Abstract

Istilah Sepsis menurut konsensus terbaru adalah keadaan disfungsi organ yang mengancam jiwa yang disebabkan karena disregulasi respon tubuh terhadap infeksi. Penggunaan kriteria SIRS untuk mengidentifikasi sepsis dianggap tidak membantu lagi. Kriteria SIRS tidak menggambarkan adanya respon disregulasi yang mengancam jiwa. Disfungsi organ didiagnosis apabila peningkatan skor SOFA ≥ 2. Dan istilah sepsis berat sudah tidak digunakan. Septik syok didefinisikan sebagai keadaan sepsis dimana abnormalitas sirkulasi dan metabolik yang terjadi dapat menyebabkan kematian secara signifikan. Dalam protokol yang dikeluarkan pada tahun 2016, target resusitasi EGDT dihilangkan, dan merekomendasikan terapi cairan kristaloid minimal sebesar 30 ml/kgBB dalam 3 jam atau kurang.

Abstract

Tujuan: Meninjau sejumlah faktor risiko terjadinya penyakit hipertensi dalam kehamilan. Kejadian hipertensi dalam kehamilan dapat dipengaruhi oleh berbagai faktor, yaitu: Primigravida, nuliparitas, usia ibu dan Indeks Massa Tubuh, yang merupakan bagian dari faktor risiko terjadinya penyakit hipertensi dalam kehamilan. Metode: Penelitian ini merupakan studi observasional dengan metode potong lintang, variabel yang digunakan berupa usia maternal, graviditas, indeks massa tubuh terhadap kejadian hipertensi dalam kehamilan. Sampel penelitian ini adalah ibu hamil yang datang ke Poli Rawat Jalan Departemen Obstetri dan Ginekologi Rumah Sakit Bhayangkara, Denpasar, pada bulan Oktober – Desember 2018. Uji statistik menggunakan Chi Square/Fisher’s Exact Test.Hasil: Faktor risiko yang penting untuk terjadinya hipertensi dalam kehamilan antara lain, indeks massa tubuh (OR 2,60 95% IK 1,36-4,95 p = 0,005) dan usia maternal (OR 2,74 95% IK 1,41-5,43 p =0,004). Sementara itu graviditas (OR 1,58 95% IK 0,70-3,57 p=0,077) bukan merupakan faktor risiko yang bermakna. Kesimpulan: Terdapat hubungan antara indeks massa tubuh dan usia maternal dengan kejadian hipertensi dalam kehamilan, dan tidak ada hubungan yang bermakna antara graviditas dengan kejadian hipertensi dalam kehamilan.A Review on The Risk Factors of Hypertension in Pregnancy at Bhayangkara Hospital DenpasarAbstractObjective: To review some risk factors of the recurrence of hypertension in pregnancy disorder. The incidence of hypertension in pregnancy can be affected by several factors: Primigravida, nulliparitas, maternal age and increased body mass index are the risk factors for hypertension in pregnancy. Method: This study was an observational study with cross-sectional the variable used were maternal age, gravidity and body mass index (BMI) on the incidence of hypertension in pregnancy. The samples were pregnant women who came to Clinic of Obstetrics and Gynecology Departement, Bhayangkara Hospital Denpasar during October to December 2018. Statistical test used was chi square factor exact tes.Result: The most important risk factors for hypertension in pregnancy such as, body mass index (OR 2,60 95% CI 1,36-4,95 p = 0,005) and maternal age (OR 2,74 95% CI 1,41-5,43 p =0,004) mean while gravidity (OR 1,58 95% CI 0,70-3,57 p=0,077) is not a significant risk factor. Conclusion: There is a correlation between maternal age and body mass index with hypertension in pregnancy and  no correlation between gravidity with hypertension in pregnancy.Key words: gravidity, hypertension in pregnancy, body mass index, maternal age.

Abstract

Background: pituitary tumor accounted for 15% of benign intracranial tumors. Clinical symptoms found in the form of visual disturbances, and hormonal manifestations, surgery is one of the main managements. Endoscopic endonasal transsphenoidal surgery (EETS) is a minimally invasive approach for this case. The advantages of EETS is better efficacy than traditional technique, and shorter hospital stays, less mean blood loss, and earlier return to daily activities. Case: we reported a rare example of a male patient aged 37 years old with the main clinical features were visual disturbances, sexual malfunction and bitemporal hemianopsia on physical examination. Magnetic resonance imaging (MRI) showed macroadenoma pituitary, the size 2.41 x 2.98 x 3.20 cm. We explained about minimally invasive surgery such as EETS and the patient was agreed to performed EETS. Conclusion: endoscopic endonasal transsphenoidal surgery is a minimally invasive technique for tumor pituitary cases and safe when compared to traditional medicine.

