Abstract

Peningkatan kasus kusta baru pada tahun 2021 yang tersebar di 19 Kabupaten/Kota dengandistribusi terbanyak pada Kabupaten Padang Pariaman masih menjadi permasalahan di ProvinsiSumatera Barat. Hal ini menyebabkan masih terdapatnya kantong-kantong kusta yang berperandalam penularan kusta sehingga upaya eliminasi kasus kusta di tingkat Kabupaten/Kota sulit untukterwujud. Puskesmas Ulakan merupakan salah satu Puskesmas di Kabupaten Padang Pariamandengan 12 kasus baru (19%) multibasiler. Dalam upaya melakukan pemutusan rantai penularan danmeningkatkan pemahaman kepada masyarakat tentang penyakit kusta, maka DepartemenDermatologi dan Venereologi melakukan penyuluhan tentang pentingnya perawatan diri pasienkusta yang dapat dilakukan secara mandiri. Dari 14 pasien kusta didapatkan 70% pengetahuanpasien masih rendah dan setelah kegiatan, pengetahuan rendah turun menjadi 48% dan 52% pasiensudah memiliki pengetahuan yang tinggi. Tingkat perawatan diri pasien 72% masih buruk, stigmadidapatkan tinggi pada usia <45 tahun dengan latar belakang pendidikan tinggi. Dilakukan jugaadvokasi kepada pemegang program dan Kepala Dinas Kesehatan Kab. Padang Pariaman untukmengaktifkan kembali Kelompok Perawatan Diri (KPD).Kata kunci: perawatan diri, kusta, kelompok perawatan diri

Abstract

This research is motivated by the importance of implementing an effective and fun creative active learning model in class X SMK N 1 Ampek Angkek. Various questions are raised with learning that is still far from the criteria of being active, creative, effective, and fun. This can also be seen from the learning which is still centered on the teacher and students are still busy listening to the lesson. Then the teacher also does not optimize the potential that exists in students. The type of research used is Quasi Experimental Design. The population in this study were class X students of SMK N 1 Ampek Angkek for the 2020/2021 Academic Year. Sampling was carried out using the Nonprobability Sampling technique, namely by taking non-random samples. The first sample consisted of 25 students in the experimental class using the PAKEM model and the second sample consisted of 23 students in the control class using the conventional learning model. The research data was obtained by giving student responses as many as 22 items that have been tested. Based on the results obtained from the mean of the experimental class is 71.76 and the control class is 71.52. The results of the hypothesis using the t-test at a significance level of 0.05 obtained a Sig (2-tailed) of 0.041. So that H0 is rejected and H1 is accepted because 0.041 <0.05. This shows that there is a significant difference between the application of effective and fun creative active learning models in information technology and computer subjects with conventional learning models.

Abstract

Background: Mammary Paget's disease (MPD) with skin invasion is a rare condition that is often associated with an underlying malignancy. However, invasive MPD without malignancy is even rarer and often misdiagnosed. Case presentation: This report presents the case of a 56-year-old woman who presented with a progressively enlarging scaly erythematous plaque on her left nipple for 6 months. Dermoscopy and histopathological examination confirmed the diagnosis of invasive MPD, but radiological examination did not reveal any malignancy. Conclusion: Invasive MPD without malignancy is a rare but important entity to recognize. Awareness of this condition can help prevent overdiagnosis and unnecessary treatment.

Abstract

Background: Hypopigmented mycosis fungoides (HMF) is an uncommon variant of mycosis fungoides (MF), a cutaneous T-cell lymphoma. It presents a diagnostic challenge due to its clinical resemblance to various benign dermatological conditions. This case report highlights the importance of a comprehensive approach to diagnosis, incorporating clinical, histopathological, and immunohistochemical findings. Case presentation: A 48-year-old Indonesian woman presented with a one-year history of progressive, asymptomatic hypopigmented patches on her extremities. Initially misdiagnosed as progressive macular hypomelanosis, the patient's condition did not improve with topical treatments. Clinical examination revealed multiple hypopigmented patches and macules on both extremities, with some lesions exhibiting fine scales. Histopathological examination demonstrated atypical lymphocytes with epidermotropism and Pautrier's microabscesses. Immunohistochemical staining confirmed the presence of CD3+ T-cells, leading to the diagnosis of HMF. Conclusion: HMF can mimic various dermatological conditions, making diagnosis challenging. A thorough clinical assessment, coupled with histopathological and immunohistochemical evaluation, is crucial for accurate diagnosis and appropriate management. This case underscores the importance of considering HMF in the differential diagnosis of hypopigmented skin lesions, particularly in individuals with persistent or atypical presentations. Early recognition and intervention are essential for optimizing patient outcomes.

