Abstract

Shift worker sleep disorder (SWSD) adalah gangguan tidur irama sirkadian yang ditandai dengan insomnia dan kantuk berlebihan yang terjadi pada mereka yang bekerja diluar periode tidur yang tipikal atau disebut juga jam-jam yang non-tradisional yaitu pukul 22.00-06.00. Pekerja shift berisiko lebih tinggi untuk mengalami tekanan darah tinggi, berat badan berlebih dan diabetes bahkan meningkatkan risiko kanker. Gangguan tidur mengakibatkan disregulasi sumbu hipotalamus hipofise adrenal. Pekerja shift harus memperhatikan tidurnya bukan hanya untuk kesehatannya sendiri namun juga untuk mencegah kecelakaan kerja yang dapat merugikan banyak orang. Terapi SWSD meliputi terapi farmakologis dan non-farmakologis. Terapi non-farmakologis mencakup pengaturan jadwal kerja, modifikasi lingkungan, dan menghindari konsumsi kafein, alkohol, serta nikotin. Terapi farmakologis meliputi konsumsi hormon melatonin dan obat-obatan yang dapat memodifikasi tidur. Shift worker sleep disorder (SWSD) is a circadian rhythm sleep disorder characterized by insomnia and excessive sleepiness that occurs in those who work outside of typical hours of sleep period, also called the nontraditional hours at 22:00 until 06:00. Shift workers are at higher risk for developing high blood pressure, excess weight and diabetes, even increase the risk of cancer. Sleep disorders cause dysregulation of the hypopituitary hypophysis adrenal axis. Shift workers must sleep properly not only to preserve their own health, but also to prevent accidents that can harm many people. Therapies SWSD include pharmacological and nonpharmacological therapy. Nonpharmacological therapy include improving work schedules, environmental modifications and also avoiding caffeine, alcohol, and nicotine. Pharmacologic therapy includes the consumption of the melatonin hormone and drugs that can modify sleep.

Abstract

Interprofessional Education (IPE) is a learning process between students from various health workers from different educational backgrounds, that interacting and collaborating to produce and provide promotive, preventive, curative, and rehabilitative health services. The implementation of the IPE program in academic education is important to help develop the concept of collaboration between health professionals to prepare and create better, professional, and competent health workers to deal with health problems. The purpose of this study was to determine how the effectiveness of interprofessional education in increasing the ability and competence of health professionals. The method in this study is a literature study article or narrative review in the form of a study of several research journals related to the effectiveness of interprofessional education on increasing abilities and competition between health professions obtained through Google Scholar, PubMed, and Science Direct. The benefits of IPE activities are students can learn to collaborate and function in a team and bring this knowledge, skills, and attitudes into future practice through enhancing several aspects such as communicating, collaborating, determining the roles and responsibilities of each profession, conflict management, decision making, sharing knowledge and skills and finally mutual respect between friends. Based on the cited literature review, it can be cited that interprofessional education (IPE) is a way to increase competence and collaboration skills between professions so that it can create health professionals who are professional, capable, and able to improve the quality of health services.

Abstract

One of the aims of Interprofessional education (IPE) is to foster collaboration amongst healthcare professionals. IPE has been adopted at Udayana University by involving seven health courses at Udayana University, Bali, Indonesia. These students were assigned to 49 groups which composed the seven health course students. They learned working in collaboration from cases identified in the community. The activities spanned for five semesters. At the beginning of their activities the students received interprofessional collaboration competencies attainment survey (ICCAS). The survey has been validated in the population and has two collaborative competencies, namely communication and collaboration. This was a report of ICCAS employment in Semester III dan IV of the first cohort of IPE Program at Udayana University. 138 students completed the survey in both semesters. A paired-sample t-test was conducted to compare the mean score differences of collaboration and communication in the first and second semesters of attending IPE learning. Whilst, there was no significant differences on the mean score for communication, there was a significant difference in the mean score for pre and post collaboration (94.7±9.7) and (91.0±10.8); t(130)=3.379, p=0.001. These results suggest that the collaboration competencies amongst the seven health courses students reduced after one semester attending IPE activities. These findings indicated that the study design and learning tasks should be amended to ensure students gain the most of their learning in collaboration.

