Abstract

Peningkatan jumlah penduduk akan memberikan dampak terhadap masalah kesehatan, salah satunya adalah lansia yang mengalami proses penuaan yang sedang dialami. Lansia yang mengalami proses penuaan akan lebih beresiko menderita penyakit degeneratif salah satunya adalah Osteoarthritis (OA). OA kronis akan dilakukan tindakan total knee replacement dan membutuhkan rehabilitasi yang tepat. Faktor yang mempengaruhi pasien post TKR untuk mobilisasi adalah ketakutan bergerak. Dengan kondisi ini menyebabkan seseorang post operasi TKR beresiko terhadap kekakuan otot dan mempengaruhi proses penyembuhan dan aktivitas setiap hari sehingga peneliti ingin mengetahui pengaruh cognitive behaviour therapy dalam menurunkan kinesiophobia pada pasien post operasi total knee replacement. Penelitian ini menggunakan metode quasy experiment dengan pendekatan pre test dan post test with control group. Dengan total sampel 15 responden untuk kelompok intervensi dan 15 responden untuk kelompok kontrol,  Pengumpulan data menggunakan lembar observasi dan kuesioner tampa scale for kinesiophobia dengan nilai validitas 0,79 dan nilai reliabilitas 0,9 kemudian analisis data dilakukan dengan menggunakan paired t-test. Hasil penelitian menunjukkan bahwa kelompok intervensi memiliki nilai p-value 0,001 yang diartikan terdapat hubungan yang signifikan pada kelompok intervensi.

Abstract

Backgrounds: Fractures represent a potential or actual threat to a person's integrity. Fracture management includes conservative measures and surgery. One of the conservative measures is the installation of protective traction. The purpose of traction training is to reduce spasm, reposition and immobilization. Immobilization is a limitation of independence, physical movement of the body or one or more extremities which is often caused by movement restrictions in the form of bed rest, physical restrictions due to external equipment (for example casts or skeletal traction), restrictions on voluntary movements, or physical disorders. There are various physical problems that can occur in a state of immobility, one of which is the problem of changes in fecal elimination, namely constipation. Abdominal massage is a complementary therapy that can be used as an effective intervention to treat constipation without causing side effects. Method: application of Evidence Based Practice (EBNP) with evidence sourcesobtained from the Pubmed journal. The article was published in 2020. Searches were carried out in several databases, namely the Cochrane Library, Biomed Central, MEDLINE (EBSCOhost), MEDLINE (PubMed), and Scopus. The author carried out screening using the PICO formulation. Results: Data analysis results were processed using Jamovi with the one sample T-Test statistical test. The average reduction in constipation was found before and after abdominal massage with a P-Value of 0.001. Conclusion: There is a significant difference in reducing constipation before and after giving abdominal massage with P-Value <0.005.).

Abstract

Fracture is a condition where the continuity of bone tissue is broken. Intertrochanter fractures are fractures that occur in the pelvic bones with a transverse line from the greater trochanter to the lesser trochanter. Broken bones are a potential or real threat to a person's integrity, so they experience physiological and psychological disorders that can cause reactions in the form of pain. Traumatic pain due to bone fractures that damage healthy tissue. The first treatment is to observe whether there are other accompanying injuries, after that skin traction is applied which is considered effective before performing surgery to reduce pain. Objective: to determine the reduction in pain scale in intertrochanteric fracture patients after skin traction was applied. The method in this research uses a pre-experimental design with a one group pretest-posttest design. Data collection techniques used questionnaire sheets and the Numeric Rating Scale (NRS) pain scale. Data analysis used paired T test. The results showed that the intensity of pain before and after being given skin traction had decreased. The conclusion in the study was that the installation of skin traction before surgery could reduce the pain scale in intertrochanter fracture patients.

Abstract

Background; Patients with postoperative femur fractures will experience limited motion in their knee function, weakness, immobility and disability as a result of limitations in caring for themselves and carrying out daily activities. This study is to determine the effect of early mobilization education on the level of independence in postoperative patients with femur fracture. Research methods; The study used a quasi-experimental design with pre and post control group design, with 36 respondents (18 experimental groups, 18 controls). Data collection using questionnaires and observation. The independent variable is early mobilization education, while the dependent variable is the level of independence. The sampling technique used consecutive sampling. The population in this study were patients with postoperative at Prof. Dr. R. Soeharso Surakarta. Data analysis using independent t-test and paired t-test. Research results; There is a significant difference in the level of independence in patients before and after being given early mobilization exercises of intervention group. The results of the independent t-test on the level of independence obtained p value (0.0001) <0.05, there is a significant difference in the level of independence in patients in the control group and intervention group. Conclusion; There is an increase in independence in patients after early mobilization education, and there is a difference in the level of independence in the control group and the intervention group.
