Abstract

Uveitis merupakan proses peradangan uvea, yang meliputi iris, badan siliar, dan koroid. Secara anatomi, terdapat empat klasifikasi uveitis, yaitu uveitis anterior, uveitis intermediet, uveitis posterior, dan panuveitis. Uveitis posterior merupakan radang lapisan koroid dan struktur sekitarnya pada posterior mata. Kelainan ini langka, namun paling sering dikaitkan dengan komplikasi kebutaan. Diagnosis uveitis posterior membutuhkan pendekatan komprehensif karena bersifat kompleks dan multifaktorial. Penatalaksanaan yang tepat penting dan terutama untuk menemukan dan mengobati penyebab dasar agar dapat mencegah perburukan dan komplikasi yang lebih berat.Uveitis is an inflammatory process in the uvea, consisted of iris, ciliary body, and choroid. Four anatomical types of uveitis include anterior uveitis, intermediate uveitis, posterior uveitis and panuveitis. Posterior uveitis is the inflammation of the choroidal layer and surrounding structures in the posterior eye. This disorder is considered rare, but most commonly related to ocular complications, such as blindness. Diagnosis requires a comprehensive approach because of its complex and multifactorial nature. Appropriate management is important. Causal reatment can prevent the worsening and more severe complications.

Abstract

Depression among orphaned teenagers is a vulnerable mental health disorder that occurs as an impact of the absence of an adequate relationship and affection from parents since childhood period. Cross-sectional analytical study was carried out, enrolling 93 orphaned teenagers from three orphanages in Denpasar City. Sociodemographic data were obtained using questionnaires and level of depression were classified based on Beck’s Depression Inventory. The level of depression among orphaned teenagers respectively, normal (60.2%), mild depression (18.3%), moderate depression (15.1%), and severe depression (6.5%). Logistic regression analysis showed that the only sociodemographic factor that significantly related to depression is gender, boys were about 39% less-likely to experience depression than girls (OR=0.39; p=0.03). Depression is prevalent among girl orphans, thus counseling and social support are needed to overcome the occurrence of depression among orphaned teenagers in Denpasar. Abstract

Congenital orbital teratoma is a rare and complex congenital anomaly due to abnormal tissue growth within the orbit. This case report aims to highlight the significance of early prenatal screening in identifying congenital orbital teratoma and its potential complications. A 6-day-old male newborn presented with macrocephaly and a unilateral massive protrusion of the left eye. Prenatal ultrasonography performed at 36 weeks gestation showed a suspicion of fetal craniofacial mass. Further brain CT scan revealed an orbital heterogeneous mass, with calcified components dominantly in the anterior and middle cranial fossa into the left orbit, which was subsequently diagnosed as congenital orbital teratoma associated with non-communicating hydrocephalus. This finding prompted an urgent ventriculoperitoneal (VP) shunt. Unfortunately, despite prompt intervention, the patient passed away after the procedure. Pathological examination was compatible with an immature teratoma. Multidisciplinary management is warranted to improve diagnostic screening and refine management strategies for better outcomes in such cases.

Abstract

Introduction & ObjectivesMicro-invasive glaucoma surgery with goniotomy is a recent surgical treatment modality that targetsthe trabecular outflow to maintain intraocular pressure. This study aims to describe the clinicaloutcomes of excisional goniotomy using KDB and to review the published literature regarding amodified goniotomy technique as a potential surgical treatment modality in Indonesia. MethodsA cross-sectional descriptive study was conducted in patients that underwent goniotomy stand-aloneor combined with cataract surgery. Main outcome parameters consist of visual acuity, intraocularpressure (IOP), number of glaucoma medications, and complications during post-operative follow-up. ResultsA total of 7 patients (8 eyes) were included in the study, consisting of 5 (71,4%) males and 2 (28,6%)females. The mean age of the subjects was 47.7 years. Subjects consisted of primary,pseudoexfoliative, steroid-induced, and angle-recess glaucoma. Mean pre-operative IOPmeasurements showed 20.74 mmHg with visual acuity consisting of 1 (14.3%) mild visualimpairment and 7 (85.7%) severe visual impairment. Postoperatively there was progressiveimprovement in the mean IOP to 13.81 and 11.31 mmHg on first-day and one-week follow-up,respectively. Visual improvement was observed in 2 (28.5%) with mild visual impairment, 3 (42.6%)with severe visual impairment, and 2 (28.5%) with hand movement due to complications of cornealedema following phacoemulsification. A week later, visual acuity improved to 4 (57.1%) with mildvisual impairment and 3 (42.9%) with moderate visual impairment. ConclusionGoniotomy is a simple surgical method that may be applied in Indonesia as a novel technique toprovide an effective intraocular pressure-lowering effect.

Abstract

Introduction : Trabeculectomy remains a gold standard of glaucoma surgical management despite a steady rate of bleb failure due to conjunctival scarring. In this case, we report a case of a failed trabeculectomy managed with ab-interno bleb needling followed by antimetabolite injection of mitomycin C. Case Illustration : A 18-year-old male patient with a history of blurry vision in his right eye and vision loss in his left eye. The patient had previously undergone cataract surgery due to juvenile cataracts and developed persistent high intraocular pressure (IOP) in both of his eyes. A trabeculectomy was performed on his right eye six months prior to the examination; however, the IOP remained at 22 mmHg. Ocular examination revealed a shallow anterior chamber and flat conjunctival bleb. An ab-interno bleb needling technique was performed followed by antimetabolite injection of mitomycin C under local anesthesia in an operating theatre. Follow-up results showed a significant reduction of IOP on the first day (13.3 mmHg) and a week after surgery (18.5 mmHg), maintained by one anti-glaucoma eye drop. Discussion : Trabeculectomy filtration surgery has a high failure rate in younger patients due to a high risk of conjunctival scarring. The ab-interno needling approach with mitomycin C injection is a novel technique that can be performed and offers the advantage of increasing aqueous outflow through a direct opening of the trabecular meshwork from within the anterior chamber. Conclusion : This technique provides effective management of failed trabeculectomy to achieve intraocular pressure lowering effect and a corresponding decrease in the risk of progressive glaucomatous optic neuropathy.
