Abstract

During the postpartum period, mothers are at risk of experiencing postpartum complications which are the main cause of maternal death in the world. Some of the things that trigger postpartum complications are postpartum bleeding, postpartum infections, urinary tract infections, uterine subinvolution, thrombophlebitis and pulmonary embolism and postpartum depression. The city of Kupang still has a high maternal mortality rate with the most common cause is bleeding which is a complication during childbirth. One of the programs that have been implemented to reduce maternal and child mortality is the KIA revolution and the Bakunase Community Health Center is one of those implementing the program. This health centre has a relatively high total number of visits for normal delivery and postpartum patients, so correct applicative measures are needed to prevent complications for postpartum mothers who undergo the delivery process here. Looking for the effect of postpartum exercise affects preventing complications in postpartum patients. This research uses a qualitative approach in the form of a descriptive case study. Results: An examination of the patient's general condition was carried out after the two activities above and the results were good and there were no signs of infection. Postpartum exercise affects preventing complications in postpartum patients so the application of good and regular postpartum exercise can increase the prevention of postpartum complications and can reduce maternal mortality due to bleeding which is still a major problem.

Abstract

Latar Belakang: Kanker serviks atau kanker mulut rahim yang disebabkan karena infeksi virus (Human Papilloma Virus) masih merupakan masalah kesehatan perempuan di Indonesia tertinggi setelah kanker payudara, sehubungan dengan angka kejadian dan angka kematiannya yang tinggi. Angka kejadian penyakit ini ini diperkirakan akan terus meningkat dalam kurun waktu 10 tahun mendatang jika tidak dilakukan tindakan preventif yang memadai. Tujuan: untuk meningkatkan pengetahuan masyarakat tentang kanker serviks sehingga meningkatkan motivasi kaum wanita untuk melakukan deteksi dini. Metode : pendidikan kesehatan melalui penyuluhan serta konseling. Hasil : Masyarakat semakin memahami tentang kanker serviks dan kaum wanita sangat termotivasi untuk segera melakukan pemeriksaan IVA. Kesimpulan : Untuk meningkatkan pengetahuan masyarakat tentang kanker serviks diperlukan berbagai cara pendekatan yaitu melalui penyampaian materi secara langsung, pemberian leaflet ataupun media lain yang mudah dipahami peserta dan juga disimulasikan tentang cara deteksi dini kanker serviks.

Abstract

This study aims to analyze the readiness of health human resources (HRH) in East Nusa Tenggara (NTT) Province to achieve the tuberculosis (TB) elimination target by 2030. A descriptive analytic design with quantitative and qualitative approaches was applied, using secondary data from the Global Tuberculosis Report 2023 (WHO), National TB Control Program Report 2023 (Ministry of Health, Indonesia), and NTT Health Profile 2023 (BPS). Results show that HRH ratios in NTT remain below national standards, with TB drug-sensitive treatment success at 84% and drug-resistant TB treatment success at 56%. Preventive therapy coverage for household contacts reached only 37%. The study concludes that strengthening HRH through recruitment, redistribution, continuous training, and community cadre empowerment is essential to achieve TB elimination by 2030.

