Abstract

ABSTRACT Nasopharyngeal carcinoma (NPC) epidemiologically occupies the fourth position as the highest number of malignancy cases in Bali, Indonesia. The most common histological type of NPC cases are non-keratinizing squamous cell carcinoma (NKSCC) undifferentiated subtype. This research aims to know the clinicopathological aspect of nasopharyngeal carcinoma at Central General Hospital Prof. Dr. I.G.N.G Ngoerah Denpasar in 2021-2022 regarding the distribution of age range, gender, education, occupation, clinical stage, histological type, as well as the results of anamnesis. The research design is a descriptive study of 70 cases of NPC at Central General Hospital Prof. Dr. I.G.N.G Ngoerah Denpasar from 2021 to 2022. Samples were collected retrospectively from secondary data using a total sampling technique through a cross-sectional study, samples were obtained from medical record data and hospital management system of Central General Hospital Prof. Dr. I.G.N.G Ngoerah Denpasar. In this study, the most cases of NPC were found in the range of 51-60 years old (32.9%), male sex (65.7%), high school education status (48.6%), private employee employment (32.9%), anamnesis results of a lump in the neck (77.1%), stage IVA (40%), and with the histological type of NKSCC undifferentiated subtype (90%). Men have twice the risk of suffering from NPC compared to women, most cases have complained of a lump in the neck and other symptoms such as nasal congestion, tinnitus, headaches. The most common histological type found is the NKSCC undifferentiated subtype, which mostly occurs in advanced stages of an IVA. Keywords : Clinicopathology, NPC, Nasopharynx

Abstract

HIGHLIGHTS Clinicopathologic characteristic of patient with LEEP procedures dominated by women above 35 years old, with total parity less than 3 and without history of contraceptive usage. The p16 expression analyzed by immunohistochemistry technique. The positive and negative reactivity of p16 ratio was 3:5 on LEEP specimen which microscopically appear as chronic cervicitis, low-grade and high-grade squamous intraepithelial lesion.   ABSTRACT Objective: The aim of this study was to reveal p16 expression with the clinicopathological characteristics of patients who underwent cervical biopsy using the Loop electrosurgical excision procedure (LEEP) at Udayana University Hospital, Bali, Indonesia, for the period 2020 – 2023. Materials and Methods: The research was conducted at Anatomic Pathology Laboratory of Udayana University Hospital, Denpasar, Bali, Indonesia. The samples were selected based on inclusion criteria such as the formalin fixed paraffin embedded (FFPE) sample from positive IVA patient and continue for LEEP procedure. Otherwise, the exclusion criteria were moldy FFPE sample and incomplete clinical data. Then, p16 immunostaining procedure was carried out manually. The interpretation of p16 results was analyzed using SPSS software, version 25 by International Business Machines (IBM) Corporation. Results: The positive p16 expression was revealed in 12 samples (38.7%), in contrast the negative staining appeared in 19 samples (61.3%). Unfortunately, p16 expression was not significant statistically based on age, parity, and contraceptive history, with p-values of 1.00, 0.45, and 0.65, respectively. Meanwhile, a statistically significant association was found between p16 expression and histopathologic diagnosis (p = 0.02, 95% CI 1.4 – 38.3). In addition, 22.2% of the variation of p16 expression based on multivariate analysis demonstrating a significant correlation (p = 0.01). Conclusion: p16 expression with histopathology diagnostic characteristic in patient who underwent Loop electrosurgical excision procedure (LEEP) was found statistically significant. Moreover, clinical application of p16 in daily practice should be performed with consideration especially for pre-cancer lesion in LEEP biopsy specimen procedure and clinicopathological approach is essential.

Abstract

Introduction: Oncocytoma is a benign tumor of the salivary glands consisting of oncocyte cells, occurring in approximately 2% of all salivary gland neoplasms, most of which occur in the parotid gland. This tumour most commonly occurs in the sixth to eighth decades of life, with a mean age of 64 years and without gender predilection. The clinical presentation is often misdiagnosed as pleomorphic adenoma, cystic lesion, and inflammation.Case Presentation: A 63-year-old male, complained of a lump under the right ear, for 7 years ago. It feels painless and slightly enlarged since a month before surgery. Based on clinical examination suspected as benign cystic lesion due to inflammation and ultrasound suggesting a benign tumour mass with multiple lymphadenopathies at the right and left submandibular and right parotid. Then, the resected tumour continued for histopathological examination. Microscopically, the tumor cells are confined to the tissue. It consists of a uniform large polygonal cell with granular eosinophilic cytoplasm without any mitosis and necrotic area. Conclusions: Oncocytoma is a rare benign tumor of the salivary glands, especially affecting the parotid gland. Clinical and radiologically it is often misdiagnosed, so histopathological examination is needed for diagnostic confirmation for the best patient outcomes

