Abstract

The Forum of International Respiratory Societies melaporkan penyakit paru obstruktif kronis (PPOK), asma, dan tuberkulosis (TB) masih menjadi penyebab utama kematian dan disabilitas di dunia. Di Indonesia, Riset Kesehatan Dasar (Riskesdas) 2018 menunjukkan preval_ensi ketiga penyakit tersebut masih tinggi, sedangkan penelitian terbaru melaporkan bahwa masih banyak kasus PPOK, asma, dan TB yang tidak terdeteksi (underdiagnosed). Riskesdas 2018 juga melaporkan bahwa rata-rata penderita penyakit tersebut berada pada kelompok usia produktif dan lansia. Upaya promotif, preventif, dan deteksi dini dini masih menjadi kunci utama dalam menurunkan angka mortalitas dan morbiditas serta meningkatkan kualitas hidup. Oleh karena itu, pengabdian kepada masyarakat melalui pembentukan “Pondok Paru Sehat” bertujuan sebagai sarana edukasi dan deteksi dini PPOK, asma, dan TB di Kelurahan Dukuh Kupang Kota Surabaya. Mitra ini dipilih karena memiliki jumlah penduduk usia produktif dan lansia mencapai 11.775 individu yang berisiko menderita ketiga penyakit tersebut. Selain itu, Kelurahan Dukuh Kupang dipilih sebagai wujud pelaksanaan tri dharma Fakultas Kedokteran Universitas Wijaya Kusuma Surabaya (FK UWKS) terhadap masyarakat sekitar. Kegiatan ini melibatkan Departemen Ilmu Penyakit Dalam dan Ilmu Kesehatan Masyarakat FK UWKS serta sebagai implementasi Mata Kuliah / Blok Respiratory System (RS) oleh mahasiswa semester 4 FK UWKS. Edukasi dilakukan dengan pemaparan materi oleh dosen FK UWKS dan dilanjutkan skrining PPOK melalui kuesioner skor PUMA yang disediakan oleh Perhimpunan Dokter Paru Indonesia (PDPI), kuesioner CAPTURE, dan pemeriksaan Peak Expiratory Flow Rate (PEFR) / Arus Puncak Ekspirasi (APE) untuk mengestimasi fungsi paru, sedangkan skrining asma dan TB dilakukan melalui kuesioner.

Abstract

Even after cessation of cigarette smoke exposure, long-term exposure leads to lung fibrous tissue growth characteristically by oxidative stress. Oat, an inexpensive dietary source with antioxidant characteristics, is expected to alleviate oxidative stress in the lung tissue after exposure to cigarette smoke has ceased. This research aims to evaluate the effect of an oat diet in alleviating oxidative stress in the lung tissue of BALB/c mice after cessation of cigarette smoke exposure. A randomized post-test-only control group design was used for this research. The sample used in this experiment is male BALB/c mice (n = 33). Mice with BALB/c strain were separated into two groups: (1) control and (2) treatment. SOD, GSH, and MDA were the variables of the study. Cigarette smoke exposure decreases the activity of SOD and GSH and increases the concentration of MDA. After cigarette smoke exposure had been stopped in the oat-treated mice group, SOD and GSH activities were enhanced, and the MDA content dropped. This research's variable outcomes were statistically significant (P0.05) from the standard feed alone-treated BALB/c mice group. Thus, consuming oats may alleviate oxidative stress implicated in lung fibrogenesis following cessation of tobacco smoke exposure

Abstract

Folic acid is a synthetic vitamin B9 that plays an important role in folate metabolism, including the synthesis of DNA and RNA, and epigenetic methylation. These processes are essential for the development of the nervous system. Impaired folate metabolism contributes to the pathophysiology of ASD through the dysregulation of neurochemical and epigenetic processes. This meta-analysis aimed to evaluate the efficacy of folinic acid in alleviating ASD symptoms.  Methods: Review Manager Software. Searches were conducted in PubMed, Science Direct, and Cochrane based on studies published in the last five years (2018-2023), with inclusion criteria of RCTs and exclusion of observational studies and non-English articles. Fixed-effects analysis was used because no heterogeneity was observed. Results: Two studies (n= 103) evaluated the effect of folinic acid on reducing ASD symptoms. Symptoms were assessed using the aberrant behavior checklist. The pooled mean difference was -0,66, with a 95% confidence interval ranging from -1,22 -0,10. The results showed statistical significance, with a p-value of 0,02. However, this analysis used only two studies with small sample sizes and modest mean differences. In conclusion, folinic acid administration has the potential to reduce ASD symptoms in children. Further research with a larger sample size is needed to generalize the clinically meaningful results.

Abstract

Penyakit tidak menular (PTM) seperti hipertensi dan hiperglikemia merupakan penyebab utama morbiditas dan mortalitas di Indonesia. Deteksi dini berbasis komunitas menjadi langkah strategis untuk mencegah komplikasi yang lebih serius. Penelitian ini bertujuan untuk mengidentifikasi tingkat risiko hipertensi dan hiperglikemia melalui pendekatan pemeriksaan kesehatan berbasis komunitas. Metode yang digunakan meliputi skrining tekanan darah, pengukuran kadar gula darah, wawancara kesehatan, serta edukasi gaya hidup sehat. Hasil penelitian menunjukkan bahwa sebagian besar masyarakat memiliki tingkat kesadaran yang rendah terhadap faktor risiko PTM, namun setelah dilakukan skrining dan edukasi, terjadi peningkatan pengetahuan serta kesadaran mengenai pentingnya pencegahan. Kegiatan ini juga menghasilkan data awal mengenai prevalensi risiko hipertensi dan hiperglikemia di tingkat komunitas. Upaya ini diharapkan dapat menjadi model pemberdayaan masyarakat dalam pencegahan PTM secara mandiri dan berkelanjutan.