Abstract

ABSTRACT Tuberculosis (TB) remains a global health problem, including in Indonesia. The East Nusa Tenggara Province Health Center (SITB) reported 10,110 TB cases (56%) in 2024. Approximately 7,851 of the 17,961 TB cases remain undetected or unreported. The number of TB cases in Kupang Regency in 2024 reached 454 (38%), with approximately 755 cases remaining undetected out of the target of 1,209 This gap is influenced by suboptimal utilization of cadre resources, weaknesses in the service system, and the lack of effective strategy development. This study aimed to analyze the relationship between managerial epidemiology and the effectiveness of the tuberculosis program in Kupang Regency in 2025. A mixed-method study with a cross-sectional design was conducted at 29 health service facilities in Kupang Regency, including 27 primary health care facilities and two referral hospitals, selected using purposive sampling. Statistical analysis showed a significant relationship between managerial epidemiology and TB program effectiveness. Of the 11 independent variables studied, 10 were significantly associated with program effectiveness (p 0.05), including planning, management-based and environmental-based implementation, intervention design, identification of causes and risks, selection of epidemiological strategies, monitoring, supervision, evaluation, and surveillance. Despite these findings, the effectiveness of the TB program still needs improvement due to limited resources, suboptimal data analysis, and high community stigma. In conclusion, epidemiologically based planning, intervention strategies, monitoring, supervision, evaluation, and surveillance are directly related to TB program effectiveness in Kupang Regency. Keywords: Managerial Epidemiology, TB Program Effectiveness, Kupang Regency.ABSTRAK Tuberkulosis (TB) masih menjadi masalah kesehatan global, termasuk di Indonesia. Pada tahun 2024, Sistem Informasi Tuberkulosis (SITB) Provinsi Nusa Tenggara Timur melaporkan 10.110 kasus TB (56%), dengan sekitar 7.851 dari estimasi 17.961 kasus yang masih belum terdeteksi atau tidak dilaporkan. Di Kabupaten Kupang, jumlah kasus TB yang dilaporkan pada tahun 2024 mencapai 454 kasus (38%), dengan sekitar 755 kasus yang belum terdeteksi dari target 1.209 kasus. Kesenjangan antara target dan capaian tersebut dipengaruhi oleh belum optimalnya pemanfaatan sumber daya kader, kelemahan sistem pelayanan, serta belum berkembangnya strategi yang efektif. Penelitian ini bertujuan untuk menganalisis hubungan antara epidemiologi manajerial dengan efektivitas program tuberkulosis di Kabupaten Kupang tahun 2025. Penelitian ini menggunakan metode campuran (kuantitatif dan kualitatif) dengan desain potong lintang yang dilaksanakan pada 29 fasilitas pelayanan kesehatan di Kabupaten Kupang, terdiri atas 27 fasilitas pelayanan kesehatan tingkat pertama dan dua rumah sakit rujukan, yang dipilih secara purposive sampling. Hasil analisis statistik menunjukkan adanya hubungan yang signifikan antara epidemiologi manajerial dan efektivitas program TB. Dari 11 variabel independen yang diteliti, 10 variabel menunjukkan hubungan yang signifikan dengan efektivitas program TB (p 0,05), meliputi perencanaan, pelaksanaan berbasis konsep manajemen dan lingkungan, desain intervensi, identifikasi penyebab dan risiko, pemilihan strategi epidemiologi, pemantauan, supervisi, evaluasi, dan surveilans. Meskipun demikian, efektivitas program TB masih perlu ditingkatkan karena keterbatasan sumber daya, analisis data yang belum optimal, serta tingginya stigma di masyarakat. Dapat disimpulkan bahwa perencanaan, strategi intervensi, pemantauan, supervisi, evaluasi, dan surveilans yang berbasis epidemiologi berhubungan langsung dengan efektivitas program TB di Kabupaten Kupang. Kata Kunci: Manajerial Epidemiologi, Efektivitas Program TB, Kabupaten Kupang.

Abstract

In archipelagic regions such as Ternate, where access to healthcare services and diagnostic facilities is limited, tuberculosis (TB) remains a public health problem with high incidence and mortality rates. The risk of TB transmission increases due to geographical conditions, shortages of healthcare personnel, and limited early detection efforts. This community service program aimed to improve early detection of tuberculosis and enhance public understanding of TB prevention and management through community-based systematic screening. The methods used included socialization, health education, screening using portable X-ray, initial examinations, assistance, and referral services for high-risk communities. The activity was conducted in Ternate from May 16 to 17, 2025, involving 112 participants. The results showed that 87 individuals had suspicious lung lesions based on portable X-ray examinations, and 56 individuals were confirmed positive for TB after further examinations. In addition, public awareness regarding TB prevention, symptoms, and transmission increased from 55% before the activity to 93% after the activity. The findings indicate that TB screening using portable X-ray technology is effective in improving early TB detection and increasing community knowledge regarding TB prevention and control.