Abstract

 AbstrakIntoleransi laktosa adalah suatu keadaan ketidakmampuan enzim laktase menghidrolisis laktosa diusus  halus,  umumnya  diderita  oleh  anak  –  anak dan  remaja.  Intoleransi  laktosa  primer  dapat disebabkan oleh berkurang atau tidak adanya aktivitas enzim laktase dan kelainan gen LCT C>T-13910. Intoleransi laktosa sekunder umumnya terjadi akibat infeksi rotavirus. Intoleransi laktosa dapatasimtomatis atau dengan gejala klinis yang bervariasi seperti mual, sakit perut, kembung dan sering flatus. Pemeriksaan penunjang yang umum, antara lain:  tes napas hidrogen, tes toleransi laktosa (TTL) dan tes Gen LCT C>T-13910. Ketiga pemeriksaan ini saling berhubungan satu sama lain. Tes genetik adalah yang paling akurat. Kata kunci: Tes napas hidrogen, intoleransi laktosa Recent Diagnostic Procedures for Lactose Intolerance AbstractLactose intolerance is a condition of lactase enzyme inability to hydrolyze lactosein small intestine, mostly in children and teenagers. Primary lactose intolerance is LCT C>T-13910 gene disorder causing reduction or absence of lactase activity, while secondary lactose intolerance is usually caused by rotavirus infection. This condition can be asymptomatic but may cause nausea, stomachache, and flatulence. Diagnositc laboratory tests such as: hydrogen breath test, lactose tolerance test and LCT C>T-13910 gene test are correlated and complementary with their different sensitivities and specificities. Abstract

Kolelitiasis adalah material atau kristal yang terbentuk di dalam kandung empedu. Beberapa faktor risiko yang sering ditemui pada kejadian kolelitiasis dikenal dengan “6F” (Fat, Female, Forty, Fair, Fertile, Family history). Keluhan klinis yang sering ditemukan adalah nyeri pada perut kanan atas, nyeri epigastrium, demam, ikterus, mual, muntah. Sampel sebanyak 102 orang dipilih secara purposif dari pasien yang berkunjung di bagian Penyakit Dalam RSUD Koja pada periode 5 Oktober sampai dengan 31 Desember 2015, desain penelitian adalah deskriptif dengan pendekatan potong lintang. Hasil disajikan dalam tabel dan grafik. Frekuensi tertinggi berdasarkan jenis kelamin sebanyak 64 pasien (63 %) adalah perempuan, umur ( > 40 tahun) sebanyak 88 pasien (86 %), frekuensi tertinggi berdasarkan jumlah anak didapatkan bahwa responden yang mempunyai tiga anak atau lebih sebesar 52 pasien (52 %), rata-rata nilai indeks masa tubuh (IMT) sebesar 24,80, tidak ada riwayat keluarga yang menderita kolelitiasis sebanyak 83 pasien (80%), dengan warna kulit kuning langsat sebanyak 70 pasien (69 %), keluhan klinis yang tersering adalah dispepsia 61 pasien (60%), dengan nilai rata rata kolesterol total 201 mg/dl. Berdasarkan hasil penelitian ini disimpulkan bahwa, pasien kolelitiasis di RSUD Koja terjadi lebih banyak pada pasien perempuan dengan warna kulit kuning langsat (fair) yang berusia lebih dari 40 tahun, dengan jumlah anak lebih dari tiga orang, memiliki nilai rata-rata indeks massa tubuh sebesar 24,80, sebanyak 83 pasien kolelitiasis tidak ditemukan adanya riwayat kolelitiasis dalam keluarga, dan ditemukan bahwa dari seluruh jumlah pasien kadar rata-rata kolesterol 201 mg/dl dengan keluhan utama dispepsia.Kata kunci: kolelitiasis, faktor risiko, RSUD Koja

Abstract

Osteoporosis is a metabolic bone disease that is caused by a decreased amount of bone mass, and microarchitectural damage to bone tissue that affects the bone to break easily. Osteoporosis can be divided into primary and secondary etiologies. Vertebral, hip, distal radius, and proximal humerus are the most common predilection of fracture complication due to osteoporosis. The diagnosis of osteoporotic fractures is established based on the identification of risk factors, general clinical manifestations, specific manifestation from each fracture predilection, and advanced examinations, including radiology imaging and dual-energy x-ray absorptiometry. The purpose of osteoporotic fractures management in elderly is a stable fixation with early mobilization that will induce an optimal activation of osteoblast for new bone formation.