Abstract

Invasive procedures remain the gold standard for lung cancer diagnosis, while new cases in 2018 reached 30,023 patients and tend to occur at a younger age, necessitating non-invasive markers for early detection of lung cancer. Currently, numerous studies have been conducted on microRNA (miRNA) as a lung cancer biomarker, including miRNA-19; however, research findings on miRNA-19 in the diagnosis of this disease remain controversial, prompting this study to assess the ability of miRNA-19 to detect lung cancer through a meta-analysis. Literature searches were conducted in three databases (PubMed, Science Direct, and Cochrane) up to March 22, 2023, using the Patient, Intervention, Comparison, Outcome (PICO) principle, with inclusion criteria for prospective, retrospective, case-control, or cross-sectional studies providing data for analyzing the diagnostic capability of miRNA-19. The methods used adhere to the 2020 PRISMA guidelines. Analysis was performed using STATA, MetaDisc, and Review Manager software. Four articles met the inclusion criteria for this meta-analysis. The combined analysis results regarding the sensitivity of miRNA-19 in lung cancer diagnosis reached 0.81 (95% CI [confidence interval]: 0.76–0.85), specificity 0.69 (95% CI: 0.61–0.76), diagnostic odds ratio (DOR) 12.62 (95% CI: 4.73–33.65), and area under the curve (AUC) 0.85. Analysis of the positive likelihood ratio (PLR) and negative likelihood ratio (NLR) yielded results... Analysis of the positive likelihood ratio (PLR) and negative likelihood ratio (NLR) yielded results 2.51 (95% CI: 1.59–3.95) and 0.29 (95% CI: 0.18–0.48). Based on the above data, miRNA-19 has potential in the non-invasive diagnosis of lung cancer.

Abstract

Fibrosis jantung merupakan respon patologis terhadap cedera kronis yang dapat menyebabkan gangguan fungsi jantung. Pajanan asap rokok atau secondhand smoke (SHS) diketahui dapat meningkatkan risiko fibrosis jantung melalui mekanisme stres oksidatif dan inflamasi. Penelitian ini bertujuan untuk mengevaluasi efek protektif ekstrak Peperomia pellucida terhadap fibrosis jantung yang diinduksi oleh pajanan SHS pada tikus Wistar. Tikus dibagi menjadi tiga kelompok: Kelompok K (kontrol), Kelompok P1 yang hanya menerima pajanan SHS, dan Kelompok P2 yang diberi ekstrak P. pellucida 400 mg/kg BB sebelum dan selama pajanan SHS. Efek fibrosis jantung dievaluasi melalui pengukuran berat jantung dan analisis fraksi area kolagen menggunakan pewarnaan Masson’s Trichrome. Data dianalisis dengan uji One-Way ANOVA diikuti dengan uji post-hoc Tukey. Hasil penelitian menunjukkan bahwa pajanan SHS secara signifikan (p > 0,05) meningkatkan berat jantung dan fraksi area kolagen pada kelompok P1 dibandingkan dengan kelompok kontrol (K). Sementara itu, pada kelompok P2 yang mendapat ekstrak P. pellucida, terdapat penurunan yang bermakna (p < 0,05) pada berat jantung dan fraksi area kolagen. Penurunan ini mengindikasikan adanya efek protektif dari ekstrak P. pellucida. Berdasarkan temuan ini, ekstrak P. pellucida menunjukkan potensi sebagai agen terapeutik dalam mencegah fibrosis jantung yang disebabkan oleh pajanan asap rokok. Penelitian lebih lanjut diperlukan untuk mengeksplorasi mekanisme molekuler yang mendasari efek protektif ini dan untuk menilai dosis serta durasi pengobatan yang optimal.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a leading cause of morbidity and mortality from non-communicable diseases in Indonesia, particularly in Surabaya. The “Pondok Paru Sehat” community service program, initiated by the Faculty of Medicine, Wijaya Kusuma University Surabaya (FK UWKS), aims to raise awareness, knowledge, and early detection of COPD through nutrition education and spirometry screening in Dukuh Kupang sub-district. This study employed a descriptive qualitative approach using the Five Levels of Prevention model, involving 40 participants from both productive age groups and the elderly. The results indicated that 28 participants had an FEV1/FVC ratio of <70%, suggesting undiagnosed airway obstruction. Nutritional status assessment using Body Mass Index (BMI) showed that 30% of participants were underweight, while 20% were overweight. Participant satisfaction reached 92.5%, with the majority considering the education and screening services highly beneficial. The program demonstrates that community-based education combined with lung function screening can enhance awareness and early detection of COPD. This model is recommended for replication in other regions with support from local health cadres, health authorities, and educational institutions.