Abstract

Leprosy is still a health problem in several countries, including Indonesia. Disability due to leprosy is a health threat for sufferers and causes quality-of-life problems that can disrupt the productivity of leprosy patients. Ocular involvement in leprosy is joint, but little data has been reported. Apart from multi-drug treatment (MDT), leprosy sufferers must also be able to carry out continuous self-care to prevent further disability. This is a collaborative activity between the Department of Ophthalmology and Dermatology Venereology of the Faculty of Medicine, the University of Andalas in Puskesmas Pauh Kamba district of Padang Pariaman. This is an advocacy activity on self-care (3M: Seeing, Examining, and Protecting) and eye examination. The activity was followed by interactive discussions and continued with a direct examination for skin abnormalities and disabilities, eye examination, and fundoscope. The activity results were obtained from 24 respondents, 11 men and 13 women. The age range is 16-59 years (67%), with the highest level of education being Elementry School graduates, which is 11 people (46%). In the self-care questionnaire of patients, 40% behaved well, and the results of eye examinations almost suffered from dry eye complaints without disabilities.

Abstract

Background: Familial hypercholesterolemia (FH) is a rare inherited disorder characterized by high cholesterol levels, primarily high levels of low-density lipoprotein cholesterol (LDL-C). This condition can lead to premature cardiovascular disease. A less recognized but significant manifestation of FH is the development of multiple tuberous xanthomas, which are exogenous masses or fatty deposits on the skin. Case presentation: This report describes the case of a 4-year-old female child who presented to the Dermatology, Venereology, and Aesthetics Department of Dr. M. Djamil General Hospital Padang with multiple yellowish, painless, and non-itchy lumps on her body. These lumps were located on her knuckles, hands, elbows, buttocks, knees, back of knees, ankles, and feet. The lumps had been progressively increasing in size over the past month. Her medical history revealed that she first developed a yellowish lump on her right ankle two years prior, which gradually spread to other areas of her body. A dermoscopic examination showed a yellow-brown structureless area, and laboratory tests confirmed hypercholesterolemia with elevated LDL levels. Her triglycerides were normal, and both of her parents also had hypercholesterolemia. A histopathology examination confirmed the diagnosis of xanthoma. The patient was diagnosed with tuberous xanthomas and treated with simvastatin 1x10 mg, along with surgical excision of the xanthomas and flap reconstruction under general anesthesia. Conclusion: This case highlights the importance of a multidisciplinary approach in managing multiple tuberous xanthomas in a child with FH. While surgical excision can effectively remove the lesions, ongoing lipid-lowering therapy is crucial to prevent recurrence and reduce cardiovascular risks associated with FH. Collaboration among dermatologists, pediatric cardiologists, and pediatric nutritionists is vital to ensure comprehensive care and enhance the patient's overall quality of life.

Abstract

Background: Malignant melanoma, a highly aggressive form of skin cancer, is rare in adolescents. Early diagnosis is crucial for the best possible treatment and prognosis. Dermoscopy, a non-invasive technique, has been shown to increase the sensitivity of melanoma diagnosis, leading to early identification and better treatment outcomes. Case presentation: A 14-year-old female presented with a painful, bleeding black nevus on her right upper arm, increasing in size over four months. Dermoscopic examination, using a two-step algorithm and the ABCD rule, categorized the lesion as malignant melanoma. Histopathological examination confirmed the diagnosis of superficial spreading melanoma (Breslow 2, AJCC stage IIB). The patient underwent Mohs surgery for treatment. Conclusion: This case highlights the importance of dermoscopy in the early recognition of melanoma in adolescents. Dermoscopy aids in differentiating between benign and malignant melanocytic lesions, enabling prompt diagnosis and treatment. Increased awareness and utilization of dermoscopy can significantly improve outcomes in this patient population.

Abstract

Background: Keloid is a process of abnormal collagen thickening during wound healing in skin tissue accompanied by the formation of new blood vessels. Many keloid therapy modalities have been developed but the recurrence rate of those treatment still ranged 1-70%. Herbal plants have been developed for various types of treatment, one of which is for treating keloids. Purpose: The alpha mangostin content in mangosteen peel is known to have antifibrotic properties, further research is needed regarding the administration of alpha mangostin on the process of keloid occurrence. Methods: The investigation was conducted in vitro on phase III keloid fibroblast cells. There were two groups, which divided into the control groups and the treatment groups. The control groups and treatment groups were given alpha mangostin extract in concentrations of 20 μM; the sample of this study was 16. For each group after 24h of the incubation, fibroblast cell proliferation was measured by Microtetrazolium (MTT) assay, fibroblast cell migration was measured by scratch assay, SMAD3 expression was measured after immunocytochemical staining, and type 1 collagen was measured by Enzyme-Linked Immunosorbent Assay (ELISA). The Ethics Committee at the Research Ethics Commission of Faculty of Medicine Andalas University has reviewed this research. Result: Alpha mangostin can reduce the average expression of TGF-β1, SMAD3 expression, type 1 collagen, proliferation, and migration.  . Conclusion: At concentration of 20 μM, alpha mangostin suppressed TGF-β1 expression, SMAD 3, collagen type 1, proliferation, and migration in keloid fibroblast cell.