Abstract

Data epidemiologi saat ini menunjukkan adanya peningkatan angka keluhan pasien terhadap kualitas tidur. Diperkirakan setiap tahun, antara 20%-40% orang dewasa mengalami kesukaran tidur, dan 17% diantaranya mengalami masalah serius. Melihat data tersebut diatas, terdapat kemungkinan orang dengan pekerjaan yang membutuhkan konsentrasi serta kesiapan fisik dan mental seperti guru SMA, justru memiliki kualitas tidur yang buruk. Penelitian ini merupakan penelitian deskriptif cross-sectional. Sampel dikumpulkan dengan menggunakan teknik total sampling dengan terlebih dahulu memperhatikan kriteria inklusi dan eksklusi. Kemudian pengumpulan data dilakukan dengan menggunakan kuesioner Pittsburgh Sleep Quality Index untuk menilai kualitas tidur, Epworth Sleep Scale untuk menilai skala mengantuk, serta Depression, Anxiety, and Stress Scale untuk mengukur skala depresi, cemas dan stres. Sebanyak 67 responden telah memenuhi kriteria sampel. 44,78% responden memiliki kualitas tidur yang buruk, dengan karakteristik: tidak terdapat perbedaan proporsi kualitas tidur buruk pada laki-laki maupun perempuan (50%), dan proporsi kualitas tidur buruk yang paling banyak adalah pada kelompok umur di bawah 30 tahun (33,3%), lama bekerja sebagai guru lebih dari 24 bulan (76,7%), bertugas sebagai guru mata pelajaran dan bukan sebagai wali kelas (56,7%), serta status sudah berkeluarga (73,3%). Kesimpulan yang didapatkan dari penelitian ini adalah bahwa proporsi kualitas tidur buruk lebih kecil dibandingkan kualitas tidur baik pada guru-guru SMA Negeri di Denpasar. Hal ini sangat mungkin disebabkan oleh karena banyaknya faktor yang menimbulkan kualitas tidur yang buruk. Karakteristik kualitas tidur buruk pada penelitian ini tidak semuanya sama dengan hasil penelitian sebelumnya. Kata Kunci: kualitas tidur buruk, guru SMA, Pittsburgh Sleep Quality Index

Abstract

Sleep paralysis adalah suatu kondisi yang biasanya terjadi ketika tidur atau bangun di mana individu sadar tapi tidak mampu bergerak atau berbicara dan sering disertai dengan halusinasi. Sleep paralysis membuat individu merasa kelelahan, mengalami kesulitan berkonsentrasi dan penurunan memori. Penelitian ini dilakukan untuk mengetahui prevalensi dan karakteristik sleep paralysis pada remaja Sekolah Menengah Atas (SMA) Negeri di Denpasar. Penelitian ini merupakan penelitian deskriptif cross sectional. Responden adalah siswa SMA Negeri di Denpasar tahun 2017 yang dipilih dengan teknik multistage random sampling sejumlah 231 orang. Hasil menunjukkan prevalensi sleep paralysis sebesar 72,3%, dengan umur terbanyak 16 tahun, tidak ada banyak perbedaan antara laki-laki dan perempuan, paling banyak pada siswa yang tidur 5-6 jam di malam hari, pada siswa yang tidur siang selama 1-2 jam, paling sering terjadi pada posisi terlentang, pada siswa yang memulai tidur pukul 10-11 malam, lebih banyak terjadi pada siswa dengan kualitas tidur buruk, lebih banyak pada siswa yang stres, terdapat hanya beberapa siswa yang mengonsumsi alkohol dan tidak ada yang pernah mengonsumsi obat yang berhubungan dengan sleep paralysis. Pada penelitian ini ditemukan beberapa siswa mengalami hipertensi, gangguan kejang, gangguan panik, dan gangguan cemas serta tidak ada siswa yang mengalami PTSD Kata kunci : Prevalensi, Karakteristik, Sleep paralysis.

