Abstract

Background: Low self-esteem is a psychological problem commonly experienced by patients with mental disorders, particularly schizophrenia, which can negatively impact their quality of life. Positive affirmation therapy is one effective non-pharmacological intervention to enhance both quality of life and self-esteem. This therapy works by embedding positive thoughts through repeated self-statements to shape a better self-perception, boost self-confidence, and reduce negative thinking patterns. Objective: This study aims to determine the effectiveness of positive affirmation therapy in improving the quality of life of patients with low self-esteem at the Flamboyan Ward of dr. RM Soedjarwadi Mental Hospital Klaten. Method: This research employed a descriptive case study approach involving two patients diagnosed with low self-esteem. The positive affirmation therapy was administered twice daily for three consecutive days. The patients’ quality of life was assessed before and after the intervention using the WHOQOL-BREF instrument. Results: The results showed increased quality of life in both patients. Patient B improved from a WHOQOL score of 50 (moderate) to 75 (good), while patient A improved from 50 (moderate) to 100 (excellent) after receiving the positive affirmation therapy. Conclusion: Positive affirmation therapy has proven to be an effective, safe, and simple non-pharmacological method for improving the quality of life in patients with low self-esteem. This intervention can be a supportive therapy in mental health nursing, especially in strengthening patients' self-concept.

Abstract

Background: Risk for Violent Behavior (RPK) refers to aggressive actions that may result in harm to oneself or others, often triggered by unmet needs, perceived failure, frustration, or ineffective stress management. Non-pharmacological interventions play a crucial role in managing such behaviors, and one effective approach is Progressive Muscle Relaxation (PMR) therapy. Objective: This study aims to evaluate the effectiveness of Progressive Muscle Relaxation therapy in reducing signs and symptoms of violent behavior among patients at risk of aggression at Dr. RM. Soedjarwadi Regional Mental Hospital, Central Java. Methods: A descriptive case study design was employed, incorporating a pre-test and post-test approach using the RUFA (Rating Unit for Aggressive Behavior) observation sheet to assess changes in violent behavior. Two respondents were observed before and after the intervention. Results: Prior to the intervention, both respondents exhibited moderate levels of violent behavior, classified as Intensive Stage II, with baseline scores of 11 and 14, respectively. Following three consecutive days of PMR therapy, both respondents showed improvement, transitioning to Intensive Stage I (mild), with post-intervention scores of 8 and 9. Conclusion: Progressive Muscle Relaxation therapy demonstrates a positive impact in reducing violent behavior among psychiatric patients. The findings indicate a noticeable reduction in aggression levels after the intervention, suggesting that PMR is an effective non-pharmacological strategy for managing patients at risk of violent behavior.

Abstract

Background: Hallucinations are perceptual disturbances that occur without external stimuli, affecting one or more of the five senses while the individual is fully conscious. In 2024, schizophrenia patients experiencing hallucinations accounted for 2,335 (54.2%) of total admissions at Dr. RM Soedjarwadi Regional Mental Hospital, Central Java Province, making them the largest patient group treated at the facility. Non-pharmacological interventions play a vital role in managing psychiatric symptoms, and music therapy, particularly classical music has shown potential in reducing psychological distress and improving cognitive and emotional functioning. Objective: This study aims to evaluate the effectiveness of classical music therapy in reducing the severity of auditory hallucinations among patients with psychiatric disorders at Dr. RM Soedjarwadi Regional Mental Hospital. Methods: A descriptive case study design was employed, with two respondents undergoing classical music therapy based on the pretest-posttest method. The Auditory Hallucinations Rating Scale (AHRS) questionnaire was used to assess changes in the frequency, intensity, and impact of hallucinations before and after the intervention. Results: Prior to the intervention, both respondents exhibited severe levels of auditory hallucinations, with baseline AHRS scores of 23 and 26, respectively. Following the implementation of classical music therapy, scores decreased significantly to 8 and 9, both falling within the mild category. Conclusion: The findings indicate that classical music therapy, particularly the use of Mozart compositions, is effective in reducing the severity of auditory hallucinations in patients with psychiatric disorders. This suggests that music therapy can serve as a valuable non-pharmacological adjunct in the management of hallucinatory symptoms in clinical psychiatric settings.
