). Tidak ada perbedaan signifikan antara perlakuan pemberian antigen38 kDa, PPD dan tanpa perlakuan. Dapat disimpulkan bahwa antigen 38 Kda dapat menstimuli  ekspresi IFN-γ dan IL-4 limfosit TCD4+pada kultur PBMC kontak TB. Kata Kunci: Antigen rekombinan 38 Kda, Mycobacterium tuberculosis, IFN-γ, IL-4, limfositTCD4+

Abstract

The purpose of this research is to report the influence of BLS training on the knowledge and life support skills of senior high school students.. We did an experimental study in one of the senior high schools in Malang City. First-year until third-year students conducted a BLS training. The use of automated external defibrillators and chest compressions were addressed in a 3.5-hour simulation-based session. We used pre-and post-tests to evaluate the outcome of BLS training with multiple-choice questions. Student t-tests were used to compare scores. According to our analysis, the average score before and after the test went from 80 to 94.19 (p<0.001). Following the instruction, over 90% of the students were able to carry out every step of automated external defibrillation and BLS. To sum up, high school students can learn BLS through a simulation-based method; the training validates their attitude and equips them with essential knowledge and skill sets, enhancing their confidence in BLS performance.

Abstract

Background: The recombinant Ag38 protein developed from a local strain of Mycobacterium tuberculosis has great potential to be used as a sero-diagnosis agent using the antigen rapid test because it has several epitopes that bind to antibodies. However, it is not yet known which antibody Ag38-recombinant binds maximally between IgA and IgG.Objective: The aim of this study is to compare the interaction between IgG and IgA on Ag-38 rec in silico as a basis for the selection of sero-diagnosis agents in the TB rapid test.Methods: The results show that the protein PstS1 has a higher binding sensitivity to IgG based on one of the docking models which shows a docking score of -229.70, a confident score of 0.8312 and RMSD 1.060 A. The ramachandran plot also shows that testing on this model has a protein structure that is good, with disallowed regions [X,X] values of 0.5% (less than 0.8%). The results of this analysis show that the most favored regions are 90.5% with a G-factor of -0.27. The quality of the structure of the 3D mooring model can be said to be good because it fulfills the ideal structure requirements.Conclusion: Ag38-rec antigen M. tuberculosis H37Rv binds to IgG more strongly than IgA.

Abstract

AbstractBackground: Platelet aggregation and inflammation have been associated with atherosclerosis, and inflammatory parameters such as mean platelet volume (MPV), platelet distribution width (PDW), neutrophil-lymphocyte ratio (NLR), and mean platelet-lymphocyte volume ratio (MPLVR) are often used. However, the relationship between these inflammatory parameters and the SYNTAX Score in CAD remains uncertain.Objective : The purpose of this study was to investigate the relationship between angiographic SYNTAX score, NLR, and platelet parameters (MPV, PLR, MPLVR).Material and Methods : We conducted a study involving patients undergoing elective coronary angiography from January 2023 to June 2023 at Dr. Saiful Anwar General Hospital, who were 18 years of age or older and had CAD. All participants willingly agreed to be part of the study. We collected complete routine blood counts and performed blood biochemistry assessments 24 hours before the coronary angiography procedure. Pearson's correlation was used to examine the relationship between the variables.Result: A total of 366 individuals meeting the study's inclusion criteria were included. MPV, PLR, MPLVR, NLR, and PDW showed weak to negligible correlations with the SYNTAX Score, displaying correlation coefficients of 0.260, 0.190, 0.320, 0.470, and 0.290, respectively, all with p-values <0.001.Conclusion: This study in individuals with coronary artery disease revealed a limited to negligible correlation between the SYNTAX Score and inflammatory parameters including MPV, PLR, MPLVR, NLR, and PDW.Â