Abstract

Background: Articular cartilage injuries often result from trauma, genetic predisposition, and degeneration. These injuries lack inherent regeneration mechanisms due to the absence of blood vessels and limited progenitor cell entry. Osteoarthritis is characterized by gradual cartilage deterioration. Mesenchymal stem cells (MSCs), particularly their secretome including exosomes, hold promise as a regenerative therapy. This review explores the application of MSCs and their secretome to address cartilage defects.Methods: This review was conducted based on PRISMA guidelines. Animal model studies focusing on the use of stem cell secretomes for cartilage regeneration were explored. The search, encompassing PubMed, MEDLINE, the Cochrane Library, Scopus, Web of Science, and Science Direct from January 10, 2023, to July 27, 2023, was conducted utilizing Google Chrome as the search engine. Studies with outcomes based on OARSI or ICRS scores, as well as any additional outcomes related to MSC secretome utilization, cartilage regeneration, and proliferation, were included.Results: Our systematic review identified six studies using MSCs in vivo and in vitro. Synovial membrane-derived MSCs significantly enhanced cartilage regeneration by elevating chondrogenic capabilities. Hydrogel-based systems techniques and 3D-printed scaffolds have emerged for innovative delivery. Specific microRNAs, such as miR-92a-3p, have been recognized for enhancing cartilage regeneration. Strategies for the effective targeting of MSC exosomes to the precise cartilage damage site have been explored.Conclusions: The studies demonstrate the potential of MSC-derived secretomes and exosomes for knee cartilage regeneration in animal models. Further research and clinical trials are needed to refine these approaches for practical application.

Abstract

Background: The most frequent primary malignant bone tumor in children is osteosarcoma. Patients with osteosarcoma are currently treated with a combination of neoadjuvant and adjuvant chemotherapy as well as surgery, either limb-sparing surgery or limb-ablation surgery. How to manage the remaining bone growth during the limb-sparing procedure in youngsters is one of the challenges. Unfortunately, amputation is still many perform for OS patients. The purpose of this study was to inform and educate doctors about the value of early OS diagnosis and treatment in pediatric patients.Case Presentation: We presented case from a male 16 years old Balinese complaint of pain on her left knee 12 months ago. The pain was continuous and got worse at night. The patient went to a traditional masseuse and was given topical herbal ointment. Three months later, a lump appeared on her knee at the size of a small ball and getting bigger. We did a review slide of the previous biopsy sample by the pathology of anatomy in our hospital. The result came out as osteosarcoma with osteoblastic cells. We performed to amputated the limb-affected tumor following wound care routinely controlled.Results: Intraoperatively, we performed wide excision, without patellar tendon, medial collateral ligament, and cruciate ligament preserve. We amputated the limb-affected tumor. The patient routinely controlled to our outpatient clinic every week. Unfortunately, the patient was died after 3 weeks of admission.Conclusion: To reduce the risk of misdiagnosis or delayed diagnosis of osteosarcoma, every source of knee pain with a mass in pediatric age groups should be thoroughly explored with a high index of suspicion for malignant lesions. Therefore, amputation is a cutting-edge osteosarcoma treatment that can be avoided if the patient can be identified early and treated quickly in pediatric oncology malignant instances.Keywords: osteosarcoma, long bone, diagnosis, tibia, pediatricCorrespondence: Komang Septian Sandiwidayat. Abstract

Background:  Total knee arthroplasty (TKA) is a widespread surgical procedure for severe arthritis. By 2030, primary TKA would reach over 1.2 million procedures annually in the United States. In Indonesia, the prevalence of joint disease is about 19.5 million cases. This study aims to evaluate the clinical demographics characteristics of TKA in Prof. Ngoerah General Hospital, Bali, Indonesia.Subjects and Method:  This is a descriptive study using a Cross-Sectional method with a total 57 adult patients undergoing TKA caused by Osteoarthritis grade III/IV in Prof IGNG Ngoerah Hospital from June 2020-June 2022. The dependent variable is the TKA procedure. Independent Variable is Age, gender, grade of knee osteoarthritis, comorbidities, and complications Data from the medical record was collected and stored in a Microsoft Office Excel 365 spreadsheet, the distributive table was analyzed and collected by using SPSS.Results:  Patient characteristic is dominated by an elderly female (84.2%), with age group 61-70 (38.6%). Comorbidities in this study include hypertension (28.1%), type II diabetes (10.5%), and rheumatoid disease (1.8%). The main complication recorded in this study includes joint stiffness (3.5%), and persistent pain (5.3%).Conclusion:  After Total Knee Arthroplasty was performed on 57 patients with a wide range of comorbidities, only a small number of complications occurred. This study implied that TKA could be performed even if the patient had some comorbidities as long as the patient pursued a complete pre and post-operative evaluation algorithm of the procedure.Keywords:  knee arthroplasty, osteoarthritis, human and medicine, rehabilitationCorrespondence:  IGN Wien Aryana, Consultant of Sports Medicine, Dept.
