Abstract

Length of stay (LOS) can increase hospital rates and become a cost issue for patients, potentially resulting in a discrepancy between hospital rates and INA-CBGs rates at the hospital. This study aims to determine the difference between the actual costs and BPJS claim costs for diabetic patients based on the length of stay and class level. The research method is observational with a descriptive approach. The data collection technique used retrospective medical record data from 50 patients. The results showed that the largest average difference between hospital costs and BPJS claim costs was for patients with a length of stay >5 days in the VIP class, amounting to IDR 9,562,850.00 with an average claim of IDR 6,146,000.00. The highest cost was for patients with a length of stay of 1-5 days in Class III, amounting to IDR 110,718,506.00 with a total claim of IDR 107,948,500.00. The discrepancy between hospital rates and INA-CBGs rates was caused by factors such as control, excessive use of drugs, and disease complications. Therefore, measures to control these cost discrepancies include cross-subsidies, adherence to the BPJS E-catalog, medication based on appropriate duration of administration, Clinical Pathway, referral management, as well as financial collaboration with social funds from the Muhammadiyah Zakat Infaq and Shadaqah Institution (Lazismu). The length of hospitalization for diabetes mellitus patients can increase the hospital rates to be paid and have implications for reducing the effectiveness and efficiency of services. Thus, hospital managers need to implement quality control and cost control for hospital services.

Abstract

Abstract

The cost of treating DM disease is one of the portraits of chronic diseases that require significant medical expenses The burden of treatment costs for DM patients will be more expensive if complications occur because it has an impact on the need for drugs and therapies that last a lifetime. This study aimed to identify the cost components of inpatient treatment of Type II DM patients with Peripheral Circulation complication according to treatment class at PKU Muhammadiyah Bantul Hospital. Qualitative research design with case study design. The sampling technique was carried out by purposive sampling and the data collected were 50 patients. The main data sources were obtained from Medical Records and Financial Administration data from Medical Records, Pusjamkes, and Finance units The cost components analyzed included consultation fees, action fees, treatment fees, support and accommodation with comparisons between inpatient classes. This study showed that the largest cost treatment component of DM patients was Very Important Person (VIP) patients with an average doctor’s consultation fee per patient (Rp.660.500), accommodation costs (Rp2.006.000), and action costs (Rp.4.415.000). Meanwhile, the largest average supporting costs (Rp.3.300.844) and treatment (Rp527.695) were found in class II patients. This indicates that the tendency of the supporting cost components is very dominant because it is influenced by the specifications of the services needed such as laboratory examination costs, and radiology costs which are influenced by blood transfusion factors, hemoglobin and albumin, and the types of antibiotics needed during treatment. Hospital managers can carry out quality control and especially control the costs of drugs and medical consumables by complying with the provisions of patient treatment and care.

Abstract

This study aims to explain the management strategy for assessing the needs of grade VIII students in planning Islamic group counseling services at SMP IT Cahaya Robbani Kepahiang. The background of this research is based on the importance of structured needs assessment as a basis for designing counseling guidance services that are relevant to student development. The research method used is a qualitative descriptive approach with a case study design. Data was collected through interviews, observations, and documentation, then analyzed using the Miles and Huberman analysis model which included data reduction, data presentation, and conclusion drawn. The results of the study show that the management of student needs assessment is carried out in a planned manner through three main stages: planning, implementation, and analysis of assessment results. Needs data were obtained through surveys, observations, and interviews, and then used as a basis for designing Islamic value-based group counseling programs. This strategy has proven effective in directing counseling services to align with the real needs of students, while strengthening their involvement and spiritual awareness in dealing with personal and social problems. Research confirms that a good and structured needs assessment is essential in the development of contextual and meaningful Islamic counseling services for students. The recommendation of this study is that schools should continuously integrate needs assessment in counseling guidance programs, improve the competence of BK teachers in an Islamic approach, and develop an Islamic value-based counseling model that is adaptive to the dynamics of student needs.