Abstract

Introduction: Vibration is a mechanical motion that oscillates around a fixed point. Whole Body Vibration (WBV) can be defined as vibrations that pass through the surface, backrest, or the floor, either in standing or reclining position. WBV has been proved to lower the level of awakening, inducing sleepiness by providing sense of comfort, and improving insomnia. This study aims to construct a bed with Whole Body Vibration (WBV) which was expected to provide comfort, lowering sleep onset, as well as inducing sound sleep, thus becoming a non-pharmacological treatment option for sleep disorders. Method: This study was conducted in two stages which began with designing and manufacturing of the vibration bed and followed by a performance test. Assesment was conducted based on subjective interviews. Performance test results were presented descriptively. Result: According to the functionality testing, the vibration bed could produce vibration frequency between 1-4 Hertz and displacement 0.094-0.397 mm on vertically directed vibration. According to performance test, as many as 94.1% of subject’s preferred 1 Hertz frequency compared to other frequencies. Furthermore, 88.23% felt comfortable using the vibration bed, 76.47% experienced improvement in onset of sleep and also more soundly sleep. There were only 23.52% of subjects that experienced no change from previous sleep pattern. No side effects were found during the performance test. Conclusion: Lower frequency Whole Body Vibration (WBV) bed with vertical vibration effectively enhanced the subjects’ sense of comfort and improved sleep quality with no observed side effect.Keywords: Whole Body Vibration, Vibrating bed design, Insomnia, Performance test.

Abstract

Disruptions in sleep-wake cycles are particularly prevalent after a Traumatic Brain Injury (TBI). Sleep disruptions may occur as a main impact of brain damage or as a result of other neuropsychiatric sequelae of TBI, such as anxiety disorder, depressive disorders, substance abuse, chronic pain, and/or medication consumption. Chronic discomfort (headache and broad pain, presumably of central origin) and/or sleep difficulties (insomnia, disturbed breathing, periodic limb movements) occur in about one in five people with TBI. Disruptions in sleep-wake cycles linked with TBI need therapy. Although data specific to individuals with TBI is currently limited, cognitive-behavioral treatment and medication may be beneficial in alleviating sleep-wake problems in people who have sustained a TBI. This article aims to raise awareness of sleep disturbance after TBI to enhance diagnosis, assessment, and therapy and disclose new research opportunities.

Abstract

Diabetes melitus (DM) merupakan salah satu penyakit metabolik menahun yang paling sering di dunia, dengan tren yang semakin meningkat dan juga berdampak pada masyarakat di Indonesia. Komplikasi dari penyakit DM dapat berdampak pada angka morbiditas dan mortalitas yang tinggi. Penyakit DM memiliki komplikasi yang berdampak ke sistem saraf seperti stroke, gangguan fungsi kognitif, gangguan tidur, serta nyeri neuropati diabetik (NND). Komplikasi diabetes sering tidak diketahui oleh masyarakat. Kurangnya pemahaman masyarakat indonesia terhadap DM, komplikasi dan upaya pencegahan menjadi salah satu kendala yang masih terjadi. Komplikasi ini semakin meningkat risikonya pada kelompok usia lanjut (lansia) dan menganggu kualitas hidup hingga segi ekonomi di masyarakat. Peran fase kesehatan pertama yaitu Puskesmas sangat berperan penting dalam upaya pencegahan penyakit diabetes, seperti deteksi dini sehingga dapat pencegahan dan penanganan kepada pasien diabetes. Tujuan dari kegiatan ini yaitu untuk meningkatkan pengetahuan masyarakat tentang komplikasi DM pada sistem saraf di area kerja Puskesmas I Denpasar Selatan. Metode kegiatan yang akan dilakukan yaitu ceramah, diskusi, hand-on dalam penegakan diagnosis dan kuisioner pre dan post-test terselenggara pada tanggal 30 Oktober 2022 di Banjar Pegok, Denpasar. Hasil kuisioner pre dan post-test didapatkan adanya peningkatan nilai pengetahuan peserta setelah dilakukan pelatihan. Simpulan dan implikasi yang didapatkan yaitu diharapkan setelah adanya pelatihan ini, para masyarakat di Puskesmas I Denpasar Selatan dapat lebih memahami tentang penyakit DM dan komplikasinya terhadap sistem saraf.