Abstract

Introduction: GISTs usually occur in older adults with a median age of about 60–65 years. The incidence rate between men and women is equal (1:1). The most common location of GISTs is the stomach which accounts for 60%, followed by the jejunum and ileum around 30%, duodenum 5%, and colorectal 5% and a small percentage of cases occur in the esophagus, appendix, gallbladder, and extraintestinal areas such as the mesentery, omentum, and retroperitoneum. Case Presentation: A 59-year-old male, suffered from a lump under the umbilicus 3 months before admission to the hospital. The results of the ultrasound examination showed a mass in the superior area of the bladder, and the mass did not appear to infiltrate the intestines, bladder, umbilicus, and surrounding organs. The biopsy tissues showed spindles and round-to-oval morphology with eosinophilic cytoplasm. Mitoses 65/50 High Power Field In addition, immunohistochemistry examination showed positive stains in DOG 1, CD117, SMA, CD 34, and negative for S100.Conclusion: Extragastrointestinal stromal tumor is a rare tumor. Based on, clinical, imaging, histopathological, and immunohistochemical examination, this case was diagnosed as an extragastrointestinal stromal tumor, spindle cell type.

Abstract

Background: Cervical cancer is a malignant tumor of the cervix with a high mortality rate, primarily identified by vaginal bleeding. Human papillomavirus (HPV) infection was found in 99.7% of cervical cancer cases. Other risk factors include age at first sexual intercourse, parity, smoking, weak immune system, use of oral contraceptives, and a high number of sexual partners. This study aims to analyze and describe the clinicopathological features of cervical carcinoma, including patient demographics, primary symptoms, histopathological types, and tumor staging, at Prof. Dr. I.G.N.G. Ngoerah Hospital from 2021 to 2022.Method: This study employs a descriptive research method with a retrospective approach, utilizing secondary data and comprising 283 samples. Inclusion criteria included patients diagnosed with cervical carcinoma confirmed by histopathological evaluation and complete medical records documenting age, primary complaints, histopathological classification, and TNM staging. Data were processed and presented using descriptive statistics. Results: The results showed that the age group with the highest number of cervical carcinoma cases was the 40-49 years age group (36.0%), the most common main complaint was bleeding (68.9%), the most common histopathological type was squamous cell carcinoma (78.8%), and the most common TNM stage was T1 (52 cases) and N0 (87 cases). Conclusion: This study highlights that cervical carcinoma predominantly affects women aged 40–49 years, with abnormal vaginal bleeding as the most common symptom. Squamous cell carcinoma was the most common histopathological type (78.8%), with the majority of cases staged at T1 (18.4%) and N0 (30.7%).

Abstract

Introduction: Leiomyosarcoma accounts for (~40–50%) of all uterine sarcomas, but only 1-2% of all uterine malignancies. The incidence of leiomyosarcoma increases with age, reaching its peak in the seventh decade of life. Here, we proposed the importance of macroscopic and microscopic criteria of leiomyosarcoma to ensure clinical diagnosis.Case Presentation: A 58-year-old woman came with vaginal bleeding for 3 weeks before admission to the hospital.  The ultrasonography (USG) examination showed enlargement of the uterus and was clinically diagnosed as a leiomyoma. However, on histopathological examination, a proliferation of anaplastic cells was revealed, forming fasciculi that intersect each other and infiltrate into the connective tissue stroma. Mitosis 15/50 HPF and abnormal mitosis with abrupt coagulative necrosis are easily found. The histopathologic examination concluded that the patient had uterine leiomyosarcoma (pT1a), FIGO stage 1B.Conclusion: Histopathological examination and a clinicoradiological approach are key to identifying leiomyosarcoma. In most cases, although it is confined to the uterus, it has a poor prognosis. Otherwise, this case has a more favorable outcome due to the pathological and clinical stage, with 5-year survival rates of 40 – 70%
