Abstract

Citicoline has a neuroprotectant effect, especially in mild-to-moderate stroke. The presence of the ε4 allele in the ApoE gene is associated with a severe acute ischemic stroke (AIS) phenotype, in addition to neurodegenerative risk This study aims to determine the effect of citicoline on cognitive and motoric recovery in severe AIS biomolecularly, such as ApoE4 rs429358 and rs7412 polymorphisms. A 64-year-old woman was diagnosed with AIS with a 6-hour onset and with a NIHSS score of 25 and MMSE score of 21. The patient’s AIS was biomolecularly severe as proven by PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) ApoE4 which yielded 482 bp at rs429358 (codon 112) and 249 bp 167 bp at rs7412 (codon 158) on electrophoretic visualization. The patient received 3x 500 mg intravenous citicoline for 4 days followed by another 1000 mg of citicoline intravenous during polyclinic control for 5 consecutive days. On the 9th day, the patient experienced improvement in cognitive function with an MMSE score of 29 and superior (4/5) and inferior (5/5) motoric strength. Citicoline is a neuroprotectant agent that is beneficial for the recovery of cognitive and motoric function in biomolecularly severe SIA (ApoE4, C/C, Arg112/Arg158) because it can save the penumbra area from infarction.

Abstract

Background: Vertigo is a common complaint in medical settings, with the most important differential diagnosis to exclude being vertebrobasilar ischemic stroke. However, there have been no cohort studies evaluating the risk of stroke following hospitalization for acute vertigo without other neurological symptoms (isolated vertigo). Objective: To assess the long-term risk of stroke in patients with a history of hospitalization due to isolated vertigo. Methods: This retrospective cohort study was conducted at Dr. Moewardi Hospital in Surakarta, utilizing secondary data from electronic medical records for the years 2024–2025. A total of 268 patients hospitalized for vertigo and 223 control patients (elective gastrointestinal endoscopy patients) were selected based on similar age distribution. Stroke incidence data were analyzed using Kaplan-Meier methods and Cox regression, with adjustments for demographic variables and vascular risk factors. Results: Over a 2-year period, stroke occurred in 16 vertigo patients (5.8%) and 2 control patients (1.1%). After adjustment, vertigo patients had a 3.25-fold higher risk of stroke compared to the control group (HR: 3.25; 95% CI: 2.18–4.15; p = 0.002). Stroke risk significantly increased in patients with ≥3 vascular risk factors (HR: 5.01). Approximately 68% of strokes occurred within the first year. Conclusion: Isolated vertigo significantly increases the risk of ischemic stroke, particularly in patients with risk factors during the first year following the initial symptoms of isolated vertigo.

Abstract

 Epilepsi merupakan salah satu penyakit saraf yang menjadi permasalahan kesehatan masyarakat global. Prevalensi epilepsi di Indoesia berdasarkan data dari Departemen Kesehatan tahun 2018 sebesar 1.8 juta. Insidensi epilepsi berkisar 50.4 per 100.000 populasi pertahun dengan 80% pasien epilepsi di dunia terjadi di negara berkembang. Penderita epilepsi sering mendapatkan stigma negatif di masyarakat. Pentingnya penyuluhan mengenai informasi terkait epilepsi pada masyarakat khususnya pada kader PMR diharapkan dapat memberikan informasi secara lebih lengkap terkait epilepsi sekaligus sebagai pemutus stigma. Penelitian ini dibuat untuk mengetahui efektivitas penyuluhan epilepsi terhadap tingkat pengetahuan kader PMR SMA/SMK Surakarta mengenai penyakit epilepsi dan penanganan kedaruratan pada kasus kejang. Penyelenggaraan penyuluhan diikuti oleh 159 responden yang berasal dari 70 SMA/SMK se-karesidenan Surakarta dengan masing-masing mengirimkan minimal 2 perwakilan PMR. Sebelum dan sesudah penyuluhan dilakukan evaluasi terkait pengetahuan peserta mengenai epilepsi melalui pretest dan posttest. Data hasil evaluasi menunjukkan adanya perbaikan pengetahuan peserta dari skor rata rata awal  74.12 menjadi 82.5. Kata kunci : epilepsi, penyuluhan, anggota PMR

Abstract

Introduction: Individuals after Covid-19 infection are suspected to have symptoms of cognitive impairment. Researchers wanted to use the telephone version of MoCA-22 assessment to assess cognitive function. Method: Cross-sectional study at Dr. Moewardi Hospital, Surakarta. Researchers assessed cognitive function telemedicine using Telephone-MoCA 22 and then compared it with the 30-point standard MoCA-INA examination. The analysis is continued on the results of inspection of each component. Results: During 2020-2021 there were 191 Residents who were infected with Covid-19. After screening, 69 people were able to complete the study, of which 34 people with a history of Covid-19 infection (49,2%) and 35 people (50,8%) without a history of Covid-19 infection. The subjects consisted of 34 men (49,2%); 35 women (50,8%); age range 25-33 (±28.97) years; Education grade is 28 juniors, 27 intermediate, and 17 seniors Resident. In the regression test, it was found that effect of Covid-19 history on cognitive function with p-value = 0,94 if using MoCA 30, and p-value = 1,17 if using T-MoCA 22. Comparative test of the two assessments obtained p-value = 0.475. In the analysis of each component obtained less than the maximum value on components of calculation, repetition and delayed memory. Conclusion: In the study, it was found that a history of Covid-19 infection had no effect on cognitive function in research subjects tested using MoCA-INA or Telephone-MoCA. T-MoCA examination has a test value that is not significantly different from the full version of MoCA 30 points.

Abstract

Background: Spinocerebellar ataxia (SCA) is a dominant and monogenic central nervous system disorder, characterized by progressive motor disorders affecting coordination, balance, speech, and ADL. The prevalence is 2.7 out of 100,000 people. Currently, cell-based therapy is being developed for clinical improvement in SCA. Case: A 48-year-old woman with weakness in both legs and arms, difficulty walking, dysmetria, and dysarthria since 2015. Examination of SARA scoring is severe ataxia, EMG showed polyradiculopathy, muscle denervation, and suspicion of posterior ramus lesions; complete blood laboratory and tumor markers were routine. MRI brain contrast and whole spine contrast radiology imaging were also performed. Clinical improvement was achieved in 2016 following stem cell injection in Thailand (dose and type of cells unknown). However, clinical worsening occurred from 2020 to 2024. The patient received Umbilical Cord Mesenchymal Stem Cells (MSC) in October 2024, administered intrathecally at a dose of 20 million cells. Discussion: The first injection showed significant clinical improvement. The second injection showed no clinical improvement, but no worsening of symptoms was found. The difference in results may be due to variations in the route of administration, cell type, cell quality, and the dose administered. Conclusion: The administration of umbilical cord mesenchymal stem cells (UC-MSCs) in SCA is considered safe, with minimal complications, and can suppress disease progression, although it does not produce clinical improvement.
