Abstract

The eye is one of the main human visual organs whose function is to convert stimuli into visual perception. This organ is very sensitive and susceptible to various disorders. The eyes are a visual organ that allows us to observe the environment around us, obtain information and carry out various daily activities (Ameliany, 2022). Cataracts can be classified based on their severity into incipient, immature, mature and hypermature cataracts. Apart from that, cataracts can also be differentiated based on the age at which they appear, namely congenital, juvenile, presenile and senile (Khairunnisa, 2023).

Abstract

Obstructive ileus is a digestive system disorder characterized by a blockage caused by abnormalities in the intestinal lumen, intestinal wall, or foreign objects outside the intestine. Blockages in this intestinal area can cause accumulation or buildup at the site of the blockage. Patients with obstructive ileus generally experience complaints such as nausea, vomiting, abdominal distension, bowel disorders, flatus (gas) disorders, and the most common symptom is abdominal pain. The goal of this community service is to enable families to understand the definition, causes, and management of obstructive ileus and to practice applying warm compresses to minimize pain in the affected area. The methods used in this activity were counseling, interactive discussions, and practice of applying warm compresses. The media used were leaflets and flipcharts. The results obtained from this health education activity can improve patient (family) knowledge and practice in applying warm compresses to relieve pain. It is hoped that after this activity, patients (families) can apply knowledge of how to manage pain independently as an initial preventative measure to reduce pain

Abstract

Congestive Heart Failure (CHF) is a condition in which the heart is unable to pump blood adequately to meet the metabolic needs of the body. CHF is one of the most prevalent cardiovascular disorders globally, nationally, and locally, contributing to significant morbidity and mortality. This study aims to describe a comprehensive nursing care process applied to a patient (Mr. R) diagnosed with CHF at the Mawar 1 Ward of RSUD dr. Soeselo, Tegal Regency. This study used a descriptive case study design. Data were collected through interviews, physical examination, observation, and medical record documentation on February 9–11, 2026. Three nursing diagnoses were identified: ineffective airway clearance related to airway hypersecretion, fatigue related to physiological conditions (chronic disease), and knowledge deficit related to insufficient information exposure. Nursing interventions applied included airway management (semi-Fowler positioning, oxygen therapy, effective coughing technique), energy management (range-of-motion exercise, gradual activity), and health education about CHF. After 2×24 hours of nursing care, all three nursing diagnoses were resolved: airway clearance improved (SpO₂ increased from 91% to 95%, respiratory rate decreased from 28 to 23 times/minute), fatigue decreased, and patient knowledge about CHF improved. Systematic nursing care significantly improved the patient's condition in CHF management.

Abstract

Fractures are one of the most common musculoskeletal disorders caused by trauma, repetitive stress, or pathological factors. Supracondylar femur fracture is a type of femur fracture occurring at the distal 12 cm of the femur, and Open Reduction and Internal Fixation (ORIF) is the primary surgical intervention. Post-operative ORIF patients commonly experience impaired physical mobility and impaired skin integrity. This study aimed to provide comprehensive nursing care to a patient post-ORIF for right supracondylar femur fracture in the Mawar 2 Ward of RSUD dr. Soeselo, Tegal Regency. A descriptive method with a nursing process approach was used, employing interview, observation, documentation study, and literature study. Nursing assessments conducted on February 8, 2026 revealed that the patient (Ny. S, female, 68 years) experienced pain at the surgical site with a scale of 7, inability to move both lower extremities (muscle strength score 2), and a post-operative wound approximately 10 cm in length with redness and edema. Two nursing diagnoses were established: impaired physical mobility related to neuromuscular disorders and impaired structural bone integrity, and impaired skin/tissue integrity related to invasive procedure effects. Nursing interventions included mobilization support, pain management, wound care, and collaborative administration of medical therapy. After two days of nursing implementation, both diagnoses were partially resolved. Discharge planning focused on gradual mobilization, independent wound care, and high-protein and high-calorie diet to accelerate recovery.

Abstract

Hypertension is a condition in which systolic blood pressure exceeds 140 mmHg and diastolic blood pressure is above 90 mmHg. Hypertension is the leading chronic non-communicable disease among adults in Indonesia, with a prevalence of 26.5%, and tends to increase with age. Family nursing care plays an important role in helping patients manage hypertension through education, pharmacological and non-pharmacological interventions, and ongoing monitoring. This case study aimed to describe comprehensive nursing care for Ny. K, a 66-year-old patient from the family of Tn. I, diagnosed with hypertension in Desa Tonjong RT 02 RW 04, Kecamatan Tonjong, Kabupaten Brebes. Nursing care was conducted from December 27 to 29, 2025, using interview, physical examination, observation, and documentation methods. Two nursing diagnoses were identified: (1) Risk of ineffective cerebral tissue perfusion, and (2) Knowledge deficit related to hypertension. Nursing interventions included vital sign monitoring, oral medication administration (Amlodipine 10 mg), health education on hypertension, and non-pharmacological therapy using bay leaf (Syzygium polyanthum) decoction. Evaluation results showed that the knowledge deficit was resolved after one session of health education, while the risk of ineffective cerebral tissue perfusion was partially resolved, with blood pressure decreasing from 180/100 mmHg to 165/90 mmHg over three home visits. It is recommended that families continue the bay leaf decoction therapy for one week and maintain regular blood pressure monitoring at the nearest health facility.
