Abstract

Pendahuluan: Hipertensi atau tekanan darah tinggi adalah peningkatan tekanan persisten pada pembuluh darah arteri, di mana tekanan darah sistolik ≥ 130 mmHg dan tekanan diastolik ≥ 80 mmHg. pada bulan November 2017 terdapat 102 orang ibu hamil yang hipertensi sedangkan pada bulan Desember 2017 terdapat 148 orang ibu hamil yang hipertensi, hal ini menunjukkan bahwa ibu hamil dengan risiko tinggi yang disebabkan oleh hipertensi semakin hari semakin bertambah. Tujuan Penelitian ini untuk mengetahui dan menjelaskan faktor-faktor yang berhubungan dengan kejadian hipertensi pada ibu hamil di Puskesmas Kramat Jati Jakarta Timur Tahun 2019. Metode: Jenis penelitian ini adalah deskriptif analitik dengan disain penelitian cross sectional. Populasi pada penelitian ini adalah seluruh Ibu hamil yang usia kehamilannya 20 minggu keatas, dengan jumlah sampel 94 responden. Pengumpulan data menggunakan rekapitulasi kuesioner. Analisis data menggunakan uji Chi-square dan regresi logistik berganda. Hasil: Hasil penelitian menunjukkan bahwa variabel yang berhubungan kejadian hipertensi pada ibu hamil adalah riwayat hipertensi (p=0,009), paparan asap rokok (p=0,010), obesitas (p=0,000), stress kehamilan (p=0,000) dan paritas (p=0,047). Variabel yang tidak berhubungan dengan kejadian hipertensi pada ibu hamil adalah olaraga ibu hamil, konsumsi garam dan umur. Variabel yang dominan pada penelitian ini adalah obesitas OR 95%CI= 8,911 2,690-29,519. Penutup: Disarankan pada Puskemas agar melakukan promosi kesehatan tentang penyakit hipertensi, dan beberapa faktor risiko yang berhubungan dengan penyakit hipertensi seperti melakukan menjaga berat badan agar tetap ideal, mengurangi interaksi dengan perokok, serta menjaga agar tidak stress dan juga agar melalukan pemeriksaan tekanan darah saat hamil terutama bagi yang memiliki riwayat hioertensi pada kehamilan sebelumnya.

Abstract

Instruments which can be used to identify factors affecting the medication adherence of elderly with diabetes mellitus (DM) have not been discovered in Indonesia. The objective of this study was to develop a questionnaire to identify factors affecting elderly patients’ medication adherence. The development of the questionnaire was conducted based on published references attained through the exploration of research evidence with several keywords and databases. Content validity was conducted using experts’ assessment (two experts). Face validity was conducted in 13 subjects including seven (7) non-elderly subjects and six (6) elderly subjects. Construct validity test was conducted based on calculated R against table R while reliability test was conducted with Cronbach’s Alpha. There were 30 elderly subjects included in the construct validity. Present study produced a valid and reliable questionnaire comprising 35 questions in 14 subcategories from 4 factor domains with the calculated R = 0.357-0.788 and Cronbach’s Alpha = 0.934. The questionnaire can be used for further research throughout Indonesia. Hence, a complete overview of factors affecting elderly DM patients’ medicine consumption adherence can be obtained. The identification of factors related to the usage of DM patients’ medicine is expected to serve as a base for the government, both of state and local government, to produce policies related to health interventions to be implemented.

Abstract

Adherence with medication is one of the important factors in controlling blood glucose levels in patients with diabetes mellitus (DM), which in turn also determines the risk of diabetic complications. The profile and factors that influence adherence behaviour in the use of medication in elderly diabetic patients in Indonesia, especially in East Surabaya, are not yet known. The purpose of this study was to identify profiles and factors that influence the behavior of drug use adherence in elderly DM patients. A cross-sectional study was conducted in 32 Karang Werda or support groups for elderly in East Surabaya. Identification of the respondent’s level of adherence was carried out using the Adherence to Refill and Medication Scale (ARMS) questionnaire while the contributing factors towards adherence behaviour were identified using a published questionnaire. Both questionnaires have been validated in elderly people living in Surabaya. Result of adherence profile analysis was expressed in terms of percentage while determinant factors analysis was identified by using the highest mean among 14 sub-domains included in the questionnaire. In total, 281 elderly diabetic patients were recruited in this study. It was identified that 67.26% of the respondents were classified as non-adherence patients. The dominant factors that contribute to the behavior of medication adherence in elderly diabetic patients were social support, product-related factors, health workers, and emotional factors. Appropriate interventions to optimize the use of medications in elderly diabetic patients need to be identified and implemented according to the identified contributing factors.

Abstract

Background : During the COVID-19 pandemic, many individuals were concerned about being infected. Meanwhile, the elderly felt isolated due to the detrimental effect on their mental and physical health. Therefore, this study aimed to identify the mental health issues suffered by the elderly during the COVID-19 pandemic. The most frequent mental health issues assessed are sleep deprivation, loss of appetite, and psychosomatic complaints.Method : This is a descriptive-analytic study using a cross-sectional approach to find the mental health impact of the COVID-19 pandemic. The population consists of 259 older adults (≥60 years) living in West Java and Jakarta.Results : The average age of study participants is 65.3 years old (±6.8SD range 60–89 years old). The multivariable logistic regression model showed that sleep deprivation is signifantly associated with non-college education background (OR=2.28;95%CI; 1.23-4.61), anxiety (OR=7.09; 95%CI; 3.57-14.08), and the existence of chronic illness (OR=2.75; 95%CI; 1.44 -5.26). Subsequently, the psychosomatic symptom was associated with anxiety (OR=5.27; 95%CI; 2.75 -10.11) and chronic illness (OR=2.80; 95%CI; 1.47 -5.32). Loss appetite was associated with non-college education background (OR=2.50; 95%CI; 1.16-5.41), anxiety (OR=10.41; 95%CI; 5.01-21.63), and the existence of chronic illness (OR=3.60; 95%CI; 1.72-7.55). The analysis showed that none of the COVID-19 related fear is associated with a sleep disorder, loss of appetite, and psychosomatic symptoms.Conclussion : A psychosocial approach is necessary to reduce the mental health issues during the Covid-19 Pandemic, focusing on anxiety management and assisting those with chronic diseases and low education.

Abstract

This study investigated the attentiveness of elderly caregivers in long-term care facilities in maintaining oral function. A cross-sectional approach was used to achieve the study's purpose. The sample size formula accounted for 213 caregivers randomly selected from 18 long-term care facilities in 4 provinces. A self-reported questionnaire of sociodemographic characteristics and an adapted Leopold’s oral care checklist was distributed. The average age of study participants was 35.24 (±11.2 SD; aged 18–70 years), dominated by females (62.9%), and only 39% had experienced long-term care training. Multiple linear regression analysis indicated that younger caregivers likely pay more attention in the swallowing periods (β = -.182), females pay more attention in meal preparation (β = .146), and environmental observation (β = -. 238). Caregivers working in public long-term care facilities and having longer working experience paid more attention in environmental observation (β = .172) and (β = -.161), while training experience showed a significant association with feeding and swallowing periods (β = .291) and (β = .211). This study highlights the importance of training programs for care capacity, particularly oral care.
