Abstract

Abstract

This study aims to analyze the influence of healthcare service quality, healthcare facilities, and community health center (Puskesmas) image on outpatient satisfaction at Sebangki Community Health Center, Landak Regency, West Kalimantan. The research employed a quantitative approach with data collected through questionnaires distributed to 128 outpatient respondents. Data analysis was conducted using validity and reliability tests, multiple linear regression, correlation coefficient, and coefficient of determination. The results indicate that service quality, healthcare facilities, and Puskesmas image have a positive effect on patient satisfaction, both partially and simultaneously. The findings affirm that patient satisfaction can be enhanced through improvements in service quality, the availability and comfort of facilities, and the strengthening of a positive institutional image in the community. Among the three variables, healthcare service quality showed the lowest average value, suggesting the need for greater attention in continuous improvement efforts. The implications of this study may serve as valuable input for Puskesmas management in formulating more effective service strategies to improve patient satisfaction and loyalty.

It is motivated by the importance of effective leadership and organizational support in improving team performance and patient therapy outcomes. This study employed a quantitative method with descriptive and verificative approaches. The population consisted of pharmacy personnel, with total sampling used to obtain 48 respondents. Data were analyzed using multiple linear regression and mediation analysis with the PROCESS Macro (Hayes). Statistical tests included validity, reliability, t-test, F-test, and coefficient of determination using SPSS. The results showed that the head pharmacist’s leadership style and organizational support significantly affect pharmacy team performance. Pharmacy team performance also has a significant effect on the success of drug therapy. In addition, leadership style and organizational support have a direct effect on therapy success. Mediation analysis indicated that pharmacy team performance acts as a mediating variable in the relationship between leadership style, organizational support, and drug therapy success. In conclusion, drug therapy success is influenced both directly and indirectly through improved pharmacy team performance. Therefore, hospitals should strengthen leadership practices and organizational support

Abstract

Giri Emas Regional Hospital Laboratory has been applying local government retribution tariffs since 2023. With improvements in operational services and quality, along with the hospital's change in status, there is an increasing need for the implementation of competitive and strategic tariffs. Objective to identify the unit costs of routine laboratory tests using the Activity-Based Costing (ABC) method and compare them with the local government retribution tariffs in Buleleng Regency. Method: The research was conducted using a descriptive quantitative approach from secondary data on laboratory and management records on direct and indirect costs at Giri Emas Regional Hospital from November 1, 2023, to December 31, 2024. The unit cost calculations reveal that certain laboratory tests have significantly higher costs, such as direct bilirubin (IDR 177,951), triglycerides (IDR 174,955), and total bilirubin (IDR 162,541). In contrast, tests like HCG (IDR 38,972) and Widal (IDR 43,020) have the lowest unit costs. Statistically, the median unit cost at Giri Emas Regional Hospital  is higher than the local government retribution tariffs (median = 69,186 vs 44,000; U = 47, p < 0.01). Giri Emas Regional Hospital should conduct further evaluation of the differences between its unit costs and the retribution tariffs.

Abstract

Long waiting times for outpatient pharmacy services remain a major challenge in hospital service quality and patient satisfaction. At the Outpatient Pharmacy Installation of RS Prima Pekanbaru, increasing prescription volumes have not been matched by service capacity, leading to delays that exceed national standards. This study aims to evaluate outpatient pharmacy waiting times using Lean Management principles and queueing theory to identify sources of inefficiency and propose improvement strategies. A qualitative research design supported by quantitative queue analysis was applied. Data were collected through observations, document review, and in-depth interviews with 11 informants, including pharmacists, pharmacy supervisors, hospital management, prescription verifiers, and patients. The results showed an average prescription arrival rate of 1.717 prescriptions per hour, while the service rate was only 0.912 prescriptions per hour, indicating an unstable queue system. The average processing time for one prescription reached 1.09 hours, exceeding the 60-minute standard for compounded medications. Major inefficiencies were identified in the form of overproduction, particularly at the packaging stage, overprocessing due to repeated data entry caused by incomplete implementation of electronic prescriptions, and defects related to administrative and service flow errors. These findings indicate that optimizing workflow, strengthening human resources, and fully integrating digital systems are essential to reducing waiting times and improving pharmacy service performance.
